Induction of antitumor activity in macrophages by mycoplasmas in concert with interferon.

The in vitro growth of tumor cells infected with mycoplasmas was suppressed by macrophages pretreated with interferon (IFN), but the growth of mycoplasma-free tumor cells was not suppressed. Pretreatment of macrophages with IFN plus mycoplasmas or their soluble factors either simultaneously or sequentially, IFN first and mycoplasmas second, but not in the reverse order, was effective in activating macrophages to suppress the growth of mycoplasma-free tumor cells. Macrophages from C3H/HeJ mice (which respond only slightly to lipopolysaccharide) were activated by IFN plus mycoplasmas or their soluble factor, and their action was not influenced by the addition of a lipopolysaccharide-neutralizing agent, polymyxin B. These results suggest that the macrophage-activating agent in mycoplasmas does not mimic lipopolysaccharide. The administration of mycoplasmas plus IFN to mice with ascitic or solid tumors resulted in the reduction of tumor growth. The survival rate of tumor-bearing mice was improved by the administration of mycoplasmas, and this was synergistically enhanced by the addition of IFN. These results indicate (a) that mycoplasmas can be useful as a biological response modifier, and (b) that care should be taken to prevent contamination with mycoplasmas in experiments on macrophage activation.

Laryngeal epithelial changes on effects of smoking and drinking.

The human larynx is essential not only for speech, but also for swallowing and respiration. Its tissues are affected by the presence of alcohol and cigarette smoke. In this report we present our findings on the effects of smoking and drinking on histological changes in laryngeal tissue. The larynges studied were those of autopsy patients: cases with damaged tissue were excluded. We studied the larynges of 84 men (non-smoker and non-drinker, 22 cases; smoker and drinker, 62 cases) and 48 women (non-smoker and non-drinker, 40 cases; smoker and drinker, 8 cases) between the ages of 12 and 88. The squamous metaplasia of laryngeal epithelium was examined using the gross staining method (Pyronin Y) of STELL et al. (J. Laryngeal. Otol. 86: 589-594, 1972). The squamous epithelium is unstained, whereas the respiratory epithelium is a brilliant red. The area of metaplasia in the supraglottic region increased with aging, tobacco and alcohol consumption. Microscopic changes of the laryngeal epithelium were investigated using the hematoxylin-eosin staining method. The thickness of epithelium of the supraglottic region and vocal cord increased with age. There is a significant difference in the thickness of the epithelium of the supraglottic region and vocal cords of smoking and drinking patients, when compared with the same tissues of non-smokers and non-drinkers. Furthermore, there is a significant difference in those tissues of the supraglottic region of heavy smokers, when compared with light smokers, but no significant difference when compared with drinkers. However, there is no significant difference in the thickness of vocal cord tissues when comparing light smokers and drinkers on the one hand, and heavy smokers and drinkers on the other.

Increased activity on radiocolloid scintigraphy in splenic hamartoma.

A higher uptake of radiocolloid in a splenic tumor than in the rest of the spleen was observed in a 17-year-old boy. The pathologic diagnosis of the surgically resected tumor was hamartoma composed of anomalous mixtures of splenic elements, and radiocolloid was considered to be distributed to reticuloendothelial cells in the tumor. Radiocolloid scintigraphy, which can demonstrate the function of the reticuloendothelial system, is helpful in the diagnosis of splenic hamartoma.

Mid-cervical malignant pleomorphic adenoma presenting with upper airway obstruction.

The case of a 1 year and 6 months old girl with pleomorphic adenoma arising from the upper middle part of the neck is reported. Although the tumor invaded the hyoid bone, it was removed completely and symptoms of upper airway obstruction disappeared. We suspect it may have originated from thyroglossal duct structures.

[Studies on the pathogenic mechanism of beta-lactam hypersensitivity by the detection of leucocyte migration activating factor and leucocyte migration inhibitory factor].

