Comparison of two techniques of suturing in microsurgical anastomosis of the rabbit oviduct.

Microsurgical reconstruction of the rabbit oviduct was undertaken utilizing either through-and-through sutures including the mucosa, or sutures penetrating the serosa and myosalpinx but not the mucosa. Patency and pregnancy rates did not seem to vary with the suturing technique. However, scanning electron microscopic observation revealed abnormal mucosal fold patterns at 3 weeks after surgery. Fibrinous exudates over the surface of the intraluminal sutures increased with time until the entire surface was thickly veiled by 9 weeks. The observation also revealed regeneration of epithelial cells along the suture. These cells included many abnormal forms such as giant cells and misshapen cells. The majority of these were nonciliated. In theory, the intraluminal suture could form a nidus for epithelial hyperplasia which might cause future tubal obstruction. However, the patency and pregnancy rates obtained with through-and-through suturing were similar to those obtained when the endosalpinx was excluded, both in this study and in previous studies from these laboratories.

Effect of clomiphene citrate on serum prolactin in infertile women with ovarian dysfunction.

Serum prolactin (PRL) levels were determined by radioimmunoassay in 12 normal, menstruating women and in 26 infetile women with ovarian dysfunction for one cycle or for about 30 days in amenorrheic women. Galactorrhea was not observed. No significant change in serum PRL levels was observed throughout the normal, menstrual cycle. Of the 26 patients with ovarian dysfunction, seven showed PRL levels higher than normal. Twelve of the 26 patients were treated with clomiphene citrate (Clomid), 100 mg, daily for 5 days. Ovulation occurred in seven, and pregnancy was achi-ved in two of them. Serum PR;, follicle-stimulating hormone, and luteinizing hormone were measured for a preceding control cycle and during the cycle following Clomid treatment in the 12 patients. The serum PRL levels were normal during the control cycle in five of the patients who ovulated with Clomid and high in four patients who failed to ovulate. Although serum PRL levels were not significantly changed by Clomid in the patients who ovulated with the drug, they were markedly decreased during and immediately after Clomid treatment in patients who failed to ovulate with Clomid.

Studies on drug-induced lipidosis: VII. Effects of bis-beta-diethyl-aminoethylether of hexestrol, chloroquine, homochlorocyclizine, prenylamine, and diazacholesterol on the lipid composition of rat liver and kidney.

4,4'-Bis (beta-diethylaminoethoxy)-alpha, beta-diethyldiphenylethane (DH), which had been shown to induce a type of lipidosis resembling Niemann-Pick disease, was given to rats at a dose of 20, 50, 100, and 150 mg/kg body weight per day for 1 or 2 weeks. An enlargement of the liver with marked increases in free cholesterol, total phospholipids, and phosphatidylinositol took place by administration of a larger dose. The increase in bis (monoacylglyceryl) phosphate (BMGP), which is peculiar to this kind of drug-induced lipidosis, was dependent upon the dose of the drug as well as the length of time. Similar changes were also observed in kidney. Among several other drugs tested, chloroquine and diazacholesterol brought on as much increase in BMGP as treatment with DH.

An eosinophil chemotactic factor present in blister fluids of bullous pemphigoid patients.

Tissue eosinophilia is often found in the inflammatory lesions of bullous perphigoid. A study made on naturally occurring eosinophil chemotactic activity in the blister fluids of four bullous pemphigoid patients revealed the existence of this activity in all of them. Sephadex G-25 column chromatography showed that the greater part of this eosinophil chemotactic activity was composed of low molecular substance of which the weight was close to that of vitamin B12 (m.w. 1357). The blister fluids and the sera of these patients contained elevated levels of IgE. An IgE anti-skin basement, membrane antibody was found in two if the four sera, and deposits of IgE were detected along the basement membrane zone of the involved skin in one of the patients. On the basis of these findings, we have reason to believe that an eosinophil chemotactic factor of anaphylaxis (ECF-A) participates in the accumulation of eosinophils in the lesions of bullous pemphigoid.

Critical flicker frequency (CFF) and subjective fatigue during an oxyhelium saturation dive at 62 ATA.

Two divers spent over 50 hours at 610 msw in a helium-oxygen mixture (PO2:0.38-0.52 ATA). The dive duration was 27 days, including predive stages of confinement, compression, time at maximum pressure, and decompression. The divers were asked to answer 30 questions on their feelings of mental and physical fatigue and to indicate on a nine-point scale their estimation of a general feeling of fatigue. Subjective feelings of fatigue reported in this dive suggested that the divers were in good condition. Hyperbaric arthralgia and physical complaints were reported, especially during decompression, with some postdive persistence, but they should be considered as distinct signs of feelings of fatigue. The critical flicker frequency (CFF), measured throughout the dive for the two divers, showed systematic variations and a relationship between compression and pressure. These variations were grossly parallel to EEG modifications reported in other studies and probably reveal neurophysiological troubles that were not apparent from subjective reports.

Effect of CB-154 (2 Br-a-ergocryptine) on serum follicle stimulating hormone, luteinzing hormone and prolactin in women with the amenorrhoea-galactorrhoea syndrome.

Four women with the amenorrhoea-galactorrhoea syndrome were treated with 2-Br-a-ergocryptine (CB-154). In 3 patients, lactation was inhibited and ovulation was induced. Furthermore, pregnancy was achieved in one of them. In the remaining case, neither was lactation inhibited nor ovulation induced. Serum follicle stimulating hormone (FSH), luteinizing hormone (LH) and prolactin (PRL) were measured sequentially in the 4 CB- 154 treated women. They showed high serum PRL levels and acyclic patterns of serum FSH and LH. CB-154 suppressed PRL secretion and stimulated FSH and LH secretion in 3 cases where lactation was inhibited and ovulation was induced. In the one unsuccessful case, serum PRL decreased only slightly and the serum FSH and LH showed no marked changes.

Multiclinical open studies on the effect of beta-methyldigoxin on congestive heart failure with atrial fibrillation.

Clinical open trials of beta-methyldigoxin were carried out in 15 institutions in order to examine the effect, usefulness and ease of its oral administration. In the case of oral digitalization with 0.2 mg, 3 times daily, an effect was obtained in all of 13 cases of congestive heart failure accompanied by atrial fibrillation or flutter. The average time and dose required for digitalization were about 50 hours and 1.27 mg respectively. In 9 of the 13 cases, the effect was achieved within 48 hours. The average maintenance does of beta-methyldigoxin in 102 cases of congestive heart failure with atrial fibrillation was 0.177 mg per day. About 75% of the cases were maintained with 0.15 to 0.2 mg. This range of dose of beta-methyldigoxin was much smaller than that of digoxin in our series. This can be ascribed to a higher absorption rate of beta-methyldigoxin from the digestive tract. Studies on the cases in which patients previously treated with other glycosides were switched over to beta-methyldigoxin revealed that 1 mg of beta-methyldigoxin is equivalent to 1.8 mg of digoxin or to 0.59 mg of digitoxin. The usefulness and ease of beta-methyldigoxin in maintenance was evaluated as being somewhat superior to other cardiac glycosides, according to the global judgement of the physicians. The observed side effects were similar to those of other glycosides in frequency and character.
