Comparison of plasma oxytocin levels during spontaneous labor and labor induced by amniotomy, prostaglandin F2 alpha, and prostaglandin E2.

Plasma concentrations of oxytocin in nine spontaneous labors (group 1), nine amniotomy-induced labors (group 2), six prostaglandin F2 alpha-induced labors (group 3), and five prostaglandin E2-induced labors (group 4) were determined at the stages of 1 to 7 days preceding the onset of labor, onset of labor, and established labor, by means of unextracted radioimmunoassay. The levels of oxytocin in the stage preceding the onset of labor in group 1 were not significantly different from the levels of oxytocin in the corresponding stages in groups 2, 3, and 4. However, the levels of oxytocin in the stages of onset of labor and established labor in group 1 were significantly higher than those in the corresponding stages in groups 2, 3, and 4. These results suggest that oxytocin plays a leading role in the onset and progress of spontaneous labor, whereas something other than oxytocin might be involved in the labor induced by amniotomy alone or by prostaglandins.

Effects of estrogen and progesterone on thymidine kinase activity in the immature rat uterus.

The effects of progesterone and/or 17 beta-estradiol on thymidine kinase activity and autoradiograms were investigated in immature rats. Thymidine kinase activity increased more than thirtyfold above the control level 30 hours after 17 beta-estradiol injection. The enzyme activity induced by 17 beta-estradiol was suppressed by progesterone, the dose of which was approximately 1,000-fold that of 17 beta-estradiol. The specific thymidine kinase isozyme, which was separated from 17 beta-estradiol-induced uterine thymidine kinase by diethylaminoethyl (DEAE) cellulose column chromatography and not affected by deoxycytidine triphosphate, was involved in the DNA replication and inhibited by progesterone. The autoradiogram revealed many grains due to 3H-thymidine in the endometrial epithelium, stroma, and the myometrium in the immature rat 30 hours after 17 beta-estradiol injection, whereas progesterone reduced remarkably the number of grains induced by 17 beta-estradiol in the epithelium. Progesterone seems to inhibit the increment of the specific thymidine kinase isozyme induced by 17 beta-estradiol in the endometrial epithelium.

Long-term toxicity and carcinogenicity test of ammonia-process caramel colouring given to B6C3F1 mice in the drinking-water.

Caramel colouring (ammonia process) was given at levels of 0 (control), 1.25 and 5.0% in the drinking-water to groups of 50 male and 50 female mice for 96 wk, and then all all the animals were maintained without caramel for a further 8 wk. Males given 5.0% caramel showed increased cumulative mortality from wk 100 to the end of the experiment. The white blood cell count in treated males was significantly elevated in a dose-related manner. However, these changes were not considered to be biologically significant. There were no treatment-related effects on clinical signs, body or organ weights, results of urine analyses, or histological features. Therefore, this study did not demonstrate any carcinogenic effect of caramel on mice at levels of up to 5.0% in the drinking-water.

Needle tract implantation of hepatocellular carcinoma after percutaneous liver biopsy.

The authors report a case of needle tract implantation of hepatocellular carcinoma following percutaneous biopsy of the liver. The patient, a 62-year-old Japanese male, was found to have a small hepatocellular carcinoma diagnosed by needle biopsy; this was followed by transcatheter chemoembolization, a combination of transcatheter arterial infusion of adriamycin and transcatheter arterial embolization of Gelfoam, and right lobectomy. Eight months after the biopsy and the lobectomy, a nodule of hepatocellular carcinoma, measuring 3.5 cm in diameter, was found at the site of the previous biopsy and excised. The patient is doing well without further recurrence of tumor 23 months after the lobectomy and 15 months after the excision of the implanted tumor in the subcutis.

[Laboratory and clinical studies on latamoxef in the field of obstetrics and gynecology].

Latamoxef (LMOX) is a new antibiotic synthesized by Shionogi Research Laboratory. Chemically LMOX is especially unique with a sulfur atom replacing the oxygen atom in the 1 position of the conventional cephalosporin nucleus, and in addition, this antibiotic has a cephamycin-like structure. The antibacterial activity of LMOX shows high potency against Gram-negative bacteria, but tends to be weak against Gram-positive bacteria. The tissue levels of LMOX in humans after intravenous injection of 1 g were examined. The levels in uterine and adnexa uteri tissue at 1 hour after administration were 25.4 and 27.4 micrograms/g respectively. LMOX was administered to 147 cases in infections of obstetric and gynecological field. The clinical effect according to disease was 94.6% for intrauterine infections, 95.0% for adnexitis, 87.0% intrapelvic infections, and 100% for external genital organ infections, making a total of 92.5%. The rate of occurrence of side effects or abnormal laboratory findings was similar to or slightly less than that seen with other beta-lactam antibiotics.

[Japanese-applied melodic intonation therapy for Broca aphasia].

