[Radiotherapy of recurrences of endometrial carcinoma].

In a retrospective study the data of 79 patients undergoing radiation-therapy for recurrent endometrial cancer during the years 1972-1978 were evaluated. Group I consisted of 43 patients, who primarily underwent irradiation, group II consisted of 36 patients, who primarily underwent surgery (total abdominal hysterectomy) mostly without irradiation. All patients were primarily classified as FIGO-stage I, the median age was 69 and 67 years respectively. In 32 cases (74%) of group I the localisation of the recurrence was the uterus, in 8 (19%) the vagina and in 3 (7%) the small pelvis. In group II a vaginal recurrence was found in 25 patients (69%), in 11 (31%) a pelvic recurrence. In two thirds of all cases the recurrences were diagnosed within three years of the primary treatment. Late recurrences after 5 years occurred in group I in 10%, in group II twice as often. The treatment of all recurrences was irradiation. 54 patients (68%) died within 2 years, 25 survived their recurrence for more than 2 years, (32%). In group I 23% survived for more than 2 years, in group II 42%. At the time of performance of the study the 5-year-survival-rate is 13%. Thus in two thirds of patients we succeeded in extending the survival for at least 2 years. The prognosis of a recurrence following surgery is distinctly better than following primary intracavitary irradiation. 56% of vaginal recurrences following primary surgery survived more than 2 years. The 5-year-survival-rate was similar in these vaginal recurrences however to the uterine recurrences following primary irradiation. The prognosis of patients undergoing primary surgery was the better, the later the recurrence occurred.

Endometrial bacteriology in patients with endometrial cancer before and after primary intracavitary irradiation using IR-192 and an afterloading technique.

In a prospective clinical and bacteriological study the endocervical and endometrial flora of 38 patients with biopsy proven cancer of the endometrium was studied before and after intrauterine irradiation with 10 Gy (Ir-192, afterloading device, Buchler, Braunschweig). Bacteriological swabs were taken transcervically. Anaerobic transport media were used and cultures were done aerobically and anaerobically. The mean number of aerobic organisms per patient increased from 2.13 to 2.49 (not significant (n.s.)), and of anerobic organisms from 1.42 to 2.00 (n.s). Neither the recovery rate of aerobic and anaerobic bacteria nor the composition of endometrial flora was altered significantly by intracavitary irradiation. Only aerobic sporeformers increased from 5.3% to 21% (2 p less than 0.05). The most prevalent aerobic species were S. epidermidis, streptococci and E. coli, the predominant anaerobic species Pepto- and Peptostreptococcus and Bacteroides. Our results clearly show that the intrauterine administration of 10 Gy by a Ir-192-afterloading device does not sterilize the endocervix or endometrium and infections may occur after intracavitary radiotherapy.

[Intracavitary radiotherapy of benign recurrent uterine bleedings (author's transl)].

387 patients with uterine bleeding resistant to other therapy were treated by intracavitary radium-therapy. In all cases there was a contraindication for the operative removal of the uterus. In 94,3% of our cases the bleeding could be treated successfully by the radium-application. Side effects of the irradiation occurred only in a minimal and neglectable percentage. Therefore the intrauterine contact-irradiation-therapy should be remembered in cases of uterine bleeding resistant to hormonal therapy or in cases of high risk for operation. With the new and modern afterloading device the molestation for the patients could be reduced to a minimum.

[Vaginal bacterial flora of patients with operated endometrial carcinoma prior to and following intracavitary vaginal irradiation (Ir-192, afterloading)].

