Telephone care is a flourishing subspecialty of nursing requiring adaptations of bedside nursing skills and expertise in communication and telephone competencies. Telephone health care is more than "just answering the phone," even for the most experienced nurse. Knowledge of the unique skills, techniques, protocols, and legal considerations is essential in performing safe and accurate telephone triage.
Despite the increasing participation of women in medicine over the past few decades, hindrances to professional advancement for women physicians and medical students persist. The present study sought to assess the prevalence of gender bias and sexual harassment in a sample of women physicians and medical students. Within a one-year period, 54% of respondents encountered some form of sex discrimination. In addition, approximately one-fourth experienced sexual harassment (27%) and/or discrimination related to parenthood (24%). Unwanted sexual attention not viewed as sexual harassment was experienced by more than twice as many respondents (55%). Other forms of discrimination were reported as well: age (22%), sexual preference (20%), and race (7%). Perceptions of the extent of sex discrimination and sexual harassment, of their impact on professional commitment, and of the adequacy of solutions at institutions varied directly with whether or not respondents had experienced some form of employment discrimination within the past 12 months. This study points to the need for clarification of what constitutes sexual harassment and for the creation of safe, effective mechanisms to remedy and prevent all forms of gender bias.
A review of the laboratory methods and embryo transfer procedures used by seven of the foremost clinics in the world are discussed and evaluated in this article. Based on these facts an in vitro fertilization clinic was launched in 1983. In the second part of the paper the latest laboratory methods, i.e. ovulation induction plans and the embryo transfer technique of Tygerberg Hospital, are described. One hundred and thirty laparoscopies were performed since 1 May 1984-30 November 1984. Six hundred and one follicles were aspirated, 482 oocytes were obtained, 84% of all oocytes were fertilized, 112 patients reached the transfer stage and 28 pregnancies resulted. A clinical pregnancy rate of 21,4% per transfer and 18,5% per laparoscopy resulted.

A method has been developed for derivatization of monoglycerides and diglycerides to their propyl esters by using propionic anhydride in the presence of pyridine. The derivatives are volatile and thus can be separated and identified by gas chromatography (GC). Completion of the reaction is monitored by thin-layer chromatography. Monoglyceride propyl esters were positively identified by gas chromatography and by gas chromatography/mass spectrometry. No side products were observed in the derivatization reaction which requires 35 min at 75 degrees C. GC analysis of the mono- and diglyceride propyl esters can be completed in approximately 30 min.
The precision of instruments used in 3 collaborative studies conducted within the Food and Drug Administration over a 4-year period (1981, 1982, 1984) for water activity (aw) determinations according to the official AOAC method is evaluated. Calibration responses of the instruments were tested for linearity over the aw range from 0.75 to 0.97. Average absolute percent difference between predicted and assigned aw values for the linear model ranged from 0.3 to 0.7% for a fiber-dimensional hygrometer (Abbeon) and 3 electrical hygrometers (Beckman, Rotronics, and Weather Measure). The calibration responses for another electrical hygrometer (Hygrodynamics) were nonlinear. The fiber-dimensional hygrometer yielded mean aw values and precision estimates that did not differ significantly from those obtained with the electrical hygrometers for (NH4)2SO4slush, KNO3 slush, sweetened condensed milk, pancake syrup, and cheese spread. However, the mean aw value for a soy sauce was 0.838 for the electrical hygrometers compared with 0.911 for the fiber-dimensional hygrometer. The fiber-dimensional hygrometer was affected by a volatile component(s) in the soy sauce that caused an erroneously high aw value. Pooled estimates of reproducibility (Sx) in the 3 studies were 0.008 for the fiber-dimensional hygrometer and 0.010 for the electrical hygrometers; these values were not significantly different from those reported in the study that verified the current official AOAC method.
From 1 May 1984 to 30 April 1985 177 laparoscopies were performed; 684 oocytes were obtained and a fertilization rate of 85% per oocyte achieved. There was a pregnancy rate of 24.3% per embryo transfer (ET) and 20.9% per laparoscopy after 152 patients were subjected to ET. The ET technique at Tygerberg Hospital, using the Tomcat catheter, is described. An introducer--the FT introducer--designed to overcome technical difficulties in selected patients in passing the Tomcat catheter is described. Technical problems are easily overcome with this introducer. In 26 patients (17.1%) ET was performed using this instrument and 8 pregnancies resulted (a pregnancy rate of 30.7% per ET). This pregnancy rate indicates the value of the instrument.

A variety of medical therapies require the mixing of solutions from two separate bags before use. One scenario for the mixing is to drain the solution from one bag into the other by gravity through a short connecting tube. The degree of mixing in the lower bag depends on the relative densities of the two solutions, the geometry of the two bags and the connecting tube, and the placement of the connecting tube. Solutions with densities differing by as much as 12% were mixed by draining the solution from an upper bag into a lower bag for a particular geometric configuration. The two solutions had different electrical conductivities, and the conductivity of the combined solution as it exited from the lower bag was used as a measure of the effectiveness of mixing. When the more dense solution was drained from the upper bag into the less dense solution in a lower bag, mixing was very effective. The incoming jet of high density solution entrained the low density solution. Flow visualization indicated that the incoming jet penetrated to the bottom of the lower bag, and resulting large vortical structures enhanced mixing. When the less dense solution was drained from the upper bag into the more dense solution in the lower bag mixing was less effective. The buoyancy force reduced the momentum of the incoming jet such that it did not penetrate to the bottom of the lower bag, resulting in stratification of the solutions.
