Intestinal bleeding from a caliber-persistent submucosal artery in the ileum.

A 15-year-old boy developed massive intestinal bleeding. The initial mesenteric angiography and ileocolectomy failed to reveal the bleeding site. A repeat mesenteric angiography performed when bleeding recurred showed active bleeding from an ileal branch of the mesenteric artery. Histologically, the lesion proved to be a caliber-persistent submucosal artery that had ruptured. This appears to be the first report of "caliber-persistent" artery in the ileum.

Adolescent↗

Malakoplakia of the female genital tract.

A rare case of malakoplakia of the uterine cervix and the pelvis occurring in an elderly woman who also had xanthogranulomatous pyelonephritis is described and compared with the 15 reported cases of malakoplakia of the female genital tract.

Adult↗

Peritoneal carcinomatosis after prophylactic oophorectomy in familial ovarian cancer syndrome.

A woman who was at high risk for ovarian cancer because of a strongly positive family history underwent prophylactic oophorectomy, but subsequently developed and died of intraabdominal carcinomatosis. One small focus of adenocarcinoma was found on the ovarian surface in serial sections performed retrospectively on the resected ovaries. This case study emphasizes the importance of thorough histologic examination for familial ovarian cancer syndrome, including serial sectioning of the specimens removed in prophylactic oophorectomy.

Adenocarcinoma↗

Squamous carcinoma of the thymus.

A rare case of squamous carcinoma of the thymus is described and the literature is reviewed. This type of thymic carcinoma is surgically curable even in the presence of local invasion. It also appears radiosensitive with apparent cure by radiotherapy alone observed. Its prognosis is better than squamous carcinoma of the lung with mediastinal involvement, and lymphoepithelioma-like thymic carcinoma.

Aged↗

Extrarenal angiomyolipoma.

A case of angiomyolipoma arising in the anterior abdominal wall is reported and compared with other reported cases of extrarenal angiomyolipoma. None of the patients had tuberous sclerosis and no recurrence was noted following simple excision.

Female↗

Cutaneous adnexal squamous carcinoma.

A 46-year-old woman developed a squamous carcinoma of the skin that was treated with surgical excision followed by irradiation. The excised tumor showed no epidermal involvement; therefore, it was diagnosed as metastatic squamous carcinoma. However, a 4-year disease-free follow-up indicates that it represents a primary skin tumor. Since the exact histogenesis is unclear, this tumor is tentatively designated "cutaneous adnexal squamous carcinoma."

Carcinoma, Squamous Cell↗

Metastatic carcinoma in inguinal hernia sac.

A patient with unexplained low back pain, weight loss, and pulmonary infiltrate underwent an inguinal herniorrhapy. Histologic examination of the grossly normal hernia sac revealed metastatic adenocarcinoma. The patient died of cachexia 10 months later. This case observation suggests the necessity of histologic examination of grossly normal hernia sacs.

Adenocarcinoma↗

Neuromuscular hamartoma.

A rare case of neuromuscular hamartoma, a benign tumor composed of well-differentiated skeletal muscle and nerve fibers, is described and compared with four previously reported cases. The tumor arose in the major peripheral nerves. Three of the five tumors were congenital tumors. A biopsy is essential in establishing the diagnosis. However, the tumor may be left alone if it cannot be separated from the nerve with ease, since spontaneous regression has been documented.

Hamartoma↗

Bilateral malignant Brenner tumor of the ovary.

The clinicopathologic features of a case of malignant Brenner tumor with bilateral ovarian involvement are described. The tumor was apparently confined within the ovaries at initial laparotomy. However, multiple skeletal metastases developed 4 months later and the patient died of the disease 6 months after diagnosis.

Aged↗

Localized laryngeal lymphoma.

A 23-year-old man with a 3-month history of hoarseness was found to have a polypoid histiocytic lymphoma arising from the anterior commissure and the adjacent epiglottis. Work-ups including a staging laparotomy were negative. Irradiation of 4,400 rads to the larynx was given. The patient was disease-free 3 1/2 years after diagnosis.

Adult↗

Carcinosarcoma of the heart.

A unique intracardiac tumor arising in the left atrial wall is described. The patient presented with the classical clinical features of left atrial myxoma. The resected tumor was histologically malignant and showed features of a carcinosarcoma, a tumor type which has not previously been reported to arise in the heart. Histogenetically, this tumor may represent the malignant counterpart of cardiac myxoma.

Adult↗

Inflammatory pseudotumor of the liver.

The clinicopathologic features of a rare case of hepatic "inflammatory pseudotumor" are described and compared with those of three similar cases reported in the literature. In all four cases symptoms were systemic. The marked reduction in tumor size and the changing cellular composition, from a predominance of fibroblasts to a predominance of histiocytes, confirmed the reactive nature of the lesion in the present case. The etiology is obscure, and the significance of the associated occlusive phlebitis remains to be determined.

Adult↗

Multicentric Castleman's disease and Kaposi's sarcoma.

This report describes two cases of multicentric Castleman's disease associated with Kaposi's sarcoma, and reviews the literature. With the recent delineation of a multicentric variant, Castleman's disease has evolved as a disease entity with a wide clinical spectrum ranging from localized, reversible disease as originally described by Castleman et al., to a generalized disease that is rapidly progressive and fatal. The latter represents the most severe form of the disease in which the associated immune derangement predisposes the patients to infections and Kaposi's sarcoma.
