[The effect of iontophoresis with several Ca channel blockers for PHN patients].

We performed iontophoresis with Ca channel blockers for healthy adult volunteers. In this clinical study, we used iontophoresis with Ca channel blockers. Ten out patients with PHN treated at our pain clinic were treated with iontophoresis. They were randomly assigned to one of the following four treatments: (1) 5 ml of 4% lidocaine HCl, (2) 2 mg of nicardipine HCl + 5 ml of distilled water, (3) 2 mg of verapamil HCl + 5 ml of distilled water, and (4) 2 mg of diltiazem HCl + 5 ml distilled water. Iontophoresis was performed using the above four drugs on the positive pole. Using a VAS, each patient was evaluated concerning the analgesic effect. The pain before treatment (10 points) was used as the base line. Compared with the scores before treatment, VAS scores decreased significantly after iontophoresis in all four groups. In the lidocaine group, a significant decrease in VAS scores occurred immediately after iontophoresis and lasted up to 24 hours, reaching the nadir at 2 hours. In the nicardipine group, the decrease occurred immediately after iontophoresis and lasted up to one day, reaching the nadir at four hours. In the verapamil group, the decrease started 1 hour after iontophoresis and lasted up to 48 hours, reaching the nadir at 8 hours. In diltiazem group, the decrease started 1 hour after iontophoresis and lasted up to 48 hours, reaching the nadir at 4 hours. Iontophoresis with Ca channel blockers produced a prolonged analgesic effect in PHN patients. Previously we had observed the same effect in adult volunteers. Therefore, we believe that this therapy will be clinically useful.

Immunohistochemical studies on the development of the hypothalamo-hypophysial system in Xenopus laevis.

BACKGROUND: Few attempts have been made to clarify the relational development of the hypothalamo-adenohypophysial and -neurohypophysial systems in species higher than amphibians. METHODS: The appearance and topographical distribution of endocrine and neuroendocrine cells and fibers in these systems were immunohistochemically examined in the larvae of Xenopus laevis from immediately before hatching (stage 32, Nieuwkoop and Faber's classification) to the end of metamorphosis (stage 66). RESULTS: (1) Each endocrine cell differentiated until the middle premetamorphic period. MSH cells initially appeared in the posterior half of the pituitary anlage at stage 35/36, followed by the differentiation of GH cells at stage 39 in the middle part, PRL cells at stage 46 in the anterior half of the pituitary anlage, and LH cells at stage 50 in the posterior two thirds of the pars distalis. With the progression of development, the cells which differentiated at early stages shifted from their initial positions; MSH cells, to the pars intermedia; and GH cells, to the posterior half of the pars distalis. 2) Oxytocin and vasopressin fibers were observed at stage 47/48 in the median eminence, and converged to the pars nervosa at later stages. 3) Neuroendocrine fibers innervated the median eminence during the middle premetamorphic to prometamorphic period: SOM fibers, at stage 45; CRH, 47/48; GRH, 48; dopamine, 58; and LHRH, 60. The cells containing these hormones were observed in the (presumptive) preoptic and/or infundibular nuclei. CONCLUSION: These results suggest the following three chronological steps in the development of hypothalamo-hypophysial systems and their target organs: independent development of target organs at early developmental stages; appearance of hypophysial hormones to control the development of target organs at middle developmental stages; appearance of hypothalamic hormones to control the function or maturation of the hypophysis at late developmental stages.

Exocytosis of secretory granules in the juxtaglomerular granular cells of kidneys.

