Changes in nerve fibers adjacent to transplanted VX2 carcinoma in rabbit tongue.

This study was undertaken to examine the relationship between malignant tumor tissue and nerve fibers using a transplantation model and nerve fiber strains. Most of the neural tissue, as well as the other host tissues, showed severe degenerative changes as the transplanted tumor grew. Furthermore, the degenerative changes were chiefly confined to the area anterior to the tumor. These changes were probably provoked chiefly by the compressive effects of the rapidly enlarging transplant. The results could possibly explain clinical symptoms such as paralysis or disturbances in the movement of the tongue observed occasionally in some patients with lingual carcinomas.

Effect of 2-hydroxypropyl-beta-cyclodextrin on percutaneous absorption of methyl paraben.

A potential use of 2-hydroxypropyl-beta-cyclodextrin (HP-beta-CyD) to solubilize methyl paraben and to suppress its percutaneous absorption was examined, and compared with nonionic surfactant HCO-60. HP-beta-CyD significantly increased the solubility of methyl paraben in water, where the apparent 1:1 stability constant of the soluble complex was determined to be 2150 M-1. The in-vitro cutaneous permeability of methyl paraben through an isolated skin of hairless mouse was suppressed by HP-beta-CyD, thus promoting the bioconversion of methyl paraben to the less toxic metabolite, p-hydroxybenzoic acid (p-HBA) in the epidermis. These effects of HP-beta-CyD were greater than those of HCO-60. HP-beta-CyD (2% w/v) reduced the in-vivo percutaneous absorption of methyl paraben by 66% 24 h after the topical application of a solution containing [14C]methyl paraben to hairless mouse skin. Additionally, the percutaneous absorption of [14C]HP-beta-CyD was confirmed to be extremely low. These results suggest that HP-beta-CyD is useful in liquid preparations of methyl paraben for topical application.

Fluid and protein secretion by the submandibular glands of weanling rats in response to cholinergic and peptidergic agonists at various doses.

Fluid and protein secretion by the submandibular glands of 25-day-old rats were examined and compared in response to three cholinergic and four peptidergic sialogogues at various doses. All cholinergic and peptidergic agonists used were potent sialogogues for the submandibular glands of the weanling rats over the wide range of doses used. The cholinergic agonists, bethanechol and methacholine and the peptidergic agonists, substance P, substance PTyr8 and eledoisin-related peptide used intravenously, acted similarly to each other on the submandibular glands of the rats, late in the natural weaning period, but carbachol and physalaemin had slightly different effects. Of the peptidergic agonists, physalaemin was the most potent sialogogue among four tachykinins tested at the low dose. The types of protein secreted by the submandibular glands of the weanling rats in response to all sialogogues used here were typical of the beta-type. These results indicate that all agonists used could mainly stimulate the acinar cells of the submandibular glands of the weanling rats which have already fully developed functionally at this time.

Agents for the treatment of overactive detrusor. VIII. Synthesis and pharmacological properties of 4,4-diphenyl-2-cycloalkenylamines including FK584 and 3,3- or 4,4-diphenylcycloalkylamines.

This article describes the synthesis of 4,4-diphenyl-2-cycloalkenylamines (3, 5a) including FK584 (S(-)-3a) and 3,3- or 4,4-diphenylcycloalkylamines (2, 4, 5b) and their inhibitory activities against detrusor contraction. The order of inhibitory activity (i.v.) of the N-tert-butylamine derivatives against urinary bladder rhythmic contraction in rats was as follows: S(-)-4,4-diphenyl-2-cyclopentenylamine (FK584, S(-)-3a) > 4,4-diphenylcyclohexylamine (5b) = R(-)-3,3-diphenylcyclopentylamine (R(-)-4) > or = 3,3-diphenylcyclobutylamine (2) > or = terodiline hydrochloride (HCl) (1) = RS(+/-)-4,4-diphenyl-2-cyclohexenylamine (5a) > R(+)-4,4-diphenyl-2-cyclopentenylamine (R(+)-3a) > or = S(+)-3,3-diphenylcyclopentylamine (S(+)-4). Although the inhibitory activity of FK584 and compounds R(-)-4 and 5b against detrusor contraction in vitro induced with KCl in guinea-pigs was less potent than that of terodiline HCl, their inhibitory activities against detrusor contractions in vitro induced by electrical field stimulation and carbachol were more potent than those of terodiline HCl.

