Reduction of nitrite production by endothelin-1 in isolated porcine ciliary processes.

It has been postulated that endothelin-1 and nitric oxide (NO) could participate in the modulation of aqueous humor dynamic in the eye. This study investigates whether endothelin-1 can reduce the production of nitrite (a stable metabolite of NO) in isolated porcine ciliary processes. Nitrite production was measured (Griess reaction) in the medium surrounding isolated ciliary processes before and after exposure to different drugs. Results are expressed in percent of basal nitrite production. In a concentration-dependent manner (0.1 nM to 1 microM), endothelin-1 significantly decreased basal nitrite production (1 microM: 81.7+/-3.5%; P<0.001). This effect was prevented by the endothelin type A (ETA)-receptor antagonist BQ123 (1 microM: 95.6+/-4.3%; P<0.05), but not by the endothelin type B (ETB)-receptor antagonist BQ788 (1 microM: 83.3+/-4.4%; P=0.86) or by the prostaglandin F2alpha analog unoprostone (30 microM: 86.7+/-3.7%; P=0.74). The adenylcyclase activator forskolin significantly increased basal nitrite production (1 microM: 143.4+/-3.9%, P<0.001). This effect was prevented both by endothelin-1 (0.1 microM; 113.9+/-6.7%: P<0.01) or the nitric oxide synthase inhibitor L-NAME (0.5 mM: 103.3+/-8.4%: P<0.001). The inhibitory effect of endothelin-1 on forskolin-induced nitrite production was significantly reversed by BQ123 (1 microM: 132.4+/-5.1%; P<0.01), but not by BQ788 (1 microM: 112.7+/-4.1%; P=0.64) or unoprostone (30 microM: 109.3+/-4.8%; P=0.98). These results suggest that endothelin-1, through an ETA receptor activation, can reduce both basal and forskolin-induced nitrite production in isolated porcine ciliary processes.

[Application of capsular tension ring in cataract surgery for zonular dialysis].

OBJECTIVE: To evaluate the application of capsular tension ring (CTR) in the cataract surgery for zonular dialysis. METHODS: Eleven cases of partially traumatized zonular dialysis patients were managed with CTR after continuous curvilinear capsulorrhexis (CCC), cataract extraction and posterior chamber intraocular lens (IOL) implantation. RESULTS: The IOLs in all the eyes were basically in the normal position without tilting or obvious decentration. One month postoperatively, the best-corrected visual acuity (BCVA) was 0.4 in 4 eyes, 0.5-0.8 in 5 eyes and over 0.8 in 2 eyes. A side-by-side overlap of both eyelets of the ring happened in 1 eye due to the bigger diameter of CTR, and no other complications were seen except the original complication of cataract surgery. CONCLUSIONS: For the patients of zonular dialysis, the CTR can preserve the integrity of the capsular bag, provide the necessary capsular space while giving a counter-traction, reduce asymmetric capsular forces, stabilize the vitreous base, facilitate phacoemulsification, aspiration of cortex and IOL implantation, prevent the postoperative IOL decentration and allow a quicker visual recovery. The CTR is a useful device that effectively supplements the currently available cataract surgery armamentarium, and develops the application of posterior chamber IOL implantation.

Caspase-3 and its inhibitor Ac-DEVD-CHO in rat lens epithelial cell apoptosis induced by hydrogen in vitro.

OBJECTIVE: To investigate the role of caspase-3 and its inhibitor Ac-DEVD-CHO in rat lens epithelial cell apoptosis induced by hydrogen peroxide (H(2)O(2)) in vitro. METHODS: Rat lenses were incubated in modified Eagle's medium containing 2 mmol/L H(2)O(2) to induce apoptosis in vitro. Apoptosis in lens epithelial cells was assessed by transmission electron microscopy and annexin V-propidium iodide (PI) double staining flow cytometry after 12, 24 and 48 h of incubation. The activity of caspase-3 was analyzed by western blotting. RESULTS: Observations under transmission electron microscopy revealed that 2 mmol/L H(2)O(2) could effectively induce lens epithelial cell apoptosis in vitro. Caspase-3 activity increased during cell apoptosis and the peak measurement occurred at 24 h after treatment with H(2)O(2). Cell apoptosis was blocked by caspase-3 inhibitor Ac-DEVD-CHO. CONCLUSIONS: The activation of caspase-3 plays an important role in executing apoptosis in H(2)O(2)-treated lens epithelial cells and in the formation of cataract. The caspase-3 inhibitor Ac-DEVD-CHO may effectively prevent lens epithelial cell apoptosis caused by oxidative injury.