BACKGROUND: There is little agreement as to the secretory process of renin granules in juxtaglomerular granular cells (JG cells) of kidneys, although a large number of studies of the regulation of renin secretion have been reported. METHODS: The structural correlation between the stimuli and the secretory process was examined in mouse JG cells on renal cortical slices incubated with the beta-adrenergic agonist, isoproterenol; the loop diuretic, furocemide; the Ca2+ chelator, EGTA; and the actin filament-disrupting agent, cytochalasin B. RESULTS AND CONCLUSIONS: Treatment with isoproterenol (10(-5)-10(-3) M) or furocemide (10(-3) M) in Ca(2+)-containing medium did not significantly affect the ultrastructure of JG cells. In slices incubated with isoproterenol or furocemide in the Ca(2+)-free medium, JG cells occasionally contained a few electron-lucent granules at the cell periphery in addition to the electron-dense mature granules observed in the control slices. On rare occasions, the JG cells displayed omega-shaped cavities with electron-lucent matrices, a feature similar to the contents of electron-lucent granules. Cytochalasin B markedly promoted the effects of these stimulants in Ca(2+)-free medium. These findings suggest that participation of actin filament disassembly in the exocytotic process of the mature granules in JG cells.

Preoperative cystometrography in patients with clinical benign prostatic hypertrophy.

Preoperative water cystometrograms obtained from 437 patients with benign prostatic hyperplasia (BPH) were examined in a retrospective study. The cystometrographic results were analyzed regarding the preoperative clinical features: the patients' age, presence or absence of urinary incontinence, history of urinary retention, and rate of residual urine. Their prognostic value in terms of improvement in voiding difficulty and postoperative urinary incontinence was also analyzed at 1 and 6 months after elective transurethral resection of the prostate (TURP). Of these patients, 263 (60.2%) had detrusor instability (group I), whereas 174 did not (group II). Vesical denervation supersensitivity (Vds) to bethanecol chloride was noted in 47 (12.5%) of 375 patients. The observed difference in clinical features was significant between the two groups, with group I being older (P < 0.01) and showing a greater incidence of urinary incontinence (P < 0.001) and retention (P < 0.001). The difference seen between groups I and II in terms of mean bladder capacity (P < 0.01), compliance (P < 0.01), and a greater positive rate of Vds (P < 0.001) was also significant. The clinical and cystometrographic parameters studied worsened with advancing age of the patients. Although the majority of the patients (94.7%) were relieved of obstructive symptoms after TURP (6 months later), 113 (25.9%) showed no relief at 1 month.(ABSTRACT TRUNCATED AT 250 WORDS)

Hearing impairment in WBN/Kob rats with spontaneous diabetes mellitus.

The inner ear of spontaneously diabetic WBN/Kob rats was functionally and morphologically examined in order to elucidate the relationship between diabetes mellitus and hearing impairment. At 3 months of age, WBN/Kob rats were non-diabetic, and their hearing function was normal. At 6-7 months of age, they showed decreased glucose tolerance and an increasing tendency toward urinary excretion of glucose without high plasma concentration of glucose, and were therefore judged to be pre-diabetic. They also displayed a significant elevation of hearing threshold in the auditory brainstem response, but showed little morphological and histochemical changes in the inner ear. At 12-13 months of age, they were spontaneously diabetic and showed a more apparent elevation of hearing threshold in auditory brainstem response than that in pre-diabetic animals. In addition, they displayed a marked decrease in the number of spiral ganglion cells and oedematous changes in the stria vascularis. The stria vascularis also showed a decrease in the intensity of staining with some lectins, i.e., wheat germ agglutinin, succinylated wheat germ agglutinin, Soranum tuberosum lectin, and concanavalin A. In conclusion, hearing impairment is induced by diabetes in the WBN/Kob rats first as an elevation of hearing threshold along with glucose intolerance; secondly, as a decrease in the number of spiral ganglion cells; and thirdly, as oedematous change of the stria vascularis with decreased intensity of lectin staining.

Preferential selection of VP7 gene from a parent rotavirus strain (SA11) in sequential passages after mixed infection with SA11 and SA11-human rotavirus single-VP7 gene-substitution reassortants.

