[The value of microsurgical techniques in tubal surgery (author's transl)].

Tubal surgery by help of microsurgical techniques was performed in 86 women from January 1976 until December 1978. 70 patients suffered from tubal sterility due to pelvic inflammatory disease. 16 women asked for reversal of sterilization. As result of 83 operations 17 life births respectively undisturbed intrauterine pregnancies were achieved. Two patients had an abortion. Three more pregnancies could not be added to the success rate, because in these women we found only one blocked tube intraoperatively, while the contralateral one seemed to be intact. Up to now we haven't had an ectopic pregnancy. In 39 patients the operation was done within the last half year. The value of microsurgery in the field of tubal reconstructive surgery is discussed in regard to the literature and to our own experiences and operative results.

Long-term followup after exstrophy closure: late improvement and good quality of life.

Now that we have observed the growth and development of 17 of our anatomically reconstructed exstrophy patients through 20 or more years of followup, we are convinced that reconstruction should be the preferred primary treatment. Most of these patients are healthy, attractive, well educated adults and have achieved emotional maturity and stability, despite their many problems. Anatomical reconstruction also has considerable advantages over other current treatments for exstrophy that were not apparent initially.

[Gynecological and obstetric problems in connection with stomatology].

Hormonal changes during pregnancy produce changes in the whole organism. Every fifth to seventh pregnant woman shows hyperplastic gum changes with tendency to bleeding. Changes due to pregnancy in the oral cavity are: lowered salivary flow rate, changes of the pH towards acidity and an increase in oestrogen contents of saliva, corresponding to the increase in plasma oestrogen. Dental procedures during the first months of pregnancy could be the cause of miscarriage in rare cases; after the 28th to the 30th week of pregnancy external influences can originate premature labour pains. Endocrinological changes during the menstrual cycle give risk to recognisable cycle-dependent changes in the oral mucosa; changes in the blood clotting mechanism are minimal, so that dental interventions can be practiced independent of the menstrual cycle and the occurrence of the menstrual period. The use of oral contraceptives (pills) leads to hypercoagulability of the blood clotting system. For bigger operative interventions it is advisable to normalise the clotting factors by stopping the pill for six weeks prior to planned intervention.

[Follow-up of patients who underwent reduction mammoplasty according to Strombeck between 1972 and 1975. Postoperative complications and results. Modification of the technique. Postoperative mammographic results (author's transl)].

Follow-up examinations were carried out in 60 women who underwent a reduction mammoplasty. Their review concentrated on the questions of the frequency of postoperative complications, the possibility to avoid complications by a modification of the technique and the question of mammographic evaluation of the breast in the face of scars from the mammoplasty. There were 12 severe complications. There were 6 necroses of the nipples, there were 3 abscesses, there were 2 suture fistulas and there was one case of massive hematoma. The complications depend on the amount of tissue resected. Especially necroses of the nipples depend upon the amount of tissue resected. With increasing experience of the surgeon, the nipple is moved without tension by lateral division of tissues and omission of resection of tissue above the nipple. This reduces the risk of necrosis of the nipple. Ugly scars are avoided by additional subcutaneous dexon double O sutures and additional subcuticular structures. Prolonged immobilization and later removal of the sutures help to obtain fine scars. A comparison of the preoperative and postoperative mammographies showed that 78% of the X-rays could be evaluated for carcinoma more readily postoperatively than before. For the differential diagnosis to scars from the operative, a preoperative and postoperative mammogram is necessary for comparison.

[Reconstruction of fallopian tubes using microsurgical technics (author's transl)].

The results of reconstructive tubal surgery improved significantly since microsurgical technics were introduced. Nowadays microsurgery is the procedure of choice for salpingolysis, salpingostomy, reanastomosis reversal of sterilisation and conservative repair of an ectopic pregnancy. Successful reversals of sterilisation demonstrate the advantage of the microsurgical technics in the best way. Tubal sterilisation isn't irreversible any longer but it still should be recommended as a definitive method. Because of the high technical effort reversal of sterilisation can be done in exceptional cases only. In young women the possibility of a reversal must be considered and a sterilisation operation should be done in an adequate and careful way.

Fine structure of tarsal sensory organs in the whip spider Admetus pumilio (Amblypygi, Arachnida).

The sensory organs on the tarsi of the antenniform first legs of the whip spider Admetus pumilio C. L. Koch (Amblypygi, Arachnida) were examined with the scanning and transmission electron microscope. At least four different types of hair sensilla were found: (1) thick-walled bristles, which have the characteristics of contact chemoreceptors (several chemoreceptive dendrites in the lumen plus two mechanoreceptors at the base); (2) short club sensilla, innervated by 4-6 neurons which terminate in a pore on the tip; they are possibly humidity receptors; (3) porous sensilla, which are either innervated by 20-25 neurons and have typical pore tubules, or they have 40-45 neurons but no pore tubules; both types are considered to be olfactory; (4) rod sensilla occur in clusters near segmental borders; they are innervated by only one large dendrite which branches inside the lumen. Other tarsal receptors are the claws, which correspond to contact chemoreceptors, and the pit organ which resembles the tarsal organ of spiders. Compared to other arthropod sensilla, the contact chemoreceptors are very similar to those of spiders, while the porous sensilla correspond structurally to olfactory receptors in insects; the club and rod sensilla seem to be typical for amblypygids.

Childhood chemotherapy and later drug abuse and growth curve: a follow-up study of 30 adolescents.

The authors studies the correlation of methylphenidate medication in childhood with later drug abuse and growth in height. Thirty adolescents with histories of minimal brain dysfunction who had been treated for at least six months with chemotherapy were compared with 30 adolescents who had not been so treated. The findings do not indicate that methylphenidate prescribed in childhood contributed to later drug abuse or seriously interfered with growth in height.