[Clinical research of the implantation of phakic anterior chamber intraocular lens for high myopia].

OBJECTIVE: To investigate the efficacy and safety of the implantation of phakic anterior chamber intraocular lens (IOL) for high myopia. METHODS: A consecutive group of 73 eyes in 41 patients with -7.00 to -30.00 diopters (D) of myopia were implanted. RESULTS: All of 73 eyes were implanted successfully and have been followed-up for 3 m (months). The uncorrected visual acuity was from FC/33 cm to 0.2 pre-operatively and 0.1 to 1.0 3 m post-operatively. The best corrected visual acuity (BCVA) was from 0.05 to 1.0 pre-operatively and 0.1 to 1.0 3 m post-operatively. The post-operative BCVA of every eye was not worse than the pre-operative one. The refractive diopters were from -7.00 to -30.00 D pre-operatively and -6.00 to +2.50 D 3 m post-operatively. There were no significant differences between pre- and 3 m post-operative mean corneal astigmatism (t = 1.751, P = 0.082) and mean intraocular pressure (IOP) (t = 1.181, P = 0.240), respectively. The mean counts of endothelial cells was (2 680 +/- 538)/mm(2) pre-operatively and (2 514 +/- 420)/mm(2) 3 m post-operatively. There was no significant difference (t = 1.182, P = 0.242) though it decreased 6.19%. No severe complications occurred. CONCLUSIONS: The implantation of phakic anterior chamber IOL for high myopia is predictable, reversible and controllable with simple manipulation. No severe complication occurred in 3 m post-operatively, and long-time follow-up is still required.

[A quantitative study on early changes in rabbit lens capsule epithelium induced by low power density microwave radiation].

OBJECTIVE: To test the safety of guidelines for limiting exposure to low power level microwave radiation and to try to explore the mechanisms of early damages induced by such radiation in rabbit lens capsule epithelial cells, using flow cytometry as a quantitative method of evaluation of these damages. METHODS: The rabbit eyes were exposed to 5 mW/cm(2) (group I) and 10 mW/cm(2) (group II) power densities of microwave for 3 h. Annexin V-PI two-color flow cytometry was used to detect the changes of rabbits lens epithelial cells, apoptosis cells and secondary necrotic cells after radiation. The other eyes served as self-controls. RESULTS: In group I, the percentage of early apoptosis cells in the experimental eyes was significantly greater than that of the controls (P < 0.01), whereas no significantly difference in the percentage of secondary necrotic cells could be observed (P > 0.05). In group II, there was no significant difference between the percentage of early apoptosis cells in experimental eyes and the controls (P > 0.05), whereas the percentage of secondary necrotic cells in the experimental eyes was significantly greater than that of the controls (P < 0.05). The percentage of secondary necrotic cells (16.2 +/- 5.6)% in group II is greater than that in group I (10.3 +/- 6.8)% (P < 0.05), though there is no significant difference in the percentage of early apoptosis cells between these two groups (P > 0.05). CONCLUSION: Low power density of microwave radiation (5 mW/cm(2) and 10 mW/cm(2)) can induce irreversible damage of rabbit lens epithelial cells in a dose-dependent manner. Therefore, the safety dosage criterion of microwave exposure needs further investigation.

[Action of miosis of 1% pilocarpine liposome in rabbits].

