For many young people, their aspiration for an environmentally compatible, and societally meaningful, field of activity is very marked and reflects the values they hold. Vocational rehabilitation should be oriented accordingly, and take up the motivational potential implied. The more accurate the knowledge of previous vocational experience, motivations, attitudes toward work, and of the subjective difficulties in coping with work-related requirements, the more detailed can vocational rehabilitation planning be devised. The potential is increased by a concept that is guided by insights from the psychology of work and emphasizes individual rehabilitation planning as well as follow-along developmental counselling. Assessment of 52 individual service instances for mentally ill youths and young adults, of Berlin-Kreuzberg, revealed an appropriate placement quota of 67%. The developmental course taken in the individual cases shows that satisfactory long-term outcomes can be achieved through careful entry diagnostics, individual rehabilitation planning, ongoing developmental counselling, selective educational intervention, industrial try-outs, as well as stabilizing aftercare.
Eighteen patients on chronic dialysis (15 haemodialysis, three peritoneal) were evaluated to determine whether certain clinical features and/or in vitro indicators of immune function correlated with the response to hepatitis B vaccine. Only 44% of patients developed antibody to hepatitis B surface antigen (anti-HBS) following vaccination and neither duration of dialysis, history of renal transplant, peripheral T cell numbers, T4/T8 ratios, nor mitogen induced lymphocyte proliferation correlated with response to vaccination. Advanced age, history of multiple blood transfusions and depressed levels of IgM may be associated with a poor antibody response to the vaccine. Future studies need to consider these variables when evaluating immunogenicity of new hepatitis B vaccine preparations.
Bovine antithrombin III (AT III) interaction with the luminal surface of bovine aortic segments with a continuous layer of endothelium was examined. Incubation of 125I-AT III with vessel segments, previously washed free of endogenous AT III, demonstrated specific, time-dependent binding to the protease inhibitor to the endothelium. Half-maximal binding was observed at an added AT III concentration of 14 nM. Binding of 125I-AT III to the vessel wall was reversible (50% dissociated in 4 min), and addition of either heparin or Factor Xa accelerated displacement of 125I-AT III from the vessel segment. Dissociation of 125I-AT III from the vessel segment in the presence of factor Xa coincided with the formation of a Factor Xa-125I-AT III complex. Inactivation of Factor IXa and Factor Xa by AT III was facilitated in the presence of vessel segments. Pretreatment of vessel segments with highly purified Flavobacterium heparinase precluded the vessel-dependent augmentation of AT III anticoagulant activity as well as specific binding of 125I-AT III to the vessel endothelium. In contrast, pretreatment of the vessel segments with chrondroitinases (ABC or AC) had no detectable effect on 125I-AT III binding or on AT III anticoagulant activity. AT III binding to vessel segments was competitively inhibited by increasing concentration of platelet factor 4. Binding of the protease inhibitor to vessel segments was inhibited by chemical modification of AT III lysyl or tryptophan residues. These AT III derivatives retained progressive inhibitory activity. These data suggest that heparin-like molecules are present on the aortic vessel wall and mediate binding of AT III to the vessel surface, as well as enhancing the anticoagulant activity of AT III at these sites.
Male and female mice of strains C3H/eB and C57BL/6 were injected subcutaneously with methylcholanthrene (MCA) dissolved in olive oil; mice injected with olive oil served as controls. After intervals of from 2 to 13 weeks, experimental and control mice were injected intraperitoneally with 51Cr-labeled rat red cells (RRC) and killed 24 h later. Radioactivity assayed in spleen and liver and expressed as percent of total-body radioactivity served as indicator of phagocytic uptake. Significant decreases in splenic and, to a lesser extent, hepatic phagocytosis were observed at some time intervals in all strain-sex combinations, except for female C57BL/6 mice. The phagocytic activity of untreated C57BL/6 mice exceeded that of C3H/eB mice. Phagocytosis was greater in female than in male C3H/eB mice. In the experimental design employed, no direct correlation was apparent between impairment of phagocytosis and resistance to carcinogenesis.
