[A case study with bladder metastasis of renal cell carcinoma and stomach cancer].

A 64-year-old woman received nephrectomy and lymph expurgation surgery for renal cell carcinoma on Jury 1, 1981. The pathologic diagnosis was adenocarcinoma of the clear cell type at Robson's stage 2. She next visited the Department of Gastroenterology complaining of stomach discomfort on November 5, 1981. Stomach cancer of Borrmann's type IV was identified in the lesser gastric curvature, but only biopsy was performed because it was inoperable. The pathologic diagnosis was undifferentiated adenocarcinoma. On January 23, 1982, there was microscopic hematuria. A cystoscopic examination revealed one soy bean-sized, smooth, pedicle tumor to which coagula were partially adhered in the center of the triangular region. After TUR-Bt performed on March 3 the pathologic diagnosis was adenocarcinoma of the clear cell type with no submucosal infiltration. Based on these findings, the patient was diagnosed as having suffered metastasis of renal cell carcinoma to the bladder. She died of bleeding from stomach cancer on June 15. Based on the fact that the tumor was localized in the bladder mucosa, implantation through the urinary tract was strongly suspected as the metastatic route of the renal cell carcinoma to the bladder.

[Clinical studies on multiple lacunar state].

Sixty-five cases of clinically diagnosed multiple lacunar state have been analysed. The clinical course of multiple lacunar state is usually progressive in nature, simulating degenerative diseases, in contrast with the mode of acute onset seen in the other vascular diseases. Average age at the initial visit was 63.8 year-old, and sex ratio showed marked male preponderance, being 12 for males and 1 for females. Gait disturbance and speech disturbance are the most frequent initial symptoms, followed by slow motion, emotional lability and swallowing difficulty. Neurological manifestations are dysarthria, short-stepped and apraxic gait, hyperreflexia, positive Babinski and Chaddock reflexes, minimal spasticity, dementia, positive palmo-mental reflex, emotional lability, fixed face, rigidity, bradykinesia, foot grasping, dysphagia, positive Myerson's sign and tremor, in the order described. The important point is that the rigidity is paratonic and the tremor is action or postural, not the cogwheel rigidity or resting tremor like Parkinson's disease. The appearance of pathological reflexes (Babinski and Chaddock reflexes) are quite important, especially Chaddock reflex, which can frequently become positive despite negative or equivocal Babinski reflex. Hypertension, especially longstanding in nature, is the major contributing factor in this disorder. CT scan showed the presence of one or more lacunes in 52 out of 65 cases (80.0%). The detection of lacune can be influenced by the quality of CT scan, and the high resolution CT scanner is greatly useful for that purpose. It is stressed that the detailed neurological and computed tomographic evaluations will make it possible to reach the clinical diagnosis and appropriate treatment of multiple lacunar state.

Determination of the rate of rRNA synthesis in Xenopus laevis triploid embryos produced by low-temperature treatment.

Triploid embryos of Xenopus laevis were obtained by cold-temperature shocking of the fertilized eggs, and the rate of the ribosomal RNA (rRNA) synthesis was determined for comparison with that in diploid embryos. For this purpose, both triploid and diploid embryos were dissociated into cells at the neurula stage, and then labeled with (3H)uridine for varying lengths of time. The rate of rRNA synthesis, as estimated after determination of (3H)UTP specific radioactivity and the total label incorporation into the purified rRNA, was about 0.1 pg/cell/hr for both diploid and triploid embryo cells. Nuclei of triploid embryo cells contained three nucleoli of apparently similar sizes--an indication of the functioning of all the three rRNA gene clusters to a more or less similar extent. Also, rates of synthesis of 4S RNA and 5S RNA were determined: Both rates did not change appreciably between triploid and diploid embryo cells. Based on these results, it appears that transcription of these redundant genes occurs at a constant rate on a per cell basis irrespective of the presence of 1.5 times as many genes as the control.

Combination chemotherapy for advanced bladder cancer with adriamycin, cyclophosphamide, and 5-fluorouracil.

Combination three-drug chemotherapy with adriamycin (ADM), cyclophosphamide (CPM), and 5-fluorouracil (5-FU) was performed in 24 cases of advanced bladder cancer who underwent surgical treatment, and three cases with recurrent or metastatic bladder cancer. The average age (25 men and 2 women) was 53. Of the 24 cases, nine were in stage T2, 10 in T3, and five in T4. One course consisted of a combination of 30 mg/m2 of ADM, 300 mg/m2 of CPM, and, 250 mg of 5-FU, administered five times. The combination was administered to three groups: every day for 5 days consecutively in group A, twice a week for 21/2 weeks in group B, and once every 4 weeks for 16 weeks in group C. After injection of ADM, CPM, and 5-FU, 200 mg/day of 5-FU was administered PO daily in all three groups. The 5-year survival rate of the 24 cases (apart from 3 cases with measurable metastatic tumor) was 58%. The 5-year survival rate for stage T2 was 88%, and that for stage T3 was 62.5%; all patients with stage T4 disease died before 3 years and 6 months. Partial response was seen in two out of three patients with recurrent or metastatic disease. Alopecia was observed in all cases as a side-effect of the chemotherapy. Also anorexia, nausea, and myelosuppression were observed in many cases, though the degree was tolerable. However, there were no disorders of the cardiovascular system, except for one case with transient hypotension.