In a prospective bacteriological and clinical study the vaginal bacterial flora of 35 patients with endometrial carcinoma, who underwent surgery 4 to 6 weeks earlier, was investigated prior to and following intracavitary vaginal irradiation with 10 Gy in 0,75 cm(Ir-192, afterloading device, Buchler). Bacteriological swabs were taken prior to and following ther insertion of a tube applicator. Anaerobic transport-media were used an cultures were performed aerobically and anaerobically. The mean number of aerobic species per patient increased slightly from 3.26 to 3.60 (n.s), anaerobic species remained constant (1.36 before and 1.30 after irradiation). Also the frequency of isolation of most aerobic and of all anaerobic species was statistically not altered following irradiation. Staphylococcus epidermidis decreased from 51.4% to 22.8% significantly (2 p less than 0.05), Micrococci increased from 0.0% to 11.4% (2 p less than 0.05) and aerobic sporeformers form 0.0% to 34.3% (2 p less than 0.01). Among the anaerobes Pepto- and Peptostreptococci were isolated prior to and following intracavitary irradiation in 11 to 14%, Bacteroides-species in 11 to 20%. Our bacteriological results suggest, that intracavitary irradiation in the therapeutically usual doses has no sterilizing effect on the vaginal flora. This flora plays an important role in infection following gynecologic radiotherapy.

[Therapeutic results in ovarian cancer after replacement of conventional radiotherapy by Co-60 irradiation (author's transl)].

The use of Co-60 screen and pendular irradiation instead of conventional radiotherapy has brought about an improvement in the 5-year survival rate in patients with more advanced stages of ovarian cancer. Although the general side effects of Co-60 irradiation are considered to be more unpleasant than those of X-ray and radium therapy, severe, reversible or irreversible damage to the urogenital tract and intestinal organs was diminished by accurate planning of Co-60 therapy, so that these complications occurred less frequently than with conventional radiotherapy.

[The possibilities of optimal therapy of corpus uteri cancer (author's transl)].

During the years 1960-1974 out of 1002 patients with endometrial carcinoma 256 underwent primary surgery and 846 primary irradiation. The evaluation of the results of 4 different surgical methods showed despite a different distribution of the stages of the cancer that Werthein's operation and abdominal hysterectomy and adnexectomy with simultaneous lymphnodectomy had statistically significant better results than abdominal hysterectomy without lymphonodectomy. By surgery a 5 years survival time was achieved in 83.6% (214 cases) with a primary mortality rate of 0.39% and an incidence of histologically proved lymphnode-metastases of 6.78%. Out of the 846 patients with generally or locally inoperable endometrial carcinomas, who underwent primary intracavitary irradiation 411 (48.6%) achieved a 5 years survival time. After irradiation in 9.9% reversible reactions (severe proctitis and cystitis) and in 2% irreversible complications (fistulas) were observed. After an observation period of 5 years 625 of the total of 1002 treated patients (62.4%) were alive and free of symptoms.

[Irradiation therapy of recurrent cervical carcinoma (author's transl)].

A report is presented of 141 irradiated cases of recurrent cervical carcinoma, 48 (34%) of the patients have survived for more than 2 years and 21 (15%) have survived yet more than 5 years. It is noticed, that the survival-rate is neither dependent on the localisation of recurrence, nor the beginning date nor the method of radiation therapy. It is pointed out, that the re-irradiation of primarily irradiated patients give also good results.

[Radiation therapy of cervix cancer with special reference to radiation effects upon the intestine (author's transl)].

The evaluation of the therapeutical preference of succession in the treatment of the uterine cervix, the radiation therapy today presents a special importance. This is established upon two facts; that is, the age-distribution and the incidence of the inoperability of the cases (one patient in every three cases of the cervical cancer is over 60 years of age; nearly 60% of the cases are in an inoperable stage). Although in our department only partially or completely inoperable cases went through a radiation therapy, 757 ( = 47,7% out of 1586 cases) achieved to survive for more than five years. At a discussion concerning the intestinal complications which occur due to irradiation, it has been proved, that 18,6% show reversible reactions. The permanent reactions like recto-vaginal-fistula etc. were only seen in 2,4%. It is interesting to note, that the patients subjected to intestinal complications were seen by advanced cases (stage III: 65,1%) and 69% of them were over 60 years of age. Irradiation consequences are by younger patients and by cases with a possible local operability rather rare. At the end, physiopathology and the treatment of the intestinal post-irradiation effects are discussed.

