[Pituitary apoplexy of giant pituitary adenoma--case report (author's transl)].

Pituitary apoplexy, characterized by sudden onset or acute progression of signs and symptoms of pituitary adenoma, is rare yet well-known. One such case is presented with clinical course, neuroradiological findings and autopsy. A 53-year-old female complained of sudden onset of severe headache, and rapidly deteriorated in the level of consciousness. Five days later, she became comatose and was admitted to National Sendai Hospital. Plain skull films and carotid angiograms revealed a remarkably large mass in the pituitary fossa. Death came ten hours after admission. Autopsy revealed a giant tumor (10 X 5 X 7.5 cm) destroying the sella turcica and protruding toward both the nasopharyngeal cavity and the hypothalamic region. Microscopically, diagnosis was made as chromophobe adenoma with fresh hemorrhage in the tumor.

Feed-back control of cholesterol synthesis in partially hepatectomized rats.

Partial hepatectomy caused a marked stimulation of cholesterol and fatty acid syntheses without affecting serum total cholesterol, total phospholipid and triacylglycerol concentrations of rats so far examined 48 h after the operation. Serum free cholesterol level, however, was increased by the treatment and the ratio of lysophosphatidylcholine to phosphatidylcholine was concomitantly decreased, suggesting the impairment of serum lecithin: cholesterol acyltransferase activity. The lipid content in the liver, especially triacylglycerol and ester cholesterol, was increased markedly by the operation. Feeding of a high cholesterol diet which elevated serum cholesterol and phospholipid levels to the partially hepatectomized rats, accelerated the accumulation of hepatic triacylglycerol and ester cholesterol by the partial hepatectomy. The weight of the regenerating liver was not influenced by cholesterol feeding, which suggested that cholesterol feeding did not inhibit the regeneration mechanism of the liver. The increase of cholesterol synthesis after partial hepatectomy was inhibited by cholesterol feeding. Therefore, it is conceivable that the negative feed-back control of cholesterol synthesis is induced by cholesterol feeding under the stimulated cell divisions of the liver after partial hepatectomy. It is suggested from the present data that a large amount of the cholesterol which is necessary for cell growth can be taken up from serum, when serum cholesterol concentration is high.

Metabolic products of microorganisms. 155. Modes of action of rinamycin and derinamycin.

Rinamycin interfered with DNA-polymerase I of Escherichia coli in cell-free systems more effectively than actinomycin C. Its inhibitory action could be relieved by increasing the concentration of polymerase. The rinamycin concentrations that caused effective inhibition of the RNA-polymerase reaction in vitro were approximately 10 times higher than those required to inhibit the DNA-polymerase reaction in vitro.

Cardiac myosin from pig heart ventricle. Purification and enzymatic properties.

A method is described for the preparation of high purity myosin from the left ventricle of pig heart. The purified myosin was free from nucleic acid, actin, tropomyosin, troponin, the 150,000 molecular weight protein and other contaminants. Analyses of subunits in the purified myosin were carried out on 3.5% acrylamide gel with 0.1% SDS. Of the total protein present in myosin, 11.3% was in the light chains; light chain 1 (LC1), 5.9% and light chain 2 (LC2), 5.4%. Urea gel electrophoresis of the purified myosin showed three closely spaced bands corresponding to the 20,000 dalton, the charge-modified 20,000 dalton and the phosphorylated 20,000 dalton components. The properties of the Ca2+-activated and K+-activated ATPases [EC 3.6.1.3] of the purified myosin were also studied. The Km values were 27 and 55 muM and the Vmax values were 0.263 and 0.317 mumole P1/mg/min for the Ca2+-activated and K+-activated ATPases, respectively. The pH-activity profiles and the effects of SH modification were of the skeletal myosin type except that the activities were lower.

Occurrence of renovascular hypertension in two sisters.

Renovascular hypertension in two sisters, aged 22 and 20, respectively, has been described. Renal artery stenoses were observed unilaterally in the elder patient and bilaterally in the younger one. In both patients, the functional significance of unilateral stenosis of the renal artery was documented by the renal vein renin ratio between the affected side and the contralateral or less affected side. High blood pressure and elevated plasma renin activity have been normalized with a unilateral revascularization in the elder patient, and with the treatment of propranolol in the younger one. The histological examination of the stenotic renal artery in the elder patient showed a finding comparable to the perimedial fibroplasia in Harrison and McCormack's classification of idiopathic fibromuscular stenosis.

