Deformities in the orbital area can also be corrected by craniofacial surgery. Early surgery within the first year of life is indicated in cases with intracranial pressure if permanent damage to the eye is to be avoided. Otherwise, corrective surgery should be performed in early childhood, although it can also be performed on adult patients. More severe strabismus cannot be treated, but disturbance of lid function can be positively influenced. From the esthetic point of view considerable improvement can be achieved. These very protracted operations require intensive interdisciplinary cooperation.
Craniopharyngioma, primarily a benign tumour, is associated with a variety of serious problems due to tumour localization. Access to the tumour during surgery is difficult and total resection can rarely be achieved. Growth of the remaining tumour continues and symptoms of increased intracranial pressure, further visual field loss and hormonal disturbances reappear. However, conservative operation in conjunction with radical irradiation shows good results as regards tumour regression and survival with satisfactory quality of life. Irradiation with 50 Gy on average was performed in 5 out of 6 children with remaining tumour or tumour recurrence, and satisfactory results were obtained. In all cases tumour size regressed. Endocrine disturbances could be ascertained by the combined LHRH-TRH-insulin test and the clinical picture. In nearly every case there was finally a complete lack of hormones of the pituitary-hypothalamic axis. Substitution of various hormones became necessary in all cases. Meticulous surveillance of children still undergoing developmental maturation is mandatory. All six children are well and lead a nearly normal life.

Cerebral vasospasm is caused by many factors. Primarily it is caused by Noradrenalin (NA) localised in the vessel walls and by platelet aggregation, with raising of the concentration of 5-HT in the intima. Its prolongation is based on blood breakdown products and or biochemical disturbances of brain metabolism. The most potent vasoconstrictors are acting through the vessel's own receptors (Serotonin, NE, Histamin), so the most effective therapy is based on the contractile system of the vessel. By blocking the alpha-receptors by Phenoxybenzamine, blockade of the Phosphodiesterase by Papaverin, Euphyllin to raise the concentration of cAMP and stimulation of Adenyl-Cyclase-System by beta-stimulation, it is possible to get good results in the treatment of cerebral arterial vasospasm.
Craniofacial surgery deals with the correction of congenital acquired deformities of the skull and face. This kind of surgery together with microsurgery can be regarded as the most outstanding development within the past decade. Plastic surgery, as the center of organisation is responsible for coordination with many other medical disciplines. Since 1978 an interdisciplinary group for craniofacial surgery started with operations at the University Hospital for Plastic Surgery in Innsbruck and the Department for Plastic Surgery at the Klinikum rechts der Isar in Munich. In this article first experiences are reported and three cases, a Morbus Apert, a Morbus Crouzon and a Hypertelorismus presented. The indication for such treatment is directed for functional disturbance but also for aesthetical reasons to improve the psycho-social structure of the patient. An early operation is indicated in stenosis of cranium and face to avoid damage of the brain and eyes. If not urgent, these interventions should be started beyond the age of about 8, after dentition.

Myelography with water soluble contrast is a valuable method of diagnosis for suspected disc prolapse. In our experience complications are rare and therefore the indications for contrast examinations should not be too strict. Only in this way is it possible to localise the level accurately, so that surgical intervention can be confined to the appropriate level.
Circumscribed intraventricular haemorrhages are very rare. The clinical picture showed the symptom of the subarachnoid haemorrhage and additional local symptoms (e.g. hemiparesis, psychic disorders). Neuroradiologically, signs of a space-occupying process in the ventricle are found. A differential diagnosis is not possible. The operation sucks off the haematoma and removes the thin membrane of the haematoma. Sources of the haemorrhage may be damage of the vascular wall, microangiomas and microeneurysms which are destroyed by the haemorrhage.
