Patterns of jaw reflexes induced by incisal and molar pressure stimulation in relation to background levels of jaw-clenching force in humans.

Patterns of jaw reflexes induced by periodontal stimulation were examined in ten adults. Surface electromyograms (EMGs) from the masseter and anterior temporal muscles were recorded when pressure stimulation was applied to either an incisor or a molar. Reflex responses to periodontal pressure stimulation varied, depending on the background levels of jaw-clenching force that preceded stimulation (background clenching force, BCF). At low BCF, excitatory reflexes were elicited from the jaw-closing muscles and jaw-clenching force. However, the magnitude of excitatory reflexes varied with the location of the stimulated tooth along the dentition. While excitatory reflexes were induced equally in the masseter and temporal muscles during incisal stimulation, stronger excitatory reflexes were induced in the temporal muscle than in the masseter muscle during molar stimulation. At high BCF, inhibitory reflexes in the jaw-closing muscles and jaw-clenching force were elicited in eight subjects (group A) during periodontal stimulation. However, excitatory reflexes in the muscles and force were elicited in the remaining two subjects (group B). In the subjects of group A, stronger inhibitory reflexes were elicited in the temporal muscle than in the masseter muscle, and jaw-clenching force also decreased during both incisal and molar stimulation. In the subjects of group B, the magnitude of excitatory reflexes decreased with increases in BCF.

Comparison of neural network and k-NN classification methods in medical image and voice recognitions.

We make a comparison of classification ability between BPN (Back Propagation Neural Network) and k-NN (k-Nearest Neighbor) classification methods. Voice data and patellar subluxation images are used. The result was that the average recognition rate of BPN was 9.2 percent higher than that of the k-NN classification method. Although k-NN classification is simple in theory, classification time was fairly long. Therefore, it seems that real time recognition is difficult. On the other hand, the BPN method is long in learning time but is very short in recognition time. Especially if the number of dimensions of the samples is large, it can be said that BPN is better than k-NN in classification ability.

Differences in response of patient's blood cells with bronchial asthma to diverse house dust mite allergens by histamine release assay.

A HPLC method was developed for the determination of histamine released from washed blood cells of allergic patients after exposure to allergens in the presence or absence of plasma. The blood cells used were preserved in a vital state at 4 degrees C during at least 6 hours after blood-collecting. The responses of blood cells to an appropriately diluted series of a mite feces extract (Dff) solution were evaluated in three patient's groups (68 cases of hyposensitized, 59 cases of newly visited and 44 cases of the others) with bronchial asthma. Washed blood cells from the hyposensitized groups had a tendency to release histamine easily even with lower concentration of the Dff solution. Both the sensitivities of washed blood cells to Dff or mite body extract (Dfb) and the responses of the cells to diverse antigens in neither extract differed largely in individual cases. The titer of blocking antibody could be expressed as the amount proportional to the concentration of Dff or Dfb.

Immunohistochemical analysis of myoblast proliferation and differentiation in experimental skeletal muscle regeneration.

We investigated the onset and duration of DNA synthesis and differentiation of muscle precursor cells in the early stage in muscle regeneration after bupivacaine induced muscle necrosis using monoclonal anti-PCA and anti-desmin antibodies. The first positive PCNA reaction indicating the commencement of proliferating cycle of muscle precursor cells appeared at 24 hours after injury, and were most prominent at 60-72 hours after injury. Thereafter the reaction for PCNA disappeared as the maturation of regenerated myofibers progressed. Some nuclei of regenerated myotubes in the early stage were positive for PCNA contrary to the generally accepted concept that myonuclei after fusion were out of cell cycle. The first desmin expression was observed in mononuclear presumptive myoblasts at 48 hours after injury, some of which were also positive for PCNA in the double immunostained sections for desmin and PCNA. This result suggested that differentiation of presumptive myoblasts could already initiate during their proliferative phase.

[A case of Rendu-Osler-Weber disease associated with pulmonary arteriovenous fistula].

A 60-year-old female was admitted with a chief complaint of left hemothorax. Chest X-ray films revealed an abnormal shadow when the pleural effusion disappeared. On the physical examination, multiple telangiectases were found at the nasal mucosa, tongue and stomach. The family history showed that her son experienced frequent episodes of epistaxis. The patient was diagnosed to have Rendu-Osler-Weber disease. CT scanning and pulmonary angiography showed an arteriovenous fistula of the left lower lobe (S10) and cerebral angiography did a small arteriovenous fistula in the cerebellum. Pulmonary partial resection was performed to prevent re-hemorrhage. Pulmonary arteriovenous fistula was 2.5 x 1.5 cm in size. It located in the left lower lobe exposing through the surface of the lung over the diaphragm. The postoperative course was uneventful and she was discharged on the 18th postoperative day without any complications.

