Clinical review of aclacinomycin A in Japan.

Single agent activity of aclacinomycin A or aclarubicin (ACR) for acute leukaemia in adults was as follows: complete remission was achieved in 8 of 21 (38%) with untreated patients and 7 of 41 (17%) with prior chemotherapy; thus the overall complete remission rate was 24%. The optimal dose schedule was 14 mg/m2/d daily i.v. administration, and a median total dose of 200 mg/m2 and 16 days were necessary for induction of complete remission. In combination, with behenoyl ara-C, ACR, 6-mercaptopurine and prednisolone, complete remission was achieved in 40 of 60 (67%) previously untreated patients, and 41 of 65 (63%) with prior chemotherapy; thus the overall rate was 65%. In a phase II study of ACR for solid tumours, response was achieved in carcinoma of oesophagus (1/3), stomach (12/84, 14%), gall bladder (1/4), pancreas (1/8), lung (4/30, 13%), breast (6/33, 18%), uterus (1/4), ovary (3/9, 33%), head and neck (1/5) and sarcoma (1/5). Side-effects of ACR most frequently observed were nausea and vomiting (around 30%) and a moderate grade marrow suppression was noted. An ECG change was observed in 7%, but there were no cases of chronic heart failure.

[Intra-arterial infusion chemotherapy using a subcutaneously implanted silicone reservoir--with reference to the chemotherapy protocol].

Sixty patients with advanced cancers were treated with intraarterial infusion chemotherapy using a subcutaneously implanted silicone reservoir. Forty-five patients received a low dose-intermittent intraarterial infusion chemotherapy employing either combination MFC (MMC 4 mg, 5-FU 500 mg, Ara-C 40 mg) at intervals of two weeks or MMC 4 mg weekly. The results were as follows. (1) Among 30 cases, where objective evaluation of the initial response was possible, partial responses (PR) were observed in 15 cases (50%). (2) No severe complication was observed using this protocol. Only one case out of the 45 needed to discontinue the medication because of bone marrow suppression. (3) Among 28 cases, where follow-up data were available, the average hospital-free interval was 207 days. The average ratio of hospital-free interval to survival period was 56.4%. The results indicate that our method is beneficial as a maintenance chemotherapy for patients with advanced cancers, because it is effective, safer and improves the quality of life.

[A controlled study of surgical adjuvant chemoimmunotherapy with FT-207 and OK-432 for advanced stomach cancer: the fourth study].

A prospective randomized trial of surgical adjuvant chemoimmunotherapy was conducted in patients who had undergone palliative gastrectomy for previously untreated advanced stomach cancer. Immediately after surgery, one hundred and forty-seven patients were randomized to receive either chemoimmunotherapy with FT-207 plus OK-432 (group A) or chemotherapy with only FT-207 (group B). The number of patients subjected to this analysis was 134 cases (group A: 70 cases, group B: 64 cases) because thirteen patients were excluded. FT-207 was administered orally and OK-432 was administered intracutaneously. There were no differences in the background factors influencing survival time between the two groups. While the survival rate of group A patients was higher than that of group B patients, the difference in the survival rate between the two study groups was not statistically significant. However, in patients with less differentiated cancer (poorly differentiated adenocarcinoma, signet-ring cell carcinoma) the survival rate of group A patients was significantly (p less than 0.05) higher than that of group B patients. The results of the present study show that OK-432 administered intracutaneously is effective in elevating the survival rate of patients receiving chemotherapy with FT-207 for advanced stomach cancer (especially for less differentiated cancer).

[Intra-arterial infusion chemotherapy using a subcutaneously implanted silicone reservoir--with special reference to the method].

Sixty patients with advanced cancers were treated with intraarterial infusion chemotherapy using a subcutaneously implanted silicone reservoir. Forty-nine patients were catheterized from a branch of the left subclavian artery. The results were as follows. (1) In spite of some technical difficulties, catheterization from a branch of the left subclavian artery was minimally invasive, and catheter trouble using this route was less frequent than that occurring when the profound femoral arterial route was employed. (2) Implantation of the catheter end using a silicone reservoir liberated the patients from restrictions in their daily lives, and with this method, intraarterial infusion chemotherapy could be carried out easily when required without hospitalization. We conclude that this procedure is a beneficial method of intraarterial infusion chemotherapy for patients with advanced cancers.

