Neuropsychological assessment in patients with relapsing-remitting multiple sclerosis and mild functional impairment: correlation with magnetic resonance imaging.

Forty one moderately impaired patients with clinically confirmed multiple sclerosis (MS) and a relapsing-remitting course were submitted to a neuropsychological battery and magnetic resonance imaging (MRI) to correlate the neuropsychological performances with the degree of cerebral demyelination. The neuropsychological results were indicative of a very mild overall impairment. The patients were subdivided into two groups (extensive periventricular demyelination or discrete lesions on MRI) and the results of neuropsychological tests compared. Patients with extensive periventricular demyelination had an inferior performance on concept formation, non-verbal reasoning and verbal memory tests.

[Structure of the endosperm in the mature seed of Melilotus alba].

The results of the exam at the light, the fluorescence and the scanning electron microscope of the endosperm of Melilotus alba mature impermeable seeds are reported. Cryostat sections, semithin sections and squashes are observed. Melilotus alba endosperm is variable in thickness and envelopes cotyledons and radicle. Its "aleurone" layer is one-cell thick, while the number of layers of its internal cells varies in relation to the location in the seed. In the aleurone cells, the cytoplasm and the outer portion of the wall are autofluorescent; tannic acid-ferric chloride stains the outer portion of the wall and allows to see clearly the inner thickenings, DAPI and haematoxylin demonstrate the presence of the nucleus. The cytoplasm of these cells is coloured by Sudan black b, and its fluorescence is enhanced by auramine and calcofluor white. Calcofluor white enhances the fluorescence of the outer portion of these walls, too, but is without effect on the non-autofluorescent thickening, indicating presence of cellulose only in the first case. Callose is absent. Also the thin autofluorescent walls of the endosperm inner cells react positively to calcofluor. These cells are very large, almost completely filled with "gelatinous" substances--the galactomannans--and very rarely contain a nucleus.

[Adolescents and breast feeding].

The attitudes and future decisions of teenage girls towards infant feeding methods, breast-feeding or bottle feeding have been investigated. A self-conducted questionnaire was completed by 146 secondary school girls ranging in age from 16 to 18 years. Economic convenience, better nutritional value, deeper emotional link with the infant, own and friends past experience nearly always in favour of breast-feeding, represent for the girls the main advantages of breast-feeding. On the other hand, many girls apprehend breast-feeding for various possible physical, psychological and social implications. Some aspects of the problem studied in this paper show a significant correlation with the future choice of infant feeding. The girls would be interested in receiving information at school. Suitable information during school health lessons may positively influence teenage girls to adopt breast-feeding as the infant feeding method of choice.

Interobserver agreement in the diagnosis of multiple sclerosis.

Interobserver agreement in the clinical diagnosis of multiple sclerosis (MS) among six neurologists was evaluated. Three of them participated in a study of the clinical diagnosis of MS, the Italian Multicenter Study (IMS). The raters examined the clinical forms of MS of 50 patients randomly selected from among 430 patients recruited from the IMS. For each patient, neurologists were asked to make a diagnosis according to the McDonald-Halliday classification system of MS. The overall agreement on the diagnosis (MS present or absent) was fair, with no difference noted between the two groups of raters. Considering the six diagnostic levels instead, the reliability was higher for the neurologists participating in the IMS program. These neurologists agreed particularly on the Clinically Definite and Progressive Possible classifications. Complete disagreement was observed for the Early Probable or Latent and Progressive Probable classifications. Because of the different level of agreement on diagnosis, we suggest separate consideration of Clinically Definite and Progressive Probable MS cases in clinical trials and epidemiologic studies of this disease.

The influence of nutritional status on skin anergy in experimental cancer.

