Behavior of propylene glycol (PG) in dermis after treatment of rat intact skin surface with fatty acids, fatty amines or azone dissolved in PG.

Rat abdominal intact skin was treated with fatty acids, fatty amines, or Azone which were dissolved in propylene glycol (PG) and PG appearing in the rat dermis was studied. Analysis was done by Fourier transform infrared/attenuated total reflection (FT-IR/ATR) spectroscopy. The appearance of PG with time seemed to be in three phases when the skin sample was treated with a skin penetration enhancer such as oleic acid: (1) in the first stage, PG penetrated the skin barrier which was not substantially altered, and gradually appeared in the dermis; (2) in the second stage, it rapidly distributed in/throughout the dermis, and this rapid distribution was probably due to the alteration of the dermal structure: the penetration enhancing effect of the enhancer was thought to reach maximal; and (3) in the third stage, PG was saturated in the dermis. The value of T(max alteration), at which the alteration of the dermal structures is completed, showed that the action of both oleic acid and oleylamine were more rapid than other enhancers. Both the value of PG peak area(max) at the third stage which reflects the distribution volume of PG in the dermis and the value of T(sat) at which PG is saturated in the dermis were calculated, and the results suggested that both the distribution volume of PG in the dermis and the time of the saturation varied depending on the enhancer. In conclusion, our present work indicated the importance and necessity of evaluating the rate and extent of appearance of a drug in the dermis to characterize an enhancer.(ABSTRACT TRUNCATED AT 250 WORDS)

Toxicity and properties of the extract from Sarcocystis cruzi cysts.

The extract from Sarcocystis cruzi cysts in bovine muscle was subcutaneously injected to mice, guinea pigs, chickens, and rabbits to detect its toxicity. Only rabbits showed reactions after administration of the extract at a dose of 25 micrograms. The main clinical signs of the rabbits were depression, reduction in body temperature and intermittent diarrhea and the hematological findings observed were elevation in WBC, RBC, PCV, TP, BUN, AST, AUT and creatinine values and reduction in glucose, K+ and pH of blood. The extract, crude toxin, was a water soluble, acid-alkali stable and thermolabile protein and estimated to be a molecular mass of 15-16 kd.

Relationship between degree of dehydration and serum electrolytes and acid-base status in cows with various abomasal disorders.

In 368 cows with various abomasal disorders--left abomasal displacement (LAD), right abomasal displacement (RAD), abomasal volvulus (AV) and abomasal impaction (vagal indigestion)--the degree of dehydration was compared with electrolytes and acid-base status of the cows. No clinical symptom of dehydration was observed in 62% of cases with LAD and 43% of cases with RAD. The cows were moderately and severely dehydrated in 86% of the cases with AV and 84% with abomasal impaction. Hypochloremic alkalosis was observed in cows with LAD only when the cows were severely dehydrated, but cows with RAD revealed less electrolytes and acid-base abnormalities despite the increase in the dehydration score. In cows with AV, the acid-base status could not be predicted by the degree of dehydration. Profound hypokalemia and hypochloremia were observed in cows with abomasal impaction for all dehydration states. These findings indicated that hypochloremic alkalosis could be predicted in cows with abomasal impaction for all dehydration states and in LAD cases with marked dehydration, but not in RAD cases.

Effect of different suture patterns on wound healing of the teat in dairy cattle.

Experimentally-induced perforating wounds in 4 Holstein Friesian lactating cows were sutured using Gambee, continuous 2-layer (Cushing for submucosal layer and continuous horizontal mattress for skin and intermediate layer), separated 2-layer (simple continuous for mucosal layer and vertical mattress for skin and intermediate layer) and 3-layer (simple continuous for mucosal and intermediate layers, simple interrupted for skin) suture patterns. Wound healing was evaluated by radiograph, blood flowmetry, tensiometry, measurement of hydroxyproline content and histopathology. The present results suggest that 3-layer pattern provided the best healing of the entire teat. Mucosal hyperplasia was observed in Gambee and continuous 2-layer pattern while eversion of the skin, presence of suture tracts and greater amount of granulation tissue were observed in continuous and separated 2-layer patterns.

Subjective vertical and vestibular lesion.