[Biorhythm theory and primary irradiation of inoperable cancer of the cervix (author's transl)].

Hundred patients with inoperable cancer of the cervix, who had been treated by primary irradiation, were reviewed with regard to biorhythm on the days of radium application. 5-year survival was lower in patients with unfavorable biorhythm; the differences, however, were not significant. It is concluded that the calculation of biorhythm for the day of radiotherapy has little or no influence on the results of therapy.

[Results of operation and irradiation in 683 cases with ovarian carcinoma (author's transl)].

A report is given on 683 cases of ovarian cancer, who underwent postoperative radiation therapy at the Radiation Department of the 1st University-Clinic of Obstetrics and Gynecology in Vienna. An uniform staging and histological classification is complicated by the fact, that in recent years the rate of cases operated elsewhere and subsequently admitted to our clinic went up to 76%. Of all 683 cases, 193 (28,3%) reached a five-year survival time. Because of these still unsatisfying results of postoperative radiation treatment in ovarian cancer the importance and necessity of randomised studies with different therapeutic programs is emphasized.

[The necessity of preventive gynecological studies in the aged].

According to epidemiological data the majority of the old women suffers from mammary cancer or gynecological malignoma. In cervical uterine cancers this ratio reaches more than 30% and in uterine body, ovarial and breast cancers more than 60%. Thus, it is significantly clear, that gynecological preventive medical examinations are so important. For cervical cancer cytology and for breast cancer mammography and thermography are valuable and satisfactory methods. Because of the lack of a practicable preventive examination method for corpus cancer and ovarial carcinoma it is evident, that regular clinical investigations are of vital importance in this field. Vulva and vaginal cancers are to be mentioned as real old women's neoplastic disease, because it starts mostly between 7th and 9th decade of life.

[High dose gestagen therapy following primary radiation therapy of endometrial carcinoma (author's transl)].

By a high dose gestagen therapy following primary radiation therapy of endometrial carcinoma it was possible to reduce the recurrence-rate 12 months after therapy in comparison with a control-group without gestagen therapy by more than 50%. Therefore this method is recommended not only for treatment of far advanced carcinoma of the endometrium but also as prophylactic therapy in cases of early endometrium cancer.

[Geriatric gynecology with special reference to malignant tumors].

After the enumeration of every single problem encountered in the geriatric gynecology, it has been proved, that the malignant tumors of such cases are of a special significance. according to the epidemiological facts of the Austrian Cancer-Registration-Statistics between the years 1971--1975 (even if the cancer of cervix uteri is excluded) more than 60% of the cases are still seen in the geriatric group. Increasing risk (by rate of incidence) of malignant tumors of internal genitals or the breast in aged women is also known and is proved to be very specific. Inspite of the progress won in geriatric surgical gynecology, the post-operative complications are still the negative factors, which limitate the success. Therefore today radiotherapy of the malignant tumors especially in geriatric-gynecological patients is gaining more and more importance.

Age Factors↗

[The significance of primary radiotherapy in cervical carcinoma in older women].

In spite of the increasing interest in geriatric-operative therapy primary irradiation therapy still has its important place in the treatment of cervical cancer in cases of higher age. About every third patient with cervical cancer belongs to the age group over 60 years, which is more or less excluded from operative therapy. The advantage of irradiation therapy is to be seen in the low post-therapeutic mortality and the remarkable survival and healing rates.

[Therapy with gestagens for gynecological tumors sensitive to hormones (author's transl)].

Our method of irradiation of the endometrial carcinoma is described. The patient collective concerned represents a negative selection, as only inoperable cases with an endometrial carcinoma undergo primary irradiation. The 5-year survival rate amounted to 50.4% of 802 patients in the period from 1950 through 1970. Additional therapy with gestagens is discussed and recommended. The histological alterations in the material obtained by so-called "second-look curettages" are reported. Application of gestagens to other hormonesensitive tumors such as carcinoma of the ovary or adenocarcinoma of the cervix uteri is considered.
