4000 male probands aged 45 to 69 were investigated in a field trial according to random distribution for undetected intestinal bleeding with the Haemoccult - method. With a participation of 28% only 39 out of a total of 65 positive cases could be evaluated. Four cases of colon or rectal carcinoma and four cases of colonic polyps were ascertained. In view of complete lack of experience with false negative test results the Haemoccult test is at present a useful addition to the diagnostic possibilities. However, in suspected cases even a repeatedly negative test cannot replace complete colo-rectal investigation.

The concentration of selenium in human red cells was measured by a fluorometric determination method after wet digestion of the biological material. In the red cells from the same blood samples the activity of the selenoenzyem glutathione peroxidase (EC 1.11.1.9) was determined with tertiary butyl hydroperoxide as acceptor substrate. The results show that only 10% of the total selenium content of human red cells is fixed to the enzyme. No correlation between the enzyme activity and the selenium concentration could be found. On the basis of these results can be presumed that the protective effect of selenium in glutathione peroxidase against oxidant damage of cells is not the only biological function of this element.

This article offers criteria for the surgical treatment of pituitary-dependent Cushing's syndrome. Patients (n = 6) who showed no secondary complications of Cushing's disease underwent an implantation of 90Y and/or 192Ir into the pituitary gland. After implantation the excessive secretion of ACTH was suppressed to normal values and the symptoms of Cushing's syndrome disappeared. Following operation several patients received substitute therapy with other pituitary hormones. Fertility was not always conserved. Total adrenalectomy as alternative therapy was performed in adults with threatening symptoms of the disease (n = 4) and in children (n = 4). The symptoms of Cushing's syndrome disappeared. In two cases the development of a Nelson's tumor was observed. One was treated by implantation of 192Ir into the pituitary gland; the second needs no therapy up to now.
A combined xylose-lactose tolerance-test and a duodenal biopsy were performed in 68 children with suspected malabsorption-syndrome. The purpose of the present work was to assess the diagnostic value of xylose concentrations in blood at different times and to determine the additional discriminatory value to glucose levels. The contribution of the glucose rise to a calculated discriminant function is statistically significant but practically negligible and therefore does not justify its determination. A small-bowel biopsy is recommended if the concentration of xylose after 60 min is less than 26 mg/100 ml or the increment of xylose concentration above fasting level is 18 mg/100 ml or less.
The in vitro activity of doxycycline and minocycline (Klinomycin) was determined by serial dilution test in 100 strains of E. coli, 101 strains of enterobacter, 91 tetracycline-sensitive and 52 tetracycline-resistant strains of staphylococci. Only staphylococci were more sensitive against minocycline than against doxycycline whereas other species showed nearly the same sensitivity against both antibiotics. After i.v. infusion of 200 mg minocycline (during 1 h) mean serum levels fell from 3.5 mug/ml to 0.6 mug/ml (after 24 h). Half-life was calculated as 15.7 h, urine recovery as 5.9%. After oral application of 200 mg minocycline serum level peaks were 2.7 mug/ml, serum levels after 24 h 0.7 mug/ml. At repeated administrations daily dosage of 100 mg was too low of 200 mg sufficient to obtain the same serum levels as after the initial dose of 200 mg. CSF levels after oral administration of 0.4 g minocycline were 0.74 +/- 0.09 mug/ml (in serum at the same time 2.2 +/- 0.2 mug/ml). Half-life of minocycline in chronic renal failure (7 adult patients) was not prolonged (15--20 h). Minocycline is especially suitable for treatment of infections of unknown bacterial origin (including such caused by staphylococci). I.v. infusion is indicated only in unconscious or vomiting patients.
Hundred non-hospitalized smokers with an average consumption of 20 cigarettes per day were treated with an alcoholic extract of Avena sativa for disaccustoming. The study was double blind. By using query-sheets personal contact was excluded. The first result was a placebo-effect of 35% for disaccustoming of smokers and no statistically significant effect of the extract of Avena sativa. The second result was a difference of disaccustoming between light and heavy smokers. It was shown, that the rate of disaccustoming was higher for light smokers than for smokers with a high consumption of cigarettes.
Local toxic reactions from adrenaline following nerve block anaesthesia have been observed by many workers. Because of this potential danger we studied the changes of anaesthetic solutions containing adrenaline during a longer storage period. The measurements show a drop in the pH of the solutions caused by decomposition of adrenaline. This effect of storage is not detectable with the naked eye but makes analytical procedures necessary. Molecular oxygen and light decompose the solutions rapidly. After a storage period of some months the acidity of the solutions is comparable to concentrated acetic acid, a fact which could lead to local complications.
The technique of intraoperative autotransfusion (AT) is being used to date only in a few individual clinics. Since there is a clinically safe and economical AT-apparatus available the routine application of this rational blood replacement method is easy to perform. The authors have used this technique altogether in 111 patients, applying the Bentley-ATS-machine in the last 69 cases. In a previous series of 42 cases AT was used for ruptured ectopic pregnancy, in the series of 69 surgical cases for hemothorax or intraabdominal hemorrhages of mainly traumatic origin. The most frequent indications for AT in emergency surgical operations were ruptures of spleen and liver, and in elective surgery for portocaval shunt. Altogether 247 litres of blood have been retransfused with an AT-volume per patient ranging from 0,5 to 15 litres. For anticoagulation generally ACD was used, only in vascular surgery was heparin preferred. Methodical complications have not been seen. Technique, indications, consequences and possible complications of AT are described. The main advantages of autologous intraoperative transfusion are the immediate availability of blood, the absence of the risk of hepatitis and of incompatibility reactions, reduction of pressure on the blood banks and lower transfusion costs. The authors therefore believe that the possibility of intraoperative AT should exist at every surgical and gynecological clinic.

In young adults i.v. injection of 200 mg doxycycline was followed by a rapid fall in serum levels in the first 30 minutes and than a slow decrease (serum concentration after 24 h on average 1.0 mug/ml). Biological half-life was 11.9 h, final distribution volume 50.0 litres and urine recovery 70%. After oral administration of 200 mg doxycycline the serum peaks measured were 3.4 mug/ml (2 1/2 h later). With repeated doses of 100 mg every 24 h serum peaks were 2.8 mug/ml and serum concentrations after 24 h 0.8-0.9 mug/ml (urine recovery 43%). The area under the curve was 25.5% less than after i.v. injection. In 7 adults with chronic renal failure the half-life of doxycycline varied between 10 and 24 h. With repeated oral administration of doxycycline (100 mg every 24 h) there was no accumulation of the drug in blood. During hemodialysis (Stuttgart kidneys, Rhône-Poulenc plates) doxycycline injected i.v. was eliminated as rapidly as before. In renal failure doxycycline may be given in the same dosage as where renal function is normal. In 12 geriatric patients without renal disease, serum levels of doxycycline after i.v. injection of 200 mg were not significantly different from those of young adults (distribution volume 46.2 +/- 16.2 litres). It can therefore be assumed that tissue penetration of the drug is similar in the elderly and in young adults.
