The magnitude and durability of functional improvement after total shoulder arthroplasty for degenerative joint disease.

So that patients with degenerative glenohumeral joint disease who wish to consider total shoulder arthroplasty will be better informed, we sought to document the magnitude and durability of the improvement in shoulder function after this procedure. The function of 124 shoulders with primary degenerative joint disease was documented by patient self-assessment with the Simple Shoulder Test before and sequentially after total shoulder arthroplasty performed with a standardized technique. Patients reported that they could perform 3.8 +/- 0.3 (SEM) of the 12 Simple Shoulder Test functions before surgery. The total number of performable functions was consistent at different follow-up intervals: 8.0 +/- 0.4 at 6 months, 9.5 +/- 0.4 at 1 year, 10.0 +/- 0.3 at 2 years, 9.2 +/- 0.4 at 3 years, 9.6 +/- 0.4 at 4 years, and 10.0 +/- 0.4 at 5 years. We conclude that total shoulder arthroplasty can provide substantial and durable improvement in shoulder function.

HIV infected women and women's services.

HIV/AIDS is increasing faster in women than any other segment in the population. The typical woman who is HIV infected is young, poor, a minority, and in the childbearing stage of life. Only 1 study was found that explored medical services in relation to usage and cost by men and women infected with HIV (F.J. Hellinger, 1993). The purpose of this study was to examine the types of services and the groupings of those services within facilities that were offered for women who were HIV positive. Seventeen of the 28 facilities (61%) that provide care to this population responded to a telephone survey. The research suggests that care for women who are HIV positive is fragmented, and there are access barriers to care and support. Many services that are especially important to HIV-infected women were not found or were not made available by many facilities.

The role of diacylglycerol kinase activation and phosphatidate accumulation in interleukin-2-dependent lymphocyte proliferation.

The biological effects of interleukin-2 (IL-2) were examined in T lymphocytes. IL-2 binding induced the rapid activation of diacylglycerol kinase in both cytosolic and membrane subfractions. This enzyme utilized diacylglycerol from multiple endogenous and exogenous sources for the synthesis of phosphatidic acid. Phosphatidic acid was, in turn, shown to stimulate the accumulation of c-myc mRNA and augment cellular proliferation when added to IL-2-dependent cell lines. These results link previous observations of IL-2 and glycosylphosphatidylinositol-dependent diacylglycerol production to phosphatidic acid accumulation, and suggest that diacylglycerol kinase activation is part of an intricate IL-2 signaling cascade that utilizes phosphatidic acid as an effector molecule.

The legislative process.

These are years of rapid change in health care. Nurses face many challenges and have many opportunities to participate in and influence the formation of health care policy. Nurse citizens can become very effective in their lobbying efforts, whether on the federal, state, or local level, when they are armed with information about the key players and the processes and their belief in what nursing can contribute to the health care system.

Effect of malnutrition at the time of diagnosis on the survival of children treated for cancer in El Salvador and Northern Brazil.

PURPOSE: To investigate the relationship between survival and malnutrition at the time of diagnosis among children treated for cancer in two developing countries. PATIENTS AND METHODS: We studied 443 children treated for cancer between 1995 and 1998 at two centers in San Salvador, El Salvador, and Recife, Brazil. Median age at diagnosis was 4.9 years; 283 children had leukemia and 160 had solid tumors. Z-scores were calculated for weight for age (WAZ), height for age (HAZ), and weight for height (WHZ) at diagnosis. Z scores <-2 indicated malnutrition. Patients were also stratified by low-risk disease (solid tumors: stage I, stage II, or localized; acute lymphocytic leukemia: white blood cell count <25,000/microL, no central nervous system involvement, no mediastinal mass and age >1 and <10 yrs) and high-risk disease (all other patients, including those with acute or chronic myelocytic leukemia). RESULTS: Z-scores indicated malnutrition in 23.5% (WAZ), 22.8% (HAZ), and 15.7% (WHZ) of patients. Z-score was not significantly related to overall survival rates, to survival rates analyzed by type of malignancy or risk status, or to survival rates at the end of the first month of treatment. CONCLUSIONS: We found no relationship between nutritional status and survival in these patients. This implies that future protocols for use in developing countries can be designed to provide optimal treatment intensity despite the high incidence of malnutrition.
