[On a case of a repidly proceeding lethal intoxication due to n-propylajmalium bitartrate (author's transl].

A case of N-Propylajmalinum bitartrate intoxication (Neo-Gilurytmal) is reported in which the death occurred within 20 minutes after the consumption of the drug. The quantitative estimation of the drug after isolation using Extrelut--columns was carried out in stomach spectrometrically, in the blood, bile, liver, kidney, and brain by gaschromatography after trifluoroacetylation. A high Prajmalium-bitartrate concentration was found in the blood (3,85 mg/l), negative results in bile and brain, low levels in liver (0,29 mg/kg wet weight) and kidney (0,06 mg/kg wet weight). Prajmalium-bitartrate was ingested together with beer, and a blood alcohol concentration of 0.061% was determined. Furthermore, a quantity corresponding to six tablets of Neo-Gilurytmal was still found in the stomach. The rapid fatal progress of the intoxication is explained as a strong effect of the drug on the heart due to the alcohol accelerated resorption of the substance.

Medicaid-funded private psychiatric care in New York City. The role of foreign-trained physicians.

Data were obtained from the tax records and the Professional Registry maintained by the New York City Department of Health as well as from the Biographical Directory of the American Psychiatric Association to determine which psychiatrists were providing care under the Medicaid Program and how they compare with the pool of psychiatric practitioners in New York City. The data reveal that psychiatrists educated in foreign medical schools provide disproportionately more care to Medicaid recipients than would be anticipated from their numbers, and are a majority of those who received more than +30,000 in payments in 1976. Furthermore, these psychiatrists account for 80 per cent of those delivering care at the two largest "shared health facilities," commonly referred to as "Medicaid mills." These findings indicate that although the Title XIX Program has facilitated the access of the poor to the private psychiatric sector, in New York City their access is biased toward treatment by foreign-educated psychiatrists.

[The induction of new bone formation after transplantation (author's transl)].

Bone induction by implantation of decalcified bone is described by many authors. This statement was proffed by implantation of decalcified pig bone in the abdominal wall muscle of rats. An enchondral osteogenesis was found following the implantation in a time intervall of 12 weeks according to the results of many other authors.

[Proton relaxation times of water in normal and transformed cells infected with temperature sensitive virus mutants (author's transl)].

Nuclear magnetic resonance spin-lattice relaxation times (T1) of tissue water protons are elevated in samples from tumors when compared to the corresponding normal tissues. In order to exclude differences between the normal and the tumorous tissue samples other than transformation as far as possible the T1values of cells infected by temperature sensitive transforming mutants of Rous sarcoma virus were measured. In this model genetic homogeneity of the host cells as well as identity of the viruses in the normal and transformed state is assured. In the transformed cells the protons relaxed more slowly than in the cells of normal phenotype which were kept at an only slightly higher incubation temperature.

[Reversal of small bowel segments to delay intestinal passage after total colectomy (author's transl)].

Five female mini-pigs underwent total colectomy including resection of the ileocecal valve. Sparing the terminal rectum, we reformed an end-to-end anastomosis between terminal ileum and remaining rectal cuff. In a second series 8 female mini-pigs have been colectomized using the same model. To delay intestinal passage, in addition a 10-12 cm long reversed segment of terminal ileum has been inserted between small bowel and rectal stump. Both groups were compared with a group of normal unoperated mini-pigs in a study up to 1 year, observing intestinal transit time, blood chemistry and general development of the animals. It could be proved that the reversal of a small bowel segment in colectomized animals leads to a prolongation of the intestinal transit time, to normalization of electrolytes and general appearance. Colectomy was only survived by animals with a reversed segment. The clinical application of a reversed small bowel segment following total colectomy in 1 patient was successful.

[Serum levels of magnesium and zinc after a controlled thermal trauma of the rat (author's transl)].

After a thermal trauma similar changes of Mg and Zn levels in serum are observable. During the initial period of shock the concentration of both elements increases as a consequence of damage of cellular and subcellular compartments. During the catabolic phase a deficiency syndrome occurs as a result of renal excretion, loss through the wound secretion, and demarcation of necrotic tissue. On the other hand increasing requirements of Mg and Zn appear because of the anabolic process of wound healing. From these reasons a substitution of both elements is recommendable in all cases of proved deficiency after burns.

Running amok.

This study examines twenty-four cases of amok, believed the largest number of cases ever collected. They were observed in Sarawak, East Malaysia. They occurred in all indigenous groups in Sarawak, excluding the Chinese, such as Malay, Sea Dayak, Land Dayak, Kayan, Punan and Melanau at frequencies more or less following the proportion of these groups in the total population. No differences were found according to religion, the Malay being Muslim and the other groups either predominantly Christian like the Iban or animistic. Only slight diminution in the frequency was observed from 1954 to 1968. The education level of the amok runners was much lower than that of the average population. The weapons used were those immediately at hand be it parang (short sword), ax, sticks, knives, guns, bare hands or a lorry. The classical four stages were largely present: (a) brooding and withdrawal, (b) homicidal paroxysm, (c) continuation of homicidal behaviour until killed, restrained or falling into stupor of exhaustion, (d) complete or partial amnesia. While in 14 no motive could be ascertained, insult, jealousy and paranoid ideation was present in the others. Both family history of mental illness and personal psychiatric history were predominant. All cases fell into accepted diagnostic categories from organic and endogenous psychosis to neurosis and behaviour disorder.

[Criteria for surgical therapy of hypothalamo-hypophyseal Cushing's syndrome].

This article tries to give criteria for the two possible modes of treatment of the pituitary-dependent Cushing's syndrome. Patients (n = 6) who show no secondary complications on Cushing's disease should undergo implantation of 90Y and/or 192Ir into the pituitary gland. After implantation, the excessive secretion of ACTH is suppressed to normal values. The symptoms of Cushing's syndrome disappear. Following the operation, certain patients must also receive hormonal replacement therapy. Fertility is not always preserved. Total adrenalectomy as the alternative procedure is performed in adults with advanced symptoms of the disease (n = 3) and in children (n = 3). Following removal of the adrenal glands, the symptoms of Cushing's syndrome disappear. The appearance of Nelson's tumor after adrenalectomy (10% in the literature) is discussed.

[Changes in liver metabolism following a standardized skin burn and intraperitoneal injection of a burn toxin isolated from burned mice skin. Studies on rat liver using isolation perfusion].

Parameters of liver metabolism were studied in the isolated perfused rat liver 5 days after a standard skin burn or an i.p. injection of a specific cutaneous burn toxin. Decreased values of the energy quotient ATP/ADP compared with normal controls were found. The absolute content of adenonucleotides was unchanged. The excretion of glucose and urea was also decreased in both groups and glucose synthesis after a lactate load was only slightly stimulated. The results show a direct influence of a burn toxin on hepatic metabolism suggesting a possible disturbance of oxydative phosphorylation.
