Development of an ultrasonic system for three-dimensional reconstruction of the fetus.

The ultrasonic diagnostic devices available at present can only represent one section of the fetus. We have developed a system for three-dimensional reconstruction of the ultrasonic fetal image in order to facilitate the understanding of the 3-D structure of the fetus and also to make 3-D recordings of this image. Either a real-time linear array probe or a convex array probe of the ultrasonic scanner was mounted on a position sensing arm of a manual compound scanner in order to detect the position of the probe. A microcomputer was used to convert the positional information to a recording of a visual image of videotape. This image was superimposed onto the ultrasonic tomographic image simultaneously using a superimposer and was recorded on the video tape, thereafter, being recalled by the image processing minicomputer. The minicomputer VAX11/780 (DEC) system was used for 3-D reconstruction and 3-D display. In the memory system the image of the anterior uterine wall was identified and subsequently excluded in order to visualise the fetus more clearly. The threshold of brightness was set to a high level so that the fetus could be separated from the amniotic fluid. The fetus was displayed three-dimensionally using computer graphics. Using this system, we have made it possible to observe the whole image of the fetus in utero non-invasively. This system offers a method for easier understanding of the 3-D structure of the fetus in utero and also makes 3-D recording possible. In the future, we confidently expect that this system will be used for screening for fetal anomalies and abnormalities of fetal growth.

Urinary orosomucoid excretion rate in patients with non-insulin-dependent diabetes mellitus.

Urinary excretion rate and clearance of alpha 1-acid glycoprotein (orosomucoid), a major serum glycoprotein which is more anionic (pI 2.7) than albumin (pI 4.7) were measured by RIA in timed overnight urine samples from non-insulin-dependent diabetic patients with different urinary albumin excretion rate and from healthy controls. The 50th percentiles of urinary orosomucoid excretion rate in patients with normo-, micro-, and macroalbuminuria were larger than those in healthy controls. Urinary excretion rate and clearance of orosomucoid increased in parallel with increase in albumin excretion rate in diabetic patients with an albumin excretion rate of more than 10 micrograms/min. On the basis of their levels of urinary orosomucoid excretion, patients with normoalbuminuria of less than 10 micrograms/min could be divided into two groups, one with a normal and the other with an elevated urinary orosomucoid excretion rate. The findings suggest that kidneys of diabetic patients with an albumin excretion rate of more than 10 micrograms/min are unable to distinguish the difference in pI between albumin and orosomucoid, and that a subgroup with an elevated orosomucoid excretion rate may be present among diabetics with normoalbuminuria.

Characterization of resistance to VP-16 in human leukemic cell line.

Resistance mechanism was studied in the VP-16-resistant human leukemic cell line (THP-1/E) which was developed by continuous drug exposure. The drug uptake and efflux studies revealed no decrease in net cellular drug accumulation. VP-16-induced DNA single- and double-strand breaks in the whole THP-1/E cells decreased significantly compared to the sensitive counterpart as assessed by alkaline elution methods. Decrease in DNA SSBs was also observable in the isolated nuclei from the THP-1/E cells. The resistance to VP-16 in THP-1/E appeared to be independent of altered membrane permeability, and more likely to be associated with decreased VP-16-mediated DNA cleavage.

[Effects of anti-Bacteroides gingivalis (Bg), and anti-Actinobacillus actinomycetemcomitans (Aa) antibodies on Bg and Aa].

The purpose of this study was to determine the effects of anti-Bacteroides gingivalis (Bg) and anti-Actinobacillus actinomycetemcomitans (Aa) antibodies on Bg and Aa respectively. Bg and Aa were resistant to complement-mediated bactericidal activity, and killing by PMN. However a significant reduction in CFU of Bg and Aa was found when these bacteria were incubated with the antibodies and not agitated. This would suggest that the antibody was capable of aggregating or phagocytizing, but not killing, these bacteria when complement and PMN were added. Moreover the antibodies had no effect on bacterial proliferation. Anti Aa antibody also had no effects on killing of Aa or PMN infiltration in experimentally established periodontal pockets in dogs.

[Two cases of ectopic prostatic tissue in the prostatic urethra].

Two cases of ectopic prostatic tissue or adenomatous polyps with prostatic type epithelium in the prostatic urethra were reported. They were 30-and 33-year-old males suffering from gross-hematuria and from urethral pain on urination with terminal gross-hematuria. Endoscopic examination revealed papillary lesions from the dilated orifice of the prostatic utricle. Biopsied pathology demonstrated a tissue similar to that of the prostatic gland. PSA (prostatic specific antigen) stain using immunohistochemical methods was positive. Histogenesis and the importance of this lesion particularly as a cause of hematuria in the young male adult were discussed.

[Studies on human spermatozoal acrosome using spermac stain in fertile and infertile men including two cases of round-headed spermatozoa].

