[A clinical report of vascularized segmental pancreatic autotransplantation in chronic pancreatitis].

A vascularized segmental autotransplantation of distal 80% of the pancreas to the right iliopelvic space was successfully performed in a 51-year-old male who had alcoholic chronic pancreatitis. Distal 80% of the gland with severe calcified pancreatitic change was resected and prepared for transplantation by injecting the splenic artery with Ringer's lactate solution containing heparin, albumin and gabexate mecilate cooled to 4 degrees C. The spleen was removed from the specimen. An arteriovenous fistula was constructed at the tail. End-to-side anastomosis of spleno-external iliac veins was carried out after small fusiform resection of the iliac venous wall for the purpose of sufficient venous patency. End-to-end anastomosis of spleno-hypogastric arteries was done. The pancreatic stump was anastomosed to the ileum. Post-operatively, the pain completely resolved. Patency of the graft was demonstrated by angiography. Higher response of the C-peptide immunoreactivity on 75 g OGTT was found in the iliac vein on the transplanted side than in another peripheral vein. The exocrine pancreatic function diagnostant test showed 78.8% post-operatively (vs. 63.2% pre-operatively). Vascularized segmental pancreatic autotransplantation offers an excellent method of relieving pain with preservation of endocrine and exocrine functions in selected cases with chronic pancreatitis.

[A comparative, well-controlled study of ceftizoxime suppository against ceftizoxime intravenous injection in infantile acute pneumonia].

We have attempted to clinically define the therapeutic usefulness of ceftizoxime suppository (CZX-S) in children with bacterial pneumonia, in a randomized trial. Intravenous injection of ceftizoxime (CZX) was used as the control. The results are summarized below. Subjects were inpatients with bacterial pneumonia, ranging in age from 9 months to 7 years and 10 months. As a rule, the daily dose was either four 250 mg (in potency) suppositories given at 6-hour intervals or 60 mg/kg body weight intravenous CZX (control) given in 4 injections at 6-hour intervals over a period of 7 days. The number of children in the study was 67. These children were divided into 2 dosage groups (suppository, 35; injection, 32) with matching pretreatment background factors. The severity of the target disease in the majority of the children was "moderate". The rate of therapeutic effectiveness was 97.1% for the suppository and 93.8% for the injection, and did not differ significantly between the 2 groups. Rates of efficacy by severity, presence or absence of underlying diseases, daily dose and/or complications were high without exception, and did not differ significantly between the 2 groups. Eradication rates for causative microorganisms, as studied in 16 children of each group, were both 93.8%. The 2 most frequently isolated causative organisms were Haemophilus influenzae and Streptococcus pneumoniae. Side effects were examined for 36 children of each group. The frequency of side effects did not differ significantly between the suppository group (2 with diarrhea and 1 with abdominal pain) and the injection group (1 with urticaria), and 8.3% and 2.8%, respectively. The frequency of abnormal laboratory test findings differed significantly (P less than 0.01) with respect to eosinophilia which occurred in 7 (20.6%) of the injected subjects but was not encountered in the subjects treated with suppositories. Other abnormal laboratory findings included thrombocytosis in 3 (14.3%) of the injection group and increased GOT in 1 (3.2%) of the suppository group. The suppository formulation of CZX appears to be a highly useful substitute for the injectable form, and should find a special use in children whose treatment with injections experiences some difficulty.

[The statistical evaluation of the female breast cancer detected by mass screening in Hokkaido].

In Hokkaido, 113, 612 women were subjected to mass screening of breast cancer from 1973 to 1983. Out of them, 74, 285 (56.3%) were examined once and the remainder twice or more: 4,798 (4.2%) were advised to receive further precise examinations and 99.8% of them underwent the second examination. Breast cancer was detected in 377, in which 291 (77.2%) were detected by the first examination and 33 (3.8%) were so-called "interval cancer" with the mean interval of 19.3 months. The detection rate was 0.3%. Histologically non-invasive cancer was seen in 26 (6.9% of 377). Despite a recent significant increase in detection rate of the earlier stage of breast cancer, the present study showed no evidence of a decrease in advanced cancer. The prognosis of the patients who had curable operation revealed 86.9% in 5 year survival rate and 40% in 10 year survival rate.

[A case of dural arteriovenous malformation with bilateral thalamic infarction].

A 59-year-old man was admitted because of frequent vomiting and obtundation in February 1982. Neurological examination on admission revealed only slight impairment of consciousness. Papilledema, meningeal irritation sign and paralysis were not elicited. The plain CT scan was normal, but the CT scan with contrast material showed patchy enhancement in the left temporal lobe and around the third ventricle. Cerebral angiography showed a dural arteriovenous malformation (dural AVM) in the left transverse sinus fed by the left occipital artery, and the retrograde flow into the straight sinus. By the third day following admission, the level of consciousness became alert. The patient did not complain of headache, bruit and visual disturbance. He showed mild disorientation and memory disturbance. But his ordinary daily-living was independent. In August 1982, the patient gradually became inactive and apathetic. At times he lay in bed with moving his eyes, swallowing foods. At other times, he lay in bed with closing his eyes, immobile, and unresponsive except to strong painful stimuli. The patient was incontinent and required nursing care. During three month periods, the patient progressively became somnolent, speechless and immobile. Eventually, he was in a state of akinetic mutism. The patient became unresponsive. The state of consciousness fluctuated within a narrow range. The pupils were isocoric and did not react to light. He sometimes moved his eyes horizontally, but the vertical eye movement was limited. Deep tendon reflexes were hyperactive with Babinski reflex bilaterally. Passive mobilization of extremities revealed hypertonic. The CT scan disclosed the bilateral symmetrical infarction of the thalamus.(ABSTRACT TRUNCATED AT 250 WORDS)

Effects of 1-O-alkyl-2-acetyl-sn-glycero-3-phosphocholine (platelet activating factor) on cardiac function in perfused guinea-pig heart.

