Retrovesical squamous cell carcinoma associated with toxoplasmosis: diagnostic images, serum detection of SCC-antigen, and successful treatment by M-VAC regimen.

A case of retrovesical squamous cell carcinoma associated with acquired toxoplasmosis is presented. To date only six cases with retrovesical carcinoma have been reported in the literature. Although it is difficult to make a preoperative diagnosis of this rare neoplasm, transrectal longitudinal ultrasonography was diagnostically valuable, and serial measurements of serum squamous cell antigen (SCC-Ag) were useful for monitoring the clinical course.

Effects of butoctamide hydrogen succinate and nitrazepam on psychomotor function and EEG in healthy volunteers.

We studied the effects of butoctamide hydrogen succinate and nitrazepam on psychomotor function and EEG in eight male volunteers aged 19-32. The hypnotic effects, effects on psychomotor performance, EEG activity and standing steadiness between BAHS 1000 mg and nitrazepam 5 mg were compared at regular intervals for 10 h. The serum levels of both drugs were also assayed. The hypnotic effects of BAHS were very weak compared to those of nitrazepam. BAHS did not exert any effects on psychomotor performance and standing steadiness during the test period. In contrast, nitrazepam impaired psychomotor performance and standing steadiness as the serum drug levels increased. Nitrazepam decreased the alpha activity and increased the beta activity in a concentration-dependent manner. BAHS did not change the alpha activity but increased beta-2 activity at Fz and Cz at 10 h of the post-drug period. BAHS was eliminated more rapidly than nitrazepam. These results indicated that BAHS, at the dose used, was less potent than nitrazepam and the effects on psychomotor performance and standing steadiness were minimal.

Arthropathy of the wrist in leprosy--what changes are caused by long-standing peripheral nerve palsy?

A radiographical screening study of 338 leprotic patients was performed. Clinically, according to a nerve score (NS) designed by us, 12.9% of 674 hands showed mild nerve palsy (NS 5 or 6), 75.9% moderate (NS 3 or 4), and 11.4% severe (NS less than 2). Twenty-nine hands of 26 patients (NS 4.2 on average) demonstrated abnormal changes of the wrist joint on radiographs. We classified them into four groups: (a) lunate collapse (four patients), (b) scaphoid nonunion (eight), (c) scaphoid cyst (three), and (d) trapezium OA (11). In the lunate collapse and the scaphoid nonunion groups, destructive and reconstructive changes as described by Eichenholtz were identified on plain film. These groups demonstrated remarkable instability of the stress and dynamic roentgenograms. In contrast, the scaphoid cyst and trapezium OA wrists showed neither fracture nor instability and fewer changes than the other two groups. We considered the destructive changes that had taken place in the lunate collapse and the scaphoid nonunion wrists to be neuroarthropathy due to long-standing nerve palsy.

Age-dependent change in activities of lysosomal enzymes in rat brain.

The age-dependent change in activities of seven lysosomal enzymes (cathepsin D, beta-glucuronidase, acid phosphatase, acid/alkaline DNases and acid/alkaline RNases) was studied in four brain regions (cerebrum, hippocampus, pons and cerebellum) of Wistar rats. The activity of cathepsin D was significantly increased with aging in the four regions. The age-dependent change in activities of acid and alkaline DNases showed the characteristic regional difference, and the ratio of acid to alkaline DNases was increased with aging in all regions. Acid RNase showed the lowest activity in 18-month-old rats, and alkaline RNase activity was decreased with aging. The activity of beta-glucuronidase was higher in 2-month-old rats in all of the regions studied. Acid phosphatase showed no significant age-dependent change except in pons. The study demonstrated that all of the lysosomal enzyme activities do not change in parallel with aging, and that the age-dependent change showed the characteristic regional difference.

Effect of YM-09151-2, a putative D2 dopamine receptor antagonist, on dopamine metabolism in the striatum of rats.

