Simplified method for total correction of interrupted aortic arch with ventricular septal defect in infancy.

Type A interrupted aortic arch with a ventricular septal defect (VSD) in a 3-month-old infant was successfully corrected with the aid of profound hypothermia, limited cardiopulmonary bypass, and total circulatory arrest. The aortic arch was reconstructed by side-to-side anastomosis of the ascending aorta and the main pulmonary artery and then creation of a tube from the anastomotic orifice to the patent ductus arteriosus (PDA) by using a superabundant flap of the anterior wall of the main pulmonary artery. The pulmonary arteriotomy and VSD then were closed. The operative field can be approached easily through a median sternotomy with minimum dissection. Cardiac catheterization and angiocardiography 2 months later demonstrated a satisfactory reconstruction of both the aortic arch and the pulmonary artery.

Synchronized rhythmic discharges of the secondary olfactory neurons in carp.

(1) Rhythmic and synchronized multiunit bursts of spike discharges, which were superimposed on the peaks of the oscillatory potentials of 4--10 Hz (olfactory tract waves), were induced in the olfactory tract fibers of the carp in response to olfactory stimulation. Suppression of the spike discharges was observed during the interburst periods of discharges. (2) Single-unit analysis revealed three types of discharge patterns in the olfactory tract fibers in response to olfactory stimulation: rhythmic excitation (Type 1), suppression (Type 2) and no response (Type 3). A synchronization of spike discharges was observed among the Type 1 units. (3) Mass potentials were evoked in the olfactory tract by electrical stimulations applied to the olfactory nerve. When the time interval between the conditioning stimulation and the test stimulation was about 200 msec, a transient recovery of the evoked mass response was observed. This interval approximately corresponded to the cycle length of the olfactory tract waves. (4) It was concluded from the results obtained that the olfactory tract waves are the summated action potentials from the axons of the secondary olfactory neurons discharging rhythmically and synchronously. (5) An inhibitory phasing mechanism in the olfactory bulb was suggested which may cause the synchronized rhythmic activity of the secondary olfactory neurons.

Treatment of seven cases of chronic granulomatous disease with sulfamethoxazole-trimethoprim (SMX-TMP).

Seven male Japanese children with chronic granulomatous disease were given sulfamethoxazole-trimethoprim (SMX-TMP) for recurrent pyogenic infections, most of which had proved difficult to control with other antibiotics. With continous treatment the children remained free of infections severe enough to necessitate hospitalization, except on one occasion. Serious complications, including hematological disorders, never occurred during therapy and there were no changes in leukocyte function during therapy. These results indicate that SMX-TMP should be considered in the treatment of bacterial infections associated with chronic granulomatous disease.

Achilles tendon thickness and ischemic heart disease in familial hypercholesterolemia.

Achilles tendon thickness (ATT) of 112 patients with familial hypercholesterolemia (FH) with and without ischemic heart disease (IHD) was measured radiographically and was compared with that of normal subjects. The mean and SD of serum cholesterol in the heterozygotes (107 cases), the homozygotes (5 cases) and the normal subjects (36 cases) were 347 +/- 63, 589 +/- 69 and 187 +/- 30 mg/dl, respectively. The mean and SEM of ATT in the heterozygotes, the homozygotes and the normal subjects were 12.5 +/- 0.4 mm, 18.6 +/- 6.6 mm, and 6.3 +/- 0.2 mm, respectively. Cutaneous xanthomas were observed in 34 out of 112 patients (30.4%). Increased ATT was observed in 95 (84.8%). IHD was diagnosed in 39 (34.8%). The ATT of FH with IHD was significantly thicker than that of FH without IHD (P less than 0.05) and that of normal subjects (p less than 0.001). Thus, the increased ATT evaluated by x-ray was the earliest clinical sign of FH and the measurement of ATT seems to be a useful adjunctive procedure for detecting familial hypercholesterolemic patients and predicting IHD in them.

Purification of ADP-ribosylated nuclear proteins by covalent chromatography on dihydroxyboryl polyacrylamide beads and their characterization.

Nuclear proteins modified by mono or poly ADP-ribosylation were selectively isolated and purified by covalent chromatography on a dihydroxyboryl polyacrylamide bead column that specifically interacts with cis-diol-containing compounds. From rat liver nuclei that had been incubated with NAD+, histones and some nonhistone proteins were extracted with 0.25 M HCl. Approximately 60% of the ADP-ribose incorporated into 20% trichloroacetic acid-precipitable material was recovered in this extract. The ADP-ribosylated material was then isolated from the extract by covalent chromatography on a borate gel column and further purified by carboxymethylcellulose column chromatography. As judged by electrophoretic mobilities in various gel systems and by amino acid compositions, approximately 50% of the ADP-ribose recovered in the carboxymethylcellulose fractions was associated with several nonhistone proteins with molecular weights of 2-6 x 10(4), while 35% aand 15% were associated with histones H2B and H1, respectively. Since the average chain length of the polymer bound to any of these proteins was less than two ADP-ribos-l units, the percentage distribution reflects the number of ADP-ribosylated sites rather than the chain length.

An explanation for the high incidence of congenital rubella syndrome in Ryukyu.

An exceptionally high incidence of congenital rubella syndrome (CRS) has been noted in Ryukan children who were infants in utero during an extensive rubella epidemic in the Ryukyu Islands in 1964 through the spring of 1965. In 1965, the Congenital Rubella Study Group of Kyushu University registered in the Ryukyus 408 cases of CRS and an incidence of 20 cases of CRS per 1000 births. The nearby Amami Islands, on the other hand, had a low incidence of CRS. Seroepidemiologic studies indicated a high prevalence of antibodies in the Ryukyus but a low prevalence in the Amamis. Since CRS is uncommon throughout Japan, some investigators have hypothesized that Japanese strains of the virus are avirulent and that a virulent American strain caused the epidemic in the Ryukyus. However, retrospective seroepidemiologic studies reported here indicate that the high incidence of CRS in the Ryukyus was more probably due to low seropositivity and a resultant high attack rate of rubella among pregnant women, rather than to a hypothetical virulent teratogenic strain of rubella virus. Conversely, the data suggest that the low incidence of CRS in Japan may reflect the infrequency of rubella nonimmunes in women of childbearing age.

Modification of immune response in nude mice infected with mouse hepatitis virus.

In nude mice experimentally infected with mouse hepatitis virus (MHV), the numbers of early and later plaque forming cells (PFC) to sheep red blood cells (SRBC) generated in the spleen were 7 to 20 times and 2 to 163 times, respectively, greater than those in non-infected nude mice, when SRBC were given at day 0 to day 21 postinfection. Splenic theta-positive lymphocytes in infected nude mice were shown to increase only at day 10 or more postinfection. PFC response to bacterial lipopolysaccharide, a T cell-independent antigen, was not modified in MHV-infected nude mice.

Membrane-limited granules in sinus hair gelatinous membrane in mice.

Electron dense membrane limited granules were observed in basal cells of the outer root sheath and in connective tissue sheath of the anagen sinus hair of mice. These granules were oval or round in configuration, and were 150--180 nm in size. They were considered to represent structures related to collagenolytic activity to accelerate elongation of anagen hair.
