Cardiovascular manifestations of Fabry's disease were studied clinically in 10 hemizygous males and 13 heterozygous females. Mitral valve prolapse was found in 5 of 9 hemizygotes and in 5 of 13 heterozygotes examined by echocardiography. Ordinary medical examinations revealed cardiomyopathy in some asymptomatic females, and the diagnosis of the Fabry heterozygote was established by demonstration of specific inclusion bodies in the biopsied myocardium and low alpha-galactosidase activity in leukocytes. Renovascular hypertension of juvenile onset and thromboembolism were also found in 7 patients. It was concluded that Fabry's disease should always be considered in cases of mitral valve prolapse, cardiomyopathy, renovascular hypertension and thrombosis of unknown etiology, and that the Fabry patients should be followed carefully for the early detection of cardiovascular involvements in this disease.

In order to perform the dexamethasone suppression test (DST) with saliva as an alternative to serum, we assayed directly the cortisol concentrations in 25 microliters saliva samples, using a commercial radioimmunoassay kit for serum cortisol with minor modifications. Cortisol in saliva showed a diurnal rhythm parallel to that of cortisol in serum samples collected simultaneously. Saliva cortisol levels increased significantly after ACTH injection, but with a 60 min delay in reaching their peak compared to peak serum cortisol levels. The increase in saliva cortisol was five-fold, while that in serum was two-fold. Saliva cortisol levels continued to increase in some subjects while serum total cortisol levels already had begun to decline. In those subjects, the correlation of saliva with serum cortisol was greater when a quadratic curve was fitted than when calculated for a linear correlation. Considerable variation was observed for within-subject correlations, ranging from + 0.48 to + 0.999. The DST with saliva sample collection was performed on 43 inpatients with affective disorders. Sensitivity, specificity and diagnostic confidence of the DST for major depressive episode with melancholia were 33%, 91%, and 78%, respectively, at the criterion value of 0.3 microgram/100 ml for saliva cortisol, which are similar to those most often reported for the DST with serum cortisol determination. These results indicate that saliva cortisol levels do not always parallel serum cortisol levels and thus are not an unequivocal substitute. The findings for the DST in psychiatric patients, however, support the practical clinical usefulness of saliva cortisol measurements.

A cellular analysis of 330 middle ear effusions (MEEs) from children with otitis media with effusion (OME) was performed with the use of cytocentrifuge prepared smears and the results were compared with their clinical data. Chemotactic activity in MEE was also determined with the use of agarose assay and compared with their cytological results. In the acute group the cellular pattern of MEE was mainly the neutrophil dominant type which was observed in 60% of this group. In the chronic group the percentage of this type of MEE decreased but was still present in 25% of them. High chemotactic activity was frequently observed especially in neutrophil dominant type MEE.
Although prostaglandins (PGs), especially PGE2 and PGF2 alpha, are thought to be associated with inflammation, there have been no papers dealing with the synthesis of PGE2 and PGF2 alpha in human tonsillar lymphocytes. The features of PG synthesis in lymphocytes were investigated by incubating cells with radiolabelled precursors. Synthesis of PGE2 was greater than that of PGF2 alpha in tonsillar lymphocytes. PGE2 was the predominant product in the tonsillar lymphocytes of the recent infection group. There was no significant difference between T and B lymphocytes in the ratio of PGE2 and PGF2 alpha formed. A significant increase in PGE2 formed with bradykinin was found in the tonsillar hypertrophy group. The possible role of PGs in the pathogenesis and mechanism of releasing free fatty acid PG precursors from cell membranes was discussed.
In general, subglottic stenosis in children is one of the most difficult problems to treat surgically. Six patients, aged 4-9, requiring a tracheostomy due to severe subglottic stenosis are reviewed. Decannulation was difficult to perform in these cases. All children in this series underwent surgical correction of subglottic stenosis. Patients had been successfully decannulated with no evidence of recurrent stenosis and were followed up for a period of 2-9 years. Our cases which were followed up for 2-9 years address the issue raised by opponents of surgical correction of subglottic stenosis in children, namely the consideration that the laryngeal growth potential may be adversely affected by surgery.

We have experimentally developed a photoperiod-controllable clean rack for SPF rats. The effectiveness of this clean rack in preventing the infection was attested to by the fact that the SPF rats and nude mice housed in this rack maintained SPF conditions after six months. And we investigated circadian rhythms of water intake and blood corticosterone levels in rats kept in the rack under various lighting conditions. Both rat groups under diurnal and reversed lighting conditions manifested significant increase in both water and blood corticosterone levels, showing a reversed phase relationship of the circadian rhythms between the two groups. These facts indicate the usefulness of the rack newly developed in the studies on circadian rhythms, reproductive physiology and behavior.

The translocation of indigenous bacteria from the gastrointestinal tract to the mesenteric lymphnodes was compared in ten strains of mice. Indigenous Escherichia coli were cultured from the mesenteric lymphnodes of only two of the six mouse strains examined. Thus, spontaneous translocation of indigenous enteric bacteria across the intestinal barrier did not occur to any significant extent in any of the mouse strains examined. Since bacterial overgrowth in the gastrointestinal tract promotes bacterial translocation, bacterial translocation was tested in ten mouse strains including B10 series after antibiotic-decontaminated and subsequent colonization with streptomycin-resistant E. coli C25. E. coli C25 populated the ceca of the mice at levels of 10(8) to 10(9) per gram and translocated to 90-100% of the mesenteric lymphnodes with mean of 10(1.13) to 10(1.86) per mesenteric lymphnode. However, there were no significant differences between mouse strains as to the translocation incidence or the numbers of viable E. coli C25 per mesenteric lymphnode. Thus, genetic differences between mouse strains did not influence bacterial translocation from the gastrointestinal tract to the mesenteric lymphnodes.
A total of 1,278 strains of Streptococcus faecalis, 688 of Str. faecium and 796 of Staphylococcus epidermidis were collected from laboratory animals of 20 colonies in 1980, and tested for their resistance to 20 drugs. Thirteen percent of Str. faecalis, 12% of Str. faecium and 25% of Staph. epidermidis were drug resistant. Multiple drug resistant organisms were frequently detected in laboratory animal colonies where antibiotic treatment had been undertaken. TC-resistant streptococci and EM-resistant Staph. epidermidis were distributed in about half of the colonies regardless of the history of antibiotic usage.

Human lymphoid cell lines were established from the peripheral lymphocytes of persons with different P blood group phenotypes by in vitro transformation with EB virus. The glycosphingolipid compositions of these cell lines were examined by TLC. The results show that P1k and P2k phenotype cells lack globoside (P antigen) and that two cell lines established from persons with the p phenotype lack both globoside and CTH (Pk antigen), while both the glycosphingolipids were detected in two cell lines established from persons with usual phenotypes (P1 or P2 phenotype). CTH synthetase activity was detected at decreased levels in two p phenotype cell lines, however, globoside synthetase activities could not be significantly detected in all the P phenotype cells.
Single radial hemolysis (SRH), neutralization (NT), and hemagglutination inhibition (HI) tests were carried out on sera from horses immunized against the Prague and Miami strains of equine influenza virus. The HI and NT tests demonstrated good sensitivity; the sensitivity of the SRH test was somewhat lower. The NT titers of individual sera were correlated very closely with the HI titers, although the NT titers were higher. SRH zone diameters of individual sera also showed significant correlation with the NT and NI titers. The SRH test appears to be suitable for large-scale serological surveys and offers the advantages of rapidity and simplicity.