In 145 suspected cases of beta-lactam hypersensitivity, the identities of the allergenic drugs were performed by leucocyte migration inhibition test (LMIT). The involvement of cell-mediated immunity using leucocyte migration activating factor (LMAF) and leucocyte migration inhibitory factor (LMIF) was investigated for each allergic symptom. The proportion of LMIT positives was 74% of all 145 cases, and more than 70% for each allergic symptom except for anaphylactic shock, and in particular more than 90% for fever and eosinophilia. LMAF and LMIF were detected 39% and 35% of 145 cases, respectively, however LMAF was more frequently detected than LMIF in cases with hepatopathy. LMAF was more frequently detected than LMIF when the latent period (the duration of sensitization) of skin eruption or fever was less than 10 days, whereas LMIF was more frequently detected than LMAF when it was more than 10 days. The same result also was obtained at the boundary between 14 days of the latent period in hepatopathy. These results indicate that in the pathogenesis of beta-lactam hypersensitivity, cell-mediated immunity plays a major role. Both LMAF and LMIF are involved, and their production is dependent on the duration of allergenic drug-sensitization, LMAF is produced during the early period of sensitization, whereas LMIF is produced during the late period of sensitization.

[Assessment of infarct-related coronary arteries using the bull's eye view method and unfolded surface mapping in thallium-201 myocardial tomography].

Infarct-related coronary lesions and collaterals were assessed by the bull's eye view and the unfolded surface map derived from thallium-201 myocardial tomography (Tl-201 SPECT) in 25 patients with anterior myocardial infarction. The patients were categorized in six groups according to their cine-angiographic findings: locations of stenosis (proximal or distal portions of the first diagonal branch or distal site of the first septal branch), and the presence or absence of collaterals. The anterior half of the left ventricle on the bull's eye view map was divided into eight regions from the anterior septum to the lateral wall and from the apex to the cardiac base. Tl-201 SPECT images were expressed as functional maps using maximum-count circumferential profile analysis: an "extent image" showing the extent of perfusion defect, and a "severity image" demonstrating the degree of hypoperfusion. The following results were obtained: 1. Perfusion defect of the "extent image" at the basal portion of the anterior septum reflected poor collaterals, with a sensitivity of 81.3%, a specificity of 77.8%, and a diagnostic accuracy of 80.0%. 2. Coronary stenosis at segment 6 (AHA classification) was differentiated from that at segment 7 by detecting hypoperfusion at the basal septum on the "severity image", with a sensitivity of 71.4%, a specificity of 77.8%, and a diagnostic accuracy of 75.0%. 3. Perfusion defect at the basal section of the proximal portion of the anterior wall on the "extent image" indicated stenosis at the proximal segment 6, with a sensitivity of 57.1% and a specificity of 88.9%.(ABSTRACT TRUNCATED AT 250 WORDS)

[Evaluation of image quality and lesion detectability on I-123 IMP brain SPECT].

Influence of image quality on lesion detectability in I-123 IMP brain SPECT was discussed in forty-two cases with various cerebrovascular disorders. Fifty nine lesions, which were considered to show decreased uptake of I-123 IMP, were analyzed in this study. In all cases, SPECT examinations were started within 30 minutes after intravenous administration of 3 mCi of I-123 IMP. To determine the lesion detectability, fourteen radiologists from nine hospitals were gathered and film reading was carried out. To classify the image quality into valuable or not, findings of five points on SPECT image were visually evaluated; 1) uniformity of cortex accumulation, 2) separation of bilateral visual cortices, 3) density of white matter, 4) separation between basal ganglion and thalamus, 5) density of basal ganglion and thalamus. Mean lesion detections were 10.3/14 for the lesions of cerebral cortex, 9 for cerebellum, 3.58 for white matter and 3.22 for basal ganglion and thalamus, respectively. The detectability was certainly influenced by the image quality in the lesion of basal ganglion and thalamus, white matter and single lobe of cerebral cortex. We concluded that detection of small lesions and deep lesions with I-123 IMP SPECT was dependent on the image quality and the proposed classification of I-123 IMP SPECT image was useful for evaluating image quality from the points of view of the lesion detectability.

[A case of subacute cerebral infarction demonstrating hyperemia and crossed cerebellar diaschisis in I-123 IMP SPECT].

