Transcatheter obliteration of the renal artery by ethanol injection: experimental results.

The sclerosing effect of the introduction of 95% ethanol into the renal artery was evaluated in eight rabbits and one pig. Longterm occlusion of the renal artery with ensuing infarction of the kidney could be produced by a small, easily tolerated dose. Several inherent drawbacks of currently used embolic sgents may be avoided with this technique, which is suggested for clinical trial.

Palliative embolization of renal tumors: follow-up of 19 cases.

Palliative renal artery embolization (without subsequent nephrectomy) was performed in 19 patients with renal carcinoma, and the follow-up is presented. Seventeen of the patients had metastatic disease at the time of embolization. Three patients were alive 11, 52, and 56 months after embolization. Gross hematuria was eliminated in 5 patients. Temporary remission of suspected pulmonary metastases was seen in 2 patients. It is concluded that transcatheter embolization alone is not curable and in this series did not affect the overall survival rate; however, local tumor symptoms may be palliated by this method.

Computed tomography, angiography and ultrasound in staging of renal carcinoma.

Preoperative evaluation of the extent of the renal tumor is of value in determining the prognosis of the patient and the most suitable therapy. CT and angiography were reviewed in 27 cases of renal carcinoma to evaluate tumor involvement of lymph nodes, renal vein, vena cava, and the perinephric space. In 26 of these cases the ultrasound examination was similarly reviewed. Comparison of ultrasound, CT, and angiography demonstrated that staging of renal tumors with ultrasound is mainly limited by its inability to demonstrate extrarenal infiltration. Both CT and ultrasound proved reliable in the demonstration of tumor spread to the renal vein and inferior vena cava. CT and angiography were equally effective in the evaluation of tumor extension; since CT is easier to perform, less invasive and less time consuming than angiography it should be the primary method for staging of renal neoplasms. Angiography offers detailed information on the vascular supply of the tumor important for the choice of surgical approach and should be performed only on special indications.

Antegrade spermatic phlebography in the diagnosis of metastases from testicular tumors.

Antegrade spermatic phlebography was performed in 18 patients in connection with orchiectomy because of testicular expanding lesions and was compared with other diagnostic methods. Fifteen of the cases were teratomas or seminomas. In 6 patients metastases were demonstrated at phlebography and later confirmed. In one case no abnormality was found at phlebography, nor at CT, but at lymphography metastases were found in not enlarged lymph nodes. Due to technical error at phlebography one case was falsely considered abnormal. The technique is described and its use as a screening procedure for metastases is advocated.

CT of soft-tissue tumors.

Computed tomography was performed on 52 patients with soft-tissue tumors, 36 malignant and 16 benign. Major advantages with CT include the possibility of differentiating between lipoma and other tumors and the ability to demonstrate the transverse location of a lesion (intra- or extracompartmental). It was not possible to relate attenuation values to histologic type or grade of malignancy. CT was found superior to conventional radiography in demonstrating additional bony destruction of the pelvis and spine. The risk of overestimating the size of high grade malignant tumors because of accompanying edema is discussed. CT should precede angiography in the investigation of soft-tissue tumors, and angiography may primarily be reserved for those lesions where vascular relationships are not adequately demonstrated by CT.

Comparison between two radiographic contrast media for hysterosalpingography.

Two water-soluble contrast media for hysterosalpingography--one with high viscosity (Perjodal H viscous) and one with low viscosity (Isopaque Amine) were compared with special reference to the development of pain in 61 patients. Patients examined with Isopaque Amine experienced significantly less pain and this contrast medium therefore is suggested as a suitable agent for hysterosalpingography in the investigation of sterility.

Pulmonary surfactant release in fetal rabbits as affected by terbutaline and aminophyllin.

Terbutaline; a beta2-adrenergic agonist, and aminophyllin, a phosphodiesterase inhibitor, were given separately, or in combination, to rabbit fetuses on the 28th day of gestation. The possibility of these drugs causing a release of pulmonary surfactant was evaluated 3 h after i.m. injection directly to the fetus. Lung compliance was studied and lung lavage fluid assessed as regards content to total phospholipid phosphorus, lecithin/sphingomyelin (L/S) ratio, and surface activity. It was found that terbutaline, in a dose of 0.1 mg, had a significant (p less then 0.05) effect on each of the four parameters studied and, when given in a dose of 0.05 mg, significantly affected all except L/S ratio. Aminophyllin, in a dose of 10 mg, resulted in a 40% mortality, but in the surviving fetuses the drug had no significant (p greater than 0.05) effect on any of the four parameters. When 5 mg aminophyllin was given together with 0.05 mg terbutaline, the effect could be attributed to terbutaline.

