Nucleotide sequence of VP4 and VP7 genes of a unique human rotavirus strain Mc35 with subgroup I and serotype 10 specificity.

The VP7 and VP4 genome segments of human rotavirus strain Mc35 (Urasawa et al., J. Infect. Dis. 166, 227-234, 1992) were investigated. Analysis of its VP7 nucleotide and amino acid sequences confirmed that it belongs to G (or VP7) serotype 10; however, the nucleotide sequence homologies between Mc35 and the four G10 bovine strains examined were lower than those among the four G10 bovine strains. The VP4 nucleotide and amino acid sequences of Mc35 were distinct (66.1-68.8% nucleotide and 64.9-70.0% amino acid homologies) from those of previously reported human and animal rotavirus strains representing different P (or VP4) types except for the strain K8 VP4, which was designated as type 3 of human rotavirus VP4 by Gorziglia et al. (Proc. Natl. Acad. Sci. USA., 87, 7155-7159, 1990). The VP4s of the K8 and Mc35 strains showed a relatively high degree of homology (77.6% nucleotide and 86.0% amino acid sequence homologies) and were presumed to represent subtypes of type 3 human rotavirus VP4 (type 3A (K8 VP4) and type 3B (Mc35 VP4)).

Hemodynamic and oxygen delivery-consumption changes during partial liver resection.

The effects of partial liver resection on hemodynamics and the oxygen delivery-consumption relationship were evaluated in ten patients with hepatocellular carcinoma. The cardiac index and oxygen delivery were increased significantly (P < 0.05) at 30 minutes after incision, 30 min after liver resection and in the recovery room. Oxygen delivery decreased significantly (P < 0.05) during liver resection. Oxygen consumption remained low throughout the procedure. We did not discover any flow-dependent change in oxygen consumption. Although our patients persisted a hyperdynamic state throughout surgery, their arterial ketone body ratio remained low. Therefore, it may be necessary to maintain a hyperdynamic state during partial liver resection in order to increase hepatic blood flow.

Efficient production of antigenic mosaic reassortants of rotavirus with the aid of anti-VP4 and anti-VP7 neutralizing monoclonal antibodies.

Anti-VP4 and anti-VP7 neutralizing monoclonal antibodies (N-MAbs) were used to select efficiently antigenic mosaic reassortants of rotavirus whose VP4 and VP7 are derived from parental strains having different antigenic specificities of VP4 and VP7. Under the selective pressure of anti-simian rotavirus (SA11) VP7 N-MAb and anti-human rotavirus (Wa or S2) VP4 N-MAb, all the clones isolated after mixed infection with SA11 and human rotavirus were found to be reassortants having VP4 of SA11 and VP7 of Wa or S2 by serological and genomic analyses. Two reassortants isolated from the mixed infection with SA11 and S2 had a segment whose different migration in polyacrylamide gel was different from those of any RNA segments of the parent strains, suggesting the occurrence of rearrangement in the process of reassortment.

Clinical applications of ICG Finger Monitor in patients with liver disease.

The indocyanine green (ICG) Finger Monitor system is a non-invasive indication of ICG concentrations in the blood. In this study, significant correlation was found between the sensor signal voltage and plasma ICG concentrations ranging from 0.04 mg/dl to 1.0 mg/dl (r = 0.998, P < 0.001) in vivo. The ICG clearance curve showed an initial sharp rise 20-30 s after bolus injection, followed by a small rise. The concentration then deceased exponentially. In 196 patients with chronic liver disease, there was a close correlation between the KICG (plasma disappearance rate) and R15 (blood retention ratio at 15 min) (r = 0.886, P < 0.001, r = 0.912, P < 0.001) and corresponding values calculated by the conventional ICG method. In 263 cases with chronic liver disease, the plasma disappearance rates calculated using this monitor (mean +/- S.D.) were 0.156 +/- 0.064 (n = 20) in the control group, 0.129 +/- 0.060 (n = 92) in the chronic hepatitis group, 0.048 +/- 0.025 (n = 59) in the cirrhosis group and 0.059 +/- 0.03, (n = 92) in the group with hepatocellular carcinoma. A significant difference in the plasma disappearance rate and blood retention ratios 15 min after injection of ICG using this system was observed between control cases and the chronic hepatitis and cirrhosis groups (P < 0.0001). In 36 cases, the time from injection to the appearance of ICG in the fingertip significantly decreased in the cirrhosis group (P < 0.01). The ICG Finger Monitor system was shown to be useful clinically as well as for research due to its accuracy and non-invasive nature.

