[The effect of succinylcholine on vecuronium and pancuronium].

In 105 adult patients under halothane anesthesia, the neuromuscular blocking effects of vecuronium and pancuronium were determined with prior succinylcholine 1 mg.kg-1 administration and without. Force of the evoked twitch increased 123.7% of control after recovery from succinylcholine-induced block. Prior administration of succinylcholine was associated with a leftward shift of dose-response curve of vecuronium or pancuronium. Onset of the force reduction from initial dose (0.08 mg.kg-1) was faster and recovery from initial and maintenance doses (0.02 mg.kg-1) were slower. This potentiating effect persisted at least 2 hours.

[Influence of hemoglobin level, arterial oxygen partial pressure and arterial oxygen content on the results of radiation therapy in lung squamous cell cancer].

It is believed that anemia is an important prognostic factor in the case of cancer patients treated by radiation therapy. One possible explanation for this is tumor oxygenation. With respect to tumor oxygenation, the arterial oxygen content (CaO2) may be more direct indicator than hemoglobin (Hb) level. Fourty-two patients with squamous cell lung cancer treated solely by irradiation were reviewed. It was found that CaO2 was more important than the Hb level in determining a tumor response to radiation therapy. This is considered as important indirect evidence of the existence of hypoxic fractions of cells in human tumors.

[Prediction of radiosensitivity by micronucleus assay].

The correlation between cell survival curve and dose response curve of MN frequency following irradiations was studied using cytokinesis-block method. Both dose response curves were analyzed by linear quadratic model, i.e. SF = exp (- alpha D - beta D2) and MN frequency = aD + bD2 + c. A good correlation between alpha/beta and a/b ratios was observed in repeated paired experiments (gamma = 0.97). When the cells were treated with BUdR, alpha-type radiosensitizer, a value in dose response curve of MN frequency increased but b value did not. In 10 renal cell carcinomas, the linear correlations between cell surviving fractions and MN frequencies were observed. When radioresponses of 5 esophageal cancer cell lines were evaluated with MN assay, wide range of a/b ratios was found. These data present that MN frequency assay using CB method is available as a tool of rapid assay of radiosensitivity of cells.

[Acute biphenotypic leukemia followed by two color flow cytometry].

In this paper is reported a case of acute biphenotypic leukemia who was treated by chemotherapy and pursued its effect by two color flow cytometry. A 33-year-old male patient was admitted due to fever and general fatigue and diagnosed as acute leukemia by hematological findings. Surface markers were investigated to find positive reaction of Leu 12 (CD19), J 5 (CD10), My 7 (CD13) and My 9 (CD33), in which Leu 12 and My 9 were simultaneously expressed on the same blast cells by two color flow cytometry. He was treated with daunorubicin, enocitabine, mercaptopurine, vincristine, and prednisolone to obtain partial remission. Then, he was administered L-asparaginase, doxorubicin, vincristine and prednisolone to reach complete remission. The effect of chemotherapy was investigated by not only bone marrow puncture, but also by two color flow cytometry. From the findings in this case, the two flow cytometry was proved to be a useful tool for not only diagnosis of acute mixed leukemia, aut also the judgement of the effect of treatment.

[Genetic markers and thrombin reaction in a family of Bernard-Soulier syndrome].

A family with Bernard-Soulier syndrome (BSS) was investigated with reference to the genetic markers and thrombin reactions. The proband was a 24-year-old man with a life-long history of epistaxis and gingival bleeding. His parents were first cousins; furthermore, his father was born to parents of second cousins. His father also had bleeding tendency and was also diagnosed as having BSS. However, his mother and elder brother were normal. Genetic marker analysis among the family members suggested that the 16th chromosome was associated with the development of BSS, because only the haptoglobin genotype coded on the 16th chromosome was the marker in both the proband and his father. In addition, they both exhibited decreased thrombin-induced platelet aggregation at a low dose, but an almost normal reaction at a high dose of thrombin.

ERCP in the assessment of patients with post-cholecystectomy syndrome: benefits and limitations.

The aim of our study was to assess the diagnostic accuracy yielded by endoscopic retrograde cholangio-pancreatography (ERCP) in a group of 41 patients presenting with persistent or recurrent abdominal pain and/or cholestasis following cholecystectomy. Each patient had previously undergone, without success, a different combination of non-invasive tests. Cannulation with adequate opacification of at least one duct was achieved in all patients. Aetiologically diagnostic findings obtained with ERCP were as follows: normal 36.8%, choledocholithiasis 34%, benign biliary stenosis 9.8%, chronic pancreatitis 4.9%, pancreatic carcinoma 2.4%, ampullary carcinoma 2.4%, cholangiocarcinoma 2.4%, miscellaneous 7.3%. ERCP gave a final diagnosis in 26 patients (63%) and in all the cases presenting with cholestasis. ERCP plays a first-line role in the diagnostic assessment of patients with the post-cholecystectomy syndrome. However, there is still a considerable part of this population in whom ERCP does not contribute to a diagnosis.