Hyperkalaemia with renal tubular dysfunction by sulfamethoxazole-trimethoprim for Pneumocystis carinii pneumonia in patients with lymphoid malignancy.

Hyperkalaemia with renal tubular dysfunction by oral therapy of sulfamethoxazole-trimethoprim (co-trimoxazole) is described in 2 elderly Japanese patients with lymphoid malignancy, who developed Pneumocystis carinii pneumonia and improved. A high dose of cotrimoxazole induced hyperkalaemia with the elevation of serum creatinine and blood urea, and increased urinary N-acetyl glucosaminase after several days of the drug administration in these patients; one patient became unconscious. Discontinuation of co-trimoxazole normalized serum potassium level and symptoms. A repeated low dose of the drug induced hyperkalaemia. Before the treatment of co-trixomazole, their serum levels of creatinine showed upper limits of normal ranges. In the present study, our cases suggested that patients receiving a high dose of co-trimoxazole should be evaluated for these potential complications during a course of treatment, particularly in elderly patients with preexisting renal dysfunction.

[Effects of prostaglandin E1 on temperature gradients between hand and foot during epidural anesthesia].

We studied the effects of prostaglandin E1 (PGE1) on hand-foot temperature gradients (foot temperature-hand temperature) induced by lumbar epidural anesthesia. Thirty-six ASA class 1 patients undergoing rotatory acetabular osteotomy were randomly assigned to receive 0 gamma (n = 13), 0.005 gamma (n = 10) and 0.02 gamma (n = 13) of PGE1. Thermometric probes were placed on the palm and sole of each patient. Bladder temperature was also monitored to evaluate the change of core temperature. Epidural catheter was indwelled at the level between L2 and L3 prior to induction and 2 % lidocaine was administered epidurally. All patients were warmed by water blanket at 37 degrees C and fluids for infusion warmed at 37 degrees C. Humidity and moisture exchanger was placed in breathing circuit. Hand-foot temperature gradients of patients given 0 and 0.005 gamma of PGE1 increased to 2.8 +/- 1.5 and 2.3 +/- 2.3 degrees C at extubation, respectively. In contrast, temperature gradient of patients given 0.02 gamma of PGE1 was 0.0 +/- 1.8 degrees C (P < 0.01). There was no difference among the values of bladder temperature in each group. The effects of PGE1 on mean arterial pressure, heart rate and central venous pressure were minimal. These data suggest that 0.02 gamma of PGE1 administration is effective to reduce hand-foot temperature gradient during epidural anesthesia.

Polylactic acid-polyethylene glycol block copolymer. A new biodegradable synthetic carrier for bone morphogenetic protein.

A new biodegradable polymer, a polylactic acid-polyethylene glycol (PLA-PEG) block copolymer, proved to be an effective and suitable carrier for bone morphogenetic protein (BMP). Composites of semipurified BMP and PLA-PEG consisting of a PLA segment with a molecular weight (MW) of 650 d and a PEG segment with a MW of 200 d (PLA-PEG 650-200) were implanted under the fasciae of the dorsal muscles of mice. Three weeks after implantation, the PLA-PEG 650-200/BMP composites were completely absorbed and replaced by newly induced bone with hematopoietic marrow. The composites induced twice as much bone as composites of BMP and a 650-d PLA homopolymer. Results indicate that of all the biodegradable synthetic polymers the authors have tested, PLA-PEG 650-200 is the most suitable and effective BMP carrier. Composites of PLA-PEG 650-200/BMP and hydroxyapatite powder (HAP) also induced ectopic bone formation. Because PLA-PEG 650-200/BMP is viscous and semiliquid and PLA-PEG 650-200/BMP/HAP is doughy and plastic, the former can be used as an injectable osteoinductive material and the latter as a plastic mold.

Gene cloning and expression of a bone morphogenetic protein derived from a murine osteosarcoma.