OBJECTIVE: To compare the action of miosis of 1% pilocarpine liposome with 1% pilocarpine solution in rabbits. METHODS: 18 white rabbits were randomly divided into 3 groups. Test group received 1% pilocarpine liposome, positive control group received 1% pilocarpine solution, negative control group received liposome. Each eye drop instilled into left eye of rabbits and sterile saline solution instilled into right eye as control. The pupil diameter was measured at time intervals of beginning, 0.25, 0.5, 1, 2, 3, 4, 5, 7 hours. RESULTS: The mean pupil diameter change of 3 groups in both eyes was not significant (P > 0.05) at beginning. The strongest action of miosis took place 0.25 h in positive control group and 0.5 h in test group after instillation. The dilation of pupil in both groups took place 1 h and 3 h, and the restoration of pupil in both groups took place at 5 h and 7 h. The mean pupil diameter of negative control group was not significant in seven hours. CONCLUSIONS: The results suggest that 1% pilocarpine liposome improves the bioavailability and prolong the duration of its action.

[Prevention of oxidative cataract by PD150606 - an inhibitor of calpains].

OBJECTIVE: To observe oxidative cataract inhibition of PD150606 by studying the opacity of the lens, the variation of protein and cells, and by further studying the role of calpains in the mechanism of oxidative cataract. METHODS: Rat lenses were cultured in vitro with H(2)O(2) and PD150606. The lenses were observed under the microscope. Simultaneously, photograph analysis was performed in order to detect the variation of opacity; the proportion of WSP was determined by the method of Lowry; the number of apoptotic lens epithelial cells (LEC) was measured by flow cytometry; the expression of Caspase 3 in LEC was determined by SAS-PAGE electrophoresis and immunoblotting. RESULTS: Opacity of lens was enhanced in 24 hours when cultured with H(2)O(2) and PD150606. There were significant differences of relative gray scale between H(2)O(2)-induced and control group at 6, 12 and 24 hours (P < 0.05), significant differences of the proportion of WSP between H(2)O(2)-induced and control group at 12 and 24 hours (P < 0.01), significant differences of the number of apoptotic LEC at 6, 12 and 24 hours (P < 0.01) and significant differences of expression of Caspase 3 at 6, 12 and 24 hours (P = 0.000, 0.005, 0.004). CONCLUSION: PD150606, an inhibitor of calpain, can inhibit H(2)O(2)-induced opacity of lens, proteolysis and apoptosis of LEC partially. The activated calpains can induce degradation of crystallins and apoptosis of LEC, which may play a role in the mechanism of oxidative cataract. Caspase 3-dependent pathway may contribute to apoptosis of LEC induced by calpains.

[Effect of low-intensity microwave radiation on proliferation of cultured epithelial cells of rabbit lens].

OBJECTIVE: To compare the effects of different doses of microwave on the proliferative activity and cell cycle of cultured epithelial cells of rabbit lens, and to investigate the limit tolerant of microwave exposure. METHODS: Cultured epithelial cells of rabbit lens were exposed to microwave radiation with frequency of 2,450 MHz and power density of 0.10, 0.25, 0.50, 1.00, 2.00 mW/cm(2) for 8 h in vitro. HE staining was used to observe the morphological changes of lens epithelial cells, the proliferative activity and cell cycle were measured by MTT assay and PI fluorescent staining. RESULTS: 8 h after radiation, 0.50, 1.00 and 2.00 mW/cm(2) microwave could decrease the proliferation of lens epithelial cells, make the cells disordered arrangement, shrinkage, detachment, and inhibit the synthesis of cell DNA. The percentage of G(0)/G(1) phase cells were 71.95% +/- 2.12%, 75.68% +/- 3.35% and 82.40% +/- 8.68% respectively, which were higher than that in control group (61.68% +/- 5.76%, P < 0.05 or P < 0.01). The percentage of S phase cells were 19.32% +/- 3.07%, 16.08% +/- 4.91% and 12.98% +/- 8.08% respectively, which were lower than that in control group (28.05% +/- 5.12%, P < 0.05 or P < 0.01). No obvious changes could be detected in 0.10, 0.25 mW/cm(2) microwave groups (P > 0.05). CONCLUSION: Microwave exceeding 0.50 mW/cm(2) may make injury to lens epithelial cells after 8 hour radiation, which may be related to the effect of microwave radiation on cell cycle.

