The pharmacokinetics and pharmacodynamics of a new thromboxane synthetase inhibitor, 6-(1-imidazolylmethyl)-5,6,7,8-tetrahydronaphthalene-2-carboxylic acid (DP-1904) in man after repeated oral doses.

The pharmacokinetics of DP-1904, a new potent and selective thromboxane synthetase inhibitor, and its effects on ex-vivo prostanoid formation have been studied in groups of Japanese normal male volunteers, who received repeated oral doses of 200 mg every 12 h for 4 doses, or 400 mg every 24 h for 2 doses, or 200 mg every 12 h for 14 doses. The drug was well tolerated by all subjects without evidence of adverse reactions. Repeated administration showed no significant changes in half-lives, tmax values, cmax values and AUC values. DP-1904 did not exhibit time-dependent kinetics. Its plasma levels were lower than the quantifiable level (50 ng mL-1) at 12 h after each dose. These data suggest no significant accumulation of DP-1904 in normal volunteers. DP-1904 reduced the serum thromboxane B2 by about 80% during the medication, the serum concentrations returning to about 44, 75 and 20% of the predrug control values at 36 h after the last 200 mg doses and 48 h after the last 400 mg dose.

Propranolol blocks cocaine-induced potentiation of the contraction in the smooth muscle of the rat vas deferens.

In the smooth muscle of the rat vas deferens, 10(-5) M cocaine shifted the dose-response curve to norepinephrine to the left and enhanced the maximal contractions to norepinephrine and methacholine. Propranolol at 10(-4) M almost completely blocked these potentiating effects of cocaine. Sotalol or lidocaine at 10(-4) M did not block the potentiating action of cocaine. The cocaine-induced enhancement of the methacholine contraction and its blockade by propranolol was observed in the dibenamine-treated vas deferens. Propranolol also blocked the depolarization-induced enhancement of the methacholine contraction. Together with the previous study demonstrating the calcium-antagonist action for propranolol, these results suggest that propranolol blocks the cocaine-induced enhancement by inhibiting the calcium influx through the potential-dependent calcium channel in the rat vas deferens.

Synthesis and anti-human immunodeficiency virus (HIV-1) activity of 3'-deoxy-3'-(triazol-1-yl)thymidines and 2',3'-dideoxy-3'-(triazol-1-yl)uridines and inhibition of reverse transcriptase by their 5'-triphosphates.

3'-Deoxy-3'-(1,2,3-triazol-1-yl)thymidines (5a, 6a, 8a, 11a, and 12a) and 2',3'-dideoxy-3'-(1,2,3-triazol-1-yl)uridines (5b, 6b, 8b, 11b, and 12b) were synthesized as cyclic analogues of 3'-azido-3'-deoxythymidine (AZT) and 3'-azido-2',3'-dideoxyuridine (CS-87) by the cyclization of 5'-trityl derivatives (1a, b) of AZT and CS-87 using alpha-ketophosphorus ylides and with acetylenic compounds followed by deprotection of the 5'-trityl group. It was hypothesized that the triazole nitrogen atoms could mimic and distorted azido group. However, no significant activity against human immunodeficiency virus type 1 (HIV-1) was observed with any of these compounds. 5'-Triphosphates (17a and 18a, b), prepared from 5a and 6a, b, were inactive against HIV-1 and Rauscher murine leukemia virus (RLV) reverse transcriptases.

Immune mediated skin lesions in a dog with autoimmune hemolytic anemia.

A canine case of Coombs' test positive and antinuclear antibody-negative hemolytic anemia was examined because of the development of skin lesions after 18 months treatment with prednisolone. Histopathological examination of biopsy specimens obtained from skin and oral mucosa revealed the acantholysis, edematous lesions of the stratum basale and mononuclear cell accumulation in the dermis. Deposits of immunoglobulin G and complement factor 3 were detected at the intercellular and dermoepidermal junction by the direct immunofluorescent test. From these results, the case was considered to be an autoimmune disease caused by distinct antibodies against different organs.

A new staining solution for the morphological studies of fungi and Prototheca.

A new staining solution named "PHOL" is described for studying the morphology of clinical and environmental isolates of fungi and Prototheca. In total 69 isolates were used for assessing the efficacy of this newly formulated stain. The solution has shown good ability to stain all 62 isolates of fungi and 7 of Prototheca, and thus helped in the identification of the organisms. The new solution has the potential to stain the young as well as old isolates of fungi and Prototheca. It is emphasized that a wider application of this newly introduced inexpensive and safe staining solution will help the mycologists to study the morphological characteristics of yeast, molds, and Prototheca obtained from man, animals and environment.