Patients with peripheral vestibular disorders may experience postural instability in the standing position, which means disorientation in their environment. The disturbed vestibular function in light reflects the postural instability in space. The present study investigates the relationship between the subjective vertical and the vestibular lesion. The subjects were patients with unilateral vestibular lesion, and the control subjects had no past history of vestibular lesion or any present vestibular lesion. A straight line consisting of 5 spotlights projected on a dome-shaped screen was concentrically moved like a stylus by pressing buttons. We measured the subjective vertical of the subjects using a 5-spotlight straight-line (spotlight-line) on a dome-shaped screen in front of which the subjects were placed. The result showed that the patients with unilateral peripheral disorder had a statistically significantly larger deviation to the impaired ear side, especially when the spotlight-line was moved from the impaired side, as compared with controls.

Influence of warm ischemia on isolation and primary culture of hepatocytes from rat liver for a hybrid artificial liver.

To assess the possibility of using hepatocytes from ischemic liver, as a bioreactor of a hybrid artificial liver, we investigated the influence of warm ischemia on the isolation and culture of hepatocytes in rats. Warm ischemia was induced by clamping the liver hilus and the animals were divided into 3 groups according to the duration of ischemia: group A (no ischemia), group B (10 minutes) and group C (20 minutes). Hepatocytes were isolated by the collagenase perfusion method and cultured for 5 days. The yield and viability of the isolated hepatocytes were lower in group C. Rate of attachment was decreased as the duration of ischemia increased. There was no significant difference observed in functions in culture. Sufficient hepatocytes, as a bioreactor, can be isolated and cultured from warm ischemic liver within 10 minutes. Though the number of available hepatocytes were diminished, hepatocytes procured from longer warm ischemic liver could be utilized as a bioreactor.

[Home parenteral nutrition for terminal cancer patients--when should we tell the truth about their disease and return them to their home with HPN].

Home parenteral nutrition (HPN) is a significant way to enable terminally ill cancer patients to live with high quality of life at home. However, the most important condition to perform HPN is the patients' desire to return to their home. For this purpose, we should consider in informing them of the truth of their disease. We had three terminal cancer patients who returned to their home with HPN. Cas 1: 52-year-old woman, recurrent stomach cancer. She did not know her cancer. She stayed at home for 45 days with HPN. Case 2: 46-year-old woman, recurrent stomach cancer. She did not know her recurrence of cancer. She stayed at home for only 15 days with HPN. Case 3: 45-year-old woman, recurrent pancreatic head cancer. She knew her disease and her life expectancy. She had 6-year-old daughter and desired to return home. She stayed at home with HPN for 8 months. As we told Case 3 truth of her disease when she was diagnosed as recurrence of cancer, she lived at home with HPN for 8 months. From the experience of three cases, we may concluded that, the sooner the patients are informed the truth about their disease, the longer they can stay at home with HPN and spend their limited time with high QOL.

Tympanometric changes following acute otitis media in Japanese children.

Using tympanometry we investigated potential changes in both middle ears developing during and after unilateral acute otitis media (AOM) in Japanese children. In 144 children with unilateral (AOM), tympanometry was performed at presentation and was repeated at the 1st, 2nd, 3rd, and 4th weeks and then every 2 weeks up to 12 weeks after entry into the study. Type B tympanograms were recorded in 81 (56%) of the affected ears and in 37 (26%) of the unaffected ears at presentation. Twelve weeks after AOM, the type B tympanogram was still observed in 21 (15%) of the affected ears and in 16 (11%) of the unaffected ears. Ears with type B changes at presentation showed a significantly slower recovery of middle ear ventilation than did those with either type A or type C1.

Peculiar structures of the blood vessels forming the secondary ossification center in the rat femoral heads.

To investigate the development of the blood vessels forming the secondary ossification center in the femoral heads of growing rats, we observed the specimens of the proximal femurs having vascular casts using both an optical and a scanning electron microscopy (SEM). Histologically, the vascular invasion occurred from the lateral part of the cartilaginous epiphysis, prior to the appearance of the ossification center. The vascular casts were prominent within the cellular spaces consisting of a few hypertrophic chondrocytes. The SEM study suggested that the vascular buds had peculiar structures and invaded the cartilaginous epiphysis by breaking through the cellular spaces. The blood vessels budded out from the arch of the lateral epiphyseal vessels toward the femoral heads. The sizes of the large-caliber vascular buds of about 50 microns in diameter were similar to those of the cellular spaces, and they penetrated into the epiphysis by repeated merging and branching. Many deep impressions were noted on the surface of the vascular buds, and the tips showed the peculiar shape similar to a suction cup.

