[Fine structure of the vomeronasal organ in the house musk shrew (Suncus murinus)].

Fine structure of the vomeronasal organ (VNO) was examined in an insectivore, house musk shrew (Suncus murinus), from a viewpoint of comparative anatomy. The VNO was a pair of tubular structure, about 6mm in length, and situated at the base of the nasal septum. The medial wall of the lumen of the VNO was lined with the sensory epithelium (SE) consisting of sensory, supporting and basal cells, while the respiratory epithelium (RE) lining the lateral wall was pseudostratified and covered with microvilli. Jacobson's glands were distributed on the dorsolateral to ventrolateral side of the VNO and opened to the lumen in the transitional region from the RE to the SE. Their secretion was PAS-positive bu talcian blue-negative. Ultrastructurally, sensory cells of the SE were bipolar neurons and covered on their free surface with conspicuously long microvilli. The other ultrastructural features in sensory, supporting and basal cells were similar to those in previously reported species. The ultrastructural features of the RE were also similar to those in previous reports except that the free surface was covered with microvilli instead of cilia. In addition, a few kinds of migrating cells were often observed in both the SE and the RE. Acinar cells of Jacobson's glands possessed a round to elliptical nucleus and many large secretory granules, about 1,000 nm in diameter. Rough endoplasmic reticulum and Golgi apparatus were well developed in their cytoplasm.(ABSTRACT TRUNCATED AT 250 WORDS)

[Lectin histochemical studies on the musk gland in the house musk shrew (Suncus murinus)].

The musk gland of the adult house musk shrews (Suncus murinus) of both sexes was studied lectin histochemically. The musk gland was a kind of scent gland, consisted of congregation of branched or unbranched simple tubuloalveolar gland holocrine in nature and was attached by an apocrine gland-like structure (sweat gland) in the deeper layer of its periphery. Acinar cells of the musk gland were distinguishable into three type from basal to luminal parts of the acinus; immature cells, mature cells and degenerating cells. There was no histological difference between both sexes. Lectin-binding pattern of the musk gland was examined in comparison with that of the sweat gland and ordinary sebaceous gland by histochemical staining techniques using seven lectins: ConA, RCA I, PNA, SBA, UEA-I, DBA, WGA, WGA and PNA labelled the duct of the musk gland more intense than the acinus. Several lectins showed a tendency to label the cells situated near the luminal surface more intense than those near the basement membrane in both the acinus and duct of the musk gland. In the sweat gland and ordinary sebaceous gland, the lectin-binding pattern was different with each other and from that in the musk gland. These findings suggest that the musk gland, sweat gland, and ordinary sebaceous gland are different to each other in nature of cells and the secretion.

[Ontogeny of the pronephros and mesonephros in the South African clawed frog, Xenopus laevis Daudin, with special reference to the appearance and movement of the renin-immunopositive cells].

Development of the pronephros and mesonephros of Xenopus laevis was morphologically studied in the larvae immediately after hatch (stage 35/36) to the end of metamorphosis (stage 66) together with the appearance of renin-immunopositive cells. The pronephros consisted of 3 pairs of pronephric tubules at stage 35/36 and became functional at stage 37/38 with the opening of the pronephric ducts to the cloaca. It increased thereafter in volume, but became degenerative at stage 56, lost a part of the pronephric ducts at stage 60, and completely disappeared at stage 64. The mesonephros displayed its anlage at stage 42, and gained the immature renal corpuscles at stage 48. At stage 49, it was equipped with the matured nephrons. The mesonephric tubules differentiated into the neck, proximal, intermediate and distal parts. The collecting tubules and ducts were also observed at this stage. At stage 56, a part of the mesonephric tubules degenerated and were displaced by newly formed tubules. Reorganization of the mesonephros was not completed at the end of metamorphosis. Renin-immunopositive cells did not appear in the pronephros, but were observed at the diverging part of the renal artery from the dorsal aorta, and in the walls of arteries and afferent arterioles within the mesonephros in a low frequency of appearance.

