[Homologous plasma cross-circulation on galactosamine induced hepatic failure rats].

Plasma cross-circulation which resembles plasma exchange clinically was carried out in between galactosamine (GalN) treated hepatic failure rats and normal rats. Twenty-four hours after the injection of GalN, plasma cross-circulation was performed at a plasma flow rate of 0.1 ml/min for 6 hours. In eighteen hours after the completion of a procedure, survival rate was 75% (6 out of 8 survived) in GalN injected rats treated with plasma cross-circulation, as compared to 40% (4 out of 10 survived) in GalN injected rats treated with sham-circulation. State 3 oxygen consumption and ATP synthesis in the liver mitochondrial respiratory function, and ATP and total adenine nucleotide contents in the liver in the former group were significantly higher than those in the latter group at the time of sacrifice of the survived animals. These appear to suggest that plasma cross-circulation enhances mitochondrial phosphorylative activity in the liver and may contribute higher survival rate of the rats. Plasma exchange using a large amount of fresh plasma, therefore, would be effective for the treatment of acute liver failure in its early stage.

The effects of vitamin E on the indicator enzymes of organella membranes in the injured liver.

Acute and chronic liver damage was caused by the administration of either galactosamine or carbon tetrachloride. Consequently, the rats with damaged livers were killed after vitamin E was administered. The livers were removed and were homogenated. Indicator enzymes (5'-nucleotidase, arylsulfatase, cytochrome C oxidase and glucose-6-phosphatase) of organella membranes were measured in the homogenates of the normal and damaged livers. The effects of vitamin E resulted in the stabilizing of the impaired membranes of plasma, lysosome, mitochondria and microsome; (1) the abnormal decrease of 5'-nucleotidase activity and glucose-6-phosphatase activity, and the abnormal increase of arylsulfatase activity, which induced galactosamine or carbon tetrachloride, and (2) the abnormal decrease of cytochrome C oxidase activity induced by galactosamine- HCl, were normalized.

A polyhydric phenol sulfokinase and chronic liver injury.

The sulfokinase, that transfers activated sulfate from PAPS (3'-phosphoadenosine-5'-phosphosulfate) to nitrocatechol sulfate was studied. 1. It is thought that the enzyme plays an important role in detoxication by the sulfation of polyhydric phenols in liver. 2. The enzyme had an optimum at pH 8.0 in Tris-acetate buffer. Km was 0.105 x 10(-3)M. The rate of conjugation was linear within 1 min. 3. The main distribution of the sulfokinase was found in the liver, lungs, spleen and various other organs in the rat. The activity of the enzyme in rat liver was the highest in soluble fraction. In chronic hepatic injury caused by carbon tetrachloride injection, the activity of the enzyme gradually decreased to below about one half of the control value.

The effect of oral administration of amphotericin B on the incorporation of 5-fluorouracil into human gastric cancer tissue.

Amphotericin B (AmB)-5-fluorouracil (5-FU) combination therapy against cancer was evaluated when both were given orally. Seven gastric cancer patients were treated in this manner, and 4 gastric cancer patients treated with 5-FU alone to serve as the control. AmB syrup and 5-FU syrup were administered orally for 3 days before surgical operation. Then, the drugs were given through a gastric tube into the stomach 4 hr before the start of the gastric surgery. Lesion tissues and healthy tissues were collected from each patient and the 5-FU titers were measured by bioassay. It was shown in the majority of the gastric cancer cases that the 5-FU levels in the lesion tissues were substantially higher than those in the surrounding normal tissues, whereas in the gastric cancer patients who received 5-FU alone, most of the lesion tissues showed lower 5-FU content as compared with that of the surrounding tissues.

Results of esophageal transection for esophageal varices: experience in 100 cases.

The results of transthoracic esophageal transection in 100 patients with esophageal varices are described. There were 11 operative deaths in this series, and the majority of patients died from hepatic failure. Esophageal varices disappeared completely in 81 percent of the patients and faded in 18 percent. Post-transection rebleeding was observed in six cases. There were 16 late deaths, caused mainly by hepatic failure and hepatoma. The 3 year and 5 year survival rates includine mortality rate, the efficacy in eliminating varices and the sufficient survival rate, it is presumed that esophageal transection is the most suitable operation for esophageal varices, even in poor risk patients.

The promoting effect of amphotericin B on the incorporation of neocarzinostatin into human gastric cancer tissue.

AmB/NCS combination therapy against cancer was evaluated. Six gastric cancer patients were treated in this manner, and 7 gastric cancer patients treated with NCS alone to serve as the control. AmB sirup was administered orally for 4 days before surgical operation. NCS was given intraveously at the onset of gastric surgery. Lesion tissues and healthy tissues were collected from each patient and the NCS titers were measured by bioassay. It was shown in the majority of the gastric cancer cases that the NCS levels in the lesion tissues were substantially higher than in the surrounding normal tissues, whereas in the gastric cancer patients who received NCS alone, no significant differences were found between the tissues.

Mitochondrial function of rat liver in biliary obstruction.

An investigation was performed of the liver mitochondrial respiratory function in rats after 1, 3 and 6 weeks of biliary obstruction and the following results were obtained: 1) Various parameters of liver mitochondrial respiratory function, such as respiratory control ratio, ADP/O, O2 consumption in state 3 respiration and adenosine triphosphate synthesis were found to decrease with prolongation of biliary obstruction. 2) The mitochondrial respiratory enzymes, cytochrome a (+a3) and cytochrome c(+c1) showed that both decreased in contents with prolongation of obstruction, particularly the latter. 3) The activation ratio of ATPase (latent ATPase/dinitrophenol stimulated ATPase) was increased after long term biliary obstruction, which was thought to indicate the severe damage of mitochondrial membrane. 4) Investigation of the respiratory function with the various respiratory substrates showed that the locations of mitochondrial respiratory inhibition in obstructive jaundice are at sites 1 and 2, which is the same as the situation seen in nonspecific damage of mitochondria. 5) There was a high degree of mitochondrial respiratory disturbance by bile acids, particularly CDCA, which is thought to be one of the causal factors of liver dysfunction in obstructive jaundice. 6) Mitochondrial respiratory function was markedly disturbed in hypotension, and the degree of which correlated with length of time of biliary obstruction.

It was shown in the majority of the gastric cancer cases that the NCS levels in the lesion tissues were substantially higher than in the surrounding normal tissues, whereas in the gastric polyp and the gastric cancer patients who received NCS alone, no significant differences were found between the tissues.

Morphometric studies on the rat liver in biliary obstruction.

The common bile ducts of the Wistar rats were ligated and severed, and liver biopsies were done weekly for 7 postoperative weeks. Light and electron microscopic specimens were prepared for the morphometric studies. The volume ratio of the hepatic parenchyma decline with the lapse of time after bile duct ligation. However, elevated mean sectional area of the nucleus, increased mitotic index and unchanged estimated weight of the hepatic parenchyma after biliary obstruction suggested that lost hepatocytes were compensated by regeneration. Mitochondrial swelling and curling of the cristae were noted in biliary obstruction in general. Moreover, both the number and volume ratio of the mitochondria were increased corresponding to the duration of biliary obstruction. These changes were interpreted as an adaptation process to mitochondrial dysfunction.