[Expression and modulation of basic fibroblast growth factor in bovine lens epithelial cells].

OBJECTIVE: To evaluate the expression of basic fibroblast growth factor (bFGF) in bovine lens epithelial cells (BLEC), the effects of fetal bovine serum on the expression of bFGF and to explore the effect of breakdown of blood-aqueous barrier on the expression of bFGF after cataract surgery. METHODS: BLEC were isolated and cultured. Western blot and immunohistochemistry method were used to determine the expression of bFGF in these cells. The dose effects (0.2%, 1.0%, 10.0% fetal bovine serum) and time effects (4, 8, 12, 24 h) of 10.0% fetal bovine serum on the expression of bFGF in BLEC were estimated by Western blot analysis. RESULTS: Immunocytochemical study revealed that the cytoplasm of BLEC could be stained positively by the anti-bFGF antibodies. Western blot analysis revealed the presence of bFGF with a molecular weight of 18 000. The expression of bFGF was upregulated in a dose-dependent and time-dependent manner by fetal bovine serum. There was statistically significant difference among the control group and the fetal bovine serum treated group (P < 0.05). CONCLUSIONS: BLEC express relatively low molecular weight bFGF. Fetal bovine serum can upregulate the expression of bFGF in the BLEC. Breakdown of blood-aqueous barrier after cataract surgery can stimulate the expression of bFGF in lens epithelial cells through increasing the serum level in the aqueous humor.

[Effect of N-acetyl-L-cysteine and catalase on apoptosis of lens epithelial cell and the activity of caspase-3].

OBJECTIVE: To investigate the effect of antioxidants N-acetyl-L-cysteine and catalase on H(2)O(2)-induced apoptosis of lens epithelial cells and the activity of caspase-3. METHODS: Rat lenses were incubated in MEM with 2 mmol/L H(2)O(2) (H(2)O(2) groups) or without H(2)O(2) (control groups), antioxidants 100 micromol/L N-acetyl-L-cysteine and 900 U/ml catalase were used to block the oxidative injury of lens epithelial cells respectively. Lens opacification and apoptosis of lens epithelial cells were detected using transmission electron microscope and AnnexinV-PI staining after 24 hours incubation. The activity of caspase-3 was analyzed by Western blot at the same time. RESULTS: The observations revealed that 2 mmol/L H(2)O(2) could apparently induce lens opacification and lens epithelial cell apoptosis in vitro, the apoptosis rate increased to (31.20 +/- 3.31)% after 24 hours incubation, which is also the time caspase-3 was activated. Treatment with N-acetyl-L-cysteine and catalase could inhibit lens opacification, apoptosis and caspase-3 activation induced by H(2)O(2). The apoptosis rate decreased to (20.90 +/- 3.16)% and (15.02 +/- 2.41)% respectively (P < 0.01). CONCLUSION: These data indicate that antioxidants N-acetyl-L-cysteine and catalase, possibly through regulation of the activity of caspase-3, can prevent lens opacification and apoptosis of lens epithelial cells.

[Aqueous humor penetration of topically applied ofloxacin, ciprofloxacin and tobramycin].