Immunohistochemical studies on the development of TSH cells in the pituitary of Xenopus laevis larvae.

Since thyroid-stimulating hormone (TSH) producing cells are thought to play an essential role during metamorphosis, their development was immunohistochemically examined to reveal the appearance and topographical changes in the distribution in the hypophysis of Xenopus laevis tadpole from hatching to the end of metamorphosis. TSH-immunoreactive cells initially appeared at stage 49 (just the beginning of development of the pars nervosa in the hypophysis) as small clusters in the middle part of the pars distalis. They showed conspicuous changes in number during late premetamorphosis: their number reached a peak at stage 51, suddenly decreased at stage 52 (just before completion of the hypophysial histogenesis) followed by gradual increase until the end of metamorphosis. At later stages, they were restricted to the posterior half of the pars distalis.

[Histological studies on postnatal development of the musk gland in house musk shrews (Suncus murinus)].

The development of the musk gland in house musk shrews aged 0 to 21 days was studied lectin histochemically. On about 3 days of age, the musk gland could be discriminated from surrounding skin. This organ developed markedly on days 3 to 7, and its development continued to 21 days of age. Histologically, on day 1, the bulges that should develop into musk glands were identified in the vicinity of hair germs. On days 3 to 7, these bulges developed markedly, and on day 21 their structure revealed almost the same morphology with that in mature musk glands. We used 7 lectins; ConA, RCA I, PNA, SBA, UEA-I, DBA, and WGA. On day 1, 6 lectins except for WGA labelled the cell of the musk gland. WGA began to label it on day 3. Until 7 days of age, lectins bound to only the cytoplasm of the cell of musk glands. On days 12, however, PNA and UEA-1 labelled the cell membrane equally, and RCA I labeled it on day 21.

Apoptosis and heterophagy of medial edge epithelial cells of the secondary palatine shelves during fusion.

Recent in vivo and in vitro studies have suggested that medial edge epithelial (MEE) cells covering the lateral palatine shelves do not undergo cell death, but migrate into the oral and nasal epithelium or transform into mesenchymal cells. We, therefore, reexamined the fate of MEE cells during palatal fusion in rat embryos by in situ 3' nick end labeling of dUTP (TUNEL), electron microscopy, and immunohisto/cytochemistry. TUNEL staining revealed positive nuclei in the medial edge epithelium immediately prior to contact, in epithelial triangles formed between the epithelial seam and nasal or oral epithelium, in epithelial pearls, and in mesenchymal tissue near the epithelium. However, these TUNEL-positive cells were rarely present in the epithelial seam. Electron microscopy revealed MEE cells showing nuclear chromatin condensation and cell shrinkage, and apoptotic bodies in the fusing epithelium; these often contained apoptotic body-like structures as heterophagosomes. By double staining using a laser scanning microscope, TUNEL-positive nuclei were co-localized with lysosomal cysteine proteinases, cathepsin B or L in MEE and mesenchymal cells adjacent to the epithelium. These results suggest that MEE cells undergo apoptosis during the palatal formation, even though they migrate into epithelial triangles or transform into mesenchymal cells. Moreover, apoptotic bodies and cellular debris were phagocytosed by adjacent MEE cells or mesenchymal cells and digested by lysosomal enzymes.

Net nutrient flux by visceral tissues of beef steers given abomasal and ruminal infusions of casein and starch.

The objective of this study was to quantify changes of net nutrient metabolism by portal-drained viscera (PDV) and liver of four beef steers (253 +/- 7 kg) in response to combinations of ruminal (R) or abomasal (A) infusions of cornstarch (S) and casein (C). The four treatments in a Latin square design were SACA, SACR, SRCA, and SRCR. Steers were fed alfalfa hay (DMI = 4 kg/d) as a basal diet in 12 equal meals delivered every 2 h and they received continuous infusion of S (800 g/d) and C (200 g/d) in 11-d periods. Digestibilities of DM, N, NDF, and starch, ruminal outflow of liquids and DM, and energy and N retention were less (P < .05) for SA than for SR. Net ammonia and glucose release from PDV were greater (P < .01) for SA than for SR. Net total VFA, acetate, and beta-hydroxybutyrate release from PDV and total splanchnic acetate and beta-hydroxybutyrate release were greater (P < .05) for SR than for SA, but starch infusion site had no effect (P > .10) on net urea N transfer or alpha-amino N release by PDV. Net release of ammonia N, propionate, and total VFA by PDV and uptake of urea N by PDV were greater for CR than for CA, but net alpha-amino N release by PDV and total splanchnic tissues were greater for CA than for CR (P < .05). Summation of energy supply by PDV indicated no difference in total supply among the treatments, but relative contribution of energy sources was affected by infusion site. Energy release by PDV per unit of DE intake was .68 and .66 for SA and SR, respectively. Net release of glucose by PDV was greater for SACA than for SACR (P < .05). These results suggest that site of digestion of starch and casein varies the relative contribution of nutrients to energy supply by visceral tissues and therefore varies N use in beef steers.