SPECT with N-isopropyl-p-[I-123]iodoamphetamine were performed in a 81-year-old man with cerebral infarction. In the subacute phase, the radioactivity was increased in the infarct area where fogging effect and remarkable contrast enhancement was demonstrated in X-ray CT. The contralateral cerebellar hemisphere showed reduced activity due to crossed cerebellar diaschisis (CCD). In the chronic phase, the decrement of the activity in the infarct was observed. Increased activity in the subacute phase was thought to reflect the hyperemia in the infarct, while CCD suggested the decreased metabolic activity in the lesion. The coexistence of the hyperemia and the CCD indicates flow and metabolic uncoupling, which means "luxury perfusion". This case was also thought to demonstrate atypical findings of CCD in SPECT imaging.

[A case of pulmonary embolism diagnosed by 111In labeled platelet scintigraphy].

Although the role of 111In platelet scintigraphy is established in the diagnosis of deep vein thrombosis, its indication in pulmonary embolism remains uncertain. We performed platelet scintigraphy in a patient with recurrent pulmonary thromboembolism, which revealed abnormal accumulation in the left hilar region. Pulmonary perfusion scintigraphy performed at the same time did not show perfusion defect. However, pulmonary angiography and thromboembolectomy demonstrated recurrent embolus in the hilus. This suggested that platelet aggregation in the left hilus shown by platelet scintigraphy was indeed related with recurrence of pulmonary embolism, and platelet scintigraphy is useful in detecting new development of thrombus in patient with recurrent pulmonary thromboembolism.

[Evaluation of pontine visualization with single photon emission computed tomography using N-isopropyl-p-(I-123)iodoamphetamine in normal and pathological cases].

Pontine visualization with single photon emission computed tomography (SPECT) using N-isopropyl-p-[I-123]iodoamphetamine (I-123 IMP) was evaluated in 99 cases including 46 cases of brain infarction, 25 of other cerebrovascular disorders and 28 of brain tumor and in 19 normal subjects. In each case, SPECT using a rotating gamma camera was started at 30 minutes after intravenous administration of 111 MBq of I-123 IMP. Transverse images were reconstructed parallel to the cantho-meatal line and displayed on films. Pontine visualization was visually classified into three grades: high radioactive accumulation confined to the pons with clear contour (Grade 3), faint accumulation with indistinct contour in the pontine region (Grade 1), the pontine accumulation was identified but its activity was less significant (Grade 2). The results showed that grade of the pontine accumulation was lower in pathological cases compared to normal subjects. Six cases with brain stem lesion demonstrated pontine accumulation of grade 1 in 4 case and of grade 2 in 2 cases. Lower grade was also revealed in 29 cases with crossed cerebellar diaschisis (CCD). No significant correlation was observed between classified grade of pontine accumulation and maximum counts of reconstructed images. We concluded that SPECT could detect brain stem lesions and that lower accumulation in pons with CCD positive cases was thought to partially reflect the reduced blood flow and metabolism in the pontine nuclei.

Quantification of area and percentage of infarcted myocardium by single photon emission computed tomography with thallium-201: a comparison with serial serum CK-MB measurements.

In order to quantify the size of the infarcted myocardium, two kinds of data processing techniques were applied to single photon emission computed tomography (SPECT) with thallium-201 and its clinical reliability was evaluated by comparing it with the infarct sizing procedure with the serial serum creatine kinase-MB measurements in 14 patients with acute myocardial infarction. After maximum-count circumferential profile analysis, short axis images were reformatted into an unfolded surface map and a bull's eye view map. The SPECT-determined infarct size was defined as the area or the percentage of hypoperfused myocardium of which the profile count was less than the mean minus 2SD derived from 8 normal subjects. The infarct area was calculated from the number of pixels with an abnormal count and expressed in an unfolded surface map. The percentage was calculated from the number of abnormal profile points and displayed in a bull's eye view map. A high linear correlation was observed between the enzymatically determined infarct size and the infarct area or the percentage (r = .947, r = .872, respectively), despite underestimations in 2 patients with accompanying right ventricular infarction and overestimations in 2 patients with prior anterior infarction. Moreover, a close negative correlation was found between the left ventricular ejection fraction and the infarct area or the percentage (r = .836, r = .821, respectively). Thus, the semiautomatic techniques for processing thallium-201 SPECT images might contribute to the quantitative estimation and display of infarcted myocardium and have high clinical reliability.