YM-09151-2, a novel benzamide derivative, significantly increased the concentrations of dopamine (DA) metabolites, both 3,4-dihydroxyphenylacetic acid and 3-methoxy-4-hydroxyphenylacetic acid (homovanillic acid). The maximum increase in such metabolites was about 4-fold the concentration in control animals, and was observed at 2 hr after oral administration. 3-Methoxy-4-hydroxyphenylethylene glycol, a metabolite of noradrenaline, was slightly increased in its concentration exhibiting a transient effect, but 5-hydroxyindoleacetic acid was not. YM-09151-2 antagonized the decreasing effects of apomorphine, a nonselective DA agonist, and LY-171555, a selective D2 DA receptor agonist, on the concentrations of DA metabolites in the brain. In contrast, SCH-23390, a selective D1 DA receptor antagonist, did not antagonize the effects of DA agonist. These results strongly suggest that YM-09151-2 is a selective antagonist of D2 DA receptor and a candidate as a new antipsychotic agent for clinical use.

Enzyme-linked immunosorbent assay for IgG and IgM antibodies against human parvovirus B19: use of monoclonal antibodies and viral antigen propagated in vitro.

Enzyme-linked immunosorbent assay (ELISA) systems for serum IgG and IgM antibodies to human parvovirus B19 were established by utilizing anti-B19 monoclonal antibodies (mAbs) and human plasma B19 antigen. The specificities of IgG and IgM ELISA were confirmed by indirect immunofluorescence staining and Western blot immunoassay with panel sera. The series of serum specimens obtained from two B19-infected patients were examined with ELISA. The IgM antibody titers increased quickly after the onset of the symptoms and returned to a negative range after five months. The IgG antibody titers also increased just after the increasing of IgM titers and the elevated levels continued for more than a year. We also established the same ELISA systems by utilizing in vitro propagated B19 antigen and similar results were obtained.

Effect of (Asu1,7)-eel calcitonin on the prevention of osteoporosis induced by combination of immobilization and ovariectomy in the rat.

Experimental osteoporosis was induced in the rat by a combination of ovariectomy and immobilization by hemicordotomy. (Asu1,7)-eel calcitonin (CT) was administered at doses of 0.1, 0.5, 1.0, 2.5, and 5.0 MRC unit/kg, and its preventive effect on bone loss was examined. Significant bone loss was observed in the untreated control group by photo-densitometry and measurement of bone mass. CT at doses of above 1.0 MRC unit/kg significantly increased femur scores and at doses of above 2.5 MRC unit/kg resulted in significantly higher photodensity. Dry weight and ash weight were significantly higher in the 2.5 and 5.0 MRC unit/kg CT-treated groups. Administration of CT had a nearly dose-dependent effect on bone mass. Histomorphometry revealed significantly lower fractional formation surface and fractional resorption surface in the 5.0 MRC unit/kg CT-treated group. Bone mineralization was studied by the quantitative analysis of tetracycline incorporation. Tetracycline uptake was significantly lower in the groups treated with CT at doses above 1.0 MRC unit/kg. The kinetic bone formation activity studied by 45Ca and 14C-proline tracer examination, in the 5.0 MRC unit/kg CT-treated group was significantly lower than that in the control group, but not lower than that in the sham-operated group.

Granular cell ameloblastoma. A rare variant.

A rare case of granular cell ameloblastoma in the anterior mandible of a 59-year-old man has been studied by light and electron microscopy. In some areas, the tumor was very similar to an oncocytoma (oxyphilic adenoma). Almost all tumor cells were full of eosinophilic granules, whereas in the common type of granular cell ameloblastoma, only the cells located in the central portion of the tumor are granular.

IL-2 receptor subunit, p75: direct demonstration of its IL-2 binding ability by using a novel monoclonal antibody.

A new monoclonal antibody (mAb), TU11 mAb, was found to be specific for the p75 subunit of human interleukin 2 receptor (IL-2R). TU11 mAb reacted with human and Gibbon ape cell lines positive for IL-2Rp75 but not with cell lines negative for IL-2Rp75 tested. TU11 mAb specifically detected a single cell surface molecule with a molecular weight of 75 kd. The 75 kd molecule was identical to p75 detected by TU27 mAb specific for human IL-2Rp75 in sequential immunoprecipitation. TU11 mAb did not block IL-2 binding to the high-affinity IL-2R at all and precipitated p75 bound with IL-2. Using TU11 mAb, we have demonstrated here that solubilized p75 has an IL-2 binding site.