Distinct yearly change of serotype distribution of human rotavirus in Thailand as determined by ELISA and PCR.

A total of 241 group A rotavirus-positive stool samples collected from diarrhoeic patients in Thailand between July 1988 and June 1991 were characterized for their serotypes by enzyme-linked immunosorbent assay (ELISA) using serotype-specific monoclonal antibodies and by a polymerase chain reaction (PCR). In July 1988-June 1989, serotype 1 was the most prevalent (63.4%), followed by serotype 4 (11.0%) and serotype 2 (8.5%). In July 1989-June 1990, 59.8% were serotype 1, 24.3% were serotype 2, and 6.1% were serotype 3. In contrast, in July 1990-June 1991, serotype 3 was detected in the highest frequency (40.5%), 29.9% were serotype 1, and 27.3% were serotype 2. Thus, a distinct yearly change of serotype distribution of rotavirus in Thailand was observed in the three consecutive years. In particular, it was of note that the prevalence of serotype 3 greatly increased, in contrast to the previous studies in which almost no serotype 3 rotaviruses were detected in the years 1983-8 in Thailand.

Short-term effects of denopamine on anaerobic threshold and related parameters in patients with chronic heart failure: a double-blind crossover study.

BACKGROUND: The short-term effects of denopamine, an orally available beta-stimulant, on exercise capacity were studied in patients with chronic heart failure. METHODS AND RESULTS: Nineteen patients entered the study. Three patients had ischemic heart disease, 13 had dilated cardiomyopathy, and three had valvular disease; 16 patients were in New York Heart Association class II, and three patients were in New York Heart Association class III. Symptom-limited exercise testing (ramp protocol) on a bicycle ergometer with gas exchange analysis was conducted 1 hour after oral administration of either 20 mg denopamine or placebo. Drug administration sequence was randomly assigned in a double-blind crossover method, with 1 week between drugs. Peak VO2 was 20.4 +/- 3.2 and 21.2 +/- 3.1 ml/min/kg, respectively, for those administered the placebo and the drug, and anaerobic threshold was 13.1 +/- 2.1 and 14.0 +/- 2.0 ml/min/kg. There was a significant increase in peak VO2 (p < 0.05) and anaerobic threshold (p < 0.01) with denopamine, whereas no significant change was observed in peak work rate or exercise time. Denopamine increased heart rate in patients with atrial fibrillation but had little effect on heart rate in patients with sinus rhythm. CONCLUSION: Data obtained from gas exchange analysis are more sensitive and potentially more useful in the detection of short-term changes in exercise capacity than data obtained from either exercise time or peak work rate, indexes that are commonly used to assess drug therapy. Patients with mild-to-moderate heart failure with sinus rhythm, but not those with atrial fibrillation because of its frequent induction of tachycardia, may be good candidates for denopamine therapy.

Independent segregation of the VP4 and the VP7 genes in bovine rotaviruses as confirmed by VP4 sequence analysis of G8 and G10 bovine rotavirus strains.

