Glossopharyngeal and tectal influences on tongue-muscle motoneurons in the Japanese toad.

Anuran tongue-muscle motoneurons receive excitatory inputs both from the glossopharyngeal nerve afferents and from the optic tectum. As a step toward elucidating the neural bases for controlling the tongue movements, we searched intracellularly for the neuronal pathways from the glossopharyngeal afferents to the tongue-muscle motoneurons in paralyzed Japanese toads. The electrical stimuli applied to the glossopharyngeal nerve (ipsilateral lingual branch) evoked polysynaptic excitatory postsynaptic potentials (EPSPs) in tongue-protractor motoneurons and mixed mono- and polysynaptic EPSPs in tongue-retractor motoneurons. Furthermore, we investigated the mode of the convergency of the excitatory inputs from the glossopharyngeal nerve and those from the optic tectum. A spatial facilitation was observed between the tectal EPSPs and the glossopharyngeal EPSPs in some motoneurons tested. These results suggest the existence of common excitatory premotor interneurons, on which the tectal descending volleys and the glossopharyngeal afferent volleys converge.

Treatment of childhood acute lymphoblastic leukemia with intermediate-dose cytosine arabinoside and adriamycin.

Eight pediatric patients with acute lymphoblastic leukemia (ALL) were treated with intermediate-dose cytosine arabinoside (ID-AraC, 1 g/m2) in combination with adriamycin except one patient. Of the eight patients, four refractory to the initial induction therapy and one in bone marrow relapse gained complete remission with two to three cycles of this therapy. Four of the five patients have been in continuous remission for 4 to 24 months with the maintenance therapy of monthly administration of ID-AraC. One patient in central nervous system (CNS) relapse has continued in remission from CNS leukemia after two cycles of the therapy. Side effects of ID-AraC and adriamycin were generally mild to moderate and tolerable in all children. These results suggest that the use of ID-AraC and adriamycin might prove effective in the treatment of ALL refractory to other regimens.

Correlation between the saliva and free serum concentration of phenobarbital in epileptic children.

Twenty epileptic children taking phenobarbital (PB) were evaluated for the concentration of PB in their saliva (Sa), total serum concentration (TS), free serum concentration (FS), and also the pH of the saliva samples. The Sa/TS ratio was 43.0 +/- 5.2% (mean +/- SD), and showed a close relationship between the two (r = 0.98). The free serum concentrations for PB were also observed to be closely correlated to the saliva concentration (r = 0.99), as well as to the total serum concentration, with the FS/TS ratio being 45.0 +/- 5.6%. However, no obvious relationship between the salivary pH and the Sa/TS ratio for PB was observed. This suggested the usefulness of monitoring the PB saliva concentrations in clinical management of epilepsy.

Protein phosphorylation of neutrophils from normal children and patients with chronic granulomatous disease.

The phosphorylation and dephosphorylation of proteins in neutrophils from normal children and patients with chronic granulomatous disease (CGD) were studied with two-dimensional gel electrophoresis and autoradiography, followed by densitometric scanning. In normal neutrophils the radioactivities of 11 spots among approximately 50 radioactive spots were changed by stimulation with phorbol 12-myristate 13-acetate (PMA) and 6 of the 11 spots were also changed by stimulation with N-formyl-methionyl-leucyl-phenylalanine (FMLP) and NaF. The phosphorylation of only two spots (Mr = 48 000 and 62 000) was inhibited by 2-deoxyglucose and N-(6-aminohexyl)-5-chloro-1-naphthalene sulphonamide (W-7), which inhibits superoxide production, while it was not affected by dibutyryl cAMP, KCN and ethyleneglycol-bis-(beta-aminoethylether)-N,N'-tetraacetic acid (EGTA), which do not affect superoxide production. The observation indicates that the Mr = 48 000 and 62 000 proteins may be involved in the activation process of superoxide production. When the neutrophils of four male and two female CGD patients were examined, the changes in 11 spots on stimulation were similar to those of normal children, indicating that the (de)phosphorylation of the proteins which seems to be involved in the activation process is not affected in CGD neutrophils.

Accordion flap.

The accordion flap is proposed as a means of covering a rhombic defect. A prominent feature of the method presented is in the preparation of a flap on one side of the skin defect, resulting in little dog ear.

Doppler detection of tricuspid regurgitation following Kawasaki disease.

Between June 1984 and April 1985, five patients (4.2%) had tricuspid regurgitation detected on pulsed Doppler echocardiography among 119 patients with Kawasaki disease. The grade of tricuspid regurgitation was moderate in three and mild in two patients; conventional cardiac examinations revealed no evidence of this complication. All patients had associated coronary artery aneurysms except one, but no patients developed subsequent myocardial infarction. The occurrence of tricuspid regurgitation following Kawasaki disease has been considered to be causally related to the carditis, a frequent complication during the acute febrile period of the illness. The present data indicate that tricuspid regurgitation is an additional cardiovascular complication of Kawasaki disease and that Doppler echocardiography is useful in detecting this disorder.

Brainstem auditory evoked potentials in children with neurodegenerative diseases.

We assessed the usefulness of brainstem auditory evoked potentials (BAEPs) for differentiating types of degenerative diseases in children. The findings of BAEPs were abnormal in one out of two with polioencephalopathies and in seven of the eight with leukoencephalopathies, showing prolonged interpeak latencies of waves I-III, I-V or disappearance of wave components. On the other hand, normal responses were observed in four with corencephalopathies and one with spinocerebellopathy. The serial BAEPs in eight patients had deteriorated with progression of the clinical symptoms. These data on 16 children suggest that BAEPs are indeed a useful tool for monitoring pathophysiologic processes of neurodegenerative diseases.

Preferential association of acidic actin with nuclei and nuclear matrix from mouse leukemia L5178Y cells.

