False triggering with a ventricular triggered pacemaker and false inhibition with a ventricular inhibited pacemaker were observed in two patients with complete heart block. These arrhythmias were considered to be attributable to false sensing with a demand pacemaker. Both in vitro and in vivo studies were performed to elucidate the mechanism of these false sensings, and the false sensings were reproduced in a series of experiments. Loose contact of the tip of electrodes to myocardial tissue, refractory period, the distance between two electrodes, and the after potential and its dv/dt of pacemaker impulses are considered to affect the performance of demand pacers.

In a series of 79 cases of primary esophageal carcinoma resected at The Center for Adult Diseases, Osaka, there were six tumors with specific histopathologic features valid for the diagnosis of argyrophil cell carcinoma. Of the 6 tumors, 3 were studied electron microscopically and assay for ACTH content was performed on 4 tumors. Clinically, the ages of the 6 patients ranged from 56 to 71 years; two were women and four men. Four of the 6 patients died with widespread tumor recurrences within 9 months of operation. Microscopically, the 6 tumors were composed largely or almost entirely of small, spindle-shaped cells resembling those of oat cell carcinoma of the lung, and were characterized by the arrangement of tumor cells in solid sheets or anastomosing cords, the presence of argyrophil tumor cells, and the deposits of amyloid. Electron microscopically, the three tumors contained neurosecretory-type granules. Using bioassay or radioimmunoassay ACTH activity in the tumor tissues was detected in 3 out of the 4 tumors determined. From the light and electron microscopic characteristics and the assay evidence, it seems reasonable to conclude that the 6 tumors are endocrine polypeptide producing tumors (apudomas) that arise from argyrophil cells normally found among the basal cells of the esophageal mucosa, and that they represent a distinct histopathologic entity clearly distinguishable from other types of esophageal carcinomas.
A case of papilloma of the gallbladder is reported. The gallbladder was associated with cholesterosis but not with stones. The causes of colicky pain in this case are discussed. Papillomas of the gallbladder exhibit no characteristic clinical picture; the symptoms are similar to those of any type of gallbladder disease. Papillomas of the gallbladder may be associated with malignant degeneration. Cholecystectomy is the treatment of choice.

The new "Occupational Safety and Health Act" was established in October 1972. According to the Act, the employers with 50 and more employees were bound to make an occupational health doctor provide health care for the employees within the enterprises. Considering the national background mentioned above, the author examined the actual status of the medical and health care services in order to discuss the professional function of an occupational health doctor by reviewing the records concerning the workers' health at a certain paper factory managed by the National Government, under the Government Health Insurance scheme. The results were as follows: 1) Aging of the workers had an important impact on medical service and health care provided. 2) The medical service by the occupational doctor attached to the factory differed much from that by other medical institutions outside the factory in several points; particularly in days of receiving medical consultation, medical expenditure expressed in "point score," number of injections given and the frequency of laboratory examinations. 3) as a whole, the occupational health doctor was taking a role as "the first contact doctor" in primary medical care, as well as a health officer preventing disease and promoting health in the factory, whereas the medical institutions outside the factory were rather clinically oriented in treating ill-health.
Cytotoxicity against tumor-specific antigen and allogeneic antigens in mice was raised by immunization with viable tumor cells in saline, but not by immunization with tumor cells in complete or incomplete Freund's adjuvant. On the other hand, strong delayed hypersensitivity as demonstrated by macrophage migration inhibition was detected in the mice immunized with tumor cells in complete or incomplete Freund's adjuvant in both syngeneic and allogeneic systems. After immunization with viable tumor cells, delayed hypersensitivity was raised to a moderate degree in allogeneic, but not at all in syngeneic mice. Cytotoxicity was retained after a booster with syngeneic tumor cells in complete Freund's adjuvant in the mice which had been immunized with viable tumor cells. Cytotoxicity was raised after a booster with viable syngeneic tumor cells in the mice which had been immunized with tumor cells in complete or incomplete Freund's adjuvant. Thus, the development of cytotoxicity depended entirely on the presence of viable tumor cells as an immunogen. Activation of helper T cells was not found in the syngeneic mice immunized with viable tumor cells or tumor cells in complete or incomplete Freund's adjuvant. In allogeneic mice, the activity was facilitated slightly by immunization with viable tumor cells or tumor cells in complete Fruend's adjuvant but not with tumor cells in incomplete Freund's adjuvant. This sytem may be useful for analytical studies on cellular immunity, since cytotoxicity, delayed hypersensitivity, and helper cell activation can be induced in dissociated forms.

Radioactive adenosine triphosphate was synthesized transiently from adenosine diphosphate and radioactive inorganic phosphate by sodium and potassium adenosine triphosphatase from guinea pig kidney. In a first step, K+-sensitive phosphoenzyme was formed from radioactive inorganic phosphate in the presence of magnesium ion and 16 mM sodium ion. In a second step the addition to the phosphoenzyme of adenosine diphosphate with a higher concentration of sodium ion produced adenosine triphosphate. Recovery of adenosine triphosphate from the phosphoenzyme was 10 to 100% in the presence of 96 to 1200 mM sodium ion, respectively. Potassium ion (16mM) inhibited synthesis if added before or simultaneously with the high concentration of sodium ion but had no effect afterward. The half-maximal concentration for adenosine diphosphate was about 12 muM. Ouabain inhibited synthesis. The ionophore gramicidin had no significant effect on the level of phosphoenzyme nor on the rate nor on the extent of synthesis of adenosine triphosphate. The detergent Lubrol WX reduced the rate of phosphoenzyme break-down and the rate of synthesis but did not affect the final recovery. Phospholipase A treatment inhibited synthesis. In a steady state, the enzyme catalzyed a slow ouabain-sensitive incorporation or inorganic phosphate into adenosine triphosphate. These results and other suggest that binding of sodium ion to a low affinity site on phosphoenzyme formed from inorganic phosphate is sufficient to induce a conformational change in the active center which permits transfer of the phosphate group to adenosine diphosphate.
The sodium and potassium ion pump is an intrinsic enzyme of plasma membranes. In these experiments it was driven backward in a transient two-step operation involving, first, phosphorylation of the enzyme from inorganic phosphate, and second, transfer of the phosphate group from the enzyme to ADP upon addition of a high concentration of Na+. There was no evidence of a significant concentration gradient across the membranes. Na+ presumably reached the solutions on both faces of the membrane simultaneously and provided the energy for synthesis simply as a consequence of ligand binding. An interaction free energy between the free energy of the binding of Na+ and the free energy of hydrolysis of the phosphate group on the enzyme was estimated. The experiments also suggested a feature of the transport mechanism. This is control by phosphorylation of access pathways from the solutions in contact with the faces of the membrane to an active center for cation binding. In the dephosphoenzyme access would be to the intracellular solution and in the phosphoenzyme access would be to the extracellular solution.
