[Pick's disease in senescence].

We report two patients with Pick's disease in senescence. Patient 1 is a 78-year-old woman. She developed abnormal behavior at the age of 76 years. Neurological examination at age 76 revealed poor rapport, easy angriness, "Denkfaulheit", oral tendency, and slight dementia [WAIS (Wechsler Adult intelligence Scale) total IQ 62]. Cranial CT scan and MRI showed bilateral atrophy of the frontal and temporal lobe, especially of the temporal lobe. Patient 2 is a 73-year-old man. He developped sexual abnormal behavior and "triebhafte Hemmungslossigkeit" at the age of 71 years. Neurological examination at age 72 revealed poor rapport, lack of spontaneity, easy angriness, "Denkfaulheit", and slight dementia [WAIS total IQ 91]. Transient "stehende Redensarten" was noticed. Cranial CT scan and MRI showed bilateral atrophy of the frontal and temporal lobe, especially of the frontal lobe. To our knowledge, Pick's disease with an onset in the senescence is very rare. Pick's disease should be included in the differential diagnosis of abnormal behavior in the senescence.

[Surgical treatment of distal aortic arch aneurysm combined with mitral regurgitation--a case using "elephant trunk" technique and concomitant mitral valvuloplasty].

We present a surgical case of a 63-year-old male with distal aortic arch aneurysm combined with mitral regurgitation. Through a median sternotomy, a mitral valvuloplasty was performed with McGoon's procedure and Carpentier's ring. Then with the aid of deep hypothermia and circulatory arrest, the aortic arch was opened. A long woven velour graft was anastomosed only proximally to the distal end of the aortic arch and was allowed to float down the descending aorta ("Elephant Trunk" technique). Anti-coagulant therapy was continued only for 4 weeks after the operation. Postoperative left ventriculogram showed no residual regurgitation. The aneurysm was confirmed fully thrombosed around the graft on computed tomogram. It is concluded that "Elephant Trunk" technique for distal arch aneurysm is useful when combined other cardiac surgery without anti-coagulant therapy, such as mitral valvuloplasty, is needed.

[Presenile dementia with marked recent memory disturbance--in relation to hippocampal dementia].

Two cases of presenile dementia with marked recent memory disturbance were reported. Patient 1 is a 54-year-old woman. She noticed forgetfulness at the age of 51 years. Neurological examination at aged 52 revealed marked recent memory disturbance, but examination by WAIS (Wechsler Adult Intelligence Scale) showed good results (verbal IQ 106, performance IQ 104, total IQ 106). There was neither disorientation in place nor character change. Cranial CT scan and MRI revealed the absence of brain atrophy. About 3 years after the onset of the disease, the degree of dementia is slight and disorientation in place does not appear. Patient 2 is a 67-year-old man. He noticed forgetfulness at the age of 63 years. Neurological examination at aged 66 revealed marked recent memory disturbance, but examination by WAIS-R showed moderate results (verbal IQ 89, performance IQ 87, total IQ 88). There was neither disorientation in place nor character change. Cranial CT scan and MRI revealed slight dilatation of the inferior horns of the lateral ventricle and slight cortical atrophy. About 4 years after the onset of the disease, the degree of dementia was slight and disorientation in place did not appear. We can not rule out the possibility that our cases belong to Alzheimer's disease, but the clinical course of our cases is peculiar. In the relation of responsible lesion in pure amnestic syndrome, hippocampal dementia, and simple senile dementia, our cases are interesting and important.

Deletion mapping of chromosome 8p in colorectal carcinoma and dysplasia arising in ulcerative colitis, prostatic carcinoma, and malignant fibrous histiocytomas.

Short tandem repeat polymorphism markers on the short arm of chromosome 8 were used to search for loss of heterozygosity (LOH) in colorectal carcinoma and dysplasia complicating ulcerative colitis, in prostatic carcinoma, and in malignant fibrous histiocytoma (MFH). Fifty percent of prostatic carcinomas (13/26), 44% of carcinomas or dysplasias arising in ulcerative colitis (7/16), and 30% (4/12) of MFH cases showed LOH for markers on 8p. Detailed mapping demonstrated variability in the size of the chromosomal region showing LOH; however, the data suggest a common 30-centimorgan region of LOH on chromosome 8p between the LPL locus and pter in colorectal and prostatic cancers. In addition, LOH was observed on 8p in both high-grade and low-grade dysplasia in ulcerative colitis, indicating that LOH on 8p may occur at an early stage of neoplastic development in this disorder. In contrast, MFH cases exhibited LOH for marker D8S87, which has been identified as being near the putative Werner's syndrome locus. These results suggest that a tumor suppressor gene, located on the distal portion of chromosome 8p, exists in common for prostatic and colorectal carcinomas, and a second tumor suppressor gene may exist linked to the Werner's syndrome locus.

The use of a hydroxyapatite block for innominate osteotomy.