Staging of bladder carcinoma with computed tomography.

In a series of 24 patients with bladder carcinoma. 17 of whom underwent cystourethrectomy, the discriminatory power of computer tomography (CT) in staging of the tumours was compared with the clinical evaluation. In low-stage tumours the accuracy of staging was not increased by CT. In high-stage tumours CT was greatly superior to clinical examination for assessing neoplastic extension. CT is recommended in bladder carcinoma for which cystourethrectomy is planned, so as to achieve maximum accuracy of information concerning depth of infiltration.

Late recurrence of giant-cell tumor of bone: pharmacoangiographic evaluation.

Late recurrence (more than five years) of benign giant-cell tumor of bone is uncommon. Two patients were evaluated by angiography, including injection of vasoactive drugs, because of osteolytic lesions developing six and seven years after primary operation for benign giant-cell tumor. Angiography established a probable recurrence in both cases. Angiography also permitted accurate evaluation of tumor extension, facilitating pre-operative planning. Giant-cell tumors are predominantly hypervascular lesions. Angiographic evaluation is therefore recommended when a recurrent lesion is suspected.

Pharmacoangiography of the kidney: an overview.

The use of various vasoactive drugs in the angiographic investigation of renal pathology is discussed. It is concluded that vasoconstrictors (e.g., angiotensin) are to be preferred in order to demonstrate neovascularity, whereas vasodilators yield more information about "medical diseases" of the kidney. In spite of the increased use of noninvasive techniques such as ultrasound and computed tomography, it is suggested that pharmacoangiography will still remain a valuable tool in the diagnostic armamentarium of urologic radiologists.

Diagnostic and prognostic value of pedal lymphangiography in testicular teratomas.

Since 1960 pedal lymphangiography was followed by retroperitoneal lymphadenectomy in 60 patients with malignant teratoma of the testis. The accuracy of lymphangiography compared to the histological examination of the retroperitoneal lymph nodes was 88%. The prognostic value of lymphangiography seems to be of importance. Cavography and urography have given little additional information while modern techniques, such as computed tomography and perhaps ultrasound, probably will be of great help in the staging of patients with testicular tumours.

Effect of glucocorticoids on fetal immunoglobulin production after premature rupture of membranes.

Cord blood IgA and IgM levels were measured in newborns of mothers with prematurely ruptured membranes (PRM) who had also received prenatal glucocorticoid treatment to accelerate fetal lung maturity. Previous data show that some newborns of mothers with PRM had elevated IgA and/or IgM levels. This increase was also found in the present glucocorticoid-treated group, indicating that glucocorticoid therapy did not appear to alter fetal humoral immune response at all.

Renal vascular changes as reflected by angiography.

Gray-scale ultrasonography and whole-body CT have narrowed the indications for renal angiography. However, as illustrated here, there remain a number of vascular disorders for which angiography still provides the only means of establishing a correct diagnosis. Some of these cases may also be treated by transcatheter embolization in connection with angiography.

Palliative embolization of arterial renal tumour supply. Results in 10 cases.

Palliative occlusion of the arterial renal tumour supply was performed in 10 patients and the follow up is reported. Nine of the patients had no subsequent nephrectomy. Spongostan (99% gelatin) was used as the embolic material in 4 patients with the addition of steel coils in 2. Bucrylate was used in 6 cases. Six patients are alive with survival rates presently ranging from 3 to 24 months after embolization. Improvement of the survival time cannot be estimated but local symptoms such as hematuria and pain may be treated in those patients with renal tumours who are not considered for surgery.

Angiographic findings in adrenal masses.

The angiographic findings in 34 patients with adrenal lesions were reviewed, including 11 adrenal metastases, 9 adrenal pheochromocytomas, 4 adrenal cortical carcinomas, 4 adrenal cortical adenomas, 3 cases of adrenal hyperplasia and 3 cases of adrenal invasion from carcinoma of the upper pole of the kidney. The results indicate differential diagnostic difficulties in distinguishing among these various entities by angiography.