Continuous record of carotid artery pulse during exercise.

The fixing apparatus for a pulse transducer was improved to record continuously and stably the carotid artery pulse (CAP) during exercise on a bicycle ergometer. The CAP changed largely at the onset and at the end of exercise, oscillating during exercise. The contour of the CAP at exercise was different from that at rest in several respects. During exercise, the incisura were lower, and/or the tidal wave occasionally exceeded the percussion wave. The present method proved to be applicable to exercise on both supine and upright bicycle ergometers with a heart rate lower than approximately 160 beats/min.

[Preoperative cystometric analysis of benign prostatic hyperplasia--special reference to the natural history and postoperative outcomes].

Four hundred and thirty-seven patients with benign prostatic hyperplasia selected for elective prostatectomy underwent preoperative cystometry. Of these patients 263 (60.2%) had bladder instability, who tended to be more irritative and more obstructive in their preoperative objective assessment than the others. Forty-seven patients (12.5%) revealed vesical denervation supersensitivity, of these patients all but one had bladder instability. Significant correlation was found between occurrence of the bladder dysfunction and the severity of obstruction. The occurrence of both bladder dysfunction developed with age. Four hundred and fourteen patients (94.7%) had successful outcome in voiding postoperatively, but elderly patients with low bladder compliance and severe emptying failure tended to need several months in their recovery. Postoperative incontinence was found in 100 patients (22.9%) with high incidence of preoperative bladder instability in the early phase, but was reduced in most cases 6 months later. Eighteen prolonged incontinent cases showed high incidence of low bladder capacity and low compliance with instability at preoperative cystometry. In these cases, the incidence of elderly patients was higher than that of dry cases. It is concluded that most of these bladder dysfunction is secondary to the infravesical obstruction which depends upon severity and duration of the disease, but aging also appears to play a role in its appearance. We assume that detrusor instability represents a kind of compensation for infravesical obstruction, and that the development of denervation supersensitivity indicates that the bladder damage is severely progressing.

[Projects in surgery for lung transplantation].

Lung transplantation was simulated in 9 essential items in the surgical side. The possibility to utilize cardiac death donor instead of cerebral death donor was discussed. The transplant window was shown in a patient with primary pulmonary hypertension. The informed consent must be authorized at the specific meeting. For the pre- and postoperative care, it is essential for surgeon to treat on the team which is consisted of members from the specific regions. With donation, a method of the standardization for graft preservation was shown by statistical analysis of many factors. The level of surgical techniques in Japan have been evaluated to be on the current level. In postoperative care, reimplantation response, infection, and rejection are important factors which have been clinically overcome in preceding institutions. However, a practical method of established diagnosis for acute rejection remain unresolved. In addition, CMV infection will be an unavoidable problem in Japan. The research of immune tolerance has been successfully underwent in experiments to overcome chronic rejection.

[Intranasal administration of nitroglycerin solution and nitroglycerin spray during general anesthesia].

Nitroglycerin spray is a newly developed nitroglycerin (TNG) form, which delivers 0.3 mg of TNG with each spray emission. We compared the efficacy of intranasal administration of TNG spray and that of TNG solution during general anesthesia. Nineteen patients were randomly assigned to receive either 0.3 mg TNG solution (Solution group) or 0.3 mg TNG spray (Spray group) intranasally. Peak elevation of plasma TNG occurred 2 min after drug administration in both groups, and the plasma level decreased to an ineffective level after 10 min. The plasma TNG of the spray group was significantly higher than that of the solution group for 3 min. Two min after drug administration, systolic blood pressure decreased significantly in both groups, and this level persisted for 10 min. Diastolic blood pressure also decreased in the spray group, but not in the solution group. Heart rate increased significantly in both groups 2 min after drug administration. Oxygen index (PaO2/FIO2) decreased significantly in both groups, and the level persisted 10 min after administration. Although no significant differences were found between two nitroglycerin forms in any of these hemodynamic and respiratory parameters, nitroglycerin spray may have clinical advantages because of reliability in its effect, ready availability, ease of application and high stability.