We determined the complete nucleotide sequences of the VP4 genes of five bovine rotavirus strains (A5, 61A, A44, B223 and KK3). The deduced VP4 amino acid sequence of strain A5 with G8 serotype specificity is 91.1% identical to that of strain NCDV of the serotype G6, and strain 61A with G10 serotype specificity has a VP4 amino acid sequence (95.2% identity) similar to that of the UK strain of the G6 serotype. In contrast, the VP4 amino acid sequences of strains A44, B223 and KK3 of the G10 serotype, isolated in Thailand, the U.S.A. and Japan respectively, have very similar sequences to each other, but less similarity (50 to 60%) to other group A rotavirus strains reported so far. Their VP4 genes are 2352 nucleotides in length and encode 772 amino acids, four amino acids fewer than the VP4 proteins of other animal rotaviruses. Thus, the presence of three different VP4 types (P types) and the independent segregation of G serotype and P type were postulated in bovine rotaviruses by VP4 sequence analysis.

Screening for growth hormone deficiency using urinary growth hormone measurement.

The diagnostic approach in growth hormone deficiency (GHD) is complicated. Two or more provocative tests are essential for definitive diagnosis of GHD. However, such testing cannot be carried out routinely on all subjects with short stature because of the need for hospitalization and blood sampling. A simple screening method for GHD would be of great value. Human growth hormone (hGH) levels were measured in the early morning urine of 192 children aged 7-15 years with height 2.0 s.d. below the mean for their ages. Sixty-eight subjects were selected because they showed a urinary hGH level < 10 ng/g creatinine. They were further examined in terms of bone age and plasma insulin-like growth factor (IGF-I) levels. In 30 subjects, the ratio of bone age: chronological age was < 0.8 and/or plasma IGF-I level was < 0.7 U/mL. Finally 24 of these subjects were examined with provocative tests and other endocrinological tests. Eleven subjects proved to have poor growth hormone secretion and one subject was diagnosed as having Turner syndrome. In conclusion, 11 patients with GHD were diagnosed from 192 children with short stature using urinary hGH measurement as the first screening method. These findings suggests that urinary hGH measurement could be a useful and simple method for detecting GHD.

Serologic analysis of human rotavirus serotypes P1A and P2 by using monoclonal antibodies.

Three human rotavirus (HRV) VP4 serotypes and one subtype have been described on the basis of a fourfold or an eightfold-or-greater difference in neutralization titer when tested with hyperimmune antisera to recombinant VP4 or VP8* (serotypes P1A, P1B, P2, and P3). To start to analyze the antigenic basis underlying serotype specificity, we produced a library of 13 VP4-specific neutralizing monoclonal antibodies (NMAbs) to two HRVs, the serotype P1A strain Wa and the serotype P2 strain ST3, and characterized the reactivity of these NMAbs with a panel of serotypically diverse HRV strains by neutralization assay and enzyme-linked immunosorbent assay (ELISA). We characterized the serotypic specificity of the NMAbs by using a fourfold or an eightfold-or-greater difference in titer against the homologous (i.e., immunogen) and heterologous strains as a criterion for serotype. Some ST3-derived NMAbs reacted specifically with serotype P2 HRVs by ELISA and/or neutralization assay, while some Wa-derived NMAbs reacted specifically by ELISA and/or neutralization assay with some or all serotype P1A HRVs. Other Wa- and ST3-derived NMAbs reacted with some or all serotype P1A and P2 HRV strains by neutralization assay and ELISA. Most NMAbs did not react with serotype P1B or P3 strains. In previous studies, three distinct operationally defined epitopes have been identified on VP4 by examining the reactivity patterns of selected antigenic variants of HRV strain KU. At least one of the NMAbs described here recognizes an epitope unrelated to these previously identified epitopes, since it neutralized both KU and its variants.

Respiratory control during exercise in patients with cardiovascular disease.