Since Albert et al. (1973) published Melodic Intonation Therapy (MIT), this new technique for Broca type aphasics has won a good reputation because of its remarkable efficiency. Many improved cases have been reported. On the contrary, there seems to be no study on MIT in Japan. This gap may be mainly caused by the great difference between two languages which makes difficult to apply original MIT to Japanese language. Through a comparative study of two languages, we have made some amendments on original MIT, which just fit for Japanese. MIT is designed to generate verbal output by embedding target phrases and sentences in a simple, nonliguistically loaded melody pattern. The pattern is consisted of the melody line, the rhythm, and points of stress. Now let us explain our Japanese version of MIT concerning these three elements. As for melody line, our version has only two pitches; high and low, since Japanese language has a pitch accent which is decided in each word. As a result, the melody line of Japanese version is far more simple than the original one, which has four pitches. The rhythm of English is called "stress-timed rhythm", which means that a part between two syllables that have primary stresses tends to be spoken at regular intervals regardless of the number of syllables between them. Consequently, the tempo of speech varies depending on the number of syllables between them. On the other hand, Japanese has "syllable-timed rhythm", which means isosyllabic.(ABSTRACT TRUNCATED AT 250 WORDS)

Effects of human fibroblast interferon on human tumors transplanted into nude mice: sensitivity of malignant melanoma.

The antitumor effects of human fibroblast interferon (HuIFN-beta) on three lines of human malignant melanoma (AM-1, SK-14, SK-2) were studied using a nude mouse-human tumor xenograft system. The sensitivity of melanoma to interferon in relation to melanin productivity was investigated. Intratumoral administration of 6 X 10(5) IU of HuIFN-beta significantly inhibited proliferation of AM-1 and SK-14, but did not inhibit that of SK-2. The sensitivity to HuIFN-beta was in the order of SK-14, AM-1 and SK-2, and was well correlated with the susceptibility of cell division observed histologically. Although these tumors differ in production of melanin, difference in sensitivity to HuIFN-beta due to melanin productivity was not clear. The relationship between the antitumor effect and the administration method was studied with SK-14, which was the most sensitive to HuIFN-beta. Antitumoral activities depending on the routes of administration were in the order of intratumoral, peritumoral-subcutaneous and intraperitoneal. Intratumoral and subcutaneous administrations of high doses brought about complete disappearance of the tumor cells or decrease in size of the tumor mass.

[Evaluation of patients with ischemic heart disease by exercise thallium-201 myocardial imaging: comparison with coronary arteriography and graded treadmill exercise testing].

One hundred and eight patients with suspected ischemic heart disease were evaluated by exercise thallium-201 myocardial imaging (T1-IM), coronary arteriography and graded treadmill exercise test (GTX). They were divided into four groups; Group I consisted of 21 patients without significant coronary artery disease (less than 75% diameter stenosis), Group II of 47 patients with significant coronary artery disease (greater than or equal to 75% diameter stenosis) but without previous myocardial infarction, Group III of 24 patients who had previous myocardial infarction with additional coronary artery disease, and Group IV of 16 patients who had previous myocardial infarction without additional coronary artery disease. In Group I, T1-IM showed positive findings in only two patients (9.5%), whereas GTX showed positive findings in seven (33.3%) and borderline findings in four (19.0%). In Group II, T1-IM showed positive findings in 34 patients (74.5%), while GTX showed positive findings in 31 (66.0%) and borderline findings in 12 (25.5%). In Group II, the sensitivity of T1-IM and GTX for identifying severe stenosis (more than 99%) were 95.0% (19 of 20 patients) and 75.0% (15 of 20), respectively, but only 50.0% (four of eight) and 50.0% (four of eight), respectively, for identifying less severe stenosis (about 75%). In Group III, the sensitivity for detecting infarcted lesion by T1-IM was 95.8% (23 of 24 patients), but the sensitivity for detecting ischemic area in the non-infarcted lesion was 41.7% (10 of 24). In Group IV, a significant defect was detected by TI-IM, corresponding to the infarcted site in 14 of 16 patients (87.5%), and periinfarcted ischemia was shown in seven of the 14 patients (50.0%). In both Group III and IV, GTX was positive in only 50% and 31.3%, respectively. We conclude that T1-IM is a useful technique in the diagnosis of myocardial ischemia, especially severe coronary stenosis, and is more specific than GTX. However, we must be careful for evaluating ischemic, but not infarcted lesion by T1-IM in patients with myocardial infarction.

"Nonalcoholic steatohepatitis" induced by massive doses of synthetic estrogen.

We have noticed that functional disorders of the liver characterized by hepatomegaly and an increase in serum gamma-glutamyl transpeptidase develop in patients with prostatic cancer who are placed under longterm therapy with massive doses of estrogen after castration. We performed laparoscopy in six cases of prostatic cancer with hepatomegaly so that we could study the morphology of the liver. Our findings were as follows. In five, the histological features of the liver biopsies were very similar to those seen in alcoholic hepatitis. In spite of this fact, two of the five had no history of alcohol consumption. Furthermore, in one other case, liver damage resembling alcoholic hepatitis developed during abstinence. The findings in these three cases suggested that long-term, massive doses of synthetic estrogen may lead to liver injury similar to alcoholic hepatitis in nonalcoholics. The ultrastructural findings of the liver cells were also suggestive of the adverse effect of treatment. All cases were negative for hepatitis B surface antigen. Recent reports have demonstrated some nonalcoholics with histological features of the liver indicative of alcoholic hepatitis. This particular condition was termed "nonalcoholic steatohepatitis" by Ludwig et al. It is quite likely that synthetic estrogen is also responsible for "nonalcoholic steatohepatitis" when it is used in massive doses.