Extraskeletal mesenchymal chondrosarcoma in a cow.

A mesenchymal chondrosarcoma found in a 7-year-old Holstein cow was studied by light and electron microscopy. There was a primary site in the subcutaneous tissue at the base of the neck and metastases in the lungs, pleura, broncho-mediastinal lymph nodes, kidneys and heart. The neoplasm was composed of undifferentiated mesenchymal tissue, myxoid tissue and chondroid tissue. The neoplastic cells of the former two tissues were characterized by desmosome-like junctions and moderate to small amounts of rough endoplasmic reticulum (RER).

Fibrosarcoma of the neck in an adult female.

This paper is a case report of a 53-year-old housewife with fibrosarcoma of the neck. On 27 August 1986, she was admitted to our hospital with discomfort on the right side of her throat and difficulty in swallowing. She had no history of radiation therapy to the neck. The tumor was below the right submandibular gland and was movable. No enlarged lymph nodes were palpated in the neck. The mean CT number of the tumor was 59.5 H.U. The tumor was almost circular and its surface was smooth. Upon operating, the tumor was observed to be connected to the greater cornu of the hyoid bone and the superior cornu of the thyroid cartilage. The tumor measured 50 x 42 x 35 mm and was well circumscribed but not encapsulated. The cut surfaces were generally fleshy, glistening and grayish-white. The tumor was composed of small amounts of collagen and reticulin, producing fairly uniform spindle cells arranged in interlacing bundles, forming a herring bone pattern. The tumor cell showed rather little atypism, but did show obvious mitotic activity. Thus, the tumor was diagnosed as well-differentiated fibrosarcoma. When the patient was last seen, in April 1989, there was no evidence of recurrence.

Application of leucocyte migration tests to detection of allergenic drugs in patients with hypersensitivity to beta-lactam antibiotics.

In 90 suspected cases of beta-lactam hypersensitivity manifesting exanthema, pyrexia and hepatopathic symptoms, the identities of the allergenic drugs were investigated by immediate type skin reactions, sensitized erythrocyte agglutination tests and leucocyte migration inhibition tests (LMIT). In addition 30 control patients were tested for hypersensitivity to ampicillin, cephalexin and latamoxef. The control patients manifested the same symptoms as the patients with suspected hypersensitivity to beta-lactam but had not been treated with beta-lactam antibiotics. In the immediate type skin reaction, all the control cases as well as all the suspected beta-lactam hypersensitive patients displayed negative reactions to each of the antibiotics tested. In the sensitized erythrocyte agglutination tests, no antibodies to any of the three drugs were detected in the control group, while antibodies with a titre of 1:2 or more were detected in only 7% (6/90) of the suspected beta-lactam hypersensitive patients. On the other hand, in the LMIT, only 4% (4/90) of the control patients displayed positive reactions to the three test antibiotics, whereas the suspected sensitizing drugs elicited a high rate of positive responses (68/90; 76%) in the group of suspected beta-lactam hypersensitive patients. Broken down into symptomatic categories, response to the LMIT was positive in 41 of 58 cases of suspected drug rash (71%), 26 of 26 cases of suspected drug fever (96%), and 20 of 24 cases of suspected drug-induced hepatopathy (83%). Thus, the pyrexial group exhibited a particularly high ratio of positive reactions, and in fact exanthemic cases with concomitant symptoms (eosinophilia, pyrexia, hepatopathy, etc.) also displayed a notably high ratio of positives (19/20; 95%). The overall results indicated that delayed type hypersensitivity (DTH) is largely involved in exanthema, pyrexia and hepatopathy induced by beta-lactam antibiotics, and that the LMIT constitutes an effective means of detecting allergenic drugs in patients with beta-lactam hypersensitivity.