Effect of Toxoplasma lysate antigen (TLA) on feline cytotoxicity against FeLV positive lymphoma cells.

The cytotoxic activities of feline spleen cells treated with Toxoplasma lysate antigen (TLA) were assayed against feline leukemia virus (FeLV)-producing lymphoma. FL74 cells, and xenogeneic target lymphoma, mouse YAC-1 cells. The TLA treatments were performed in vivo alone, in vitro alone, and in vivo plus in vitro, respectively. In vivo plus in vitro treatments with TLA induced a marked augmentation in cytotoxic activity of spleen cells to FL74 cells. The treatment with TLA in vivo alone showed an enhancement of cytotoxic activity but in vitro alone did not. The cytotoxic effects of TLA-treated spleen cells obtained from the cats which had been previously immunized with live FL74 cells were similar to those of spleen cells from non-immunized cats treated with TLA. However, no increase of cytotoxicity was shown in the response to mouse YAC-1 cells regardless of TLA treatments. These results indicated that the in vivo TLA treatment augmented the cytotoxicity of feline spleen cells against FeLV-producing lymphoma cell.

The changes of antioxidative enzyme activities in equine erythrocytes following exercise.

The change in activities of 3 major antioxidative enzymes in equine erythrocytes, superoxide dismutase (SOD), glutathione peroxidase (GSHpx), and catalase, was investigated in order to evaluate the effect of exercise. Blood samples were obtained from 11 thoroughbred horses before and immediately after vigorous exercise which induced the increase of plasma lipid peroxide (Lpx) concentration from 1.16 +/- 0.40 nmol/ml to 1.29 +/- 0.34 nmol/ml. Following the exercise, the GSHpx activity in erythrocytes was significantly reduced from 69 +/- 10 IU/gHb to 65 +/- 8 IU/gHb, whereas SOD and catalase activities were not changed. Effects of an antioxidative compound, containing selenium and vitamin E(Se-E), on the response of antioxidative enzyme activities following the exercise were examined. Seven horses were injected intramuscularly with Se-E(Se:25 mg, vitamin E:54.8 mg) and received the same vigorous exercise. After Se-E treatment, plasma Lpx levels before the exercise were decreased from 1.24 +/- 0.09 nmol/ml to 0.86 +/- 0.03 nmol/ml, however, SOD, GSHpx, and catalase activities were not varied. The Se-E treatment slightly prevented the decrease in GSHpx activity and the increase in plasma Lpx level after the exercise, having no effect on SOD and catalase activities. These results suggested that the changes of GSHpx activity in erythrocytes might reflect the protective condition against exercise-induced lipid peroxidation.

The use of calcium hydroxyapatite ceramic in bone tumour surgery.

We report 60 benign bone tumours treated by resection and curettage followed by the implantation of calcium hydroxyapatite ceramic (CHA). After follow-up of six to 60 months (average 36), no patient had local recurrence of the tumour or any adverse effects from the implants. In almost all cases radiography showed that the CHA was well-incorporated into the host bone, with new bone formation in and around the CHA. Corrective remodelling of deformed bone and normal fracture healing suggested that there was normal bone turnover in the presence of the CHA. Histology of biopsies from seven patients showed bone ingrowth into the pore structure of CHA in the central zone of some defects by one year after implantation. CHA appears to be a useful substitute for bone graft in the treatment of some benign tumours.

Chiari pelvic osteotomy for osteoarthritis. The influence of the torn and detached acetabular labrum.

We assessed the results of Chiari pelvic osteotomy in 64 hips with early osteoarthritis in terms of acetabular labral detachment detected pre-operatively by arthrography. At an average follow-up of four years, assessment by the Merle d'Aubigńe score showed 83% excellent or good results. These satisfactory results were achieved in only half the cases with a detached labrum, but in nearly all cases with normal or torn acetabular labra. Other factors such as the acetabular index, the level and angle of osteotomy, and the displacement following osteotomy did not affect the results. Chiari pelvic osteotomy is a worthwhile procedure for early osteoarthritis in selected cases, but a detached acetabular labrum increases the risk of clinical failure.

Replacing the medial collateral ligament with an allogenic tendon graft. An experimental canine study.

