IL2-induced lymphocytes (IIL) obtained from normal subjects were examined as to a possibility for induction of tumor-specific killer cells. Moreover, effects of serum on their cytotoxic activity were observed. The IIL which were pre-sensitized with mitomycin C-treated MKN-28 (IILM), showed a markedly increased cytotoxic activity against MKN-28. The IIL which were either pre-cultured with PHA or not, showed augmentation of cytotoxic activity against various human cancer cell lines, although their cytotoxic activity against MKN-28 was lower than that of IILM. The cytotoxic activity of IILM was suppressed by addition to the medium of non-specific immunosuppressive factors (IAP, ferritin, alpha-fetoprotein) or of various sera obtained from gastric cancer patients, in whom histological diagnosis was tub1, por or sig, but the suppression to the IILM activity was significantly milder than that to the activity of peripheral lymphocytes. The degree of suppression by tub1 serum was significantly higher than that by the serum from por or sig. When tub1 serum was added to the medium during pre-sensitization, the cytotoxic activity of IILM against MKN-28 was reduced significantly. However, addition of por or sig serum showed no significant reduction in cytotoxic activity of IILM.
Lymphokine activated killer (LAK) cells derived from normal subjects were examined as to the following points: 1) monoclonal analysis of LAK, 2) cytotoxic ability of LAK, 3) effect on LAK cytotoxic ability of the presence in the medium of either serum obtained from gastric cancer patients or nonspecific immunosuppressive factors (ferritin, IAP, AFP), 4) effect, on induction of their cytotoxicity, of the presence in the medium during culture of sera from gastric cancer patients, simulating the conditions of in vivo administration and 5) augmentation of cytotoxic ability of LAK by simultaneous IL2 administration. The following results were obtained. 1) Monoclonal marker analysis of LAK revealed that the ratios of OKT3+, OKT4+, OKT8+ and OKIa1+ lymphocytes were all significantly higher than those in peripheral blood lymphocytes (PBL). 2) Cytotoxic ability of LAK against various tumor cell lines (MKN-28, MKN-45, KATOIII, PC-10 and K562) was found to be higher than that of PBL. 3) Addition to medium of ferritin, IAP or AFP significantly reduced the cytotoxic ability of both PBL and LAK against various tumor cell lines. However, the degree of reduction was significantly milder in the case of LAK than in PBL. 4) The cytotoxicity-suppressing effects of gastric cancer sera (untreated, at stages III and IV) were significantly milder in the case of LAK than in PBL. 5) When gastric cancer serum was added to medium, instead of normal AB serum during induction of LAK, its cytotoxic ability against various tumor cell lines was significantly reduced. Its cytotoxic ability was nevertheless significantly higher than that of PBL. 6) When IL2 was added to medium during cytotoxicity assay, cytotoxic ability of LAK was augmented. When LAK was cultured for 1 hour before assay in fresh medium containing 1,000 U/ml IL2, its cytotoxic ability was further augmented.
At 7 days after MRMT-1 inoculation at the thigh of 5 week-old female SD rats, the tumor was treated cryosurgically by 2 cycle freezing, contact method at -170 degrees C. Weights of thymus and the spleen, histological findings of lumbar lymph nodes, PHA-induced blastogenesis of lymphocytes obtained from lumbar lymph nodes or peripheral blood, and resistance rate to tumor re-challenge were examined at 1, 3, 6, 10 and 17 week(s) after cryosurgery. Following results were obtained. (1) Weight of the thymus gradually decreased by 3 weeks after cryosurgery, while that of the spleen increased by 1 week, recovering the preoperative level at 7 weeks. (2) Paracortical hyperplasia of lumbar lymph nodes markedly increased at 1 week and sinus histiocytosis increased after 3 weeks, both keeping their high values till 10 weeks, while germinal center hyperplasia showed a high value at 2 weeks, thereafter decreasing gradually. (3) PHA-induced blastogenesis of lumbar lymph nodes significantly increased at 1 week, keeping its high value till 10 weeks. (4) PHA-induced blastogenesis of peripheral lymphocytes showed the lowest value at 3 weeks, then significantly increasing at 6 weeks. (5) Resistance rate to re-challenge showed the lowest value at 3 week, reading the highest level at 10 weeks after cryosurgery. From the above results, it was suggested that anti-tumor immunity induced by cryosurgery was at the lowest level at 3 weeks, and gradually increased starting at 6 weeks. Augmentation of helper T cell and cytotoxic T cell functions at the regional lymph nodes was also suggested to occur from 1 week to 10 weeks after cryosurgery.

A case of unresectable advanced gastric cancer (portion A, Min, S3 infiltrating into pancreas, N3, por., Borrmann III-type, hen's egg size) was treated with cis-dichlorodiammine platinum (CDDP) at a dosage of 30 mg/day for five days every month following surgery. After three courses of administration, examinations revealed that the tumor had disappeared and an evaluation of complete response was made. Four months later, a second-look operation was performed. It was found that the cancer was less than thumbsize and was able to be resected radically. No significant side effects were observed during CDDP therapy.