Structural correlations with cross-reactivity of beta-lactam antibiotics in delayed type hypersensitivity. Cross-allergenicity in hypersensitivity to cephems with a tetrazolyl group in the C-3 side chain.

Cross-reactivity associated with delayed type hypersensitivity (DTH) arising from cephem antibiotics with a tetrazolyl group in the C-3 side chain was investigated by clinical testing and animal experiments. Clinical cross-reaction testing was performed with the leucocyte migration inhibition test (LMIT) with respect to sixteen patients with hypersensitivity induced by cephems with a tetrazolyl group in the C-3 side chain. The proportion of positive LMIT cross-reactions to cephems with a tetrazolyl group in the C-3 side chain was 78% (25/32), to cephems without a tetrazolyl group, 5% (1/22), and to penams, 0% (0/12). The proportion of positives in tests performed with methyl-tetrazolethiol (MTT) and hydroxyethyl-tetrazolethiol (HTT), which essentially represent the structures of the C-3 side-chains in the allergenic drugs, was 29% (4/14), while the corresponding proportion for 7-aminocephalosporanic acid (7ACA), which represents the skeleton structure of the cephem antibiotics, was 33% (1/3). The animal experiments were performed with guinea pigs, with latamoxef, cefoperazone and MTT as the sensitizing agents and testing for cross-reactivity by means of delayed type intradermal reactions as well as the LMIT. The results of intradermal testing and the LMIT agreed almost completely, thus providing strong support for the clinical results. Latamoxef and cefoperazone displayed the same cross-reactivity, manifesting cross-reactions with MTT, HTT and 7ACA as well as with cephems having a tetrazolyl group in the C-3 side chain. Moreover, DTH induced by MTT displayed cross-reactivity with cephems possessing tetrazolyl groups in the C-3 side chain. The above results indicate that free MTT radicals, as well as the skeleton ring structure represented by 7ACA, are strongly involved in DTH arising from cephem antibiotics with a tetrazolyl group in the C-3 side chain.

Tumor-specific T cells which form clusters with dendritic cells and tumor cells and deliver macrophage-activating factors.

T cells prepared from tumor (Meth A)-bearing mice were cocultured with homologous tumor cells and splenic dendritic cells to enrich tumor-specific T cells by the separation of clusters. T blasts generated from clusters were capable of inhibiting the in vivo tumor cell growth. The culture supernatant of clustering cells (CLSN) was effective in activating macrophages (M phi) to be cytostatic and cytocidal against tumor cells. Moreover, it was found that CLSN contains at least 3 distinct factors; one was identified as interferon-gamma (IFN-gamma), and the others are so far unidentified, but one acts synergistically with IFN-gamma, possibly as the second signal, and the other cooperates with lipopolysaccharide but not with IFN-gamma. We propose that the tumor-specific T cells secrete soluble mediators which cooperate with each other in M phi activation against tumor cells.

In vivo effects of recombinant interferon alpha A/D incorporated in gelatin microspheres on murine tumor cell growth.

Intraperitoneal (ip) injections of gelatin microspheres containing a very small amount of recombinant human interferon alpha A/D (A/D-IFN) (IFN-microspheres) plus free A/D-IFN improved the survival of mice bearing ascitic Meth A-R1 cells which we had isolated as IFN-resistant cells under in vitro conditions. The dose of free A/D-IFN in one injection was 10,000 IU, which was insufficient by itself for manifesting in vivo antitumor activity. In these mice, in vivo R1 cell growth was suppressed and macrophage recruitment was enhanced in comparison with mice receiving other control agents. Administration of IFN-microspheres alone was also effective but less than that of IFN-microspheres plus free A/D-IFN. Peritoneal macrophages obtained from normal or R1-bearing mice receiving ip injection of IFN-microspheres with or without free A/D-IFN were activated to inhibit the in vitro growth of R1 cells. The intratumoral injection of IFN-microspheres strongly inhibited the growth of solid R1 tumors. Intravenous injection of IFN-microspheres was effective in preventing the pulmonary metastasis of B16 melanoma cells. These results indicate that the IFN-microsphere is much more effective against tumors than free A/D-IFN.