Based on information from partial amino acid sequences of a protein with bone-inducing activity that was purified from a murine osteosarcoma (Dunn type), a cDNA library of the sarcoma was screened to clone a gene complementary to the protein. The cloned cDNA was amplified and transfected into Chinese hamster ovarian (CHO) cells for expression. When the protein produced by the transfected cell line was implanted in combination with pure carrier collagen into allogeneic mice, ectopic ossicles consistently developed at implanted sites within two weeks. The nucleotide sequence of the cDNA and its deduced amino acid sequence were homologous to those of human bone morphogenetic protein-4 (also BMP-2B). In addition, the cDNA and deduced amino acid sequences were identical to those proposed for murine BMP-4 derived from the normal murine fetus. It is postulated that the cloned cDNA encodes the protein responsible for bone formation induced by implantation of devitalized Dunn-type osteosarcoma tissue or cells. The protein product was identified as murine BMP-4, a member of the TGF-beta gene family.

[Spontaneous remission in adult T-cell leukemia].

Spontaneous remission in a 50-year-old woman with adult T-cell leukemia (ATL) is presented. The patient was referred to our hospital because of generalized lymphadenopathy and the appearance of abnormal lymphocytes with convoluted nuclei in the peripheral blood. She was diagnosed as ATL because of the characteristic morphological ATL cells, cell surface marker analysis and the presence of serum antibody to human T-cell lymphotropic virus type-I (HTLV-I). However during the following weeks before admission, her leukocyte count and ATL cells in the peripheral blood decreased in number even though she received no therapy. After admission, abnormal lymphocytes in the peripheral blood disappeared and lymphadenopathy decreased in size, LDH level was normalized. Spontaneous remission had continued ten months without chemotherapy, but she developed recurrence thereafter and died two years later after onset with progressive disease. Gene analysis study by Southern blot analysis showed monoclonal integration of HTLV-I proviral DNA at the same positions both before regression and after recurrence. In this case, the trigger of spontaneous remission was unclear.

[Clinical evaluation of hepatic blood flow and liver function with 99mTc-DTPA-HSA].

Clinical evaluation of hepatic blood flow and liver function of 30 patients with 99mTc-human serum albumin scintigraphy (99mTc-HSA) was done. In this study we evaluated the ratio of portal venous to total hepatic blood flow as the hepatic perfusion index (HPI), and 99mTc-HSA uptake ratio of the liver to the heart at 2 hrs after bolus injection as the hepatic uptake score (HUS). In clinical study, we estimated both HPI and HUS in patients with liver cirrhosis (LC) and non-liver-cirrhosis (non-LC), and in the same way estimated these two factors in the patients before and after distal splenorenal shunt operations (DSRS). We also estimated the correlation of both HPI and HUS with other liver functions. Finally we made 3 dimensional liver imagings using 99mTc-HSA. The mean HPI was 0.42 +/- 0.24 in the LC group and 0.66 +/- 0.19 in the non-LC group (p < 0.02). The mean HUS was 0.50 +/- 0.17 in the LC group and 0.67 +/- 0.13 in the non-LC group (p < 0.02). The mean HPI decreased to 32-54% after the DSRS operation, but there was no such change with HUS. Correlation between HPI and KICG was significant (r = 0.51, p < 0.02), and there was also a correlation between HUS and PT, HPT (p < 0.02), and ICGR15 (p < 0.02). We concluded that HPI and HUS were both useful factors in estimating hepatic blood flow and liver function.

Total hip arthroplasty in the neglected congenital dislocation of the hip. A five- to 14-year follow-up study.

A retrospective study was performed on 34 hips of 28 patients with neglected congenital dislocation of the hip after cemented total hip arthroplasty (THA) using a straight-stem prosthesis in 21 and a curve-stem prosthesis in 13 hips. The mean age at operation was 49 years (28-72 years) and the mean follow-up period was 9.4 years (5.6-14 years). Functional evaluation using Merle d'Aubigné Hip Score showed 71% success rate. Radiographic loosening occurred in three femoral and eight acetabular components attributable to poor cementing in all stems and three sockets, and to dislocation due to trauma and graft collapse in two sockets. Marked acetabular deficiency showed increased incidence of socket loosening. Both types of prostheses showed the same statistical results. Localized endosteal osteolysis appeared in five hips at the sites of less than 1-mm cement thickness. Greater trochanter nonunion reduced the functional status evidenced by pain, limping, and a statistically significant Trendelenburg sign. Proper cementing, effective acetabular reconstruction, and prevention of trochanter nonunion can produce excellent long-term results.

[Concentration of mitomycin C in portal blood and peripheral blood after intra venous, intraperitoneal or intrapelvic administration].