Experimental study was performed on changes in lymphocytic cytotoxicity and phytohemagglutinin (PHA)-lymphoblastogenesis after surgical tumor excision and on a possibility of estimating antitumor immunocompetence based on the latter parameter, using 3-methyl-cholanthrene-induced mammary cancer, MRMT-1, of SD rats. When the tumor was excised at 2 weeks after inoculation, cytotoxicity showed, after a transient rise, a marked reduction at 4 weeks postoperatively in the cases of postoperative negative metastasis, while it showed a high peak at 3 weeks postoperatively, followed by a sharp descent, in the cases of postoperative positive metastasis. On the other hand, PHA-lymphoblastogenesis was found to recover a relatively high level postoperatively, in the cases of both negative and positive metastases. It appears to be difficult to estimate specific cytotoxicity from PHA-blastogenesis, but both are useful, in complementary fashion, to estimate the antitumor immunocompetence.
The effect of intraperitoneal OK-432 administration and its mechanism of action were investigated using an experimental rat tumor. On day 4 or 6 of intraperitoneal inoculation of tumor cells, 50 Klinische Einheit (KE)/kg of OK-432 was intraperitoneally administered, and intraperitoneal exudate cells were microscopically examined daily. Distribution of peritoneal exudate cells: A marked increase in neutrophils was noted in ascites at 24 hours after intraperitoneal administration of OK-432, with macrophages and lymphocytes appearing later than neutrophils. A subsequent decrease in neutrophils was accompanied by a gradual increase in tumor cells. Findings of tumor cells: When OK-432 was intraperitoneally administered on day 4 of tumor inoculation, aggregation of neutrophils around tumor cells was observed on day 6, and rosette formations around tumor cells were noted on day 7 or 8. Subsequently tumor cells that formed the centers of the rosettes were destroyed and disappeared. These reactions were not observed when OK-432 was administered on day 6, nor were they observed in untreated cases. These results suggest that cytotoxic effects of intraperitoneally administered OK-432 were displayed mainly by rosette formation by neutrophilic cells, as far as this model was concerned.
Ito cells (fat-storing cells), containing a few lipid droplets, appeared in the Disse's spaces of the Beagle dog liver. They ranged in diameter from 0.5 to 5 micron. The present study revealed that mast cells were also distributed in the Disse's spaces, while they were encountered only in the interlobular connective tissue in other animals. Although mast cells as well as Ito cells were distributed solitarily in the Disse's spaces, pairing of both types of cells could be frequently observed. Direct contact between both cells was verified using both light and electron microscopy. Also encountered were some peculiar figures of certain Kupffer cells engulfing the cytoplasmic processes and secretory granules of mast cells. These findings indicate a previously unknown intimate relationship between the mast cell and other perisinusoidal cells in the dog liver: the Ito cell, Kupffer cell and endothelial cell.

Serum IAP values were measured preoperatively in 83 gastric cancer patients. The levels were significantly higher in cases of total, stage III and IV, n (+) and non-curative resection than in normal subjects, stage I, n (-) and curative resection. There was a significant correlation between IAP on the one hand, and IgG FcR (+) T cell, alpha 2-globulin and CEA on the other. OK-432 was serially administered pre- and postoperatively (A) and only postoperatively (B). IAP was examined at one, two and four weeks postoperatively, and the findings were compared. It was increased markedly at one week and was slightly reduced at two and four weeks. These changes in IAP showed no significant differences between groups A and B.
Delayed tissue eosinophilia in DNP-ovalbumin-induced allergic inflammatory skin lesions of guinea pigs was markedly enhanced by previous treatment with alum hydroxy gel (Alum) or Bordetella pertussis vaccine. This enhancement seemed due to increased production of a lymphocyte-derived eosinophil chemotactic factor (ECF) at the skin site. Treatment of animals with Alum potentiated antigen-induced in vitro ECF production by lymphoid cells from spleen and mesenteric lymph node of sensitized animals. The co-culture supernatants of lymphoid cells from Alum-treated animals also potentiated concanavalin A (Con A)-induced in vitro ECF production. The potentiating effect of Alum on ECF production seemed to be ascribed to the release of soluble factors from macrophages of the Alum-treated animals. The macrophage-derived soluble factor ECF-potentiating factor (ECF-PF) selectively potentiated ECF production but not macrophage chemotactic lymphokine production by Con A-stimulated lymphoid cells from normal animals. ECF-PF activity was associated with two separate m.w. fractions: one was 50,000 to 70,000 and the other was 10,000 to 20,000. The present study provides one of the explanations for enhanced ECF production by adjuvants, such as Alum and Bordetella pertussis vaccine.
