[Electroresection and electrocoagulation as therapy for vulvar neoplasms].

A survey of the results attained by the most prominent authors of the individual therapy methods is given. After electro-resection and -coagulation of the vulvar tumor followed by roentgen or telecobalt irradiation, patients are almost without pain. The primary mortality in 332 patients was 1,5% (= 5 cases) because lymphonodectomy was performed in only 66 cases (20%). The period of hospitalization lasts about four weeks and cosmetic healing is excellent. This therapy method can also be recommended for largely extended tumors as lack of local symptoms can be achieved up to 97%. Of the 332 patients treated at our hospital, almost equal numbers of patients were seen with carcinoma stage I and II and stage III and IV. In stage I and II (negative lymph nodes) 74,5% and in stage III and IV (positive lymph nodes) 38,4% were cured. 80% were aged between 61 and 90 years. In spite of this negative selection of patients which was also combined with the intercurrent mortality rate of 13% (= 43 cases), 193 of the 332 patients or 58,1% survived five years or longer. In the last five years the survival rate was even 62,2%. A survey of the results obtained by electrocoagulation in the last 30 years is given. Especially the enormous increase of high risks as diabetes (42,2%) and the great number of over-aged patients (83,3%) are emphasized.

[The electroresection as therapy of the vulva carcinoma. Method and results (author's transl)].

At the Ist Gynecologic University-Clinic of Vienna 292 patients with vulvar carcinoma were admitted and treated in the years 1952--1970. The therapy was an electroresection and -coagulation of the primary tumor, followed, if necessary, by a surgical dissection of the inguinal groin. An electrocoagulation is indicated for the more extended cases, too--a local healing in 97% of the cases is obtained in this way. The inguinal dissection was progressively replaced by telecobalt-irradiation (6000 rd). Of the 292 cases 139 of 47.6% presented palpable nodes and only 56 patients were operated. 79.3% were aged between 61 and 90 years. Of the 292 cases 167 or 57.4% were cured for five years or more. The primary mortality was 1% (three cases). 40 patients (13.3%) died of intercurrent diseases.

[Multiple malignant tumours in gynaecology (author's transl)].

At the radiotherapy unit of the Ist Department of Gynaecology, University of Vienna, 8500 malignant tumours were observed in the years 1950-1975. Multiple primary neoplasms were recorded in 204 of these cases (2.4%). In 76 women (37.2%) the double neoplasms were situated in the genital region (local multiplicity), whilst in 128 cases the second neoplasm was situated in a distant organ, with cancer of the breast predominating (61 cases = 29.9%). A third of the multiple neoplasms were diagnosed simultaneously, whilst in the remaining two thirds a peak occurrence in the first year before or after the diagnosis of the gynaecological neoplasm is observed. Seven cases of triple neoplasms are presented as a separate group. Our observations are compared with the appropriate literature.

[Radium treatment of primary vaginal carcinoma (author's transl)].

Between 1951 and 1970, 263 cases with primary carcinoma of the vagina were observed at the Irradiation-Department of the Ist University Clinic of Obstetrics and Gynecology in Vienna. As method of choice the individual, fractioned and protracted Radium-Röntgen-Telekoalt-therapy is recommended. Of the 263 primary vaginal carcinomas 112 (42,5%) were cured. During the time mentioned in this report, we did not have any primary mortality; 24 women, however, could only be subjected to a palliative therapy, 20 patients died from an intercurrent disease and 20 women succumbed to a second carcinoma. Conclusively, the authors present a survey about the results in vaginal carcinoma.

[Efficiency of primary radiotherapy in generally or locally inoperable carcinoma of the endometrium].

The usual method of irradiation in the therapy of the carcinoma corporis uteri is described. It is stressed, that none but inoperable carcinomas were irradiated: this means, of course, a negative selection in disfavor of the radiotherapy. A report is given of 802 cases of carcinoma of the corpus uteri, treated by primary radiotherapy in the 1st University Clinic of Obstetrics and Gynecology in Vienna in the years 1950-1970. The 5-years-survival rate in this period was 50.4%. In the last half decade of this period the 5-years-survival rate of 300 patients went down to 48.6%. The reduction of this survival rate may be caused by the increasing age of the patients, who underwent therapy and is corresponding to world-wide experiences. Additive therapies like the "second-look abrasio" for early diagnosis of a local recidivation and the gestagen therapy for the prevention of metastatic carcinoma of the corpus uteri are discussed.

[Primary radiation therapy of sarcoma of the uterus (author's transl)].

We report 24 cases of sarcoma of the corpus uteri treated by sole irradiation. The 5-year-survival-rate was 20,8%. The prognosis seems to be poor in this group treated by sole irradiation. Only inoperable cases of sarcoma of the corpus uteri ought to be treated by radiotherapy alone.

[Irradiated cases of cervical and breast cancer II. Comparative investigation of the immune status of irradiated cases with stage III cancer of the cervix and operated, non-irradiated cases (author's transl)].