[A new application of immunoconjugate to reduce the local recurrence of colorectal cancer].

To examine whether or not immunoconjugate-A7-NCS can contribute for the reduction of local recurrence of colorectal cancer, the present study was undertaken. The study included examination of local retension, lymphatic delivery and inhibitory effect on tumor development after local administration of A7-NCS. The result showed that locally injected A7-NCS showed a high local retension, a high regional lymph node accumulation and a high inhibitory effect on tumor development. This finding indicates that A7-NCS can be a new promising tool for reducing the local recurrence of colorectal cancer.

[Assessment of the background count to measure the left ventricular ejection fraction with a nuclear stethoscope].

The nuclear stethoscope is a newly developed device for monitoring left ventricular ejection fraction (EF) with a pair of cadmium telluride detector for both left ventricular count (first channel) and background count (second channel). Although it is useful for evaluating the cardiac function during exercise, the methodology of the calculation for distinguishing the net ventricular blood count from the background count has not been established yet. In order to establish a reasonable method to calculate EF, we measured EF using a nuclear stethoscope and conventional gated blood pool scintigraphy in 20 healthy volunteers. All subjects underwent two supine ergometer exercise tests consisting of the 3 stages for the nuclear stethoscope and blood pool scintigraphy. The EF was determined with the following three method for the nuclear stethoscope; 1) Cut-off level was fixed at 70% of first channel, 2) Cut-off level at 75%, 3) Cut-off level was determined by the background count obtained from second channel. There was a poor relationship between the EFs obtained from gated blood scintigraphy and the EFs from the nuclear stethoscope calculated by any of these method. Regarding the EF calculated using the background count, however, the delta values of EF between rest and any of the 3 stages during exercise correlated closely to those EF from blood scintigraphy. When we apply the EF-at-rest value obtained from blood pool scintigraphy to analysis with the nuclear stethoscope, the EFs of 3 stages indicated good correlation (Stage I r = 0.91, Stage II r = 0.82, Stage III r = 0.69).(ABSTRACT TRUNCATED AT 250 WORDS)

[Anesthetic management of a patient for giant pulmonary bulla drainage].

Recently it is claimed that the drainage procedure may be a useful method for giant pulmonary bulla in compromised patients. We reported the anesthetic managements of three patients with giant bulla for these procedures. We chose epidural anesthesia in order to prepare for the postoperative analgesia and the expansion of surgical procedure. In selecting circulatory support method, care was taken to choose dobutamine that would cause no suppression of hypoxic ventilatory drive. There is no doubt that right ventricular function is particularly important in patients with chronic obstructive lung disease. We measured right ventricular ejection fraction by thermodilution method and end-tidal CO2 by nasal cannula. These measurements may be useful methods for perioperative monitoring.

[Anesthetic consideration of a patient with congenital insensitivity to pain with anhidrosis].

Congenital insensitivity to pain with anhidrosis is a rare disorder but several clinical anesthetic experiences have been reported in Japan. We anesthetized a patient with this disorder for orthopedic operation with a low concentration of sevoflurane under monitoring of body temperature and we measured the concentrations of plasma ADH, cortisol and catecholamines levels before, during and after the operation. There were no problems during anesthesia and the patient awoke rapidly. ADH, cortisol and epinephrine concentrations showed slight elevations during and after the operation. The management of intraoperative body temperature and the necessary level of anesthesia were also discussed.

[Continuous intra-hepatic-arterial infusion of low dose 5-fluorouracil for colorectal cancer patients with unresectable liver metastases].