[The analgesic effect of iontophoresis with magnesium sulfate].

The analgesic effect of iontophoresis with magnesium sulfate was studied in fourteen healthy adult volunteers. Magnesium sulfate 0.5 mg.5 ml-1 was locally administered by the iontophoresis with 1.0 mA electric current for 10 min. The pain recognition time was measured by the pain thermometer before and after 5, 15, 30, 60 and 120 min of iontophoresis. The pain recognition time was significantly extended and lasted for 60 min. These results support the view that the iontophoresis with magnesium sulfate has analgesic effect and produces a good pain relief clinically.

[The efficacy of transarterial immuno-embolization therapy in patients with unresectable hepatocellular carcinoma].

We devised a new therapeutic modality for multiple hepatocellular carcinoma (HCC) consisting of transarterial immuno-embolization (TIE) using OK-432 and fibrinogen, and applied the treatment to 22 patients with advanced HCC who had been insensitive to TAE, and 21 patients who had no treatment. Nineteen patients had a high AFP level of more than 200 ng/ml. The serum AFP level decreased in 15 patients after TIE, and in 7 patients the AFP level decreased to less than 30%. Furthermore, a marked reduction in tumor size was observed in 69% after TIE. We report 61% 1-year, and 3-year survival rate for cases who had been insensitive to TAE, and 86% 2-year survival rate for those who selected TIE as the first choice. A high fever of more than 38 degrees C occurred in all cases, and hypotension less than 80 mmHg was observed in 42% patients. No deterioration of liver function and no disturbance of the coagulation-fibrinolysis system due to TIE except in one patient with liver failure after TIE.

[Endourology update].

In urology, endoscopic surgery and laparoscopic surgery are generally named as endourology. Transurethral endoscopic surgery was originated in mid 19th century, and established by 1950s. At present, transurethral resection of the prostate, transurethral resection of the bladder tumor, transurethral incision of the urethral stricture, transurethral vesicolithotripsy, and transurethral ureterolithotripsy, were commonly performed in many urological clinics. Percutaneous nephrolithotripsy and extracorporeal shock wave lithotripsy were introduced in 1980s, and now, open operation for urolithiasis are rarely performed. In 1990s, as a new endourologic procedure, laparoscopic operations are being increasingly applied to the treatment of urological diseases; for example, laparoscopic pelvic lymphadenectomy, laparoscopic varicocelectomy, laparoscopic exploration of non-palpable testis, laparoscopic nephrectomy, and laparoscopic adrenalectomy etc. Of the variety of treatment modalities available, the most effective and least invasive method should be selected appropriately. So, we anticipate the further advancement in minimally invasive therapy, interdependently with our sufficient experience and skills.

Pharmacokinetics of human activated protein C. 1st communication: plasma concentration and excretion of a lyophilized purified human activated protein C after intravenous administration in the mouse and the rabbit.

