Augmentation of circulation of pedicled transverse rectus abdominis musculocutaneous flaps by microvascular surgery.

The biggest problem of the TRAM flap for breast reconstruction is distal necrosis or fat lysis due to poor circulation. In order to utilise the entire TRAM flap tissue in extensive tissue defects, the contralateral rectus muscle is used as a pedicled carrier and the ipsilateral superficial or inferior epigastric vessels are anastomosed with appropriate recipient vessels in the axilla. This procedure has been performed in three cases with no necrosis, fat lysis or hardening of the flap tissue, proving adequate circulation in the flap.

Renal effects of nicardipine in patients with mild-to-moderate essential hypertension.

We studied the renal effects of nicardipine, a calcium entry blocker, in seven patients with mild-to-moderate essential hypertension. Glomerular filtration rate (GFR) and renal blood flow (RBF) were measured by means of thiosulfate and para-aminohippurate, respectively. Intravenous administration of nicardipine hydrochloride (0.5 mg) increased RBF by 26.8 +/- 5.8% (mean +/- SEM, p less than 0.01), GFR by 35.4 +/- 12.4% (p less than 0.05), and urinary excretion of sodium by 56.4 +/- 10.7% (p less than 0.05) with a significant (p less than 0.01) reduction in systolic and diastolic blood pressure as compared to control values. Nicardipine decreased total renal vascular resistance by 30.0 +/- 3.2% (p less than 0.05) from the control value, while filtration fraction remained unchanged. Our results indicate that nicardipine has several favorable renal effects with a concomitant hypotensive action in patients with mild-to-moderate essential hypertension.

[Schizophrenia in half-sisters].

Little is known about identical schizophrenic disorders in half-siblings. In this study, we report on one family with six half-siblings (from 3 different marriages of the mother) three of whom developed schizophrenic disorders. Based on psychodynamic and psychosocial viewpoints, we studied the family environment and premorbid characteristics. We also investigated the conditions and psychoreactive factors leading up to the onset or relapse of the illness. We compared the mental developments, particularly the personal relationships of the schizophrenic half-siblings with those of the normal half-siblings. We also attempted to clarify the pathological conditions on the hand and the protective factors on the other hand.

Microsurgical technique in resection of hemangioma in infants.

A microsurgical dissection technique for residual strawberry marks applied to patients 6 years of age or older (a total of 7 cases) resulted in minimal bleeding during the operations. Therefore, the same technique was used for patients aged 4 months through 3 years and 4 months. The latter 8 patients had a minimal amount of intraoperative bleeding (no tourniquet was applied to the upper arm case), direct skin closure of the wound, and no postoperative complications. The procedure is tedious but makes dissection of the hemangioma easier because of the watery subcutaneous structure in the infant; it also is less traumatic than the usual surgical dissection technique. We found this procedure to be valuable for complete removal of the unwanted strawberry mark with subcutaneous hemangioma in infants.

[Comparative research on manic conditions between monopolar mania and manic-depressive disease].

Comparing manic states in three purely manic patients on the one hand and in three manic-depressive ones on the other, the authors conclude that the phenomenological differences between the two diseases are based on different premorbid personality structures, to be more precise: on different attitudes towards their fellow human beings. And this difference is examined with respect to the question how the en-ryo between two people works.

[Apropos of 5 cases of so-called "delusions of cutaneous and intestinal infestation"--psychopathologic and neuropsychological considerations].

Five cases with so-called "Dermato- und Enterozoenwahn" are reported, and the following themes are analysed from the "multidimensional" point of view: 1) process to build the shape of the intruder which is bothering the patients, 2) behavior against the intruding small animal and attitude towards the therapeutist; their characteristic manner to make complaints, 3) premorbid personality and 4) physical findings. In regard to one of the formation types of this disease, we have postulated through the neuropsychological analysis of case 5 (somatoparaphrenic patient) that patients of the typical cases 1, 2 and 3 suffer from a special kind of agnosia (perturbation of recognition; disturbance of aperception) in which they take their abnormal body sensations for causing by the small imaginary animals. Our cases showed the importance of a premorbid personality and present life-situations in combination with physical dissolution taking part in the pathoplastic process of this particular disease.

Fibrinolytic and plasmin inhibitor activities in patients with acute myocardial infarction.

Fibrinolytic activity and plasmin inhibitor activity were determined by Blue Dextran-fibrin method in patients with acute myocardial infarction. The normal values of fibrinolytic activity and plasmin inhibitor activity were 3.34 +/- 0.74 units and 10.10 +/- 2.39 units (mean +/- SD), respectively. The fibrinolytic activity in normal women (3.63 +/- 0.99 units) was slightly higher than that in normal men (3.17 +/- 0.79 units), but the plasmin inhibitor was not significantly different in the two sexes statistically. Neither activity varied significantly with age. The blood fibrinolytic activity was depressed in the first week after acute myocardial infarction, the decrease being maximal 3 to 4 days after the event (1.73 +/- 0.31 units). The mean plasmin inhibitor activity remained normal after acute myocardial infarction, but it was increased in 3, and decreased in 4 of 7 patients in whom it was measured 3 to 4 days after the event.

A method using fibrin-fixed Blue Dextran for determining the plasmin and plasmin inhibitor activities in human plasma.

The fibrinolytic activity of plasmin was determined by incubating with fibrin-fixed Blue Dextran as a substrate, the Blue Dextran released being proportional to the plasmin activity. The applicability of this method for rapid and accurate evaluation of fibrinolytic activity was demonstrated by dose-response curves with purified plasmin, plasmin generated by urokinase in human plasma and euglobulin. The method can also be used to determined plasmin inhibitors in plasma.