Posterior extensive simultaneous multisegment decompression with posterolateral fusion for cervical myelopathy with cervical instability and kyphotic and/or S-shaped deformities.

From June 1978 to May 1988, posterior decompression with posterolateral fusion was performed at Tenri Hospital on 63 cases of cervical myelopathy with instability and/or malalignment including kyphotic and/or S-shaped deformities. Among 46 cases followed up for more than 1 year, 41 cases (89.1%) improved; 21 cases (45.7%) were rated excellent, six (13%), good; 14 (30.4%), fair; one (2.2%), unchanged; and four (8.7%), poor. On x-ray examination, bone consolidation at the bone implanted area was satisfactory in 30 cases (65.2%). Cervical instability disappeared at 55 cervical interspaces of 40 cases (78.6%). Displacement at unstable interspace disappeared or improved at 49 of 61 cervical interspaces (80.3%). Although cervical kyphosis or S-shaped deformity could not be improved, accelerated progression of these deformities appeared to be prevented.

Characterization of monoclonal antibodies against human parvovirus B19.

Eleven hybridoma cell lines producing mouse monoclonal antibodies (mAbs) against human parvovirus B19 were established. Their specificity was as follows. Approximately 5% of fetal erythroid cells inoculated with B19 reacted with all the mAbs and with anti-B19 positive human serum, but not with negative serum by indirect double immunofluorescence staining. All the mAbs recognized both VP-1 (84 kDa) and VP-2 (58 kDa) capsid proteins of B19 virions propagated in vitro and in vivo by Western blotting, and immunoprecipitated B19 virions.

Enzyme-linked immunosorbent assay for human autoantibody to glial fibrillary acidic protein: higher titer of the antibody is detected in serum of patients with Alzheimer's disease.

We have developed an enzyme-linked immunosorbent assay (ELISA) to detect anti-glial fibrillary acidic protein (GFAP) autoantibody in human sera. The ELISA was prepared by coating microtest plates with purified GFAP from bovine spinal cord. The autoantibody activities were assayed in the serum from 219 control subjects, 39 Alzheimer's disease patients and 39 cerebrovascular dementia patients. Higher titer of the antibody was observed in the serum of Alzheimer's disease patients. Since the titer showed no significant change with aging or with sex in the control serum, we could determine a certain normal value of the antibody titer. The percentage of abnormal subjects whose antibody levels were over the normal value was 53.8% in Alzheimer's disease (presenile onset) patients, 30.8% in Alzheimer's disease (senile onset) patients, 10.3% in cerebrovascular dementia patients and 5.5% in control subjects. We discuss the relationship between the anti-GFAP autoantibody and the pathogenesis of Alzheimer's disease and suggest that the evaluation of anti-GFAP autoantibody level may be useful in diagnosing Alzheimer's disease.

Improvement of tear lysosomal enzyme levels after treatment with bone marrow transplantation in a patient with I-cell disease.

We examined the lysosomal enzyme levels in tear fluids from a patient with I-cell disease after treatment by bone marrow transplantation. Acid phosphatase, beta-D-glucuronidase, N-acetyl-beta-D-glucosaminidase, alpha-L-fucosidase and alpha-D-mannosidase activities were reduced to normal or nearly normal levels after the treatment. We believe that the measurement of tear enzyme levels in I-cell disease is useful for knowing the efficacy of the bone marrow transplantation.

Nonketotic hyperglycinemia: studies in an atypical variant.

We diagnosed a 22-year-old man with psychomotor retardation, rare seizures, hyperglycinemia, and hyperglycinuria as an atypical variant of nonketotic hyperglycinemia (NKH). Despite this clinical phenotype and a CSF/plasma ratio confirming a mild variant, measurement of hepatic glycine cleavage activity and the P-protein component indicated the more severe neonatal variant.