Assays of splenic plaque-forming cells (PFC) for sheep red cells (SRC), done in parallel in control and methylcholanthrene (MCA)-injected mice of strains C3H/eB and C57BL/6, disclosed immunodepression, the degree of which depended on strain and sex of mice, and time elapsed between injection of MCA and performance of test. The most severe and long-lasting effects were observed in male C3H/eB mice, and the least impairment was seen in female C57BL/6 mice. After surgical splenectomy for assay of PFC, MCA-injected mice were observed for appearance of induced tumors up to 52 weeks after administration of carcinogen. In most groups of C57BL/6, but only rarely in C3H/eB mice, a correlation was found in individual mice between immunodepression and appearance of tumor. Since, however, the incidence of induced tumors was higher in C57BL/6 than in C3H/eB mice, immunodepression cannot be considered a universally obligatory step in carcinogenesis, but its involvement may be limited to particular phenotypes.
A 52-year-old black man evaluated for a left cataract following an iritis was incidentally noted to have a vascularized, firm, inflammatory-appearing left caruncular mass. Removal of the cataract and of the left caruncular mass led to the discovery that the latter was composed of dilated channels containing secretory globoid bodies; the cyst wall was composed of a double layer of cuboidal epithelium, occasionally displaying apical cytoplasmic snouts. Lobules of lacrimal gland tissue were found in intimate association with the cystic spaces in multiple foci. The secretory globoid bodies had ruptured from the cystic cavity into the connective tissue of the caruncle, where a multinucleated foreign body-type granulomatous response had been elicited. This spontaneous rupture and the ensuing inflammation accounted for the inflammatory character of the lesion on clinical examination. The authors believe that secretory globoid bodies are a distinctive feature of dacryops, which normally occurs in the major lacrimal gland. When dacryops occurs in a minor lacrimal gland of the caruncle or fornices (glands of Krause or Wolfring), the presence of secretory globoid bodies may help to distinguish dacryops in these unusual locations from small conjunctival inclusion cysts, which sometimes have calcareous inclusions but lack the myriad spherical secretory bodies of dacryops.
Clindamycin HCl in a 3% formulation was applied twice daily to the back skin of three white (Yorkshire) and three black (Hampshire) pigs for 22 days. Partial thickness samples of skin were obtained by use of a dermatome on shaved opposite sides at 4 hours and 5 days posttreatment. More than twice the antibiotic activity (P less than or equal to .001) was found in thinner (254 micrometers epidermis) samples than in thicker (1016 micrometers epidermis and dermis, 1 to 3) samples. A significantly higher content of antibiotic (P = 0.015) was retained in the skin of black pigs at five days posttreatment. Groups of ten Upjohn Sprague-Dawley rats were treated similarly with clindamycin phosphate in a 3% formulation three times daily for 22 days. At termination of treatment antibiotic activity was widely distributed, whereas at five days posttreatment residual activity was confined largely to samples of treated skin. No cutaneous irritation due to treatment was observed in either study.
An unusual case of mixed bacterial osteomyelitis has been reported. The prodominant organism, E corrodens, normally is an inhabitant of the aerodigestive tract; its pathogenicity should not be overlooked in the diagnosis and treatment of oral infections. In addition, descriptions of anemia of chronic disease and of the spontaneous healing of a large defect of the bone have been presented.
Of uremic patients, 37% demonstrated the presence of CFI in their sera. The serum factor acts directly on leukotatic factors, C5f, C3f, and BF to render them irreversibly inactive. Inhibitory activity was found in five of 16 patients: 37% to 61% for C5f, 25% to 36% for C3f, and 28% to 51% for BF. There is no correlation of this inhibitory activity with BUN or creatinine levels. The presence of this chemotactic inhibitor may be responsible for the increased incidence of infection in uremia.

Fascia obtained from the abdominal wall of a rat was transplanted to the oral cavity of five dogs as a graft material for vestibular extension. In all five dogs the grafted fascia remained in place as the surgical defects were covered with epithelium, which transpired within a period of 4 weeks. After 6 months, the vestibular depth was increased significantly and the vestibule was smooth and regular.

Fascia obtained from the abdominal wall of a rat was transplanted into mucosal defects created in six dogs. In all of the dogs, the fascia was found to adhere firmly to the underlying tissue and surrounding epithelium. Biopsy specimens were taken at weekly intervals for 4 weeks. The defects healed without any apparent contracture and were covered by new epithelium. It appears that xenogenous fascia is a potentially useful graft material for intraoral soft tissues.
