The role of metoclopramide in acute and delayed chemotherapy induced emesis: a randomised double blind trial.

High dose metoclopramide is an effective anti-emetic for use with cisplatin containing chemotherapy regimens but can cause extrapyramidal reactions. Lorazepam and dexamethasone are increasingly being used to alleviate chemotherapy induced emesis. This trial has assessed the contribution of high dose metoclopramide to anti-emetic control when given with dexamethasone and lorazepam. Eight-one patients receiving chemotherapy, mainly for gynaecological malignancy, entered a randomised double blind cross-over trial comparing dexamethasone and lorazepam with or without a 24 h metoclopramide infusion. This was followed by oral dexamethasone with or without oral metoclopramide for three further days depending on the initial randomisation. Sixty-one patients were fully evaluable. Fifty-five received cisplatin containing regimens and six non-cisplatin regimens. There was a significant reduction in the number of episodes of vomiting during the first 24 h in patients receiving the metoclopramide combination (P = 0.0001). On first exposure to chemotherapy 45% of patients receiving dexamethasone, lorazepam and high dose metoclopramide had no vomiting while 67% had two episodes or less ('major control'). This compared to 11% total control and 25% major control in those receiving dexamethasone, lorazepam and placebo. The control of nausea in the first 24 h was also improved (P = 0.0001). There was no difference in the degree of nausea or vomiting during the following three weeks between those receiving oral dexamethasone alone and those receiving dexamethasone and metoclopramide. Both groups showed a significant increase in nausea in the three weeks following the second course of treatment when compared to the first (P = 0.0007). Extrapyramidal reactions were recorded in 11.5% of patients receiving metoclopramide. More patients stated a preference for the metoclopramide combination although this was not statistically significant (chi 2(1) = 0.29, P = 0.59). In conclusion the combination of dexamethasone and lorazepam can give major control of emesis in 25% of patients receiving very emetogenic chemotherapy. The addition of metoclopramide increases this to 67% on first exposure to chemotherapy, but at the expense of extrapyramidal reactions in 11.5%.

Neonatal hygroscopic condenser humidifier.

A hygroscopic condenser humidifier was developed for neonates on mechanical ventilation and was evaluated by laboratory tests and clinically. Humidification provided by the unit was measured in the 10- to 50-ml tidal-volume range at ambient temperatures of 24 degrees C and 38 degrees C. The effect of a leaking patient connection on device performance was investigated. Leakage rates were measured routinely in a neonatal ICU and surgery to determine the clinical significance. In the entire tidal volume and temperature range, the unit provided an inspiratory water content in excess of 30 g/m3 when the leak fraction (volume leaked/volume delivered at Y-piece) was less than 15%. This was found in three out of four cases. In about one out of ten cases, the leak exceeded 30%, which invariably led to corrective action, such as repositioning or changing the endotracheal tube. However, even at a 30% leak, a water content of about 26 g/m3 was still available for humidifying the inspired gas, which corresponds to normal physiologic conditions found in the trachea for nasal breathing of room air.

Central vs. peripheral effects of estrogen on food intake and lipoprotein lipase activity in ovariectomized rats.

The effects of implants of estradiol benzoate (EB) into either the ventromedial nucleus (VMH-EB) or the paraventricular nucleus (PVN-EB) of the hypothalamus were compared to the effects of peripheral EB (Per-EB) replacement in ovariectomized rats. After three days of hormone treatment, the Per-EB group and the PVN-EB group exhibited a decrease in food intake and body weight. Adipose tissue lipoprotein lipase activity was generally suppressed for the Per-EB, PVN-EB, and VMH-EB groups in the left and right parametrial and retroperitoneal fat pads. The finding that the VMH-EB and PVN-EB groups experienced similar peripheral stimulation but differed on the food intake and body weight measures would suggest that these measures are not entirely under peripheral control. The findings support the hypothesis that estrogen exerts its effects via multiple mechanisms, both central and peripheral. In addition, the PVN is shown to be an important site for further research on the modulation of feeding and metabolism by estrogen.

Antral hypertrophic gastritis: a rare cause of iron deficiency.

A case of an unusual hypertrophic gastropathy confined to the gastric antrum which presented with chronic anaemia is described. The clinical and pathological features are contrasted with Menetrier's disease and hypersecretory hypertrophic gastropathy, and a possible relation to solitary hyperplastic (regenerative) polyps is discussed.

Poliomyelitis, rubella, and dengue antibody survey in Barbados. A follow-up study.

A follow-up study of poliomyelitis, rubella, and dengue antibodies has been made in light of results obtained in a 1972 health and serological survey in Barbados, W.I. Poliomyelitis antibody neutralization tests performed on sera from 307 children under age 15 using overnight serum/virus mixtures on microtiter plates at low serum dilutions revealed the absence of polio antibody at 1:2 dilution in 13.7% for type 1, 6.5% for type 2, and 14.3% for type 3 virus. A significant correlation of the presence or absence of poliomyelitis antibody to types 2 and 3 was seen with the response to immunization histories. Forty-three of 49 girls (88%) given rubella vaccine (RA 27/3) in 1972 had demonstrable haemagglutination-inhibition antibody 4 years later. Neutralization tests for dengue antibody confirmed the results of the complement-fixation tests and indicated that type 2 was probably the sole infecting strain.

Fenoterol: clinical trial of a new long acting bronchodilator.

The bronchodilator and side effects of fenoterol and isoproterenol were evaluated in 19 asthmatic adults in a double-blind study. The study demonstrated that fenoterol has an onset of action which is just as rapid as isoproterenol and a duration of action that is markedly superior. Side effects were minimal.

Fetal and neonatal cerebral oxygen monitoring with NIRS: theory and practice.

Near infra-red spectroscopy (NIRS) is a comparatively new method for monitoring the oxygenation in blood and tissue in the brain of the fetus and the neonate. Absorption of light in the wavelength range 700-1000 nm through such tissue is measured, which is then used to calculate changes in the concentration of cerebral oxygenated and de-oxygenated haemoglobin (HbO2 and Hb) and hence cerebral blood volume (CBV). Studies carried out on several groups of newborn babies have shown clear changes in HbO, Hb and CBV with hypoxia and bradycardia. These changes may well have implications in the occurrence of hypoxic/ischaemic brain injury. Intra partum NIR measurements on the fetal brain have demonstrated clear changes in HbO2, Hb and CBVm, coinciding with the onset of contractions.

Plasmodium falciparum: increased proportion of severe resistance (RII and RIII) to chloroquine and high rate of resistance to sulfadoxine-pyrimethamine in Peninsular Malaysia after two decades.

Uncomplicated falciparum malaria patients were randomly assigned to receive either 25 mg/kg chloroquine (CHL) over 3 d or a statim dose of 25 mg/kg sulfadoxine (SDX) plus 1.25 mg/kg pyrimethamine (PYR). Patients were followed up for 28 d and the parasite response graded according to World Health Organization criteria. Overall resistance to CHL was 63.3% and 47.4% to SDX/PYR. RI, RII and RIII rates were 9.1%, 42.4% and 12.1% for CHL and 10.5%, 21.1% and 15.8% for SDX/PYR, respectively. Degree and rates of resistance to CHL were significantly correlated with pre-treatment parasite density, but not those to SDX/PYR. Plasma CHL and SDX/PYR levels were within the reported ranges and were not significantly different in patients with sensitive and resistant responses.