Primary osteosarcoma of the cerebrum with immunohistochemical and ultrastructural studies: report of a case.

A 57-year-old woman with primary intracerebral osteosarcoma is reported. The tumor was identified by computed tomography as a mass with hemorrhage in the right parietal lobe. The surgical and pathological examinations confirmed an osteosarcoma of intracerebral origin. She suffered from repeated local recurrence of the tumor and died about 1 year after the onset. The pathological findings showed features of osteoblastic osteosarcoma with numerous osteoclast-like multinucleated giant cells. Immunohistochemically, tumor cells were positive for vimentin, and partially for actin. Multinucleated giant cells were reactive with vimentin and CD68 antibodies. Ultrastructurally, tumor cells were rich with rough endoplasmic reticulum. These findings are consistent with the histological features of skeletal or extraskeletal osteosarcoma. This is the third case of primary intracerebral osteosarcoma reported in the literature and the first one analyzed ultrastructurally.

Liver resection in children, using a water-jet.

The water-jet method has been used during hepatic resection in children. The instrument cuts the hepatic tissue by means of a high-pressure fine water-jet, while the exposed intrahepatic vessels are spared injury. Physiological saline was used for the jet water, and pressure of 12 to 15 kilograms of force per square centimeter (kgf/cm2) through a 0.15-mm-diameter nozzle was found to be optimal for cutting the liver parenchyma. The authors evaluated the usefulness of the water-jet dissector (n = 8) during bisegmentectomy or trisegmentectomy in comparison to the Cavitron Ultrasonic Surgical Aspirator (CUSA) (n = 5). The mean operation time was 4.08 +/- 0.87 hours for the water-jet group and 5.08 +/- 1.33 hours for the CUSA group. The mean blood loss was 602 +/- 659 mL for the water-jet group and 1,036 +/- 521 mL for the CUSA group. Although there were no significant differences with respect to operation time, blood loss, or postoperative complications, the liver parenchyma was dissected more easily using the water-jet. The authors believe that the mechanical simplicity and safety of the water-jet method will lead to its more widespread use in liver resection in children.

Changes with aging in head and center of foot pressure sway in children.

To quantitate the development of equilibrium in children, we investigated movements of the head and of the center of foot pressure (COF) in standing subjects, using a cephalograph, a force measuring platform, and a technical computer. We studied 126 subjects, 105 healthy children, aged 4-18 years, and 21 adults, aged 20-28 years. Each subject stood with their feet together on a platform for 60 s each with eyes open or closed. The area and total length of head and COF movements decreased with increasing age, and were significantly greater in children under 12 years than in adults. The ratio of the lengths of head to COF movements was significantly larger in children under 9 years than in adults. The ratio of the anteroposterior to lateral movement of COF was also larger in children under 9 years. These observations suggested that body sway in children was not only larger than that in adults, but different in nature.

A new evaluation of vestibular compensation.

Vestibular compensation process in six patients was examined by means of static (SPG) and kinetic posturography (KPG). SPG was recorded by a stabilometer during standing and KPG was recorded by POLGON so that angular change of shoulder in the frontal plane was measured during stepping. The subjects included three cases with vestibular neuronitis, three cases with bilateral vestibular loss. In vestibular neuronitis, SPG improved earlier than KPG. As a result of vestibular training, initial high levels of SPG and KPG fall precipitously and in the case with sequela, a remarkably high level of KPG gradually decreased. In bilateral vestibular loss, abnormality of KPG tended to persist and those results would be due to the disappearance of a hip strategy. The effect of training upon the case which started from the early days was not shown. However, the case which started from 10 months after the onset showed remarkable improvement of SPG and KPG.

Acute pharyngitis, an unusual complication of intravenous hyperalimentation.

While the numerous complications of intravenous hyperalimentation (IVH) are well recognized, we encountered a unique one. A 60-year-old man developed a sore throat, neck pain and fever seven days after catheterization of the subclavian vein to provide post-operative nutrition. Marked swelling was visible at the right posterior wall of this oropharynx and hypopharynx. X-ray of the neck revealed that the tip of the catheter was positioned in the internal jugular vein, not the subclavian vein as intended. The acute pharyngitis, diagnosed as due to phlebitis of the internal jugular vein due to the malpositioned catheter, subsided within two days of catheter removal.