Pharmacokinetic studies of human activated protein C (CAS 42617-41-4, APC) were investigated in mice and rabbits with 125I-labeled compound. Plasma levels of APC were determined by three different assays: total radioactivity, APC antigenicity determined by sandwich enzyme-linked immunosorbent assay (ELISA), and the amidolytic activity which was performed by immunologically captured APC. APC concentration obtained from these assays were shown to be correlated well at early times post-dose. After intravenous administration, total radioactivity in the plasma declined tri-exponentially, but antigenicity and amidolytic activity in the plasma declined biexponentially. Plasma AUC increased proportionally with the dose, and the total body clearance and t1/2 did not change significantly. In addition, no significant difference was observed between the pharmacokinetics in male and female mice. In rabbit study, the profiles of times vs APC concentration in the plasma was similar to those in mice after single bolus injection. The plasma concentrations of APC during and after infusion in rabbits were also determined. APC concentration increased during infusion and reached almost steady state at the end of infusion. The profiles of the APC concentration in benzamidine citrate plasma corresponded to the simulated curves which were characterized by the parameters obtained from the single bolus experiment. Plasma disposition profiles of the protein were studied with high performance gel chromatography method. The radioactivity in the unchanged APC was observed at 15 min after administration. At 1 h, most of the radioactivity was observed in larger molecule fraction than the intact APC. These results corresponded to the decrease of amidolytic activity in the plasma.(ABSTRACT TRUNCATED AT 250 WORDS)

Pharmacokinetics of human activated protein C. 2nd communication: tissue distribution study of a lyophilized purified human activated protein C after single or repeated intravenous administration in male mice and placental transfer and milk passage study after intravenous administration in pregnant and lactating mice.

Tissue distribution studies of human activated protein C (CAS 42617-41-4, APC) were performed in mice after single or repeated administration, and placental transfer and milk passage study were investigated. At 15 min after a single intravenous administration of 125I-APC, radioactivity was mainly distributed to the blood and blood rich organ, such as liver, and then rapidly eliminated. The radioactivity distributed to tissues was almost negligible at 24 h after administration except for the thyroid. The qualitative study of the distribution of radioactivity to tissues by whole body autoradiography demonstrated the correspondence to the result of the quantitative assay of distribution of radioactivity after single administration of 125I-APC. The influence of repeated administration of APC on its pharmacokinetic disposition was studied by administering 125I-APC once a day to mice for 14 days. Though plasma radioactivity at 15 min in mice during repeated administration of 125I-APC was almost similar to that at 15 min after a single administration, the radioactivity at 24 h after administration was 2 times higher than that after a single administration. The profile of plasma radioactivity during and after repeated administration corresponded to the simulation curve which was described with the pharmacokinetic parameters obtained previously after the single administration. Distribution profile after repeated administration at 15 min after the 4th, 7th, 10th and 14th administration was almost similar to that at 15 min after a single administration except for the thyroid and spleen. In the thyroid, the radioactivity was 500 times higher than that after a single administration, and HPLC analysis demonstrated that the radioactivity was attributed to thyroglobulin. As to the spleen, the radioactivity was about 52% of that after a single administration. During the repeated administration, the spleen became larger than that after a single administration and the final weight was 2 times heavier than that of the non-treated animal. The decrease in radioactivity of the spleen during repeated administration was attributed to the hypertrophy of the organ. Placental transfer of 125I-APC was studied with pregnant mice quantitatively and qualitatively. Radioactivity distributed in fetuses was low at every point examined, and the result corresponded to the autoradiography. During lactation, radioactivity transferred to milk and milk to plasma ratio reached 5.7 after intravenous administration of 125I-APC. HPLC analysis of the milk radioactivity demonstrated that most of the radioactivity was present in the macromolecules produced by the lactating mother.

[Suture annuloplasty for mitral regurgitation with modified Paneth-Burr's method].

Plication of the leaflets or chordae reconstruction combined with annuloplasty cave been the standard procedure of the mitral valve repair for mitral regurgitation (MR). We have chosen the JH Kay's method for mitral annuloplasty. However, the long term results were not necessarily satisfactory because of various types of the mitral lesions. We have adopted the semicircular suture annuloplasty (Paneth-Burr's method) as the first choice for MR since May, 1991. In this paper, we demonstrate our modified operative procedure of the Paneth-Burr's method and the operative results. Also, to decide the optimal size of the annulus after annuloplasty, we evaluated the changes in size of the annulus and extent of mitral regurgitation before and about 16.3 months after surgery in 21 patients (pts) with MR undergoing Paneth-Burr's method, using Doppler echocardiography. On this annuloplasty, a double suture of EPTFE string (GORE-TEX) guarded with plegets was placed at the margin of the central fibrous body, sutured circumferentially around the annulus. Two rows of the suture were crossed in every several stitches to prevent cutting. A second suture was placed on the opposite side of the annulus in the same fashion. The MR patients were divided, by their size of the annulus after annuloplasty, into group A (less than 90% of normal annular diameter, 15 pts) and group B (more than 90% of normal annular diameter, 6 pts). In 14 of 15 pts of group A, mitral regurgitation was improved from grade 2-4 to grade 0-1 postoperatively.(ABSTRACT TRUNCATED AT 250 WORDS)

[The efficacy of midazolam-famotidine suppository for premedication in children].