In order to determine whether an allogeneic tendon could be used to replace an extra-articular ligament, the right medial collateral ligament from 11 adult dogs was replaced with a fresh-frozen allogeneic patellar tendon. At each of 3, 6, 15, 30 and 52 weeks postoperatively, one dog was killed for micro-angiographical and histological studies; at 52 weeks the remaining six dogs were killed for tensile testing. Micro-angiograms showed that the allogeneic tendon was revascularised with infiltration of the mesenchymal cells from the surrounding tissues and both ends of the graft. Histologically, the alignment of the fibroblasts and collagen bundles became more regular over time, without any immunological rejection. A biomechanical study performed at 52 weeks found no significant difference in stiffness or ultimate load between normal and reconstructed ligaments. Fresh-frozen allogeneic tendons are therefore considered useful for extra-articular ligament reconstruction.

Lipid composition of muscle and adipose tissue from crossbred ram, wether and cryptorchid lambs.

Lambs from rams (R), cryptorchids (C) and wethers (W) were examined to compare with the dietary guidelines for humans on fat and cholesterol intake. Twenty-four lambs were assigned to treatments as R, C or W at weaning (60 d). Each lamb had ad libitum access to a 77:23% forage:concentrate diet containing 11.7% crude protein and 1.84 Mcal ME/kg. The experiment was terminated when the lambs reached 50 kg body weight. Lipid composition of the longissimus muscle (LM) and s.c. adipose tissue was determined by gas-liquid chromatography. Rams has less (P less than .01) s.c. fat (cm) and i.m. lipid (%) (.26 cm; 3.*%) than W (.56 cm; 4.9%). Total polyunsaturated fatty acids were greater (P less than .01) in the LM of R (7.06%) than in LM of W (5.21%). Rams had higher percentages of C18:2, C18:3, C20:4 and C18:1 in their LM than W did. On the basis of a 100-g serving of LM, R had 1.73, C had 2.15 and W had 2.21 g saturated fatty acids. There were no (P less than .01) differences among sex types for cholesterol content of LM, which averaged 66.6 mg/100 g wet tissue. The only major difference of the s.c. fat lipid composition was in saturated fatty acids. Wethers had higher (P less than .01) saturated fatty acids (51.4%) than C 46.8%) and R (43.3%). Based on the lipid composition of these three types of lambs, the LM muscle from young R lambs approached dietary fat guidelines for humans more closely than did that from castrates.

Combined effect of radiation and YM-881 (SMANCS) on murine tumors and bone marrow.

The combined effect of radiation and YM-881 (SMANCS) was studied in vitro and in vivo. When 0.25 microgram/ml of YM-881 was simultaneously combined with radiation, during and after irradiation for 30 min in total, Dq decreased from 3.3 Gy to 1.4 Gy without changing D0 in the dose-survival curve of exponentially growing SCC VII tumor cells. Five or ten times administrations of 0.1 mg/kg YM-881 at an interval of 24 h did not inhibit tumor growth. However, administration of 0.1 mg/kg YM-881 just before every irradiation which was repeated five times at an interval of 24 h yielded dose modifying factors (DMFs) of 1.8-1.2 when the tumor response to treatment was evaluated by the time for the tumors to regrow to three times the original volume. Administration of YM-881 ten times just before every irradiation yielded DMFs of 1.3-1.2. Adverse effects of the combination on bone marrow were examined by spleen colony assay. After five injections of 0.1 mg/kg YM-881, the mean number of CFU-S per femur decreased to 77% of the pretreatment level, but this was not significant statistically (0.1 greater than p greater than 0.05). The slope of radiation response curve for CFU-S per femur was not affected by the combination.

[Aspiration biopsy of regional lymph node in bladder cancer].

Percutaneous transabdominal fine needle aspiration biopsy (FNAB) of the pelvic and retroperitoneal lymph nodes was performed in 116 patients with bladder cancer. Metastasis to the regional lymph nodes was determined by this method in 21 patients. FNAB was positive in 10 of 12 patients having unequivocally positive or highly suspicious lymphogram, and in 11 of 104 patients (11%) having normal lymphogram. The results of FNAB were compared to the finding of lymph node dissection (LND) in 51 patients. FNAB and LND were negative in 43 patients and positive in 5. Two patients were FNAB negative but LND positive, and the remaining one case was FNAB positive but LND negative. The correlation between cytological diagnosis of FNAB and histological diagnosis of LND was 94 per cent. The survival rate of the cases of FNAB positive and lymphography negative was significantly higher than that of lymphography positive (p less than 0.01). The metastatic site of 5 cases with bladder cancer who are alive now without disease after more than 3 years was under the common iliac node and the number of the involved nodes was within 3.

