[Meningeal inflammatory diseases causing transverse lesions in childhood (arachnoiditis spinalis chronica and pachymeningitis spinalis hypertrophicans). Diagnosis and treatment (author's transl)].

The Arachnoiditis spinalis and the Pachymeningitis spinals are seldom the cause of neurologic deficit. During infancy both illnesses are extremely rare. One illness with a Arachnoiditis spinalis chronica adhaesiva cystica and, for the first time one with a Pachymeningitis spinalis hypertrophicans could be observed. The signs show parallels to spinal tumors, but when inflammation is the origin, the early starting symptoms of tumors appear relatively late. Symptoms of the radix remain dominant. Myelography shows a typical drop shaped block in the case of Arachnoiditis. Diagnosis can be asured only histologically. Surgery may prevent remaining defects of the spine by decompression and may obtain a permanent success by the removal of local processes. When expanded changes occur, an additional chemotherapy with Prednisolon, perhaps in combination with Endoxan may cure the patient, as shown in our two cases.

Treatment of the traumatic cerebral vasospasm with the calciumchannel blocker nimodipine: a preliminary report.

Five patients with severe head injury, rating between 3 and 5 on the Glasgow Coma Scale, were treated with the calciumchannel blocker nimodipine continuously for up to 12 days (2 mg/h) because of severe spasm of cerebral arteries. All cases involved the intracranial carotid artery and the first segments of the medial and anterior cerebral arteries. Control angiograms performed in 4 cases showed improved or normalized vessel calibre after the treatment. Intracranial pressure as well as blood pressure were not altered during nimodipine treatment. EEG improved within 7 days after treatment in all cases. Repeated CT scans showed reduction of cerebral edema. Hypothalamic disorders observed in 3 cases were stabilized. Based on these preliminary results, calciumchannel blocker reverses traumatic vasospasm and ameliorates physiologic function of the brain structures after severe head injury.

[Cerebrospinal fluid shunt in cases of dementia (author's transl)].

The authors distinguish between two evolutive forms of dementia, one being chracterized by prpgressive deterioration (for instance in the course of arteriopathic, degenerative or infections processes) and the other in which dementia sets in after the clinical picture has improved (for instance after meningeal haemorrhage, craniocerebral injury, endocrinous, metabolic or circulatory disorders, etc.). Ventricular drainage (according to Pudenz-Heyer) was performed in 20 patients presenting dementia: the results were favourable in the seven patients presenting dementia: the results were favourable in the seven patients presenting the first evolutive form and in almost all 13 patients presenting the second form.