Malignant peripheral nerve sheath tumor with prominent intracytoplasmic vacuolation: report of a case.

A case of malignant peripheral nerve sheath tumor with uncommon features is reported. A mass was noted in the left thigh of a 16 year old man. Histologically, most areas of the tumor exhibited the typical appearance of malignant peripheral nerve sheath tumor, but some tumor cells had rounded nuclei and cytoplasm, resembling an epithelioid pattern. It was noted that some rounded tumor cells showed prominent intracytoplasmic vacuolation. Immunohistochemically, almost all of the tumor cells, including the rounded and vacuolated ones, were positive for S-100 protein and vimentin. Electron microscopic study revealed well-developed cytoplasmic processes, intracytoplasmic intermediate-sized filaments, basement lamina formation and extracellular long-spacing collagens. These findings were compatible with those of Schwann cell differentiation. Moreover, ultrastructurally, the vacuolated spaces contained a few granular materials and were derived from the dilatation of the rough endoplasmic reticulum. It is speculated that intracytoplasmic vacuolation in malignant peripheral nerve sheath tumor would be caused by degeneration of the tumor cells.

Eosinophil chemiluminescence response to cytokines and opsonized zymosans in atopic dermatitis.

Chemiluminescence (CL) responsiveness of eosinophils (Eos) to zymosan particles coated with either IgG, C3 or both and eosinophilopoiesis-modulating cytokines was investigated in patients with atopic dermatitis (AD), and an attempt was made to correlate these CL responses with serum levels of eosinophil cationic protein (ECP) which increased in response to the extent of AD severity, but not blood eosinophil counts. A high degree of positive correlation existed between serum levels of ECP and either IgG containing opsonized zymosan- or interleukin-5 (IL-5)-induced CLs. The CL values induced by these two stimuli appeared to be directly correlated with the intensity of each ligand-linked receptor expression. These results suggested that Fc gamma RII-and/or IL-5R-mediated eosinophil activation at the site of lesional skins might implicate in deterioration of cutaneous inflammation, and consequently cause an elevation of serum levels of ECP in AD.

Multiple sclerosis presented acute hearing loss and vertigo.

A case of multiple sclerosis with acute deafness and dizziness as initial symptoms was reported. The pure-tone audiometry showed a high-frequency sensorineural hearing loss; in the auditory evoked brain stem response (ABR), the left side had a normal peak I with an absence of subsequent peaks. In the equilibrium test, Bruns' nystagmus was present, and the caloric test demonstrated CP on the left side. MRI disclosed a high-intensity lesion in the cerebellar peduncle (T2-weighted image). With steroid therapy, the deafness and dizziness improved within 10 days, although, after 20 days, the findings of the ABR and caloric test remained unchanged.

Cellulose acetate polymer thrombosis for the emergency treatment of aneurysms: angiographic findings, clinical experience, and histopathological study.

Cellulose acetate polymer solution is a liquid thrombotic material that hardens into the shape of an aneurysm into which it is injected. Therapy using this solution is a rapid technique that helps prevent the rupture of aneurysms, especially those that extravasate contrast material during angiography in the acute stage of subarachnoid hemorrhage. Using this polymer solution and an endovascular technique, we treated two patients who had aneurysms of the basilar and anterior communicating arteries with extravasation of contrast material during angiography a few hours after the initial subarachnoid hemorrhage. In one patient with an aneurysm of the anterior communicating artery, the aneurysm's wall was perforated with the catheter during neurointerventional procedures. In both patients, postoperative angiograms demonstrated obliteration of the aneurysmal dome, including the site of extravasation or perforation. The parent artery and surrounding perforating branches were preserved. Although we do not advocate aggressive therapy for patients who bleed during angiography, we pursued this therapy in these two patients because of the opportunity to introduce cellulose acetate polymer in an attempt to preserve the patients' lives. Unfortunately, both patients died. Histopathological studies performed at the time of autopsy demonstrated that the luminal surface of cellulose acetate polymer was covered with thrombus by 6 days after cellulose acetate polymer thrombosis. By 10 days, the thrombus had a prominent fibrin network, a concentrated plasma component, and few fibrocytes adhering to its luminal surface.(ABSTRACT TRUNCATED AT 250 WORDS)
