[Recent advance in diagnostic imaging: NMR].

Application of NMR, whether in imaging or in spectroscopy, is evolving rapidly and its clinical feasibility is to be fully assessed. It is not clear if speculation today will hold true tomorrow, but here present status of NMR in medicine, especially in its roles in research of the central nervous system are discussed as follow; NMR imaging in brain tumors, cerebrovascular diseases, degenerative diseases, trauma, spinal cord diseases, Imaging by surface coil, Contrast material in NMR imaging, Blood flow measurement by NMR, Chemical shift imaging, Studies of metabolism by 31P NMR spectroscopy, Recent advances and possible future role of NMR in medicine.

Computed tomography of calcified gastric carcinoma.

The CT findings in a case of calcified gastric carcinoma of the mucin-producing type are described. These findings include localized thickening of the gastric wall and multiple tiny calcific nodules within it.

Successful surgical repair of left ventricular rupture after acute myocardial infarction.

A 61-year-old man with impending myocardial infarction was admitted and treated by percutaneous transluminal coronary recanalization (PTCR) therapy using Urokinase. Patient's symptoms subsided and his general condition maintained stable until the fifth hospital day, when he developed the signs of cardiac rupture. He was operated upon immediately, and the left ventricular rupture was confirmed and repaired successfully. Patient recovered from the surgery well. This report describes the rare successful surgical case of cardiac rupture secondary to acute myocardial infarction in Japan.

Association between the MB system and asthma in the Japanese.

Twenty-three unrelated Japanese patients with asthma who showed a high total serum IgE level, a strong skin test response to Dermatophagoides farinae, and a high score on a radioallergosorbent test (RAST) using Dermatophagoides farinae were typed for HLA-A locus, -B locus, and -D region antigens. No significant difference in the frequencies of HLA-A, -B, and -DR antigens were observed between the asthma patients and the healthy controls. A significant difference in the frequency of MB3', however, was found between the asthma patients and the healthy controls (corrected p = 0.04, relative risk = 18.5).

Mechanism of inducing autologous antibody formation in nephrotoxic serum nephritis.

The role of the heterologous antibody fixed in the kidney was evaluated by means of transplantation of a nephritic kidney to a normal recipient in the rat. An intravenous injection of 0.5 ml of nephrotoxic serum could not induce distinct changes in the heterologous phase, but it could cause mild or moderate changes in the autologous phase. Titration of rat antibody to rabbit IgG according to passive hemagglutination test showed that rabbit IgG fixed in the transplant could induce autologous antibody formation with a similar intensity to nephrotoxic serum given intravenously, indicating that fixed antigen played the most important role in autologous antibody formation and other factors, such as the kidney-fixing rabbit IgG remained in circulation or fixed in other tissues and the gamma-globulin other than anti-kidney antibody, might be less important. It was unclear which antibody, fixed or dissociated from the transplant, played the more important role in stimulating antibody formation.

Rapid restoration of injured kidney after immediate removal of opposite normal kidney. In comparison with delayed contralateral nephrectomy.

The difference in restoration of injured kidneys according to the time of contralateral nephrectomy was investigated in rats. When the opposite normal kidney was removed immediately or when both kidneys were rendered ischemic totally, the function of the injured kidney or kidneys was rapidly restored and showed almost normal level of blood urea nitrogen as well as normal histology one week after the ischemic damage. But delayed contralateral nephrectomy slowed down the speed of recovery, and the injured kidney did not function completely and showed severe inflammatory reaction in histology four weeks after the injury. It was discussed that retention of some factors like renal hypertrophy factor would cause rapid restoration of the damaged kidney.

[Clinical studies on 6059-S in the field of pediatrics (author's transl)].

6059-S was administered to 32 children with various acute bacterial infections (bronchopneumonia 11, bronchitis 1, pyelonephritis 5, acute enteritis 6, purulent infection 4, secondary infection due to agranulocytosis 5) at the dose of 21 to 190 mg/kg/day for 2 to 12 days. The clinical response of 6059-S was very satisfactory in all 17 cases with the injection of respiratory tract or urinary tract infection, but it was not so favourable in 5 cases of secondary infection due to agranulocytosis. The overall clinical response was excellent in 5, good in 20, fair in 4, and failure in 3 with effective rate of 78%. As to side effect, each one case diarrhea and elevation of GOT and GPT was noted.