OBJECTIVE: To compare the aqueous humor penetration of topically applied ofloxacin, ciprofloxacin and tobramycin in human eyes. METHODS: Senile cataract patients (125 eyes in 125 cases) receiving phacoemulsification were randomized into three groups: ofloxacin, 42 eyes; ciprofloxacin, 41 eyes and tobramycin, 41 eyes. Each group was randomized into five sub-groups, with 8 or 9 eyes in each sub-group. Each group received either 0.3% ofloxacin, ciprofloxacin or tobramycin topical drops preoperatively. Eyedrops were instilled one drop every 15 minutes for six times. Aqueous samples were obtained from different sub-groups during surgery, 7.5, 15.0, 30.0, 60.0 and 120.0 min after the last drop, respectively. Concentration of the antimicrobial agents was determined by the HPLC (high pressure liquid chromatography). RESULTS: Aqueous humor concentration of ofloxacin was significantly higher than that of ciprofloxacin at the same period after the last drop. The bioavailability of ofloxacin was 6 times that of ciprofloxacin. Tobramycin could not be detected by HPLC, so its concentration was lower than the minimum detectable level by HPLC (0.05 micro g/ml). CONCLUSION: Ofloxacin has the best aqueous penetration in human eyes, as compared with ciprofloxacin and tobramycin, which suggests ofloxacin could be the preferred topical medicine for the prevention and treatment of endophthalmitis.

Expression and effect of basic fibroblast growth factor on human cataract lens epithelial cells.

OBJECTIVE: To detect the expression of basic fibroblast growth factor (bFGF) in human ocular tissues, and to assess the effect of bFGF on the proliferation of human cataract lens epithelial cells (LECs) and its correlation with age. METHODS: Enucleated eyes were subjected to immunostaining for bFGF protein. Human cataract LECs were cultured in vitro, and treated with bFGF for 48 hr. Proliferation was estimated by the positive area ratio of proliferating cell nuclear antigen (PCNA) in immunohistochemistry. RESULTS: bFGF protein was found in various human ocular tissues. bFGF stimulated human cataract LEC proliferation, and there was an age-related decrease in responsiveness of human cataract LECs to bFGF (P < 0.05). CONCLUSION: bFGF might play an important role in the proliferation of residual human cataract LECs after cataract surgery.

[A long-term observation of laser in situ keratomileusis for myopia].

OBJECTIVE: To evaluate the long-term results of laser in situ keratomileusis (LASIK) for the correction of myopia. METHODS: A total of 1 285 eyes with preoperative refraction of -1.50 - -25.00 diopters were treated with LASIK. Based on the preoperative refraction, the patients were divided into 3 groups (-1.50 - -6.00 D, -6.12 - -12.00 D, -12.12 - -25.00 D). The mean follow-up was 26.7 months and the results were concluded and analyzed. RESULTS: At two years, respectively 93.4%, 86.7% and 65.5% in the above three groups were within +/- 1.00 D, 84.4%, 79.7%, 42.1% were within +/- 0.50 D; the uncorrected visual acuity was >/= 0.5 in 99.6%, 93.8% and 62.6% of the patients, and 90.4%, 68.6%, and 27.2% had vision of >/= 1.0. Thirty-nine eyes with regression or under correction, and 2 eyes with epithelial ingrowth had been retreated with LASIK. CONCLUSIONS: LASIK is effective and safe for the correction of mild, moderate, high and super-high myopia. The results of follow-up for two years are stable.

[The expression of calpain II in rat lens epithelial cells of hydrogen peroxide-induced cataract].

OBJECTIVE: To study the expression and role of calpain II in rat lens epithelial cells (LECs) of hydrogen peroxide (H(2)O(2))-induced cataract. METHODS: Rat lenses were cultured in vitro and cataract was induced by 2 mM H(2)O(2). The lenses were observed under microscope. Simultaneously, photographs and picture analyses were done in order to detect the variation of the opacity. The expression of calpain II in rat LECs was detected with immunohistochemical method and compared with a control group. RESULTS: When the lenses were cultured in 2 mM H(2)O(2) for 3 hours (h), the expression of calpain II in rat LECs was increased obviously. There was significant difference between H(2)O(2)-induced and the control group (P = 0.006). After 6 h, vesicles appeared at the equator of the lenses. There was a significant difference between the result of picture analysis of H(2)O(2)-induced and control group (P = 0.013). So the expression of calpain II in rat LECs of H(2)O(2)-induced cataract was increased before cataract occurred. After induced with H(2)O(2) for 24 h, the opacity of lenses and the expression of LECs were both increased as compared with that at 6 h (P = 0.000, 0.000). CONCLUSION: H(2)O(2) can enhance the expression of calpain II in rat LECs which may play a role in the mechanism of oxidative stress-induced cataract.