We used hydroxyapatite blocks to keep open Salter innominate osteotomies in 19 children (21 hips), avoiding the need for an autograft taken from the anterior superior iliac spine. Our results were satisfactory, with less deformation of the iliac bone, and a reduction of both blood loss and operating time.

Diode laser irradiation selectively diminishes slow component of axonal volleys to dorsal roots from the saphenous nerve in the rat.

Ga-Al-As laser irradiation (830 nm wavelength) inhibits the action potentials in the dorsal roots elicited from the saphenous nerve of the rat. Following laser irradiation to the saphenous nerve, the amplitude of slower conduction parts of action potentials (conduction velocity < 12 m/s) were suppressed. This suppression was irradiation time dependent. After 3 min irradiation, slowest conduction velocity group (< 1.3 m/s) were totally diminished and 1.3-12 m/s group were reduced to 12-67%. In contrast, faster component (> 12 m/s) was unaffected by laser irradiation. These findings suggest that laser irradiation may selectively target fibers conducting at slow velocities which include afferent axons from nociceptors.

MR imaging findings of tremors associated with lesions in cerebellar outflow tracts: report of two cases.

Two cases with severe tremors were studied by means of electromyograms using surface electrodes and also by magnetic resonance (MR) imaging. The first case was associated with multiple sclerosis and demonstrated a severe postural cerebellar tremor and an alternate activation of antagonist muscles in the right arm. The second case, with hemorrhage in the brainstem, demonstrated a severe tremor at rest and mixed synchronous and alternating activation of antagonist muscles in the left forearm. MR imaging studies localized lesions possibly responsible for these tremors. In the first case a lesion was located in the superior cerebellar peduncle just under the decussation, and in the second case a lesion was found between the red nucleus and the thalamus, with possible involvement of both the cerebellothalamic and nigrostriatal pathways. The first case accords with the theory that a lesion located in the dentate nucleus and its projection can cause severe postural cerebellar tremor. The lesion demonstrated in the second case may be responsible for "cerebellar tremor at rest" or "static (resting) cerebellar tremor."

Secretion of calf chymosin from the filamentous fungus Aspergillus oryzae.

Active calf chymosin was secreted from Aspergillus oryzae transformants when the chymosin cDNA was expressed under the control of glucoamylase gene (glaA) promoter. Secreted prochymosin was autocatalytically activated to the chymosin (0.07-0.16 mg/l). Western blot analysis showed that a secreted protein immunoreactive with an anti-chymosin antibody was of similar size to authentic chymosin. Northern blot analysis revealed that mRNA of the chymosin cDNA was expressed at as high level as that of the glaA gene. The size and the level of the transcript were different among transformants, due to the integration position of the plasmid on the chromosome.

MRI appearances of calcified regions within intracranial tumours.

The MRI appearances of calcification within intracranial tumours was assessed by reviewing MR images of 11 calcified tumours documented by CT. The signal intensity of the calcified regions was varied and nonspecific on both T1- and T2-weighted images. They were seen as signal void exclusively on T2-weighted images in only 2 patients. Gadolinium enhancement of the calcified portion occurred in 7 of 10 patients. These findings reflect the presence of tumour parenchyma within the calcified region, as proved in 5 lesions examined histologically.

Regularities and irregularities in the structure of the seminiferous epithelium in the domestic fowl (Gallus domesticus). I. Suggestion of the presence of the seminiferous epithelial cycle.

A cellular association demarcated by two perpendiculars which were drawn between adjacent bundles of elongate spermatids from the tubular lumen to the basement membrane, was made the unit of histometrical observation in this study (provisionally called a "column"). Cell counting revealed that the average numbers per column of various types of germ cells do not show any significant differences among 5 fowls and between paired testes. The frequency of spermiogenic steps (numbered 1-8) was investigated in each column. A definite and common pattern was found in the frequency distribution in the 5 fowls observed. A relationship between spermiation and younger spermatid steps was also investigated in each column. The spermiation was found at different steps, but most frequently at step 2 (30.6%). Based on these observations and referring to other author's information, an average time interval between two successive spermiations was calculated roughly at 3.3 +/- 1.2 days. Theoretically, this value is equal to an average length of one epithelial cycle. Such a variable cycle may have caused irregular cellular associations in this species.

Accommodative adaptation and age of onset of myopia.

Tonic accommodation in an empty field and accommodative adaptation, defined as accommodation shift after 2 min viewing of a 4 D near task, were measured using an infrared optometer. Subjects were classified into three types: emmetropes (n = 18), early onset myopes (n = 18) and late onset myopes (n = 15). The relationship between accommodative adaptation and the onset age of myopia were compared. There was no statistical difference in the pre-task tonic accommodation among the three subject groups. However, the accommodative adaptation differed in the three groups during the 5 min post-task period. The average accommodative adaptation of emmetropia, early onset myopia and late onset myopia were 0.66, 0.27 and 1.19 D, respectively. Accommodative adaptation in the late onset myopia group was significantly higher than in the emmetropia group. Furthermore, the accommodative adaptation in the early onset myopia group was significantly lower than in the emmetropia group. Two possible aetiologies of myopia are discussed: one for the group which shows low accommodative adaptation that has no relationship with the age of onset and another for the group which shows high accommodative adaptation that has a relationship to an onset age of 15 years or later. The aetiology is also considered with regard to a potentiating chemical effect on the synapses of the ciliary muscle.

