Significance of echocardiographic evaluation on surgery to mitral stenosis.

1) Diastolic descent rate (DDR) of the anterior mitral leaflet improved after the surgery. However, this improvement was not modified with a) atrial fibrillation b) damages of the subvalvular supporting tissue c) valvular calcification d) the method of the operation, namely the open or closed commissurotomy. 2) DDR was fairly well correlated with the mitral orifice area, the cardiac output and the pressure gradient across the mitral valve. Therefore, the echocardiographic diagnosis and the follow up evaluation of the mitral stenosis after the surgery were thought to be very reliable, considering the accuracy of this diagnosis as disclosed in this study. The contributory effect to DDR was discussed.

Touch smear cytology for endoscopic diagnosis of gastric carcinoma.

A retrospective study was made on the accuracy of histology and touch smear cytology of the specimens obtained by endoscopic target biopsy. Sixty-five cases of small size excavated-type gastric carcinoma, including 27 cases of early carcinoma were studied. By histology alone, 89% of cases (58 cases) were diagnosed accurately, 85% (55 cases) by touch smear cytology alone and 95% (62 cases) by combined examination. By touch smear cytology, 10 out of 16 specimens were diagnosed as positive, although extensive necrosis of the specimens prevented histologic study. The study demonstrated that the floor as well as the edge and margin of the excavated lesion is an important target site for biopsy. Touch smear cytologic technic is an aid to the diagnosis of gastric malignancy.

The effect of dipyridamole on the coronary hemodynamics in man.

The effects of dipyridamole (Persantin) on the coronary blood flow (the great cardiac vein flow and coronary sinus ostial flow), the femoral arterial pressure, the coronary vascular resistance and the heart rate were measured continuously in man. The newly devised method of the continuous local thermodilution method enabled us to perform the exact measurement of the above mentioned variables in man. The increase of the coronary blood flow was seen definitely in the non-ischemic group and, therefore, the decrease in the coronary vascular resistance occurred in 30 sec and continued for more than 20 min. In the myocardial ischemic group, some patients showed the similar response to the non-ischemic group. However, some patients showed no increase of the flow, even though the coronary vascular resistance decreased in all cases in this group. The heart rate increased slightly in all cases.