Effect of a spironolactone derivative (SC 14266) on Plasma renin activity in adrenal regeneration hypertension.

The effects of treatment with a spironolactone derivative (SC 14266) for 9 experimental weeks on blood pressure and plasma renin activity (PRA) were investigated in unilaterally nephroadrenalectomized rats drinking 1% saline ("unilater nephrectomy"), unilaterally nephroadrenalectomized and contralaterally adrenal-enucleated rats drinking 1% saline ("adrenal enucleation") and intact rats drinking tap water ("normal"). The development of hypertension in "adrenal enucleation" rats was prevented by treatment with SC 14266 and the drug did not significantly affect the blood pressure in either "unilateral nephrectomy" or "normal" rats. SC 14266 did not influence a low level of basal PRA and the blunted response of PRA to furosemide administration in either "unilateral nephrectomy" or "adrenal enucleation" rats. On the other hand, PRA after furosemide administration in "normal" rats receiving SC 14266 was significantly higher than that in those rats treated with vehicle. The results suggest that the mineralocorticoid(s) secreted by the enucleated adrenal has a hypertensogenic property but no effect on the suppression of renin secretion under a high sodium intake and the unilateral nephrectomy.

Effects of synthetic progestational steroids on liver and serum lipids in rats.

Effects of norethisterone (NT) purified norethisterone (pure NT), norethynodrel (NE), medroxyprogesterone acetate (MAP), chlormadinone acetate (CMA) and desoxycorticosterone acetate (DOCA) on serum and liver lipid levels and serum lipoproteins were examined in both intact and estradiol-treated male rats. NT and NE caused a decrease in serum cholesterol and phospholipid levels, an increase in liver cholesterol level, no significant change in triglyceride levels of both serum and liver, with a significant change in serum lipoprotein patterns; a decreased in alpha- and beta-lipoproteins and a marked increase in pre beta-lipoprotein. Pure NT decreased serum cholesterol without causing any change in lipoprotein pattern. MAP, CMA and DOCA causee almost no effect on lipid levels in serum and liver, but CMA and DOCA increased alpha-lipoprotein and decreased beta- and pre beta-lipoproteins. An acute treatment with estradiol caused a decrease in alpha- and beta-lipoproteins and an increase in pre beta-lipoprotein with a decrease in serum lipid levels and an increase in liver lipids. By contrary, a chronic treatment with a marked hypercholesterolemia. This increase of alpha-lipoprotein in estradiol-treated rats was prevented by NT and NE, not affected or rather decreased by MAP but further increased with CMA and DOCA. These data suggest that the effects of synthetic progestational steroids on lipids are classified into two groups, 19-nortestosterone derivatives and 17alpha-hydroxyprogesterone derivatives including DOCA. The former caused a decrease in serum lipid levels with an increase of pre beta-lipoprotein and adecrease of alpha-lipoprotein. The latter caused almost no change or a slight increase in serum lipid levels with a decrease in pre beta-lipoprotein and an increase in alpha-lipoprotein, though it was not found in MAP.

Changes in plasma renin activity and plasma aldosterone in the induced paralytic attack of thyrotroxic periodic paralysis.

Changes in serum electrolytes, haematocrit, plasma renin activity and plasma aldosterone induced by glucose and insulin (GI) infusion were serially investigated in seven patients with periodic thyrotoxic paralysis. An attack which developed into complete quadriplegia was induced within 90 min after the beginning of the GI infusion in four out of seven patients. Only a slight paralysis of the legs was produced in another two patients and induction of an attack did not materialize in one. In four patients with complete quadriplegia, the mean values of serum sodium and potassium concentrations, haematocrit, plasma renin activity and plasma aldosterone slightly decreased immediately after the beginning of the GI infusion. Induction of a paralytic attack was not accompanied by any significant changes in serum sodium concentration, haematocrit, plasma renin activity and plasma aldosterone either 15 min before or after the onset of attack, while the serum potassium concentration progressively decreased, and an increase in plasma aldosterone associated with an increase of haematocrit and plasma renin activity reached a peak level at the stage of complete quadriplegia. On the other hand, in the three patients in whom an infusion produced slight or no paralysis of the legs, changes in the serum sodium concentration, haematocrit, plasma renin activity and plasma aldosterone were insignificant and the serum potassium concentration was slightly but insignificantly decreased. These results suggest that hyperaldosteronism may not be a trigger for the induced paralytic attack but a phenomenon secondary to volume depletion and a change in potassium homoeostasis induced by GI infusion.