Nuclear matrix prepared from mouse leukemia L5178Y cells contained not only the two common actin isomers, beta and gamma actins, but also two additional acidic species of actin (pI 5.1 and 5.3). An anti-actin antibody recognized these acidic species as well as beta and gamma actins on a nitrocellulose filter following western blotting of two-dimensional electrophoresis. These acidic species were co-purified with beta and gamma actins using DNase I-Sepharose affinity chromatography on the nuclear matrix. Limited digestion of the acidic actin with protease V8 or trypsin gave very similar peptide fragments as did digestion of beta and gamma actins. These acidic actins were found to be distributed in the nuclear fraction, but were scarcely detectable in the cytoplasmic fraction. One of the acidic actins (pI 5.3) was found in all subnuclear fractions (DNase extract, high-salt extract and nuclear matrix), while the other species, the most acidic actin (pI 5.1), was localized predominantly in the nuclear matrix.

Causes of death in patients with familial hypercholesterolemia.

Five out of 15 homozygotes and 41 out of 527 heterozygotes of familial hypercholesterolemia (FH) died during the past 10 years. Sudden death or heart failure was the cause of death in each of the 5 deceased homozygotes. Twenty heterozygotes died of myocardial infarction, 9 of sudden death, and 1 died after AC bypass surgery. Thus, 30 heterozygotes (73.2%) died of coronary heart disease (CHD). The mean age of death was significantly younger in male heterozygotes (54 years) than in the females (68 years). Rate of death from CHD in heterozygotes was 11 times higher than in the general population in Japan. Rate of death from pancreas cancer in FH was significantly higher than in the general population. These results suggest that FH is highly associated with pancreas cancer as well as CHD.

Three-dimensional prediction of mandibular movement in the treatment of prognathism.

The three-dimensional movements of the mandible that might occur during surgical correction of mandibular prognathism were analyzed preoperatively in 40 patients. Direct measurements were made on the dental models used to predict postoperative occlusion, and the results were transferred to the cephalometric tracings using three-dimensional coordinate transformation. Menton, gonion, and two bony points on the posterior borders of the ascending rami were chosen on the frontal and lateral cephalograms for prediction of the antero-posterior, vertical, and lateral movements. In most cases a reasonable amount of movement without much lateral shift at the posterior border of the mandible was predicted to occur when the postoperative position of the mandible was established to provide a stable occlusion. However, there were five cases in which predicted lateral movement at one of the posterior borders exceeded 5 mm, and, in four of them, protrusion of the mandible was not accompanied by lateral deviation. It was concluded that careful preoperative planning and postoperative treatment are necessary in these cases because a large amount of lateral movement at the site of operation might lead to postoperative relapse following ramus osteotomy.

Clinical characteristics of myocardial infarction following Kawasaki disease: report of 11 cases.

Between 1979 and 1984, 11 patients with myocardial infarction following Kawasaki disease were seen in our hospital. There were seven boys and four girls, aged from 3 months to 6 years. This cardiovascular complication developed in the early stage, from 19 days to 6 months of illness, in all but three patients. Significant clinical symptoms were recognized in only five patients. Two patients died, the conditions of two are well controlled with anticongestive therapy, and the remaining patients are asymptomatic. The diagnosis of myocardial infarction was confirmed by the following clinical findings: typical ECG patterns (10/11), abnormality of the left ventricular wall movement by serial two-dimensional echocardiography (9/10), elevated value of cardiac enzymes (6/6), perfusion defect by thallium 201 myocardial scintigraphy (6/8), and coronary artery occlusion or ventricular aneurysm by angiocardiography (9/9). All patients had markedly dilated and multiple coronary aneurysms during the course of the illness. Because myocardial infarction is frequently associated with the rapidly dilating coronary artery during the acute stage, ECG monitoring, preparation of resuscitation equipment, and use of antithrombotic agents are recommended for those high-risk patients. When a myocardial infarction has been diagnosed, measures including cardiac monitoring, use of vasodilators, inotropic agents, and urokinase may be valuable.

Incidence of congenital rubella syndrome in Japan (1965-1985). A nationwide survey of the number of deaf children with history of maternal rubella attending special schools for the deaf in Japan.

Accurate congenital rubella syndrome surveillance in Japan is of great interest because of the hypothesis that the Japanese rubella virus was less virulent than the strains prevalent in Europe and the Americas 20 years ago. A nationwide survey of deaf children with a history of maternal rubella in special schools for the deaf in Japan yielded a total of 365 deaf school children with congenital rubella syndrome born between 1963 and 1982. The overall average prevalence was 0.31 case per 100,000 population, and the incidence was 0.1-6.1 cases per 100,000 annual live births in Japan. If one considers the decrease in the number of deaf children attending special schools for the deaf because of the recent adoption of the policy of integrating deaf children into ordinary schools, the number of cases of congenital rubella syndrome might actually be much greater than that revealed by this survey and much greater than the 88 cases previously reported in Japan. Among the 365 cases who had deafness, 8.2% were found to have cataract (with and without congenital heart disease), and 11.0% were found to be complicated by congenital heart disease (but without cataract).

Flexible rhombic flap.

A flexible rhombic flap which can be designed in accordance with the degree of tension of skin bordering a part of the defect has been described. This procedure provides sufficient coverage of a skin defect with very little dog-ear.

Vascularized periosteal grafts: an experimental study using two different forms of tibial periosteum in rabbits.

Three groups of rabbits were studied to investigate the osteogenic capacity of vascularized periosteum. It was found that such tissue, especially in the form of musculoperiosteal flaps, produced a significant amount of bone. It is concluded that vascularized periosteal flaps have an excellent osteogenic capacity even though they are not subjected to weight or stress.
