An epidemiological survey on ossification of ligaments in the cervical and thoracic spine in individuals over 50 years of age.

An epidemiological survey on ossification of the spinal ligaments was performed on a total of 1,058 subjects over the age of 50 years by means of roentgenography of the cervical and thoracic spine. Ossification of the posterior longitudinal ligament (OPLL) of the cervical spine was detected in 34 subjects (3.2%) with a predilection for men, whereas OPLL in the thoracic spine was found in 8 (0.8%). There were 325 cases (30.7%) of ossification of the anterior longitudinal ligament (OALL) of stage II or above by Forestier's classification in the region from the cervical to thoracic vertebrae, and these cases included a significantly greater number of men. Ossification of the ligamenta flava (OLF) was observed in 48 cases (4.5%). As for the coexistence of ossification of these ligaments, 364 individuals (34.4%) had at least one instance of OPLL and OALL (stage II or above) in the region from the cervical to thoracic spine, and OLF in the thoracic spine.

[Two cases of thrombosis of an aneurysm in the acute stage].

Thrombus formation within aneurysm is common and has generally been proved to be a slowly progressive phenomenon. We report two cases of acute completely thrombosis of aneurysms. The initial angiogram failed to reveal aneurysm and other etiology of subarachnoid hemorrhage. CT revealed diffuse cisternal blood and acute hydrocephalus. We decided to operate on these patients in the acute stage because of the generally-known strict correlation between the amount of cisternal high density on CT and the subsequent development of vasospasm and ischemic event. Case 1. A 68 year-old female was admitted on Day 1 with neurological deterioration of Grade 3 (H & K). Complete angiographic study was done, including the basal view and stereotechnique, but failed to reveal aneurysm. On Day 2 we operated on this patient and discovered a thrombosed aneurysm of the anterior communicating artery (3 X 7 mm in size). Case 2. A 51 year-old male suffering from subarachnoid hemorrhage was transferred to our hospital on Day 0. Emergency angiography gave no information, but the pattern of cisternal clots on CT was suggested the existence of an aneurysm of the right internal carotid artery. The next day we performed angiography once more, at that time 2 X 5 mm internal carotid bifurcated aneurysm was revealed. Shortly thereafter we operated and found the intra-aneurysmal clot. Through consideration of these we have reached the following conclusions. When CT findings in a patient with subarachnoid hemorrhage show the diffuse cisternal clots due to rupture of aneurysm, we should consider immediate surgery even if the angiogram is negative in order to lessen the risk of delayed vasospasm.(ABSTRACT TRUNCATED AT 250 WORDS)

Preliminary X-ray diffraction studies on a ferredoxin from the thermophilic archaebacterium, Thermoplasma acidophilum.

A ferredoxin from the thermophilic archaebacterium, Thermoplasma acidophilum, is supposed to contain two (4Fe-4S) active centers; one center could be linked by four cysteine residues to the protein and the other bonded with three cysteines and an unknown group. This ferredoxin has been crystallized by salting-out against 2.3 M-ammonium sulfate solution. The space group is P21212 with cell dimensions of a = 59.20 A, b = 52.77 A and c = 41.28 A. Four molecules pack in the unit cell with Vm = 2.03 A3/dalton.

Purification of protein phosphatase from hen oviduct.

Two protein phosphatases of 103 and 29 kDa as determined by gel filtration, were purified from hen oviducts. The 103 -kDa phosphatase was purified 7300-fold to near homogeneity and dissociated into two polypeptides in the presence of SDS. Molecular masses of these polypeptides were estimated to be 60 and 38 kDa by SDS-polyacrylamide slab gel electrophoresis using the buffer system of Laemmli, but 68 and 35 kDa using the buffer system of Weber and Osborn. The stoichiometry of these polypeptides was approx 1:1 according to the densitometric analysis of gels at 550 nm. The 29 -kDa phosphatase was purified 2900-fold. Both phosphatases dephosphorylated the alpha-subunit of phosphorylase kinase more rapidly than the beta-subunit.

Proteolysis of liver acetyl coenzyme A carboxylase by cathepsin B.

Proteolysis of acetyl-CoA carboxylase was examined with cathepsin B. When chicken liver acetyl-CoA carboxylase was incubated with cathepsin B at pH 6.3, the native 220-kDa polypeptide was primarily cleaved into two polypeptides of 125 and 115 kDa, and further degraded to polypeptides of 100-50 kDa.

On fluctuation of hearing in otitis media with effusion after myringotomy.

Otitis media with effusion (OME) is known to occur in school or preschool children. Its chief symptom is hearing disturbance without otalgia. One of its effective therapies is myringotomy and suction of secretion out of its morbid tympanic cavities. This improvement differs with frequencies. The purpose of this paper is to present the mechanism of hearing recovery after myringotomy. The mean audiogram of OME showed a flat shape with 0 dB of bone conduction and 37.5 dB of air conduction. Hearing level after myringotomy recovered at high tones, but not recovered at low ones. One week later, the mean audiogram without perforations of eardrums showed hearing improvement at low tones. These findings suggested that slower recovery at low tones might be due to incision of an eardrum and closure of its perforation. To confirm this hypothesis we performed an experiment, by which the vibratory mechanism of a thin membrane was disclosed. The result showed that the vibratory modality of a membrane was influenced by incision of it. In conclusion, it is considered that though myringotomy of OME gets hearing improvement, it could result in hearing impairment, probably due to the vibratory modality of an eardrum.

Amino acid sequence of a ferredoxin from thermoacidophilic archaebacterium, Sulfolobus acidocaldarius. Presence of an N6-monomethyllysine and phyletic consideration of archaebacteria.