Eleven patients with stage-III cancer of the cervix were investigated before, during and after radio-therapy in regard to their state of humoral immunity on the basis of determinations of the serum IgG, IgM and IgA concentration, of hetero- and isoagglutinins, of tetanus antitoxin before and after vaccination with toxoid, of measles antibodies and of the percentage of lymphocyte membrane fluorescence. The cellular immunity of the same patients was investigated by determination of the percentage of spontaneously-rosetting lymphocytes, of skin-test reactivity with DNCB before and after sensitization, of skin-test reactivity with candida, trichophyton, varidase, OT and staphylo antigen. The function of polymorpho-nuclear leucocytes was investigated by means of the NVT test and St. aureus, E. coli and latex particles. All investigations were performed both before, and 3, 6, 9 and 12 weeks after the commencement of radiotherapy and the results were compared with those of an operated, non-irradiated group (stages I b and II a). Two types of noteworthy results were observed: 1. A decrease in immunological reactivity, probably in connection with cancer, since this reaction was observed both in irradiated and in non-irradiated cases, characterized by lowered or absent immune answer to tetanus toxoid, lymphopenia, decrease in sensitization to DNCB and less positive skin tests to old tuberculin and varidase. 2. An additional inhibition (although in one investigation stimulation of the immune answer was also seen), probably in connection with radiotherapy, characterized by an additional decrease in immune answer to tetanus toxoid, in skin sensitivity to DNCB sensitization and in tests with old tuberculin, and an augmented lymphopenia, as well as an increase in positive skin tests with varidase. No significant changes were observed with any other method.

[The development of gynaecological radiotherapy at the 1st department of obstetrics and gynaecology University of Vienna (a review of the past 25 years) (author's transl)].

The radiotherapy unit of the Ist Department of Gynaecology, University of Vienna, was founded 25 years ago and its direction was entrusted to me. The initial period was extremely strenuous and difficult, especially in view of the fact that in our country the radiotherapeutic specialist in general - and the specialist in gynaecological radiotherapy in particular - is confronted with many excellent, absolutely surgically-oriented colleagues. Hence, it was necessary to prove on the basis of our resultes over the years, that this form of therapy in gynaecology, all initial conditions being equal, is just as effective as surgery, or even the treatment of choice in certain forms of malignancy. As a measure of the success of radiotherapeutic managment in this field, it would not occur to any experienced gynaecologist today to treat, for instance, a case of vaginal carcinoma surgically or to be satisfied with mere tumorexcision in a case of carcinoma of the vulva. An attempt has been made in this paper to outline the different methods used in the treatment of various gynaecological tumours and to show that well-planned, individually-tailored, fractionated radium therapy, combined with conventional X-ray therapy brings therapeutic results, which compare favourably with the best in the world literature. A further improvement in the cure rate - at least in progressed cases - may be anticipated following the introduction of supervolt therapy. This report only deals with the contribution made in the course of treatment of more than 8500 cases of the radiotherapy unit of the Ist Department of Gynaecology, University of Vienna towards advancement of gynaecological radiotherapy.

[Therapy and results of primary irradiated cervix carcinomas in the years 1966 to 1968].

This is a report about 442 patients with a carcinoma of the cervix uteri, who underwent a primary radiotherapeutic treatment in the period from 1966 to 1968. 342 (77,4%) were at the age between fifty and seventy years. 226 (60%) of the women were in the stages III and IV. A five-years-healing could be obtained with 234 patients (45%). Nobody died in consequence of the treatment, i.e. primaryly. 34 (7,6%) died intercurrently. A rectovaginal fistula appeared on 10 women (2,2%). No vesicovaginal fistula was observed. 58 women (13%) suffered from an alimentary diabetes. It is determined that this grouping represents a negative selection of patients as the stages I and II have been operated according to Wertheim, if no contraindication for the operation was existent; for that reason, the intercurrent rate of mortality in the stage I was relatively high.

[Results of surgical radiotherapeutic management of cervix carcinomas during 1966-1968].

From January 1st 1966 until December 31st 1968, 593 women with carcinoma of the cervix uteri were admitted to and treated at the I. Universitäts-Frauenklinik of Vienna. 151 cases underwent a radical operation (Wertheim) resulting in a relative 5-years-healing of 85.43% and 442 women were exclusively treated by a combined radium-X-ray-therapy. From the 442 inoperable cases in 234 cases--which is 53%--a 5-years healing was reached. Of all the 593 cases together, 363 are living without relapse after 5 years, which is equal to 61.21% absolute healing.

[Primary radiotherapeutic eutic results of uterine carcinoma in the years 1960 to 1968].

A report is given of 448 cases of corpus carcinoma, treated by primary radiotherapy in the I-st Gynecological University Hospital in the years 1960 to 1968. The 5-years-healing rate in this period was 50.4 percent. It is emphasized that the cases represent a negative selection, as only inbetween the ages of 61 and 90 years. Two women died primarily from pulmonary embolism (0.4 percent). In two patients a rectovaginal fistula developed. The increasing number of patients with diabetes mellitus (23.2 percent) is striking.