We studied the competitive growth among SA11-L2(G3) and its single-human VP7 gene-substitution reassortants SA11-L2/KU-R1(G1) and SA11-L2/DS1-R1(G2), which have the genetic background of SA11-L2, during sequential passages after mixed infection. When the same infectious units (m.o.i. of 5 p.f.u./cell) of SA11-L2 and a reassortant SA11-L2/KU-R1 were inoculated onto and passaged in MA104 cells, 88% of the virus clones isolated from the culture fluid at the 3rd passage belonged to G3, and all the clones from the 10th passage had G3 specificity. Even when SA11-L2/KU-R1 with titer 10 times higher than that of SA11-L2 was used in the coinfection, the predominance of clones with G3-VP7 was observed. Although G2 clones slightly surpassed G1 clones in number in the mixed culture of SA11-L2/KU-R1 and SA11-L2/DS1-R1, G3 clones predominated in the virus progeny from a mixed culture infected with the same titers of SA11-L2, SA11-L2/KU-R1, and SA11-L2/DS1-R1. However, no significant difference in viral growth was detected among SA11-L2 and the two reassortants.

Pleomorphic nonfunctioning islet cell tumor of the pancreas.

A nonfunctioning islet cell tumor of the pancreas in a 68-yr-old female is reported. All of the laboratory data, including serum hormones, glucose, and tumor markers, were within normal limits. A pancreatoduodenectomy was performed. Cut sections of the specimen revealed a cystic mass measuring 6.0 x 4.5 x 3.5 cm. Histologically, the tumor consisted of large acidophilic cells with bizarre giant nuclei. The tumor cells were positive for chromogranin-A, synaptophysin, neuron-specific enolase, and S-100 protein. Multifocal proliferation of endocrine cells, centroacinar cells and changes in acinar cells were found in the adjacent uninvolved pancreatic tissue. Ultrastructural studies of the tumor revealed clear nuclei with no zymogen, but immature neurosecretory granules in the cytoplasm of the tumor cells. These findings were consistent with those of nonfunctioning islet cell tumors, especially of the pleomorphic type. To the best of our knowledge, this is the first report on pleomorphic nonfunctioning pancreatic islet cell tumors.

Amplifying effect of endothelin-1 on serotonin-induced vasoconstriction of human umbilical artery.

OBJECTIVE: The purpose of this study was to evaluate the role of endothelin-1 in serotonin-induced vasoconstriction in human umbilical artery. STUDY DESIGN: Umbilical arteries collected from 41 normal subjects at term were cut helically and suspended in an organ bath for recording isometric mechanical activity. In study 1 we measured the concentration-contraction response to serotonin or endothelin-1 according to a cumulative concentration schedule. In study 2 vessels were preincubated with a subthreshold concentration of endothelin-1 (100, 250, or 500 pg/ml), and then serotonin was added cumulatively. In study 3 vessels were preincubated with H-7 (3 x 10(-6) mol/L), an inhibitor of protein kinase C, or with 12-tetradecanoylphorbol-13 acetate (10(-9) to 10(-7) mol/L), an activator of protein kinase C; then serotonin was added cumulatively. In study 4 vessels were suspended in a calcium-free solution containing 2 mmol/L ethylene glycol-bis(beta-aminoethyl ether) N,N,N',N'-tetraacetic acid and 60 mmol/L potassium chloride, and then a cumulative concentration-response curve to calcium chloride (10(-7) to 10(-3) mol/L) was constructed for vessels pretreated with endothelin-1, 500 pg/ml or 5 ng/ml. RESULTS: The threshold concentrations of serotonin and endothelin-1 were 5 and 1 ng/ml, respectively. A subthreshold concentration of endothelin-1 (250 or 500 pg/ml) potentiated significantly the concentration response to serotonin (p < 0.084, p < 0.009, by analysis of variance). Pretreatment with H-7 significantly suppressed the amplifying effect of endothelin-1 on serotonin-induced contraction. Treatment with 12-tetradecanoylphorbol-13 acetate (10(-7) or 10(-8) mol/L) significantly potentiated the contractile response to serotonin (p < 0.0003, p < 0.015). The sensitivity of the arterial segments to calcium chloride in the presence of a subthreshold concentration of endothelin-1 did not differ from that in the control group. CONCLUSION: Endothelin-1 at a subthreshold concentration amplified the smooth muscle contraction induced by serotonin. The mechanism of action may be mediated by activation of intracellular protein kinase C.

