Outbreak of scombroid fish poisoning, Taiwan.

Fish implicated in outbreaks of scombroid poisoning have usually undergone prolonged storage at an inadequate temperature. We report an outbreak of scombroid poisoning in which fish stored on ice for two days caused illness after standing at room temperature for only three to four hours; fish from the same catch did not cause illness after standing at room temperature only one hour. This outbreak suggests scombrotoxins may be produced more rapidly than previously thought possible.

[Hepatitis-related viral markers in patients under long-term hemodialysis].

Antibody profiles for hepatitis B virus (HBV), hepatitis A virus (HAV), cytomegalovirus (CMV), Epstein-Barr virus (EBV) and human immunodeficiency virus (HIV) were determined on 55 serum samples collected from patients with chronic renal failure who were on long-term haemodialysis for periods ranging from 8 months to 5 years and 3 months. The exposure rates for HBV, HAV, CMV, EBV and HIV were 94.5%, 100%, 94.5%, 94.5% and 0% respectively. Among the 7 HBsAg carriers, 1 and 3 were positive for e antigen (HBeAg) and antibody to HBeAg (anti-HBe), respectively and three negative for both. These 7 carriers were also negative for anti-delta antibody. A comparison of the above antiviral profiles to those of voluntary blood donors and general population in this district revealed tht there is no difference for HBV, HAV, CMV and EBV exposure rates, VDRL, alpha-fetoprotein and CEA were also tested and the results showed no abnormalities. Only 3 patients had abnormally elevated SGOT and SGPT levels; the causes were undetermined because all of them gave positive HBV, HAV, CMV and EBV antibody profiles. In conclusion the screening of HBsAg and VDRL in the blood banks virtually eliminated possible infections of HBV and spirochate by blood transfusion and the patients with chronic renal failure who are maintained on long-term haemodialysis are generally not at higher risks of hepatitis-related viral infections.

Peritoneal papillary serous carcinoma with long-term survival.

Three cases of peritoneal papillary serous carcinoma with a complete response to chemotherapy and long-term disease free survival are reported. All three patients presented with multifocal or diffuse peritoneal involvement. Microscopic ovarian serosal surface involvement was demonstrated in two cases. The ovaries were uninvolved in one case. All patients were treated with combination chemotherapy consisting of Adriamycin (doxorubicin) and cisplatin, with or without cytoxan. All three patients survived for 5 years or longer with no clinical evidence of persistent or recurrent tumor.

Congenital hemangiopericytoma.

The clinicopathologic features of a case of congenital hemangiopericytoma are described and compared with those of previously reported cases. Features connotative of malignancy in adult or childhood hemangiopericytomas, such as high mitotic activity, increased cellularity, and tissue necrosis or hemorrhage, are frequently observed in congenital tumors. However, the congenital tumors are biologically benign. Spontaneous tumor regression observed in our case further suggests a conservative approach in the treatment of congenital hemangiopericytomas.

Localized thyroid plasmacytoma.

Two cases of localized thyroid plasmacytoma are described and the literature is reviewed. In most cases the tumors arise in thyroid glands with lymphocytic thyroiditis. Thyroidectomy with regional lymph node biopsy seems to be the initial treatment of choice. Postoperative irradiation appears to be indicated in cases with residual thyroid tissue remaining and in those with documented regional nodal involvement.

Pilomatrix carcinoma.

The clinicopathologic features of two cases of pilomatrix carcinoma are described and compared with those of the 11 previously reported cases. In contrast with its benign counterpart, pilomatrix carcinoma occurs in older patients and there is a male-predominating sex ratio. The rate of local recurrence is 60%. A single instance of distant metastasis occurred when the tumor recurred locally for the second time. Pilomatrix carcinoma can be accurately diagnosed histologically. The consistent features present in pilomatrix carcinoma but absent in the benign counterpart are increased mitotic activity and cytologic abnormalities including cellular and nuclear enlargement, and vesicular appearance of the nuclei. The primary lesion should be treated aggressively to prevent local recurrence, which may be a factor favoring distant tumor metastasis.

Female genital tract tumors in Peutz-Jeghers syndrome.

Multiple genital tract tumors, including bilateral ovarian sex cord tumors with annular tubules, adenoma malignum of the cervix, and bilateral ovarian mucinous tumors, developed in a female patient with Peutz-Jeghers syndrome. Although each of these tumor types has been reported to occur in this syndrome, the present case appears to be the first in which the syndrome was complicated by tumors of all three types.

Adenocarcinoma↗

Hepatocellular adenoma of the placenta.

A unique primary placental tumor, designated "hepatocellular adenoma of the placenta," is reported. The tumor was composed of cells resembling fetal hepatocytes. The hepatocellular nature of the tumor cells was supported by ultrastructural and immunohistochemical findings. Histogenetically, this tumor most likely represents a highly specialized monodermal teratoma.

Hemangiosarcoma complicating Paget's disease of the bone.

An 87-year-old man with Paget's disease of the left humerus was found to have a Paget's sarcoma upon pathologic examination of the left upper limb, which was disarticulated for severe pain and recurrent fractures. The histologic type of this tumor, a hemangiosarcoma, was unique among Paget's sarcomas.

Familial plantar fibromatosis.

A family in which three siblings developed plantar fibromatosis is described. This occurrence confirms the importance of heredity in the etiology of plantar fibromatosis.

Coccidioidomycosis of prostate.

A rare case of coccidioidomycosis of the prostate is described. This case appears to be the ninth case of clinically diagnosed prostate coccidioidomycosis in the literature. The presence of extragenital site involvement was associated with a high rate of mortality.

Hodgkin's disease complicating lymphomatoid papulosis.

A patient with a 14-year history of lymphomatoid papulosis developed localized subcutaneous lymphoma with morphologic features of Hodgkin's disease at the age of 46 years. The lymphoma was clinically atypical in that it remained localized in the subcutis for 10 years before involvement of lymph nodes occurred. A review of the literature indicates that patients with lymphomatoid papulosis, especially those with plaque or nodular lesions in addition to papular lesions, are given to develop malignant lymphomas of various histologic types.

Clear cell myeloma.

We report a case of clear cell myeloma misinterpreted at initial biopsy as liposarcoma. The patient had lytic skeletal lesions and monoclonal IgA, kappa, serum immunoglobulin. The tumor cells contained cytoplasmic vacuoles that produced a clear histologic appearance. The light-microscopic and ultrastructural findings are compared with those of other cases of lymphoplasmacytic disorders with prominent cytoplasmic inclusions.

Rhabdomyosarcomatous uterine adenosarcoma.

A case of rhabdomyosarcomatous adenosarcoma of the uterine cervix is described and the literature reviewed. This variant of adenosarcoma does not appear to be more malignant than other adenosarcomas. It can and should be differentiated from embryonal rhabdomyosarcoma and malignant mixed müllerian tumor because of the different prognostic implications and therapeutic approaches.