The pathophysiologic mechanism for exertional dyspnea, the main symptom of patients with heart failure, has not been fully clarified. To determine the relationship between exercise hyperpnea and the lactic acidosis in patients with heart failure, we evaluated ventilation during incremental exercise both below and above the lactic acidosis threshold in 16 normal subjects and in 48 patients with cardiovascular disease while expired gas was analyzed continuously. The peak oxygen uptake and oxygen uptake at the lactic acidosis threshold decreased significantly as the New York Heart Association (NYHA) functional class severity increased. the slope of the increase in ventilation to the increase in oxygen uptake (delta VE/delta VO2) at work rates below the lactic acidosis threshold did not differ between normal subjects and patients with heart failure. Above the lactic acidosis threshold, however, the slope of delta VE/delta VO2, which was higher than that below the lactic acidosis threshold in each of four groups, was steeper in patients in NYHA Class II (60.8 +/- 17.9) and Class III (66.5 +/- 21.2) when compared with that in the normal subjects (46.6 +/- 13.5) or the patients in NYHA Class I (46.1 +/- 10.3). The lactic acidosis caused by decreased oxygen transport to working muscles accounts for the higher ventilation during exercise in cardiac patients. These data suggest that the increased ventilation during exercise, which must be related to exertional dyspnea, in patients with cardiovascular disease is primarily the consequence of a stimulus to regulate arterial pH.

Fastigial pressor response observed during an operation on a patient with cerebellar bleeding--an anatomical review and clinical significance.

The fastigial pressor response (FPR) consists of an abrupt rise in blood pressure and tachycardia with hyperpneic response. The FPR is an important sign in posterior fossa surgery for respiration in postoperative patients. The authors experienced a distinct pressor, tachycardiac, and hyperpneic response elicited when controlling bleeding using a bipolar coagulator forceps at the subfastigial white matter in a 72-year-old man with cerebellar hemorrhage. This response was confirmed by bipolar electrodes, which produced a weak current under deeper anesthesia by increasing the concentration of enflurane. The postoperative course of this patient was uneventful. A FPR was suggested to be mediated by axons passing between the nucleus fastigii and the fourth ventricle. These commissural fibers mediating the FPR were suggested to be countercurrent efferent pathways from both sides of the nucleus parabrachialis lateralis, located at the dorsolateral corner of the rostral pons. The authors conclude that the partial destruction of the subfastigial commissural fiber bundle did not result in permanent respiratory deficit. Fatal respiratory disturbance might be anatomically suggested by completely cutting off this fiber bundle.

Synthesis and antiinflammatory and analgesic properties of 2-amino-1H-benzimidazole and 1,2-dihydro-2-iminocycloheptimidazole derivatives.

2-Amino-1H-benzimidazoles (3) and 1,2-dihydro-2-iminocycloheptimidazoles (4) were synthesized and evaluated for antiinflammatory and analgesic activities. The compounds in the series 3 were synthesized via phenylthioureas (6) or 2-chloro-1H-benzimidazole (12). Most of 4 were synthesized by two methods. One was the reaction of carbodiimides (14) with 2-amino-2,4,6-cycloheptatrien-1-one (method A). The other was the reaction of guanidines (15) with 2-chloro-2,4,6-cycloheptatrien-1-one (method B). Some of the compounds 3 and 4 exhibited potent antiinflammatory and analgesic activities when compared to timegadine (1) or tiaramide hydrochloride (HCl) (17). It was of interest that 1-(2-benzothiazolyl)-2-cyclohexylimino-1,2-dihydrocycloheptimid azole (4e) showed superior analgesic activity to timegadine or tiaramide HCl (ED50 = 1.7 mg/kg p.o. in the acetic acid-induced writhing test, ED30 = 14.0 mg/kg p.o. in Randall-Selitto method) in spite of no effect on prostaglandin E2 synthesis.

Fine structure of the septal olfactory organ of Masera and its associated gland in the golden hamster.