Immediately before closing the abdomen in a curative operation for colorectal cancer, 20 mg of Mitomycin C (MMC) was administered by intravenous, intraperitoneal or intrapelvic routes, and portal blood and peripheral blood were simultaneously collected to investigate the disposition of MMC. After intravenous administration, the MMC concentration in portal and peripheral bloods changed at the same levels, showing a biphasic pattern. The maximum concentration (Cmax) seen shortly after administration was about 2 micrograms/ml. The blood halflife in phase alpha was about 5 min, and that in phase beta about 30 min. After intraperitoneal administration, the MMC concentration in portal blood was over 60 min higher than that in peripheral blood. Cmax in portal blood, seen within 5 min after administration, was about 0.5 micrograms/ml, and that in peripheral blood was about 0.2 micrograms/ml around 10 min. The half-life was about 30 min in both types of blood. After intrapelvic administration, the MMC concentration in portal blood changed at higher levels than in peripheral blood, as after intraperitoneal administration. The Cmax in portal blood, obtained within about 5 min, was about 0.3 micrograms/ml, and that in peripheral blood was about 0.1 micrograms/ml at 12 min. The half-life was about 30 min in both types of blood. As described above, the disposition of MMC was investigated on the basis of the concentration in portal and peripheral blood. As a result, intravenous MMC showed a similar change, whereas intraperitoneal or intrapelvic MMC was higher in portal blood than in peripheral blood, indicating that the disposition of MMC was different with the route of administration.

[Effect of combined spinal and epidural anesthesia on blood loss during total hip replacement].

Ninety total hip replacements (THRs) performed by one surgeon were reviewed to compare the effects of different anesthetic techniques on the perioperative bleeding. Half of the THRs were performed under either combined spinal and epidural (CSE) anesthesia with lidocaine or general anesthesia with N2O/O2, vecuronium, enflurane or isoflurane. Deliberate hypotensive technique was not employed in any THR. All patients were female and had suffered from osteoarthritis of the hip joints. The patients in the two anesthesia groups were similar as to age, body weight, height, perioperative hemoglobin level, duration of anesthesia and operation, and blood transfusion requirement. A positive linear correlation existed between total operative blood loss and operation time in each group (P < 0.01). The rate of operative blood loss was significantly higher in the CSE anesthesia (6.2 +/- 3.0 ml.min-1) than in the general anesthesia (5.1 +/- 2.0 ml.min-1), while the amount of blood loss itself was without statistical difference. A significant increase in total volume of perioperative blood loss was also observed in the group with CSE anesthesia (1520 +/- 90 ml) compared with the general anesthesia group (1279 +/- 58 ml). These results show that spinal and/or epidural anesthesia can not always lead to reductions in operative blood loss during THR.

[Effects of prostaglandin E1 on renal function during deliberate hypotension with epidural anesthesia].

We studied the effects of prostaglandin E1 (PGE1) on renal function during and after deliberate hypotension induced by epidural anesthesia. Twenty-seven ASA class 1 patients undergoing rotatory acetabular osteotomy were divided into three groups according to the dose of PGE1. Deliberate hypotension was induced and maintained during the surgery by epidural anesthesia combined with general anesthesia. PGE1 increased urine output and creatinine clearance (Ccr) during hypotension in dose dependent manner, and the changes were statistically significant compared with control at the dose of 0.02 micrograms.kg-1 x min-1 (gamma). PGE1 also tended to increase postoperative urine output and Ccr. Regardless of PGE1 administration, urine NAG/Cr ratio tended to increase during and after deliberate hypotension. Though total dose of local anesthetics was attenuated by PGE1, serum catecholamine levels of all patients were within normal ranges. In conclusion, PGE1 0.02 gamma is effective to maintain urine output during deliberate hypotension with epidural anesthesia. However, PGE1 is unlikely to prevent renal tubular damage at the dose less than 0.02 gamma.

[A case of aortic dissection (DeBakey-I) occurring 20 months after aortic valve replacement].

Simple aortic valve replacement was performed in a 59-year-old man whose ascending aorta was moderately dilated (45 mm) because he had no characteristics of Marfan's syndrome or other connective tissue diseases. However, about 20 months later, reoperation needed to be performed for aortic dissection of DeBakey type I. The pathological findings of the aortic wall obtained at the initial operation revealed cystic medial necrosis. The occurrence of aortic dissection after aortic valve replacement is uncommon but not rare, especially in patients with cystic medial necrosis and/or dilated aortic root. Therefore, in these patients, wrapping or replacement of the ascending aorta may be required at the same time of aortic valve replacement.