Using a new acrosome stain named "Spermac stain", the relationship between the staining rate of sperm acrosome and conventional semen parameters, i.e., sp. density and sp. motility efficiency index (SMEI) was studied. The mean positive staining rate of sp. acrosome in ten fertile men was 81.5%. In 72 infertile men, a significantly higher (p less than 0.001) correlation was noted between the positive staining rate and sp. density and SMEI. 5 out of 8 male partners of unexplained infertility couples showed low staining rates (40-60%), in spite of normal sp. density and SMEI. Ejaculates from two infertile men with "round-headed" spermatozoa were examined by Spermac stain and electron microscopy. The staining rates of sp. acrosome in these two cases were 4.5% and 16.5%. The fine structures of the sperm and the spermatid were presented. Because Spermac stain is quick, reliable and easy to use, this staining method is recommended for evaluating sp. acrosomal morphology in practical use.

[Arrhythmia during extraction of the third molars under intravenous sedation. A case report of postoperative tetralogy of Fallot].

Extraction of the third molars [formula: see text) was done under local anesthesia with intravenous sedation using diazepam in a 14 year-old girl. She had the history of radical operation for tetralogy of Fallot at 4 years of age. During extraction procedure, this patient showed many multifocal premature ventricular contraction in ECG. These arrhythmias were able to control with intravenous lidocaine. In this case, the preoperative evaluation of her cardiac function was not pertinent possibly due to the judgment by her ability of physical exercise in the past history. We reflect that inadequate preoperative evaluation of cardiac function may result in inadequate management.

[A case report of IgD myeloma with acute renal failure and deposition of lambda light chain along the tubular basement membrane].

A case of IgD lambda type multiple myeloma that developed acute renal failure was presented. Histopathologic studies revealed apparent myeloma cast nephropathy with typical giant cells and light chain deposition along tubular basement membrane and inter-peritubular interstitium. Glomerular deposits of light chain were absent by electron and immunofluorescent microgram. The presented case showed a very uncommon histopathologic findings of unusual deposits of light chain, to tubulo-interstitium except for glomeruli, in addition to vertically rare IgD myeloma. A sixty-year old female who was suffered from acute renal failure and treated by hemodialysis for one month was admitted to our institute. Physical examinations revealed ill-apparent and emaciated uremic patient on admission. Laboratory and radiological studies demonstrated IgD lambda type monoclonal protein and numerous punched out appearance of skull bone. The diagnosis of IgD lambda myeloma was confirmed with aspiration bone marrow test. Anti-myeloma therapy of melphalan and prednisolone have started, however, fatal arrhythmia suddenly suffered the patient and resuscitation attempts failed to get any success one week after admission. Renal necropsy was performed and specimens were studied by light, immunofluorescent and electron microscope.(ABSTRACT TRUNCATED AT 250 WORDS)

The importance of the worldwide contraceptive experience ot Japanese physicians.

Oral contraceptives (OCs) have been used by more than 50 million women worldwide. About 30 years ago, Japan also wished to develop and introduce oral contraceptives. Regrettably, the government was not supportive. As a result, Japanese women have not had access to this form of contraception. After two years of intensive discussions at the Special Committee level, the Japanese Ministry of Welfare (KOSEISHO) published guidelines in 1987 to allow clinical trials of oral contraceptives to begin. This article discusses the relevance and importance of the worldwide experience with oral contraceptives to the Japanese physician. Despite the long wait, the Japanese physician looks forward to being able to apply clinically what has been learned worldwide.

[A trial of ACNU and radiation therapy with sensitizing agents for malignant gliomas].

Twelve cases of malignant gliomas (anaplastic astrocytoma 4, glioblastoma 8, recurrent 3, primary 9) were treated with ACNU and radiation with sensitizing agents after the surgical removal of the tumor. BUdR, Vidarabine (Ara-A), Aciclovir (ACV) were applied for sensitizing agents. BUdR was administrated intraarterially prior to radiation (380 rad, two times a week), and Ara-A and ACV intravenously during and after the radiation. Total dosage of the radiation was 50-60 Grey for each case. All recurrent and eight primary patients died. The mean survival time of the recurrent patients was 17.7 months, while that of the primary patients was 13.4 months. One of the primary patient was glioblastoma and is still surviving more than 24 months by now. The complete response (CR) rate of the primary tumor patients observed by computerized tomography (CT) scan was 5/9. We can expect the availability of this trial for malignant gliomas because of high CR rate in primary tumor cases.

[Experimental study on the influence of various dental luting cements on the crown elevation during cementation].