A case of left ventricular apical diverticulum with marked hypertrophy of the left ventricular apical wall revealed by thallium-201 myocardial emission CT is reported. A 23-year-old woman was admitted to our hospital for evaluation of chest oppression. She was known to have had a heart murmur soon after birth, but she grew uneventfully, partaking in normal exercise. At the age of 21, she began to feel chest oppression during exercise. As the attacks became frequent, she was admitted to our hospital. Physical examination revealed an ejection systolic murmur in the second left intercostal space. Electrocardiography showed ST depression and T inversion in leads III, aVF and V4-6. M-mode echocardiography was normal. Two-dimensional echocardiography showed a small diverticulum at the apex of the left ventricle, which was also recognized by left ventriculography. It was about 8 X 12 mm in size. Thallium-201 myocardial emission CT disclosed marked uptake in the apex of the left ventricle, suggesting apical hypertrophy. Stress thallium-201 myocardial emission CT was negative. Coronary angiography was normal. The cause of chest oppression in this patient is uncertain, but the small diverticulum and hypertrophy of the cardiac apex may play a role in its pathogenesis.
Twenty cases of metastatic bone tumors were treated with Elcatonin together with irradiation, achieving pain relief in 47.3% of cases, improved bone scintigrams in 16.7% of cases and radiographic improvement of invaded bone in 7.7% of cases. It has reported that calcitonin is effective for pain relief in 50% of cases but for the radiographic improvement of bone lesions in only 0-10%. The life quality of patients with bone metastases is controversial since treatment techniques for advanced oncological patients has progressed remarkably. The pain relief obtained with calcitonin contributes significantly to improving the quality of life for such patients when administered in combination with conventional treatment modes.
Human malignant cells, obtained from surgical specimens or pleuro-peritoneal exudate in 105 cases, were cultivated. Tumor-specific immunity was estimated by cytotoxicity assay using mixed autologous lymphocyte-tumor cell culture in 27 cases (40 times in all). Relations of the observed specific immunity to various nonspecific immunological parameters and to clinical responses to immunotherapy were examined. Positive reaction was observed in 14 examinations. Eight of 25 digestive tract cancers and 6 of 15 other cancers were positive. Cytotoxic activity was significantly higher in the group of favorable clinical response than in the group of progressive disease. In the group of positive cytotoxicity, incidence of clinical response was significantly higher than in the group of negative cytotoxicity. A significant positive correlation was found between cytotoxicity and T cell ratio. Incidence of positive cytotoxicity tended to be higher in the cases of single OK-432 administration (6/11). Incidence of clinical response was higher in the cases of combined OK-432 and PSK administration group (4/14). In the positive cytotoxicity group, 3 examinations showed progressive disease and 5 showed no change. It has been suggested that a higher incidence of favorable clinical response is to be expected in case of positive cytotoxicity, if some therapy will be combined with the conventional immunotherapy.
Treatment of guinea-pig spleen cells with glucocorticoids, such as dexamethasone (DEX), reduces concanavalin A (Con A)-induced production of chemotactic lymphokines (CLK), such as eosinophil chemotactic factor and macrophage chemotactic factor. The decreased CLK production is not caused by a direct effect of DEX on the spleen cells producing CLK, because Con A-induced CLK production is suppressed when the cells are cultured together with cell-free culture supernatants of the spleen cells which had been pretreated with DEX. A soluble suppressive factor, termed CLK-SF, with a MW of about 20,000, seems to be responsible for the suppression of both CLK production. CLK-SF is produced from DEX-treated T lymphocytes. CLK-SF probably exerts a critical role in the early stage of CLK production. In contrast, CLK-SF fails to inhibit Con A-induced lymphocyte proliferation, although DEX itself suppresses lymphocyte proliferation. This suggests that DEX suppresses Con A-induced CLK production by a different mechanism from that for lymphocyte proliferation.
Guinea-pig spleen cells treated with complete Freund's adjuvant (CFA) produce eosinophil-directed chemotactic inhibitory factors (ECIF). The inhibition is selective for the response of eosinophils to delayed ECF-a, which had been isolated from 24-hr-old inflamed skin lesions induced by DNP-Ascaris extract in sensitized animals and had been confirmed as being a T lymphocyte-derived lymphokine. ECIF activity is absorbed by incubation with eosinophils, but not with macrophages or neutrophils. The cells spontaneously release ECIF; pretreatment with a protein synthesis inhibitor reduces ECIF production, indicating that protein synthesis is essential. The source of ECIF seems to be lymphocytes, probably T lymphocytes. ECIF activity is recovered in two separate fractions: one elutes near bovine serum albumin (MW 70,000) and the other near cytochrome c (MW 12,500). ECIF binds to peanut agglutinin- or Limulus polyphemus agglutinin-coupled agarose beads. Furthermore, ECIF activity is blocked when eosinophils are incubated with D-galactose or sialic acid. These results suggest that ECIF derived from T lymphocytes of CFA-treated animals modulate the delayed ECF-a-mediated tissue eosinophilia, and that terminal galactose and/or sialic acid residues are essential for ECIF activity.