The direct cardiac action of 1-O-alkyl-2-acetyl-sn-glycero-3-phosphocholine (PAF) was studied in isolated perfused guinea-pig heart preparations. PAF produced a fall in left ventricular pressure, decreases in the rate of rise of the left ventricular pressure (dp/dt) and coronary flow, but had no effect on heart rate. These results indicate that PAF is a cardiodepressant with inotropic selectivity and this effect on heart is blocked by CV-3988, a specific PAF antagonist.

Malignant fibrous histiocytoma of bone. A review of 13 cases and an ultrastructural study.

Observations on 13 patients with primary malignant fibrous histiocytoma of bone were reported. Included were nine male and four female patients, ranging in age from 6 to 81 years (mean, 44.8 years). Pain was the most common complaint. The interval from the first symptom to the initial treatment varied from 2 months to 20 years. Seven tumors arose in the knee region. Roentgenologically, most of the lesions presented with an osteolytic and destructive appearance. Histologically, highly variable morphologic features existed. The storiform-pleomorphic pattern was found in every tumor, although it was not necessarily pathognomonic for malignant fibrous histiocytoma of bone. Surgery, radical or incomplete, was the primary treatment for all but one patient. Lymph node metastasis was present in three. Five patients died of the disease from 3 to 79 months after the diagnosis (mean, 28 months), all exhibiting metastasis to the lung. Gaucher's body-like structure demonstrated in the electron microscopic study shows the histiocytic quality of malignant fibrous histiocytoma of bone.

Seroepidemiology of human T-lymphotropic retrovirus type I (HTLV-I) in residents of Niigata Prefecture, Japan. Comparative studies by indirect immunofluorescence microscopy and enzyme-linked immunosorbent assay.

A large sample of carriers of human T-lymphotropic retrovirus type I (HTLV-I) in Niigata Prefecture was examined for the detection of natural antibodies to HTLV-I-related antigens in sera using both indirect immunofluorescence microscopy (IFM) and enzyme-linked immunosorbent assay (ELISA). The present findings are based on multiple surveys, using each assay technique at least twice. Although Niigata Prefecture has been considered a non-endemic region for HTLV-I, Sado Island has been proven by this study to be a relatively endemic pocket within this non-endemic area. Seropositivity was highest in residents of Sado Island; 97/1, 117 (8.7%) by IFM and 33/1,061 (3.1%) by ELISA; followed by Niigata City, 18/650 (2.8%) by IFM and 16/638 (2.5%) by ELISA; and lowest in the remaining areas, 57/2,631 (2.2%) by IFM and 20/2,551 (0.8%) by ELISA. Seropositivity was demonstrated in 172/4,398 (3.9%) by IFM and 69/4,250 (1.6%) by ELISA in Niigata Prefecture taken as a whole. In general, the incidence of seropositive residents increased gradually with age. The sex difference was not significant. The serum samples tested were categorized into 4 groups; (1) IFM and ELISA both positive, (2) IFM positive but ELISA negative, (3) IFM negative but ELISA positive, and (4) IFM and ELISA both negative. By absorption tests, IFM and ELISA seemed to recognize different specific antibodies in sera; IFM recognized antibodies to HTLV-I-related cellular antigens in addition to HTLV-I viral antigens, but ELISA recognized antibodies to HTLV-I viral antigens alone. Thus, IFM detected a broader spectrum of antigens, resulting in recognition of more positive sera than those detected by ELISA.

Clinical features of OKT4+/OKT8+ adult T-cell leukemia.

Adult T-cell leukemia (ATL) has a range of clinical characteristics. Phenotypically the leukemic cells usually express the helper/inducer associated antigen OKT4 with lack of OKT8. We have observed three patients with acute ATL cytologically indistinguishable from OKT4+/OKT8- ATL but whose neoplastic cells had the unusual phenotype, OKT3+, OKT4+, OKT6-, OKT8+ OKT9+/-, OKT11+, Tac+/-, TdT-. All patients had abnormal karyotypes and antibodies against anti-ATL associated antigens as well as proviral DNA of human T-cell leukemia virus in the leukemic cells. The clinical course was complicated by skin eruptions, hypercalcemia, pulmonary infection and disseminated intravascular coagulopathy. All died of complications shortly after diagnosis. The clinical features of these patients were similar to those of OKT4+/OKT8- ATL. However, their acute course suggests that co-expression surface antigens OKT4 and OKT8 may be a sign of aggressive nature of the disease with poor prognosis.