Carbonic anhydrase-III immunohistochemical localization in human skeletal muscle.

Using the indirect immunoperoxidase method, we studied the localization of carbonic anhydrase-III (CA-III) in frozen sections of biopsies of human skeletal muscle which had no definite pathology. CA-III was found to be localized in Type-I muscle fibers when compared with serial sections stained with myosin ATPase and other reactions. Our finding was in accordance with the biochemical data so far reported. It was though that CA-III could be used as a marker for abnormal Type-I muscle fibers in several neuromuscular diseases.

Contribution of maternal mRNA for maintenance of Ca2+-dependent reaggregating activity in dissociated cells of Xenopus laevis embryos.

Dissociated Xenopus laevis blastula cells, where reaggregation was inhibited in Ca2+-free medium, reaggregated immediately after the addition of Ca2+. This reaggregation was not inhibited by cordycepin or actinomycin D treatment during culture, although cycloheximide and puromycin were inhibitory. The reaggregation was not inhibited even when fertilized eggs were microinjected with cordycepin and their RNA synthesis was continuously inhibited through cleavage to blastula stages. In neurula cells, cordycepin treatment induced significant reduction in sizes of aggregates formed. These results suggest that the Ca2+-dependent reaggregating activity of blastula cells is maintained by the translation of maternal, rather than newly synthesized, mRNA.

Evaluation and management in consultation-liaison psychiatry.

The subjects were 95 medical and surgical inpatients referred to Kyushu University Hospital Psychiatric Consultation Service. The authors studied the mental status of each referred patient and elicited some Unclassified mental status cases. It was recommended, therefore, to bring in new concepts of normal condition and polymorphous condition in dealing with those Unclassified mental status cases which are often recognized as problematic both by the patient's primary physician and the psychiatric consultant. The ordinary style Single contact consultation was also compared with the Active follow-up consultation regarding the function of psychiatric consultations. The authors confirmed that most referring physicians who were supported by the Active follow-up consultation appreciated highly the evaluating function of a psychiatric consultant.

Calcification of the ligamentum flavum of the cervical spine. Report of four cases.

Four cases of calcification of the cervical ligamentum flavum are reported, all in women over 60 years of age. Neurological findings were not significantly different from those of other cervical compressive diseases. Among radiological examinations, computerized tomography was the most valuable diagnostic tool. Calcification might have been induced by the degeneration or abnormal nutritional state of the ligamentum flavum. Endocrine abnormalities and inflammatory processes might also have been contributory factors.

Pyrrolomycins F1, F2a, F2b and F3, new metabolites produced by the addition of bromide to the fermentation.

New antibiotics, pyrrolomycins F1, F2a, F2b and F3 were produced by Actinosporangium vitaminophilum sp. nov. when bromide ion was added to the fermentation medium. Addition of other halide ions such as chloride, iodide and fluoride ion showed no effect on pyrrolomycin production, affording polychlorinated pyrrolomycins A, B, C, D and E, but no F components. All four new antibiotics contain 2 approximately 4 mol of bromine, and their substitutional position was determined by X-ray analysis and synthesis, supported by spectroscopic analysis. They are strongly active against Gram-positive bacteria and fungi.

The mediation of tissue eosinophilia in hypersensitivity reactions. V. Comparative study of tissue eosinophilia in the skin lesions of local and systemic passive cutaneous anaphylactic reactions.

Tissue eosinophilia in active cutaneous anaphylaxis reactions is biphasic: the early phase (6 hr) is induced by a low molecular (mol. wt. 300) factor (early ECF), and the delayed phase (24 hr) is mediated by synergy of two different factors (delayed ECF-a and -b). In this study, we report the mediation of tissue eosinophilia in passive cutaneous anaphylactic (PCA) reaction sites. Tissue eosinophilia in systemic PCA showed two phases with peaks at 6 hr and 24 hr, while that in local PCA was monophasic and peaked at 12 hr. A dialysable eosinophil chemotactic factor was isolated from the early stage (0-6 hr) of local PCA skin sites, and another chemotactic factor (mol. wt. 15,000), sharing a common antigenicity with guinea-pig serum C5, from 12-hr-old local PCA skin sites. On the other hand, a different chemotactic factor with a mol. wt. of about 70,000, sharing a common antigenicity with delayed ECF-b isolated from active cutaneous anaphylactic skin lesions, was isolated from 24-hr-old systemic PCA skin lesions. Although the dialysable factor was also isolated from systemic PCA skin sites, the factor from systemic PCA delayed skin sites may not contribute to delayed tissue eosinophilia, since the activity paralleled the intensity of basophil accumulation but not to that of eosinophils. It is thus suggested that tissue eosinophilia in systemic and local PCA reactions is mediated by different chemotactic factors.