Five cases of colorectal cancer with unresectable liver metastases treated from April 1992 to April 1993 in Osaka National Hospital were summarized in this paper. A silicone catheter was placed in the hepatic artery through the gastroduodenal artery by operative procedure and connected to a subcutaneously implanted reservoir. 5-FU was administered ambulatorily using Baxter Infusor (multi day type) according to a regimen of 5-day continuous infusion and subsequent 2-day rest. The patients were 4 men and 1 woman, and from 51 to 65 years old (average: 62.4 y.o.). According to criteria for antitumor effectiveness by CT scan, one patient was judged CR, two were PR, and one was PD. One case could not be estimated because of catheter obstruction. The total efficacy rate was 75%. The serum CEA level was reduced in 3 patients. As for complication, obstruction of catheter, damage to reservoir and segmental necrosis of liver were observed in 3 patients. In conclusion, our ambulatory therapy for colorectal cancer patients with liver metastases was considered to have a high potential of not only effectiveness for cancer lesion but also the improvement of patients' quality of life.

[Intra-arterial infusion chemotherapy for recurrent breast cancer via an implantable system].

Intra-arterial infusion chemotherapy via implantable port catheter has been applied in 14 patients suffering from recurrent breast cancer. The regimen consisted of at least 2 cycles of epirubicin (1 cycle: 150 mg) and subsequent maintenance infusion (30 mg/2 weeks). The results were as follows: 1) The response rate (CR+PR) was 50%: 42.9% CR, 7.1% PR, 14.3% MR, and the median duration of response was 6 months. 2) Intractable pain, severe lymphedema and infection were reduced in 42.9% (6/14). 3) Leucopenia was the dose-limiting factor, and it appeared with an incidence of 78.8%. Catheter or portal trouble was observed in 28.6% of the patients. Despite the existence of several unanswered questions, intra-arterial infusion chemotherapy via implantable system is promising with regard to therapeutic effect and quality of life.

[A clinical study on lomefloxacin to find appropriate dose levels in the treatment of respiratory tract infections].

A comparative clinical study of lomefloxacin hydrochloride (LFLX) was performed to confirm the optimal dose of LFLX for treatment of respiratory tract infections. Daily dosages of LFLX, 400 mg (200 mg b.i.d.) and 600 mg (200 mg t.i.d.) were allocated by the randomization method. Treatment period was 7-14 days. Of a total of 136 cases, 14 cases were excluded from evaluation of clinical efficacy and 4 cases from evaluation of safety. There were no differences in the background factors of the patients between the 2 dosage groups. The clinical efficacy rates, judged by a committee, were 76.4% (42/55) for the 400 mg group and 74.6% (50/67) for the 600 mg group. No significant difference was observed between the 2 groups (P = 1.000). Incidences of adverse effects were 1.6% (1/64) for the 400 mg group and 2.9% (2/68) for the 600 mg group. Incidences of abnormal laboratory test values were 3.4% (2/58) for the 400 mg group and 1.6% (1/63) for the 600 mg group. No significant differences were found between the 2 groups. These adverse effects and abnormal laboratory test values improved with the cessation of the administration of the drug. From these results, a dose of 400 mg (200 mg b.i.d.) LFLX seems to be better than that of 600 mg (200 mg t.i.d.) LFLX in the treatment of respiratory tract infections.

[Cortisol responses to rapid ACTH test in patients with multiple organ failure].

Cortisol concentrations are usually elevated in sepsis, during major surgery and in burns. Adrenocortical response to stress seems to be essential for survival. To investigate the endogenous adrenocortical response to multiple organ failure, rapid ACTH stimulation tests were used in 15 patients. Patients were divided to nonsurvival group and survival group. Patients in both groups appeared to have increased adrenocortical activity, because basal cortisol concentrations were above normal (non-survival group: 33.8 +/- 13.7 micrograms.dl-1, survival group: 19.1 +/- 3.8). There are no significant differences between the two groups. We could not find absolutely adrenal insufficient patient. Plasma cortisol concentrations increased significantly in patients of both groups following ACTH stimulation. We found poor response to ACTH in two patients in non-survival group with very high basal cortisol concentrations. But there is no significant difference of cortisol response between the two groups. We could not determine whether the high mortality is associated with poor response to ACTH or not.
