The biological reaction to polyethylene wear debris can be related with oxidation of the UHMWPE cups.

BACKGROUND: Oxidised UHMWPE due to gamma irradiation in air has a greater susceptibility to wear than non-oxidised UHMWPE (ethylene oxide, EtO). AIM: To evaluate, the biological reaction of loose implants with oxidised and non-oxidised PE components. MATERIALS AND METHODS: Ten loose PE cups sterilised by EtO (group 1) and 13 sterilised by gamma irradiation in air (group 2) were studied. PE wear and oxidation were related to biological reaction. RESULTS: Group 1: PE cups had low wear score and no oxidation. In the oldest implants, a few PE particles and macrophages were observed in the interface membrane. Group 2: PE inserts had medium-to-high PE wear score and variable oxidation. In the interface membrane, the number and total area of PE particles were high and variable; giant cells were more numerous than macrophages. CONCLUSION: Junctional tissues around loose oxidised PE components contain more PE debris and giant cells than membranes around PE components that are not oxidised.

[Outlining passive information offered by the Drug Information Center of the Federal University of Bahia College of Pharmacy].

OBJECTIVE: To outline passive information offered by the Drug Information Center (DIC) of the Federal University of Bahia College of Pharmacy. METHODOLOGY: Descriptive and prospective study of requests received at the DIC from March, 1997 to December, 2003. Results, represented as frequencies, were obtained by using Microsoft Excel. RESULTS: 1,267 requests were received in the study period. Thirty seven percent (37%) of these involved patients; 73% were done by telephone and 66% were answered the same way; 68% of the requests were made by pharmacists. About 39% of all requests were originated in hospitals and 23% in community pharmacies. A total of 1,459 questions were received, showing a proportion of 1.2 questions per information request, of which: 18% were related to drug indication/use; 13% to adverse reactions; 17% to identification of national and foreign products and active substances; 8% to posology and 6% to drug interactions. Fifty nine percent of the requests were answered within 23 hours.

Thyroid autoimmunity: really an important cause of sporadic congenital hypothyroidism?

The transplacental transfer of maternal antithyroid antibodies has recently been hypothesised as an aetiological factor in CH. In order to test this hypothesis, mothers and newborns identified by neonatal screening as suffering from hypothyroidism were tested for TgAb, MAb and TSHBAb. Significant titres of MAb and TgAb antibodies were found in 5% of the newborns and their mothers. TSHBAb was found in 1 out of 18 newborns and 1 out of 14 mothers. A causal link was found between the transplacental transfer of IgG inhibiting thyroid growth and function induced by TSH from a mother with Hashimoto's thyroiditis a newborn with CH and in her CH newborn. However the present series did not reveal the high percentage of cases with CH and thyroid antibodies reported by others and particularly not among the neonates born to mothers without any kind of maternal thyroid pathology. It therefore seems that the role of autoimmunity in the pathogenesis of CH and TH is yet to be clarified.

Herpes zoster and controlateral hemiplegia in an African patient infected with HIV-1.

One of the neurologic complications of human immunodeficiency virus infection are cerebrovascular accidents. In HIV infected patients, ischemic strokes have been reported secondary to nonbacterial thrombotic endocarditis and cerebral arteritis. We describe an unusual cause of stroke in HIV-1 infection: Herpes Zoster ophtalmicus with contralateral hemiplegia.

The biomechanics of pelvic osteotomy according to the Chiari method.

The authors analyze the biomechanical effectiveness of pelvic osteotomy according to the Chiari method. This type of operation reduces joint loading and increases the joint support surface. The authors examined 18 patients aged an average of 31 years. This series shows that the effectiveness of pelvic osteotomy according to the Chiari method is particularly observed in cases in which joint morphology is best preserved.

[Urinary tract infections in newborns: sensitivity, specificity, and predictive value of urinary screening with the reagent strip test].

Important considerations in evaluating newborn urine examinations by dip-stick method are: ease of performance, the volume of the specimen required for test, the time required to perform the test and the cost. In considering the dip-stick test for the early diagnosis of Urinary Tract Infections (U.T.I.), it has never been tested with later follow-up to evaluate its sensitivity and specificity, also in regard to reduce the risk of further complications for urinary tract and kidney.