An immunohistochemical and ultrastructural study of vasomotor nerves in the microvasculature of human dental pulp.

Immunohistochemical methods for S-100 protein and neurone-specific enolase showed two types of nerve endings around the pulp microvasculature: the free endings, which comprise the major neural component and are distributed in all types of microvessel such as arterioles, venules and capillaries; and the varicose endings. The varicose ending was a less frequent, minor component observed only in the arterioles. Both immunohistochemical and ultrastructural observations confirmed that the varicose endings were the terminal axons of efferent vasomotor nerves. Further extensive ultrastructural examinations on the vasomotor nerves added the following new findings to our previous reports. Vasomotor nerves sometimes ramified into more than two terminal axons around arterioles, and most of these ramified axons ended in the adventitia-media junction of the arteriolar wall; however, nerve endings occasionally penetrated into the media. These findings suggest an intimate structural association between vasomotor nerves and arterioles in regulating the arteriolar microcirculation in human dental pulp by stimulating smooth-muscle cells not only of the outermost but also of the inner layers. Furthermore, the deep penetration of terminal axons into the arteriolar wall seems to provide effective regulation of pulpal blood flow under physiological and pathological conditions.

Ultrastructural relation between nerve terminals and dentine bridge formation after pulpotomy in human teeth.

Close association between nerve terminals and preodontoblasts, odontoblasts and predentine was observed during healing after pulpotomy. The nerve terminals frequently contained large numbers of synaptic vesicles. Terminals with many vesicles tended to be fewer in the predentine than in the odontoblastic layer. The distribution of terminals was more dense at the stage before the regenerated odontoblasts became arranged regularly beneath the predentine. It is suggested that these terminals have some efferent role(s), especially during collagen synthesis at the early stage of dentinogenesis. The nerves may release their abundant synaptic vesicles, in addition to serving a sensory role for monitoring the increased sensitivity in the injured areas.

The effect of intravenous and intra-tumoural chemotherapy using a monoclonal antibody-drug conjugate in a xenograft model of pancreatic cancer.

In order to investigate the efficacy of the intra-tumoural administration of an anticancer drug-monoclonal antibody conjugate in athymic nude mice bearing xenografts of a human pancreatic carcinoma, we examined the clearance of the murine monoclonal antibody A7 from the xenografts after intravenous or intra-tumoural administration and measured the antitumour effect of neocarzinostatin conjugated to MAb A7 following intravenous or intra-tumoural injection. Compared with 125I-labelled normal mouse IgG, a larger amount of 125I-labelled A7 remained in the tumour after both intravenous and intra-tumoural injection, and a significantly larger amount of 125I-labelled A7 remained in the tumour after intra-tumoural injection than that after intravenous injection. Moreover, a larger amount of 125I-labelled A7-NCS localized in the tumour after intra-tumoural injection than that after intravenous injection. Neocarzinostatin conjugated to MAb A7 showed greater activity against human pancreatic cancer than neocarzinostatin alone after both intravenous and intra-tumoural administration. Tumour growth was suppressed completely by the intra-tumoural administration of A7-NCS at a dose that did not suppress tumour growth via the intravenous route. These observations suggest that the intra-tumoural injection of neocarzinostatin conjugated to MAb A7 offers promise in treating pancreatic carcinoma.

Analysis of methicillin-resistant and methicillin-susceptible Staphylococcus aureus by a molecular typing method based on coagulase gene polymorphisms.