Cementing manipulation is an important final step in adapting crown prosthesis to various oral environments. Preventing elevation of full cast crowns during cementation and improving adaptability of the margin after cementation are especially critical. The purpose of this article is twofold: 1) To clarify the relationship between the viscosity of dental cement and the extent of crown elevation, with special attention to viscosity changes occurring in various cement materials during the hardening process. 2) To study the effects on cemented-crown elevation of the space created between the abutment tooth and the crown, which may be partly related to dental cement behavior during cementation. A total of 4 cement materials was used: Elite Cement 100 as a zinc-phosphate cement, Super Bond C & B and Chemiace as MMA-resin cements containing 4-META, and Panavia EX as a phosphoricester cement. Viscosity was measured by means of MR-3 Soliquidmeter with a cone and plate system (Reheology Engineering). A brass material was processed to experimental crowns cemented to abutment teeth of brass. Crown elevation was measured by comparator (Measurescope Model II, Nikon). In addition, effects of the resistance of dental cement during cementation on crown elevation were studied for the sake of a comprehensive understanding of the elevation phenomenon. 1. Changes in dental-cement viscosity during the hardening process 1) Elite Cement 100 demonstrated the highest initial steady flow coefficient of viscosity. It was followed by Super Bond C & B, Chemiace, and Panavia EX, in that order. The t-test revealed significant differences among these cement materials (significance taken to be 1%). 2) As experimental temperature increased, the viscosity of all tested cement materials increased with progressive hardening. 3) With the passing of time, changes in torque, which represents a change pattern in viscosity, indicated that the tested cement materials have different viscosity-change patterns during the initial setting period. 4) From a clinical viewpoint, Super Bond C & B and Elite Cement 100 had very limited working time, whereas Chemiace and Panavia EX had relatively long working time. 2. Elevation after cementation of experimental crowns 1) For Elite Cement 100, mean elevation was 334microns, for Super Bond C & B 281 microns, for Chemiace 164microns, and for Panavia EX 130microns. The t-test revealed significant differences among these cement materials (significance taken to be 1%). 2) In all the cement materials tested, with a reliability of 99%, sequential correlation was observed between crown elevation and steady flow coefficient of viscosity.(ABSTRACT TRUNCATED AT 400 WORDS)

[The therapeutic effects of OK-432 combined adoptive immunotherapy (AIT) against liver metastases of breast cancer].

We studied the effect of OK-432 combined AIT in 24 cases of liver metastases of breast cancer. Eleven of the 16 patients (69%) who received intraarterial transfer responded to this therapy. On the other hand, no patients responded to intravenous or intraportal transfer. The minimum cell number for a therapeutic response was 10(9) cells. Four patients had abscopal effects after therapy. The serum CEA level paralleled the therapeutic effects. There were no severe side effects accompanying this therapy. These results indicate that intra-arterial OK-432 combined AIT should be the first choice therapy against liver metastases of breast cancer.

[Effect of recombinant human erythropoietin administration on cardiovascular system in patient with chronic renal failure: an analysis of the blood pressure upward].

To investigate the mechanism of blood pressure upward after recombinant human erythropoietin (EPO) administration in patient with chronic renal failure (CRF), hemodynamic changes following on the improvement of anemia was evaluated by the echocardiography. Fourteen (5 males, 9 females) normotensive patients with less than 23% of hematocrit (HT) were administered 1,500 approximately 3,000 units of EPO three times a week. Four patients with systolic (greater than 160 mmHg) or diastolic (greater than 90 mmHg) hypertension after EPO administration was named group H, the remaining patients was named group N. Blood pressure, heart rates and parameters of echocardiography examined after the improvement of anemia were compared to those examined before EPO administration, respectively. Patients had an increase in Ht from 20.0 +/- 1.6% to 33.0 +/- 1.9% (p less than 0.01). LVDd, LVEDV, CO and flow velocity in group N were significantly decreased after EPO administration; while those in group H did not show the significant changes. IVSth and LVPWth in each group were significantly increased after EPO administration. These data showes that decrease in CO maintains the patients normotensive after EPO administration.

[Scintigraphic estimation of dearterialization after transcatheter hepatic arterial embolization therapy].

Radionuclide angiography was performed by bolus injection of 99mTc-phytate in 10 patients with liver neoplasms before and after the transcatheter hepatic arterial embolization therapy. The ratio of the slopes of the upstrokes for the hepatic arterial and abdominal aortic first transit curves constitutes the regional hepatic arterial to aortic blood flow ratio (RHA/A ratio), which represents quantitatively the arterial blood flow of hepatic region of interest as a fraction of aortic blood flow. The degree of embolic dearterialization was estimated by the change of this ratio derives from the same region of interest for each patient before and after the embolization. The values and post-embolic reduction of the ratio correlated well individually with the angiographic features and other patients' data. The arterial blood supply to the tumor could not be reducted using Lipiodol alone as the embolic material in only 1 case. The finding that the splenic arterial to aortic blood flow ratio (S/A ratio), calculated in the same way, was practically unchanged after the hepatic arterial embolization indicates the good reproducibility of these measurements. This noninvasive method may be helpful to evaluate the therapeutic effect of hepatic arterial embolization.