Control of the activity of branched-chain alpha-keto acid dehydrogenase complex in primary cultured rat hepatocytes.

When isolated rat hepatocytes were cultured in the medium containing 0.5 mM clofibrate (dissolved in ethanol) for 6 hours, the activity of branched-chain alpha-keto acid dehydrogenase (BCKADH) complex in the cells increased to more than two times that of control cells. Immunochemical determination with an anti-BCKADH IgG revealed that the activity increase occurred without increase in the enzyme amount. On the contrary, BCKADH activity in control cells (cultured in the presence 0.4% ethanol) decreased to two-third the initial activity at the end of 6-hour incubation whereas the activities of other enzymes tested were practically unaltered either in control or in clofibrate-treated cells. When the control cell extract was incubated with rat liver protein phosphatase, the BCKADH activity increased near to that of clofibrate-treated cells, but the same treatment of clofibrate-treated cell extract produced practically no increase in the activity. These observations indicate that clofibrate addition brought about the increase of BCKADH activity through activation-inactivation mechanism based on dephosphorylation-phosphorylation, and that this mechanism might function on liver BCKADH complex in vivo as a short term control of the activity in some conditions.

[Acute myeloblastic leukemia associated with chronic thyroiditis].

A 57-year-old woman who suffered from acute myeloblastic leukemia during the course of chronic thyroiditis, is described. The patient was diagnosed as having chronic thyroiditis in 1984 when she was 53 year-old, and was treated with L-T4.Na. She admitted in July 1988 because of general fatigue, fever, cough and sore throat. On admission, hematological examination in the peripheral blood showed marked anemia and increased leukocytes with 20.5% leukemic cells positive for peroxidase staining. Bone marrow aspiration showed 38.8% leukemic cells. She was diagnosed acute myeloblastic leukemia. She reached complete remission after combination chemotherapy. The case of acute myeloblastic leukemia associated with chronic thyroiditis is rarely reported. We reviewed the literature and discussed acute myeloblastic leukemia associated with chronic thyroiditis including this case.

Expression of Forssman glycolipid and blood group-related antigens A, Le(x), and Le(y) in human gastric cancer and in fetal tissues.

The expression of Forssman glycolipid antigen in human gastric cancers was investigated by thin-layer chromatogram immunostaining of glycolipid fractions. Incompatible blood group A antigen, Le(x) and Le(y) antigen were also studied in comparison with Forssman antigen. Forssman glycolipid as the pentaglycosylceramide was demonstrated in nine out of 12 gastric cancers, and in three out of 10 adjacent uninvolved tissues. In most cases the content of Forssman glycolipid was increased in the cancers compared with that in the uninvolved counterparts. Forssman pentaglycosylceramide was also detected in some normal gastric mucosae (two out of four), and in a fetal gastrointestinal tract tissue. In immunohistochemical examination of gastric cancer tissues, sialidase treatment revealed a positive staining with anti-Forssman antibody. Some cancer tissues from patients with blood group O were found to contain blood group A-active glycolipids, which could be distinguished from Forssman glycolipid by thin-layer chromatogram immunostaining. The incidence of incompatible A-active glycolipids was two out of 10 cancers from patients with blood group O or B. Le(x)- and Le(y)-active glycolipids were detected in most of the preparations and were not accumulated consistently in the cancers.

[Clinical evaluation of buprenorphine suppositories in postoperative patients--plasma concentrations and effects on the respiratory and circulatory system].

The plasma concentrations of buprenorphine were measured and blood gas analysis was done after administration of buprenorphine suppositories (0.2 and 0.4 mg) or its intramuscular injection (0.2mg) in postoperative patients. The C max for 0.4mg suppositories was comparable to that for 0.2mg injection, whereas the T max for suppositories was as large as about 2 hrs in contrast to 1.5 hrs for 0.2mg injection. The size of AUC was in the following order: 0.4mg suppositories greater than 0.2mg injection greater than 0.2mg suppositories. Neither notable increase in PaCO2 nor decrease in respiration rate was observed after administration of the suppositories, indicating that the formulation will not cause clinically significant respiratory depression. No significant changes were observed in blood pressure or pulse rate. Buprenorphine suppositories are considered to be safe for use in postoperative patients. Percent pain relief in the suppository group was smaller than that in the injection group. This appears to be due to a slower rate of increase in the plasma levels of buprenorphine after administration of suppositories than that after intramuscular injection. Therefore, it seems practical to give this drug to postoperative patients before the start of pain.