A molecular typing method for Staphylococcus aureus based on coagulase gene polymorphisms (coagulase gene typing) was evaluated by examining a total of 240 isolates which comprised 210 methicillin-resistant S. aureus (MRSA) and 30 methicillin-susceptible S. aureus (MSSA) collected from a single hospital. By AluI restriction enzyme digestion of the PCR-amplified 3'-end region of the coagulase gene including 81-bp repeated units, the MRSA and MSSA isolates examined were divided into 6 and 12 restriction fragment length polymorphism (RFLP) patterns, respectively, whereas five patterns were commonly detected in MRSA and MSSA. MRSA isolates that showed a particular RFLP pattern were considered to be predominant in the hospital. Coagulase typing with type-specific antisera was also performed for all S. aureus isolates for comparison. Coagulase types II and VII were most frequently detected and included isolates with four and five different AluI RFLP patterns, respectively, whereas each of the other coagulase types corresponded to a single RFLP pattern. These results indicated that RFLP typing was more discriminatory than serological typing, for typing S. aureus and demonstrated its utility in epidemiologic investigation of S. aureus infection in hospitals.

Biodistribution of murine and chimeric Fab fragments of the monoclonal antibody A7 in human pancreatic cancer.

Much recent research has focused on the use of monoclonal antibodies (MAbs) in the immunodetection of solid tumors. Fab fragments of MAbs are more suitable for immunoscintigraphy than intact MAbs. Recently, human-mouse chimeric antibodies have been developed in an effort to reduce human antimouse antibody (HAMA) production by murine MAbs in humans. In this study, 125I-labeled murine and chimeric Fab fragments of the MAb A7 were injected i.v. into nude mice bearing a human pancreatic cancer (HPC-YS) xenograft. The radioactivity in tumors and in normal tissues was subsequently measured. The tumor tissue/blood ratio (T/B) of 125I-labeled murine and chimeric Fab fragments of MAb A7 increased with time in a similar manner and reached 9.68 +/- 2.54 and 10.49 +/- 1.50, respectively, 24 h after injection. Moreover, the T/Bs of 125I-labeled murine and chimeric Fab fragments of MAb A7 were greater than the T/B of intact MAb A7. When mice bearing tumors that did not react with MAb A7 were studied, 125I-labeled murine and chimeric Fab fragments did not localize specifically to the tumors. These results suggests that chimeric Fab fragments of MAb A7 are useful carriers of radionuclides for the immunodetection of human pancreatic cancer, with equivalent activity to murine Fab fragments and less theoretical potential to induce a HAMA response.

Intra-abdominal fat in obese children.

We investigated the distribution of abdominal fat accumulation in obese children to know whether a clustering of coronary risk factors was demonstrated in visceral fat obesity as reported in adults. The relative indicator of intra-abdominal fat accumulation was obtained from computed tomography scans at the umbilicus level in 36 obese subjects (24 males, 12 females). There was no visceral fat obesity in this study by reported criteria. All metabolic variables except triglyceride did not correlate significantly with intra-abdominal fat accumulation. We conclude that visceral fat obesity is a rare status and has no close relationship to coronary risk factors in childhood.

Vasospastic action of serotonin on the umbilical artery in normal and preeclamptic patients.

The purpose of this study was to determine whether elevated plasma-free-serotonin (5-HT) levels in preeclampsia could evoke abnormal vasoconstriction in the umbilical artery (UmA). Threshold concentrations of 5-HT in the UmA from 12 normal pregnant (NP) women were 5 ng/ml. Plasma-free 5-HT levels in the UmA (14.38 +/- 5.68 ng/ml) in severe preeclampsia (SP) were higher than the threshold value. Plasma-free 5-HT levels in UmA blood (1.6 +/- 1.2 ng/ml) in NP women were lower than the threshold value. There were significant increases in sensitivity to 5-HT in the UmA from SP; the threshold value of 5-HT was low (1.0 ng/ml), and the EC50 value (5.2 +/- 2.4 ng/ml) was significantly lower than that of NP women (13.9 +/- 3.0 ng/ml, p < 0.001). These results strongly suggest that increased plasma-free 5-HT and vascular hypersensitivity to 5-HT in preeclampsia can cause vasospasms and a disturbance of the feto-placental circulation.