Fine structures of the septal olfactory organ of Masera (MO) and its associated gland, a kind of olfactory glands, were examined in the golden hamster in comparison with those of the olfactory epithelium (OE) and vomeronasal organ (VNO) and their associated olfactory glands. Bipolar neurons of the MO were divided into two types according to their apical morphology, one similar to the receptors of the OE and the other to those of the vomeronasal sensory epithelium (VSE). The former was dominant and interpreted as main receptors of the MO. The other was less in number, but also regarded as a kind of olfactory receptors. The ultrastructural features of supporting cells of the MO were similar to those of the OE rather than to those of the VSE, while those of basal cells were almost in common in the OE, VSE and MO. The associated gland of the MO was positive to both PAS and alcian blue as the Bowman's gland of the OE. The fine structure of the associated gland of the MO was also similar to that of Bowman's gland. The present findings on the fine structure of the MO and its associated gland strongly suggest that the MO fulfills an olfactory function similar to that of the OE.

A minor prevalent strain in a severe outbreak of foal diarrhea associated with serotype 3 rotavirus.

An epizootic of foal diarrhea due to serotype 3 rotavirus (RV) was observed in 89 of 168 cases (53%) during the period from March to July in 1987. A total of 51 strains of RV were isolated from the 62 diarrheal feces examined, and one isolate (CH-3) showed a unique electropherotype of viral RNA which differed from the others that widely prevailed on this farm. No positive reaction was observed between strain CH-3 and each of the antisera against serotypes 1 to 12 of human and animal RV in neutralization tests. However, dsRNAs of the CH-3 virus were hybridized with a probe prepared from a strain of equine serotype 3 RV.

[Immunohistochemical and morphometric studies on the development of the thyroid, parathyroid and ultimobranchial body in Xenopus laevis Daudin].

The development of the thyroid, parathyroid and ultimobranchial body was examined by immunohistochemistry and morphometric analysis in the larvae of Xenopus laevis Daudin from the time immediately after hatch to the end of metamorphosis. The thyroid appeared at stage 43 and began to secrete thyroglobulin from stage 47/48. Both the volume of the thyroid and the height of the follicular epithelium reached the peak at stage 61 and decreased thereafter towards the end of metamorphosis at stage 66. The parathyroid first appeared at stage 43 as the thickening of the wall of the third and fourth visceral pouch. The volume of the parathyroid gradually increased with the progress of development until the end of metamorphosis. The ultimobranchial body appeared at stage 45 as a recess of the epithelium lining the floor of the pharynx. It became follicular in the structure at stage 53. Its volume reached the peak at stage 61 and decreased thereafter towards the end of metamorphosis. Calcitonin-immunoreactive cells first appeared in the ultimobranchial body at stage 47/48, showed the most intense reaction at stage 59, and rapidly decreased in number thereafter towards the end of metamorphosis.

[Morphometric studies on the effects of dehydration on the renin-immunopositive cells in the kidney of the Mongolian gerbil (Meriones unguiculatus)].

Effects of dehydration on the total area of the kidney occupied by renin-immunopositive cells were morphometrically studied on parasagittal sections by an image-analyzing system in the Mongolian gerbils. Changes in the body weight and plasma osmolarity were simultaneously measured to compare with morphometric data. The body weight decreased rapidly at the beginning, and gradually after 10 days of dehydration. On the other hand, the plasma osmolarity showed no significant change at 2 days, increased gradually after 5 days, and reached a plateau after 15 days. Similarly, the total area of renin-positive cells showed no significant changes at 2 days, increased remarkably after 5 days, and reached a plateau after 15 days. These findings suggest that the plasma osmolarity and total area of renin-positive cells showed no significant changes at the beginning of dehydration because the increase in the plasma osmolarity was compensated by the expenditure of inter-and intra-cellular fluid, and the total area of renin-positive cells was increased in response to the acceleration of renin synthesis stimulated by the increase in the plasma osmolarity after a certain period of dehydration. The total area seemed to be maintained on a certain level because the further increase in the plasma osmolarity was suppressed by the excretion of electrolytes from the body in proportion to the water loss after a long period of dehydration.
