A clinicopathological study on cardiac lesions in 64 cases of disseminated intravascular coagulation.

Diagnosis of disseminated intravascular coagulation (DIC) was made in 64 cases (16.2%) among a total of 395 autopsy cases. There were 31 men and 33 women. Their ages ranged from 31 to 91 years (mean 76.3). Underlying diseases were mainly malignancy and sepsis. Fresh cardiac lesions were found in 40 cases (62.5%). Coronary thrombosis was found in 13 cases (20.3%) and myocardial necrosis in 24 cases (37.5%), with acute myocardial infarction in 9 and focal necrosis in 15. Nonbacterial thrombotic endocarditis was found in 17 cases (26.6%), mural thrombi in 11 (17.2%), and bleeding of the heart in 11 (17.2%). Platelet count, fibrinogen and euglobulin lysis time were not correlated with myocardial necrosis nor coronary thrombosis. Increase of fibrin degradation products correlated with the presence of coronary thrombosis with or without myocardial necrosis. DIC was found with a high incidence in the aged, and many of them were complicated with fresh cardiac lesions. Development of acute myocardial infarction depends on the small thrombi in the severe stenosis of the main coronary arteries or on the multiple microthrombi in the peripheral coronary branches.

Clinical and electrophysiologic studies on the Wolff-Parkinson-White syndrome in aged cases.

Clinical and electrophysiologic studies were performed in 10 aged cases (50-82 years of age) with WPW syndrome and following results were obtained: 1) Episodes of paroxysm of supraventricular tachyarrhythmias were observed in 7 and complication of sinus node dysfunction was noted in 3. 2) P-delta intervals on electrocardiogram exceeding 0.12 sec were observed in 4 cases. 3) Patterns of normalization of QRS complex in 4 cases were characterized by tachycardia-dependent in 2 and bradycardia-dependent in 2. 4) Effective refractory period of the accessory pathway (AP) in antegrade direction exceeded that of A-V node in 5 cases and the former exceeding 500 msec was observed in 2 cases. The relations between the changes of the electrophysiologic properties of the AP and altered clinical manifestations of the WPW syndrome in aged cases were discussed.

Activities of superoxide dismutase and glutathione peroxidase in the red cells of Japanese acatalasemia blood.

The activities of superoxide dismutase and glutathione peroxidase in the red cells of normal and acatalasemia bloods were compared. Superoxide dismutase activity in the red cells of Japanese acatalasemia blood was higher than in normal control, while glutathione peroxidase activity remained within normal limits. Cyanide sensitivity of superoxide dismutase in the red cells of acatalasemia blood was similar to that in normal control.

[Thymus dependent immunity against Tyzzer's disease in the mouse].

Nude (nu/nu) mice fail to resist to challenge infection of Tyzzer's disease after pretreatment with formalin-killed organisms that was effective for protecting heterozygous haired (nu/ł) mice from challenge. Resistance was induced nu/nu mice after the transfer of spleen cells from immunized nu/ł and concomitant formalin vaccine treatment.

Chronic fatal pneumocystosis in nude mice.

A chronic pulmonary disease was encountered in nude mice of a barrier sustained colony, and Pneumocystis carinii was identified as the causative agent histopathologically as well as on impression smear preparations in the affected lungs. Fatal infection was seen only in old nude mice aged more than 6 months, while focal pulmonary lesions were developed without clinical signs in young adult nudes 2 to 3 months of age. The lesions produced in aged nude mice were characterized by propagation of mononuclear cells with the presence of foamy masses of P. carinii. Heterozygous littermates were much less susceptible to the infection but pneumocystic lesions could be produced readily by multiple treatment with immunosuppressants. The infection could be transmitted without immunosuppressant to non-infected nudes but not to heterozygous littermates after intranasal inoculation of affected tissue emulsion or by cage mating with severely affected nudes.

Septicemic infection in nude mice caused by a virulent strain of Mycobacterium bovis.

Following i.v. infection with 2.3 to 2 X 10(6) viable organisms of Mycobacterium bovis strain Ravenel, congentitally athymic nude (nu/nu) mice of BALB/c background died earlier than their heterozygous (nu/+) counterparts, showing necrotic nodules in the heart and kidney with the involvement of liver and spleen. Pulmonary lesions in nu/nu mice, however, were less severe than those of dead nu/+ mice. In the reticuloendothelial organs of nu/nu mice, macrophages loaded numerous acid fast bacilli were transiently proliferated and later, necrotic-exudative lesions were produced without epithelioid granuloma formation, while necrotic lesions appeared at very early stage of infection in the other organs as well as adipose tissues. After thymocyte-reconstitution nu/nu mice were capable of developing epithelioid granulomas as seen in nu/+ mice, though having more numerous bacilli in granulomas than those of nu/+.

[Clinical experience with S-6436 in acute pyelonephritis (author's transl)].

S-6436 (a preparation consisting of 30% of non-enteric coated granules and 70% of enteric coated ones) was orally administered to the patients with acute pyelonephritis, and the following results were obtained: 1. Subjective symptoms: Very good in 13 cases (65%), good in 1, fair in 2 and poor in 4. 2. Urine finding: Very good in 13 cases (65%), good in 3 and poor in 4. 3. Disappearance of causative organisms: Organisms which were identified in the first examination were 17 strains of E. coli, one each of Pr. vulgalis, Klebsiella and Staph. epidermidis, 3 of Strept. faecalis, and 3 of mixed infections. Very good effect was observed in 16 cases, good in 1, fair in 1 and poor in 2. 4. Total evaluation: Very good in 13 cases (65%), good in 4 (20%) and poor in 3 (15%). 5. Slight discomfort in abdomen was observed in only one patient, but disappeared by administration of digestive. 6. S-6436 was given to the patients with acute pyelonephritis twice a day expecting that the drug has satisfactory clinical efficacy.