Substrate specificity of alkaline proteases from Cephalosporium sp. KM388.

Serine alkaline proteases from Cephalosporium sp. KM388 were specific against esters of aromatic and hydrophobic amino acids. Against oxidized insulin B-chain, the enzymes initially cleaved the site of Leu-Tyr(15-16). The cleavage specificity of KM388 protease D was broader than those of other alkaline proteases, and the site of Arg-Gly(22-23) was cleaved, which is a specific site for trypsin-like protease.

Isolation and identification of mycoplasma strains from various species of wild rodents.

Attempts were made to isolate mycoplasmas from respiratory and urogenital tracts of 35 apparently healthy wild rodents comprising 7 species under 4 genera. Mycoplasmas were isolated from nasal and oral cavities, tracheas, vaginas and penises of the wild rats: ricefield rats (Rattus argentiventer), roof rats (R. rattus) and Polynesian rats (R. exulans), but none was isolated from brown rats (R. norvegicus), house mice (Mus musculus), smithi's voles (Eothenomys smithi) and soft-furred field rats (Millardia meltada). These mycoplasma strains were identified as Mycoplasma pulmonis and M. arthritidis on the basis of their biological and serological properties.

Biochemical polymorphisms in wild rats (Rattus norvegicus) captured in Oita city, Japan.

The investigation of biochemical markers in wild rats (Rattus norvegicus) is thought to be useful in the efficient establishment of inbred strains with a wide range of genetic variability of wild rats. In this study, 21 marker proteins were analyzed by electrophoresis in nine wild rats captured in Oita city. It was found that eleven marker samples in six animals were heterogeneous. We further found that the investigated wild rat population possessed the interesting polymorphisms of esterase-4 (ES-4) with high frequency, which are very rare in laboratory rats.

[A successful repair of tetralogy of Fallot with aortic valve regurgitation, coronary artery-pulmonary artery fistula and absent pulmonary valve in a 54-year-old man].

A 54-year-old man suffered from tetralogy of Fallot associated with aortic valve regurgitation, coronary artery-pulmonary artery fistula and absent pulmonary valve underwent intracardiac repair. The operation was completed with ligation of the fistula, aortic valve replacement using a SJM 27 A, patch closure of ventricular septal defect (perimembranous outlet type) and right ventricular outflow reconstruction using a large transannular monocusped-patch, 4 cm x 6 cm in size. Postoperative course was uneventful and he is doing well now. There was no previous reports about TOF associated with AR, the absent pulmonary valve and the coronary anomaly in the adults in the literature.

[Cranial MRI in hepatic disease--relationship between MR imaging and clinical symptoms and laboratory analysis of liver function].

Magnetic resonance imaging studies on 12 cases with hepatic disease were performed. In 11 adult patients with chronic hepatic failure, T1-weighted images demonstrated increased signal in the globus pallidus in 7 patients (63.6%) and in some parts of the cerebral peduncles in 6 patients (54.5%), in the substantia innominata in 4 patients (36.3%). There might be some correlation between the abnormality of MR imaging and clinical symptoms of hepatic encephalopathy. There was a significant correlation between the intensity of the signal and Fischer's ratio of amino acid analysis. In one patient with Wilson's disease, who developed symptoms of central nervous system, T1-weighted images demonstrated increased signal in the globus pallidus. After treatment of D-penicillamine, the signal of the globus pallidus decreased.

[Sudden exacerbation in atypical syringomyelia with a unilateral superficial sensory disorders as the principal manifestation].

We report a 22 year-old woman with syringomyelia, who complained of a sudden abnormal sensation in the left neck and upper extremity after maintaining her neck in flexion for sometime. Neurologic examination revealed superficial hypesthesia on the left from C2 down, but normal motor function. The mode of onset in our patient was atypical. The clinical manifestations of syringomyelia are usually slowly progressive. On the basis of X-P film of the cervical spine and cranial MRI, a diagnosis of syringomyelia with Chiari malformation (type 1) was made. The syrinx cavity extended from C1 to L1. On the transaxial image of the cervical cord, the syrinx cavity was demonstrated in the posterior horn area ipsilateral to the sensory disturbance. The CSF flow-void sign was present in the syrinx cavity, probably reflecting pulsation of the syrinx fluid. An abnormally high signal intensity area adjacent to the syrinx cavity on T2-weighted sequences indicated damaged cord. We speculate that dynamic factors produced by neck flexion and fluid pulsation explain the sudden exacerbation in our patient.