The amino acid sequence of a ferredoxin from a thermoacidophilic archaebacterium, Sulfolobus acidocaldarius, was determined by a combination of various conventional methods to be as follows: Gly-Ile-Asp-Pro-Tyr-Arg-Thr-His-Lys-Pro-Val-Val-Gly-Asp-Ser-Ser-Gly-His- Lys-Ile -Tyr-Gly-Pro-Val-Glu-Ser-Pro-Lys(Me)-Val-Leu-Gly-Val-His-Gly-Thr-Ile-Val -Gly-Va l-Asp-Phe-Asp-Leu-Cys-Ile-Ala-Asp-Gly-Ser-Cys-Ile-Thr-Ala-Cys-Pro-Val-As n-Val-P he-Gln-Trp-Tyr-Glu-Thr-Pro-Gly-His-Pro-Ala-Ser-Glu-Lys-Lys-Ala-Asp-Pro-V al-Asn- Glu-Gln-Ala-Cys-Ile-Phe-Cys-Met-Ala-Cys-Val-Asn-Val-Cys-Pro-Val-Ala-Ala- Ile-Asp -Val-Lys-Pro-Pro. It was composed of 103 amino acid residues giving a molecular weight of 10,908 excluding Fe and S atoms. This ferredoxin contained an N6-monomethyllysine residue at position 29 which was determined by a comparison of the elution profile of the acid hydrolysates of the protein and peptides on an amino acid analyzer with three methyl derivatives of lysine and also by field desorption mass spectrometry of a purified peptide. The ferredoxin has only 7 cysteine residues, which probably participate in constructing the Fe-S clusters of this ferredoxin, indicating the presence of a unique chelate structure. Comparison of this ferredoxin with other archaebacterial ferredoxins indicated that the archaebacteria might have multiple origins in an evolutionary tree.

The sulfhydryl groups of ferredoxin-NADP+ oxidoreductases: is a disulfide bond really present?

The sulfhydryl groups of three ferredoxin-NADP+ oxidoreductases [EC 1.18.1.2] (FNR) two from blue-green algae, Spirulina sp. and Synechococcus sp., respectively, and one from spinach, were analyzed by carboxymethylation and titration with sulfhydryl reagents under different conditions. Five sulfhydryl groups in Spirulina and spinach enzymes and four in Synechococcus enzyme were found to be in reduced forms. There was no disulfide bond in any FNR tested. The results in the present experiment contradict those reported by Karplus et al. [Biochemistry 23, 6576-6583 (1984)].

Improvement of abnormal pyruvate metabolism and cardiac conduction defect with coenzyme Q10 in Kearns-Sayre syndrome.

In a patient with Kearns-Sayre syndrome, concentration of coenzyme Q10, a component of the mitochondrial electron transport system, was decreased in serum and in the mitochondrial fraction of skeletal muscle. Serum concentrations of lactate and pyruvate were abnormally high, especially after exercise or oral glucose loading. Levels of folic acid in plasma and CSF were decreased. ECG showed a first-degree atrioventricular block. After administration of coenzyme Q10 60 to 120 mg daily for 3 months, serum levels of lactate and pyruvate became normal, with improvement of atrioventricular block and ocular movements.

Effects of propranolol on portal hemodynamics in patients with chronic liver disease.

To elucidate the mechanism by which propranolol reportedly affects portal hemodynamics, we investigated the effect of propranolol on systemic and splanchnic hemodynamics in 15 patients with portal hypertension and esophageal varices by simultaneous catheterization of the portal vein and the right hepatic vein and measurement of portal venous flow using an ultrasound doppler system. Infusion of 5 mg of propranolol significantly decreased pulse rate (-12.6%), cardiac output (-24.5%), portal venous flow (-22.3%), portal venous pressure (-13.3%), and gradient between portal venous pressure and free hepatic venous pressure (-24.8%). Thus, propranolol seems to decrease portal venous pressure by reducing portal venous flow, at least in part, as a result of reduction of cardiac output due to its beta 1 adrenergic receptor blocking action.

[Clinical evaluation of TMS-19-Q, a new macrolide antibiotic, in otorhinolaryngological infections].

The clinical study for TMS-19-Q.O tablet was performed with multicenter trial. The results obtained were as follows; Final global improvement rating in 332 cases of otorhinolaryngological infections were excellent in 99, good in 142, fair in 40 and poor in 51 and the effective rate was 72.6%. Those of 266 cases with acute infection were excellent in 93 and good in 121 and the effective rate was 80.5%. Optimum daily doses would be 600 mg based on the analysis of 144 cases of the acute infection with sensitive bacteria (MIC: less than or equal to 3.13 micrograms/ml). In acute infection, major causative bacteria were Gram-positive cocci (GPC) indicating the frequency of 72.0% in total isolates and 87.5% in singly isolated cases. In chronic infection, although GPC were also dominant, Gram-negative bacilli were observed in 31.9%. Clinical and bacteriological effective rates of 160 cases of acute infection with single species were 80.6% and 90.3%, and those of 43 cases in chronic infection were 44.2% and 72.7%, respectively. The resistant rates of isolates in acute infection to TMS-19-Q were 13.3% in S. aureus, 7.7% in S. epidermidis, 6.0% in S. pyogenes and 0% in S. pneumoniae. Those in chronic infection were 20.0% in S. aureus and 25.0% in S. epidermidis. Slight adverse reactions, such as skin eruption or gastrointestinal disorders were observed in 14 cases and no severe one was observed. Slight elevation of GOT, GPT, Al-P, BUN, S-Cr. or eosinophil were observed in 12 cases. These results suggest that TMS-19-Q would be useful antibiotic for otorhinolaryngological infections.