Malignant tumours as well as severe malnutrition can interfere with carcass weight, serum albumin and delayed hypersensitivity tests. In order to identify the separate effects of these two phenomena, Wistar rats were submitted to various nutritional manipulations prior to Walker-256 carcinosarcoma inoculation (TU). Six groups of well-nourished rats with positive response to the Keyhole Limpet Haemocyanin (KLH) test were studied. Three TU-bearing groups were established, [1] protein-deprived before and after TU (group DDT), [2] protein deprived before TU and later brought back to normal chow (group DNT), and [3] those receiving a complete diet throughout the experiment (group NNT). Identical control groups without TU, sham-injected, were called DDC, DNC and NNC. Tumour size and body weight were measured daily. KLH tests were done sequentially and serum albumin was determined at sacrifice. Results indicated that cancer produced marked changes in immune response. Malnutrition was responsible for anergy and weight loss, and further aberrations tended to occur when both conditions were present. Feeding was able to restore body weight and KLH response in tumour-bearing as well as in control rats, but subsequent deterioration occurred in cancer bearing rats after the first week of tumour development. It is concluded that cancer potentiates the effects of malnutrition, on body weight and immune response, but temporary recovery could be demonstrated after adequate refeeding.

Inter- and intra-pathologist variability in the diagnosis of gestational trophoblastic neoplasia.

All 190 cases of gestational trophoblastic neoplasia diagnosed in the Baltimore metropolitan area from 1975-1982 were identified. Histologic slides were requested and reviewed independently by two pathologists who agreed upon uniform criteria for the diagnosis of hydatidiform (complete) mole, invasive mole, and choriocarcinoma. A representative sample of the slides was selected and resubmitted to one of the study pathologists for a second review. The inter- and intra-pathologist variability in the diagnosis of gestational trophoblastic neoplasia was calculated using the kappa statistic (K). Our findings indicated that the variability in the diagnosis of gestational trophoblastic neoplasia was low whereas that for the related tumor of incomplete mole was high.

Neurological complications of cerebral angiography.

We report a prospective study of 218 consecutive patients undergoing cerebral diagnostical angiography, before during and 24 hours after the procedure, to identify the neurological complication rate and the risk factors related to the patients and to the procedure. We observed 15 neurologic accidents (6.9%) with permanent sequelae in one case (0.4%). Two risk factors proved to correlate significantly with accidents, i.e. the time that the catheter remained within a vessel and difficulty in performing the procedure.

Ovarian function after pelvic lymph node irradiation in patients with Hodgkin's disease submitted to oophoropexy during laparotomy.

Staging laparotomy for Hodgkin's disease was carried out on 110 women in fertile age at the General Surgery Department of Padua Medical School from 1972 to 1977. An oophoropexy was done during the operation. According to the stage the patients underwent radiation therapy that was associated in some cases to chemotherapy. Complete evaluation was possible only in 66 patients. Eighteen out of the 66 had mantle field and para-aortic lymph node chain radiotherapy. One case of amenorrhoea (5.5%) and five pregnancies (27.8%), were found among them. Nineteen had the same type of radiotherapy with MOPP. 3 cases of amenorrhoea (15.8%) and 3 pregnancies (15.8%) were found. Five had total nodal irradiation (TNI) according to Kaplan and all of them had amenorrhoea (100%). Twenty four had TNI associated to MOPP with 24 cases of amenorrhoea (100%). Our results slightly differ from those of other Authors. They showed: 1) that the oophoropexy is not effective using standard surgical technique (probably because of the size of the central shielding) as it causes amenorrhoea in 100% of the cases; 2) the incidence of permanent amenorrhoea from chemotherapy with MOPP is the same as reported by other Authors; 3) the absence of teratogenic effects by chemotherapy in newborns of these patients.

Androgen producing adrenocortical carcinoma.

Two cases of androgen secreting adrenocortical carcinoma have been described by light and electron microscopy. The histological and ultrastructural features of the tumour cells were similar to those of compact cells of zona reticularis and to those described in virilizing adenomas. They possess numerous mitochondria with lamellar and tubular cristae, abundant smooth endoplasmic reticulum, lipofuscin bodies and scanty lipid. Irregularly shaped, crenated mitochondria, with outpouchings of the outer limiting membrane have also been observed. The clusters of neoplastic cells were surrounded by basement membrane which demonstrated a focal discontinuity, probably reflecting malignancy of the tumours. Hyperplasia of smooth endoplasmic reticulum and the presence of outpouchings of the mitochondrial outer limiting membrane might be the morphological manifestation of endocrine activity of the tumours.