[An endoscopy-guided plan of radiotherapy for detecting esophageal cancer using a lugol staining].

The Lugol staining method has lately come to play an important role in the detection of superficial esophageal cancers which are extremely difficult to detect by using only a barium double-contrasted esophagiogram and conventional esophagioscopy. Thus we have tested an endoscopy-guided plan of radiotherapy for detecting an esophageal cancer using Lugol stain. This method was found useful in detecting intraepithelial small lesions, such as intramural metastasis or multiple origin cancers. Additionally, it has been found useful in determining the definite shape of superficial malignant lesions by evaluating biopsy specimens taken from the border of the non-stained regions.

Conjunctival allergenicity-supporting factor (CASF) in DIIIa as the sea squirt antigen capable of inducing asthmatic attack and conjunctival reaction in sea squirt asthma.

Among three glycoproteins, DIIIa, Ei-M, and Gi-rep, isolated from sea squirt as antigens capable of eliciting skin reaction specific to sea squirt allergy, only DIIIa induces asthmatic attack and conjunctival reaction, whereas Ei-M and Gi-rep do not. Periodate oxidation eliminated not only the conjunctival allergenicity but also the skin allergenicity from DIIIa, suggesting that the conjunctival and skin allergenicity could both be ascribed to the epitope residing in the carbohydrate chains of DIIIa. On the other hand, proteolysis with Pronase E and chemical modifications of the carboxyl or amino groups by various methods eliminated the conjunctival allergenicity from DIIIa, whereas the skin allergenicity was still retained. Therefore, the expression of conjunctival reaction was expected to be supported by a relatively small structural factor residing in the protein moiety of DIIIa, which we have tentatively designated as conjunctival allergenicity-supporting factor (CASF). CASF might contribute to the permeability of the antigen into mucosa, enabling the carbohydrate epitope to interact with the allergy-specific IgE distributed in conjunctiva and bronchi . In specificity analysis of a rabbit anti-DIIIa serum, a periodate oxidation- and acid-stable but heat- and alkali-unstable epitope (type gamma) in the protein moiety of DIIIa was also characterized as a useful marker of the antigen. However, the type gamma epitope was apparently independent of CASF that was substantially stable to alkali and heat.

Ultrastructure of surface epithelial cells of the normal human rectum.

Luminal surface epithelial cells, excluding a few endocrine cells of the normal human rectum, were studied electron microscopically and 5 types of cells were recognized with special reference to some structure containing mucous substances. Principal-1 cells showing few tiny vesicles and Principal-2 cells containing some tiny vesicles seemed to belong to the absorptive cell group. Vesicle cells having numerous tiny vesicles, and Columnar mucous cells accompanied by numerous tiny vesicles and some round or oval mucous vacuoles, seemed to be labelled as of the secretory cell group. The common features of the epithelial columnar cells, except for the Goblet cell, were columnar shape, microvilli whose length and density had considerable variation, glycocalyceal bodies around the microvilli, and thick surface coat. Goblet cells were characterized by a goblet shape which was expanded by numerous mucous droplets. It is of special interest that 4 different types of columnar epithelial cells are recognized on the luminal surface of the normal human rectum, and that Vesicle cells and Columnar mucous cells are first observed on the luminal surface of the large intestine. Similar epithelial cells have only been reported in the crypt of the large intestine and not on the luminal surface.

[Thrombotic thrombocytopenic purpura (TTP) in which plasmapheresis was ineffective but vincristine slow infusion therapy was effective with long-term remission].

A 38-year old female patient with thrombotic thrombocytopenic purpura (TTP) did not respond to plasmapheresis and treatment with corticosteroids and antiplatelet agents but did respond to subsequent high-dose gamma-globulin therapy and vincristine slow infusion therapy. Remission, thought to be due to vincristine, was achieved. The administration schedule of vincristine was 1 or 2 mg once a week by intravenous drip infusion over a period of 6 to 8 hr. Improvement tended to be seen after the 6th administration, and remission was achieved after the 12th administration (total dose: 15 mg), and continues to the time of this writing. In this case, it was thought that vincristine inhibited a certain excessive reaction in the process of PAIgG-related platelet consumption. We conclude that vincristine should be administered at least 5 to 6 times, and the therapeutic method should be established based on the experience in a larger number of clinical cases.