We evaluated the efficacy of midazolam-famotidine suppository (M-F suppository) for premedication in children. After obtaining informed parental consent, we studied children aged 5m-7y, ASA I status, scheduled for minor elective surgery. The suppository group (n = 26) was given suppository of both midazolam 0.5 mg.kg-1 and famotidine 2 mg.kg-1, and the intramuscular injection group (n = 19) was given hydroxyzine 1 mg.kg-1. In the suppository group, gastric pH (3.90 +/- 0.34) was significantly higher, and gastric volume (1.88 +/- 0.54 ml) was significantly less than in the intramuscular injection group (1.82 +/- 0.15, 8.42 +/- 1.76 ml). The M-F suppository may offer similar sedative effect as an intramuscular injection of hydroxyzine. We concluded that the M-F suppository is an effective premedicant for pediatric patients.

[The comparison of hypnotic potency of nitrous oxide with that of sevoflurane: evaluation by the middle latency auditory evoked response].

In 10 consented patients (ASA I-II) we quantitated the hypnotic potencies of 0.7 MAC nitrous oxide and 0.7 MAC sevoflurane by measuring auditory evoked response (AER), and compared the hypnotic potency of nitrous oxide with that of sevoflurane at the same MAC. In each patient, measurements of AER waves were performed in the following three stages, 0.7 MAC nitrous oxide, 0.7 MAC sevoflurane and 0.7 MAC nitrous oxide after induction of anesthesia. Analysis of variance was used to compare the hypnotic potencies between two inhaled anesthetics. Pa and Nb latencies with sevoflurane were longer than those with nitrous oxide, and Pa and Nb amplitudes with sevoflurane were greater than those with nitrous oxide. However, we found that there were no statistical significances in our results. In conclusion, nitrous oxide exerts as much hypnotic effect as sevoflurane.

[Essential thrombocythemia transformed to minimally differentiated acute myeloid leukemia].

A 64-year-old female diagnosed for essential thrombocythemia was treated with MCNU 50 mg four times in the course of the disease. Six months after the last administration, in May 1991, she was admitted because of decreasing thrombocyte count and appearance of blasts in the peripheral blood. On admission, laboratory findings were as follows: WBC 700/microliters with 5% of blasts, RBC 331 x 10(4)/microliters, and PLT 17.9 x 10(4)/microliters. Bone marrow aspiration revealed hypocellular marrow with 39% blasts. About 5% of the blasts were positive for myeloperoxidase by electron microscopy analysis. Leukemic cells were positive for CD 7, 13, 33 and 34, negative for other lymphoid lineage markers, and demonstrated no rearrangement of TCR-beta, gamma and IgH genes. Although she was treated with low-dose cytosine arabinoside, no response was observed. Subdural hematoma and sequential pneumonia developed and the patient died eight months after leukemic transformation. In conclusion, we think that the leukemic transformation might have been developed in the natural course of essential thrombocythemia in the present case. However, we cannot exclude the influence of MCNU.

Persistence of a single electropherotype and serotype (G6P5) of bovine rotavirus in calves on a closed dairy farm from 1990 to 1993.

A virologic survey was conducted on calves with diarrhea associated with bovine rotavirus (BRV) on a closed dairy farm. The BRV was detected from 32 of 219 (14.6%) fecal specimens repeatedly collected from 56 calves born during the years 1992-1993, regardless of whether they had diarrhea. Most of the 32 strains were isolated from fecal specimens obtained from 2- to 6-week-old calves. After electrophoresis of double-stranded viral RNA from the 32 strains, genomic RNA migration patterns were similar to those of the predominant BRV strains isolated at the same farm during the years 1990-1991. All representative strains were identified as G serotype 6 (G6) and P type 5 (P5) by results of the virus-neutralization test and polymerase chain reaction procedure. Thus, BRV had no change in genomic RNA electropherotypes and serologic antigenicities in a closed dairy herd over a period of several years.
