Coronary artery bypass patients and work status.

Retrospective surgery data were obtained from 358 coronary artery bypass surgery patients 4-22 months (mean 12.5 months) after surgery. Before surgery, 69% of the subjects were working, 10% were unemployed, and 21% were retired. At follow-up, only 58% were working and 29% were retired. Employment before surgery was the most significant predictor of work status after surgery. Other factors that were positively related to returning to work include higher presurgery income and job classification, higher pre- and postoperative ejection fraction, and subjective improvement in tolerance for physical activity. Factors that were negatively related to returning to work were age, number of chronic medical problems, presence of cardiac symptoms, and receipt of disability compensation. Reported improvement in symptoms was not related to postoperative work status. Job classification, physical demands of the job, and time out of work before surgery were also related to how soon patients returned to work. Participation in an outpatient rehabilitation program was significantly related to postoperative work status for men employed before surgery. Questions were raised regarding the role of outpatient cardiac rehabilitation in altering patient perceptions and maintaining "work habits" that might facilitate work resumption after coronary artery bypass surgery. The results suggest that multiple physical, social and psychological factors interact to influence postoperative work status.

Combined effect of cytostatic drugs and E-N-L-trimethyl lysine in healthy and transplantable tumor bearing mice.

E-N-L-trimethyl lysine (TML) decreases the toxicity of Vincristin, Cyclophosphamide and Doxorubicin (Adriamycin) when administered simultaneously to healthy mice. Simultaneous treatment of L1210 ascites leukemia bearing mice with 100 mg/kg TML and 2, 2.5, 3.2, 3.5, 4 mg/kg Vincristin or 10-15; 20 mg/kg Doxorubicin increases significantly the survival of the animals when compared with untreated and Vincristin or Doxorubicin treated mice. Repeated impulse treatment of S-180 subcutaneous sarcoma with 100 mg/kg TML and 50-100 mg/kg Cyclophosphamide results in a significantly higher surviving time and surviving rate than Cyclophosphamide treatment alone.

[Action of N-dinitrosopiperazine and N-nitrosodiethylamine on aquarium guppies].

Tumors and precancerous lesions of the gastrointestinal tract were induced in aquarium fish guppy by dinitrosopiperazine and nitrosodiethylamine dissolved in aquarium water. Nitrosodiethylamine appeared to produce a stronger blastogenic effect than dinitrosopiperazine. Since tumors develop several weeks after exposure to carcinogens, this animal species may be used for express testing nitrosocompounds for blastogenicity.

Morphologic and clinico-pathologic features of thymus tumours.

A total of 37 thymic tumours was studied out of which 29 proved to be of epithelial origin. Thymomas may be accompanied by a minimal, marked or an overwhelming lymphoid reaction, pointing to the degree of the cellular defensive mechanism. According to the findings, two main types of thymoma with minimal lymphoid reaction (light and dark cell types) can be distinguished. All the 7 light cell thymomas with minimal lymphoid reaction proved to be malignant in their clinical course. Thymomas of light cell origin with marked lymphoid reaction may be clinically benign or malignant. Thymomas with overwhelming lymphoid reaction were clinically benign. The clinical behavior of the dark cell thymomas was benign. The dark cell thymomas could be divided into subgroups with solid, lacunar and spindle cell forms. Three non-Hodgkin type thymic lymphomas were observed. The T cell origin of one of these tumours was demonstrated immunologically. All the five cases of Hodgkin type thymic lymphoma were morphologically identical to the modular sclerotic form of lymphogranulomatosis.

[Influence of chloroquine on the hexachlorobenzene-induced porphyria. Investigations in the skin, liver, and urine (author's transl)].

Rats were fed with a diet containing hexachlorobenzene (HCB) for about 60 days. At this time the porphyria was manifst as shown by significantly elevated porphyrins. Thereafter, chloroquine (CQ) was additionally given over a period of at lest 6 weeks. At the end of the experiment the urinary porphyrin excretion and the porphyrin content in lijver and skin were diminished in HCB-CQ-treated animals by about 50% compared to the HCB controls. The relative porphyrin distribution pattern was not influenced by CQ. In a further investigation the prophylactic effect of CQ could be demonstrated. Rats given CQ simultaneously from the beginning of the HCB feeding showed a significantly delayed onset of porphyria. It is concluded from our results that CQ does not only form complexes with porphyrins during the treatment of th HCB porphyria. We rather assume an effect of CQ on the metabolism of iron.

Hepatic lesions in Lebistes reticulatus induced by N-nitrosodiethylamine.

The effect of N-nitrosodiethylamine (NDEA) on the aquarium fish, Lebistes reticulatus (guppy) has been investigated. The aquarium water contained 10 to 200 parts per million of NDEA. The LC50/30 for NDEA was 100 ppm. Sub-chronic experiments were carried out using 25 and 50 ppm for 90 days. The liver alterations were investigated morphologically. Initially, diffuse necrosis of hepatic cells, disorientation of hepatic structure and cyst-formation appeared. The degenerative lesions were followed by multiple foci of regenerating hepatic cells. In the later period of the experiment, hyperplastic nodules of hepatocytes, cholangiofibrosis, cystic cholangiomas, hepatocellular and cholangiocellular carcinomas could be observed in the treated animals. The following conclusions have been reached: 1. Aquarium fish can be used as a model for short-term, in vivo carcinogenesis experiments. 2. Lebistes reticulatus is a suitable strain for use as a positive control with NDEA. 3. With this model, the process of carcinogenesis can be followed histologically. 4. The process is irreversible and some of its features may be used as an early indicator in the screening of possible carcinogenic substances.

Enzyme induction in man: a study of the inducible systems of drug elimination.

The individual enzyme inducibility was studied with methods of menthol loading and sulphobromophthalein in groups of patients with low and average metabolism. Both methods are suitable for testing inducible conjugation systems by providing indirect information on the rate of drug elimination. In the low-metabolism groups the response in per cent correlated inversely with the initial value, and the changes were significant not only in the low but also in the average-metabolism group at the end of 30 days of phenobarbital treatment. The results are discussed with regard to the avoidability of undesired drug effects.

Enzyme-inductive effect of a hypolipidemic compound N-bis-(p-chlorophenoxy)-acetyl-urea in man and rat.

Besides its antilipidaemic effect, the new clofibric acid derivative (N-bis-(p-chlorohenoxy)-acetyl-urea) has an enzyme-inductive effect. The drug was administered (100 mg/kg orally) to male, Wistar rats for three days. The treatment raised the weight of the liver, the content of liver microsomal protein and cytochrome p-450 and shortened the hexobarbital sleeping time. The increase of cytochrome p-450 dependent biotransformation was found by in vitro methods in 9000-g supernatant of liver homogenate. There was a growth in biotransformation of substrates of type I (ethylmorphine, aminopyrine) and an extreme increase in reduction of nitrobenzene. We did not find any change in biotransformation of the type-II substrate aniline. In 16 patients suffering from Gilbert's syndrome, there was a decrease in the level of serum bilirubin, and increase of D-glucuric-acid output in urine and bromsulphophthalein transport maximum following the treatment of this drug given in 150 mg/day orally for three weeks. After this treatment, the level of gamma-glutamyl-transpeptides did not change. The authors highly recommend the serious consideration of metabolic interaction during the clinical application.
