Diagnostic evaluation of hepatobiliary scintigraphy using 99mTc-N-pyridoxyl-5-methyltryptophan in acute cholecystitis.

Hepatobiliary scintigraphy using 99mTc-N-pyridoxyl-5-methyl-tryptophan (99mTc-PMT) was carried out in 80 patients with benign biliary diseases, excluding cholecystectomized patients, for the diagnosis of acute cholecystitis. Using the conventional diagnostic criteria based on nonvisualization of the gallbladder in acute cholecystitis, the specificity, efficiency and positive predictability test results were 63%, 72% and 45%, respectively. When delayed intestinal transit of radioactive material was added to this criteria, the diagnostic value was significantly improved (p less than 0.01); the specificity, efficiency and positive predictability test results were 97%, 97% and 90%, respectively. Delayed intestinal transit of radioactive material should be considered in the diagnosis of acute cholecystitis.

In vivo and in vitro effects of zinc oxide-eugenol (ZOE) on biosynthesis of cyclo-oxygenase products in rat dental pulp.

To determine the in vivo effects of a zinc oxide-eugenol mixture (ZOE) on the cyclo-oxygenase system in dental pulp, we used radioimmunoassay to measure the levels of prostaglandin E2 (PGE2), 13,14-dihydro-15-keto-PG (DHK-PG), thromboxane B2 (TXB2), and 6-keto-PGF1 alpha in the dental pulp of rats. When the dental pulp was irritated by a hole made in the dentin of the mandibular incisors without use of any coolants, the levels of these cyclo-oxygenase products in the pulp were increased to, respectively, 2.8, 1.7, 10.0, and 2.6 times those in the normal pulp at six hr after treatment. In contrast, these increases in cyclo-oxygenase products disappeared immediately when the artificial cavity in the dentin was filled with ZOE (P/L; 1 g/0.25 mL), but were not altered when the cavity was filled with zinc oxidewater (ZOW, 1 g/1.5 mL). Most of the eugenol portion of ZOE was released into the pulp within two hr after the cavity was filled with ZOE. The maximal eugenol content was 35 pmol per mg of pulp. Furthermore, when the cavity was filled either with ZOE or by the addition of 10 mumol/L eugenol to the pulp homogenate, biosynthesis of 14C-6-keto-PGF1 alpha, PGF2 alpha, and PGE2 from 14C-arachidonic acid in the homogenate was inhibited. These results suggest that eugenol released from ZOE in the cavity prepared in the dentin inhibited the biosynthesis of cyclo-oxygenase products during pulp irritation.

The effects of oxygen toxicity on the pregnant rat using an apparatus designed to measure oxygen consumption method.

To evaluate the effects of oxygen toxicity in the pregnant rat at high and low concentrations of oxygen, an oxygen-consumption apparatus was devised to measure the consumption of oxygen continuously over long periods. Oxygen consumption, partial oxygen pressure on the skin (tcPo2), and lipid peroxide levels in the serum were measured. There was a close correlation between oxygen consumption and body weight of rats that weighed between 150 g and 450 g. Oxygen consumption during the later stages of pregnancy increased by 1.8 ml/day, an increase of 8%. Oxygen consumption by rats during parturition increased markedly for up to 1 h and then reached and remained at a plateau value until the end of delivery. Exposure of pregnant rats to low concentrations of oxygen resulted in a marked depression in oxygen consumption and tcPo2 during the exposure time. A significant increase in the lipid peroxide level in serum was observed in the mother rat after birth and in the newborn offspring of pregnant rats exposed to 16% oxygen for 3 h.

[A case of intrahepatic bile duct cancer responding to 5-fluorouracil, adriamycin and mitomycin C chemotherapy].

A 64-year-old woman who was diagnosed as inoperable intrahepatic bile duct cancer with massive liver metastasis was treated with 5-fluorouracil (5-FU), adriamycin (ADM) and mitomycin-C (MMC). Obstructive jaundice was reduced and performance status was soon improved by this regimen. The reduction rate of the tumor was noted to be more than 50% on CT images, so this patient was considered to have achieved a partial response. In histological examination, well differentiated tubular adenocarcinoma cells were degenerated and replaced fibrous tissue. Thus, it was considered that the reduction of the tumor was caused by these phenomena. This case suggested the activity of 5-FU, ADM and MMC combination for bile duct cancer.

High frequency of loss of human kappa light chain expression in mouse-human heterohybridomas.

The expression of human immunoglobulin (Ig) chains in human-mouse heterohybridomas was investigated on the days after cell fusion. Heterohybridomas were made by fusion of mediastinal lymphocytes from lung carcinoma patients and murine (BALB/c) myeloma P3U1. Most (more than 76%) of heterohybridomas easily lost the light chain rather than the heavy chain expression in the early stages after cell fusion. Frequency of loss of kappa-chain expression was predominant, whereas the expression of heavy chain and lambda-chain was rather stable in comparison with that of kappa-chain. These were also confirmed at the clonal level, demonstrating that only lambda-chain but not kappa-chain producers were successfully enriched. In fact some heterohybridomas producing human Ig of lambda-chain type established were found to be quite stable for more than 2 years without any recloning procedures.