[Expression and significance of vascular endothelial growth factor in rat cornea after cautery with alkali].

OBJECTIVE: To investigate the expression of vascular endothelial growth factor (VEGF) in rat cornea after cautery with alkali. METHODS: In Sprague-Dawley rats, inflammatory corneal neovascularization was induced by cautery with alkali. VEGF was detected in corneal samples at different times by Western-blot, and immunohistochemistry method was used to investigate the distribution of VEGF at rat cornea after cautery with alkali. RESULTS: The normal rat corneas did not express VEGF. The expression of VEGF increased with time after corneal cautery. The rat corneas were infiltrated by massive inflammatory cells that, especially adjacent to the cautery lesion, showed staining for VEGF. CONCLUSION: VEGF production by inflammatory cells participates in inflammatory angiogenesis in rat corneas. VEGF has the effects on the induction and supporting of cautery-induced angiogenesis in rat cornea.

[Incidence of vitreoretinal pathologic conditions in myopic eyes after laser in situ keratomileusis].

OBJECTIVE: To determine the incidence of vitreoretinal pathologic conditions in myopic eyes after laser in situ keratomileusis. METHODS: Vitreoretinal pathologic conditions of 1981 consecutive eyes (995 patients) having undergone laser-assisted in situ keratomileusis for the correction of myopia were studied. Preoperative and postoperative basic examinations included visual acuity, manifest and cycloplegic refraction, slit-lamp microscope examination, applanation tonometry and a fundus examination after pupil dilatation by indirect ophthalmoscopy and biomicroscopy with spherical lens of + 90 diopters. Before laser in situ keratomileusis, preventive treatment was carried out for predisposing lesion of retinal detachment in 8 eyes: 6 eyes for lattice degeneration and 2 eyes for atrophic holes. Postoperative examinations were conducted at 1, 3 and 12 months and once a year thereafter. All eyes were followed up for >/= 12 months. RESULTS: Eyes were followed for a mean of (18.40 +/- 4.50) months (range 12 - 28) after the surgery. Sixteen eyes of 13 patients (0.81%) developed vitreoretinopathy after LASIK, including 6 eyes with lattice degeneration (0.30%) in which one of them had previous laser treatment, 2 with posterior vitreous detachment (0.10%), 2 with macular hemorrhage (0.10%), 4 with rhegmatogenous retinal detachment (0.20%), and 2 with retinal tear without retinal detachment (0.10%) in which one of them had previous laser treatment for lattice degeneration. Five patients were males (5 eyes involved). Others were females. Mean age of the group with vitreoretinal pathologic conditions was 31.80 +/- 5.85 years (range 22 to 43). The interval between refractive surgery and development of vitreoretinal complication was (10.38 +/- 6.20) months (range 1 to 24). The eyes that developed vitreoretinopathy had myopia -4.75 to -15.00 diopters (mean -9.45 +/- 2.61 D) before LASIK. The comparison of incidences of vitreoretinopathy after LASIK between the group of >/= -6.00 D and < -6.00 D before surgery showed significant difference (P < 0.01, chi(2) = 60.78). The comparison of incidences of vitreoretinopathy after LASIK had also significant difference (P < 0.01, chi(2) = 138.64) between the eyes with pre-LASIK lattice degeneration and dry hole and eyes without such lesions. The cases of lattice degeneration and retinal tear were treated with laser retinopexy. All cases of rhegmatogenous retinal detachment were managed with cryoretinopexy and scleral buckling. Retinal reattachment was attained in all eyes and good visual acuities were recovered. CONCLUSION: No direct cause-effect relationship between LASIK and vitreoretinopathy can be proven from this study. Although the incidence of vitreoretinal pathologic conditions in myopic eyes after laser in situ keratomileusis is low, it is necessary to strictly filter candidates. Preoperatively and postoperatively, pay attention to the lattice degeneration and other retina lesions, and long-term follow-up is important.