[Phase II study of VP-16 (capsule) in malignant lymphomas. A cooperative study].

A new semisynthetic podophyllotoxin derivative VP-16 has been evaluated in a phase II study on malignant lymphomas. The drug was administered orally by capsule at a dose of 100mg twice a day for 5 consecutive days at 3-4-week intervals to a total of 53 patients. Objective responses were noted in 9 patients (25%), 4 with CR and 5 with PR, out of 36 patients with non-Hodgkin's lymphoma, while none of 5 patients with Hodgkin's disease responded. Two of 5 patients with adult T-cell lymphoma attained a PR. Overall objective response rate was 24%. Dose-limiting toxicity was hematologic and alopecia was also observed. Gastrointestinal toxicity was moderate. These results demonstrated that VP-16 administered as an oral capsule is effective against malignant lymphomas and suggested a lack of clinical cross-resistance to standard chemotherapeutic agents.

[Effect of ethyl-O-[N-(p-carboxyphenyl) carbamoyl]-mycophenolate against L1210 leukemia].

Ethyl-O-[N-(p-carboxyphenyl) carbamoyl]-mycophenolate (CAM), a mycophenolic acid (MPA) derivative, has been evaluated in terms of influence of treatment schedule and route of administration on antitumor activity in mice bearing L 1210 leukemia and cross-resistance to 6-MP was studied. The results obtained were as follows; The effect of oral administration of CAM was identical to that of MPA or FT-207. Both CAM and MPA were most effective when given on day 1, 5 and 9 following tumor inoculation. Intraperitoneal administration with MPA was marginally effective, while the activity obtained with intraperitoneal administration of CAM was superior to that for oral administration. CAM did not show any cross-resistance to 6-MP.

Fine structure of the lutein cell in the house musk shrew, Suncus murinus.

The ultrastructure of lutein cells during pregnancy and the post partum period was examined by transmission electron microscopy in the house musk shrew, Suncus murinus. Lutein cells on day 13 of pregnancy contained an extensive system of anastomosing tubules or cisternae of smooth ER and many enlarged mitochondria with numerous tubulovesicular cristae. From day 13 on, the number of small granules, possibly lysosomes, increased gradually. Between day 20 and 25, the loss of smooth ER began and elongated, or flattened mitochondria increased. Regressing lutein cells observed after parturition were characterized by abundant large dense bodies, bizarre mitochondria and a decrease in the amount of smooth ER. Unusual forms of mitochondria were always observed after day 5 of pregnancy. Two types could be distinguished; one, found frequently in the second third of pregnancy, was ring-, disc-, cup- or dumb-bell-shaped with tubulovesicular cristae, and the other, found exclusively in the last third of pregnancy and after parturition, was elongated, flattened and sometimes twisted. The paucity of lipid droplets was a characteristic feature of the lutein cells of this species. The significance of these ultrastructural changes of cellular organelles is discussed in relation to the ovarian and plasma levels of progesterone.

Bone changes and aortic calcification in aging inhabitants of mountain versus seacoast communities in the Kii Peninsula.

Bone changes and aortic calcification were compared radiographically for different age groups of the inhabitants of two communities on the Kii Peninsula at the central southern tip of the Japanese mainland, one a mountainous Shichikawa village with a traditionally restricted nutritional intake and the other a seacoast village on Oshima Island with an abundant nutritional supply. Changes in the physical properties of the bone were also assessed by resonance measurement. In the mountainous village, bone loss appeared to occur more rapidly than in the seacoast village, as indicated by a significantly higher frequency of thoracic vertebral deformity and more pronounced age-bound decrease of ulnar resonant frequency. Aortic calcification was also more frequent in Shichikawa than on Oshima Island. Such differences were more evident in women than in men. The poorer nutritional intake (especially of calcium and vitamin D) of persons living in the mountainous village (which receives less sunshine) might be responsible for the accelerated age-bound degenerative changes observed in their skeletal and vascular systems.
