Actinomycosis. Surgical aspects.

Actinomycosis is an anaerobic infection caused by actinomycetes, which are part of the normal flora in the oral cavity and intestine. Antecedent disease or surgery predisposes to infection, and involved tissue becomes indurated and forms multiple draining fistulae discharging characteristic sulfur granules. Three principal clinical syndromes are described: cervicofacial, thoracic, and abdominal. Recently pelvic actinomycosis has become more prevalent and associated with women who use the intrauterine device. The diagnosis of actinomycosis usually is made at surgery. Biopsied material histologically demonstrates sulfur granules and filamentous gram-positive rods. The differential diagnosis includes cancer and other chronic infections. Treatment consists of appropriate antimicrobial therapy and often surgery including incision and drainage or excision of abscesses, drainage of empyemas, and removal of persistent sinuses.

Mycoplasmal pneumonia. Three severe cases of a common infection.

Mycoplasma pneumoniae is one of the most common causes of community-acquired pneumonia. Infection is usually self-limited, but fulminant pneumonia and extrapulmonary complications can supervene. Usually, unilateral confluent, patchy, or nodular infiltrates of the lower lobe are seen on roentgenograms. Diagnosis is achieved by isolation of the organism or by serologic methods. Treatment with either erythromycin or tetracycline is effective, although organisms can be recovered during therapy. In the three cases of mycoplasmal pneumonia reported here, infection was resistant to initial therapy but patients recovered when appropriate antimicrobial therapy was instituted.

Malaria: recent experience in a community.

Although a global resurgence of malaria has occurred along with the appearance of drug resistant organisms, this illness rarely is encountered by physicians in the United States. We report our experience with 15 cases of malaria seen during the last five years. The diagnosis should be considered in any febrile patient who has visited or resided in an area endemic for malaria, has received a transfusion, or is a narcotic addict. Confirmation of the diagnosis is obtained by blood smears for malaria parasites. Treatment is determined by the species and drug resistance of the parasite. Chemoprophylaxis is urged for visitors to countries endemic for malaria.

Clostridium septicum arthritis.

This case of anaerobic septic arthritis caused by Clostridium septicum is apparently the first reported. It emphasizes the need for obtaining anaerobic as well as aerobic cultures on all suspected cases of septic arthritis.
