[Clinical evaluation of CA-50 in cases with colorectal cancer].

Serum CA-50 has been evaluated in 67 healthy donors and in 46 patients with a colorectal cancer by an enzyme immunoassay (EIA Kit, Mitsui pharmaceuticals, Inc.). The mean value of the healthy donors (n = 67) was 10.6 +/- 6.1 U/ml and for patients with a colorectal cancer 31.7 +/- 55.2 U/ml. Both in healthy donors and in patients, the mean value was found to be higher in females than males. The overall sensitivity in the colorectal cancer patients was 26.1 percent, the percentages for those in stage I, 20, II, 9.1, III, 40, IV, 20, and V, 50% respectively. In a correlation between serum CA-50 and CA 19-9, the correlation coefficient was 0.82 (p less than 0.01), and in between CA-50 and CEA, 0.51 (p less than 0.01). Thus a combination assay among CA-50, CA 19-9, and CEA has proved to be of significant value in cases of a colorectal cancer.

[A case of mediastinal cystic lymphangioma--case reports and review of the literature in Japan].

A 15-year-old woman was admitted to our hospital with complaints of left anterior chest pain and an abnormal shadow on her chest X-ray film. At the left thoracotomy, a large cyst (20.5 x 15 x 3 cm) was located in the left anterior mediastinum and contained translucent yellow fluid. Histological diagnosis was cystic lymphangioma. Mediastinal cystic lymphangioma is very rare among mediastinal tumors. Altogether 32 cases of mediastinal cystic lymphangioma including our case were collected from the Japanese literature and reviewed.

[Surgical treatment of bilateral spontaneous pneumothorax].

88 cases of bacterial spontaneous pneumothorax, 20 cases, occurred same time and 68 cases, occurred different time, were treated in our Department of Thoracic Surgery from 1975 to July 1987, and represented 16.6% of all cases of spontaneous pneumothorax. The characteristics of bilateral spontaneous pneumothorax, occurred same time, were as follows; 1) frequent in teens, 2) often occurred with history of unilateral spontaneous pneumothorax, 3) slightly collapsed. For the treatment of bilateral spontaneous pneumothorax, occurred same time, bilateral thoracotomy was necessary. The characteristics of bilateral spontaneous pneumothorax, occurred different time, were as follows; 1) frequent in the twenties, 2) often occurred the other side after unilateral thoracotomy within a year. For the treatment of bilateral spontaneous pneumothorax, occurred different time, if the patient is under 20 thoracotomy must be carried out, but if the patient is over 20 at the side of pneumothorax must be operated.

[Study about development of extracapillary blood flow type oxygenator--comparison with HF-4000, Capiox II, and Silox].

Results about comparison of new extracapillary blood flow type polypropylene hollow fiber oxygenator (PHO) with HF-4000, Capiox II and Silox for effects on blood components, hemolysis, immunoglobulin and high molecular weight proteins, such as complements: 1) Platelet protection was about equal. Silox was best in leucocyte protection. 2) PHO was superior to HF-4000. The extracapillary type was better for hemolysis. 3) Silox was best in immunoglobulin protection. Effect on C3 and C4 was equal, and Silox was best in protection of CH50. Slight differences were caused by silicone membrane, not caused by blood flow type. 4) PHO was superior to HF-4000 (commercial extracapillary type), and almost equal to Capiox II (intracapillary type).

Dynamics of the spinal cord: an analysis of functional myelography by CT scan.

The antero-posterior movement of the spinal cord with flexion and extension of the neck was analyzed in order to clarify the mechanism of spinal cord compression in cases with postoperative spinal deformity, and to contribute to the improvement of the surgical methods of conventional laminectomy. The control subjects were 47 cases without cervico-thoracic neurological symptoms, who underwent CT myelography in flexion and extension of the neck; the cervical spinal cord was examined in 27 of these cases and the thoracic cord in the other 20. CT myelography was also carried out in 16 patients with cervical myelopathy and in 5 patients after posterior decompression surgery (suspension laminotomy). CT sections in flexion and extension of the neck were analyzed for 1) change of configuration of the dura mater and the spinal cord, and 2) antero-posterior shift of the spinal cord in the subarachnoid space. In the control subjects, the configuration of the dura mater was slightly flattened at C5/6, C6 and C6/7 in extension of the neck. The cervical spinal cord shifted anteriorly in flexion and posteriorly in extension of the neck, and was flattened at the midcervical level in flexion in the control subjects. There was a statistically significant correlation between the location of the spinal cord and the adjacent intervertebral angles at the levels of C4, C5 and C6. These results were compared with the results from the 16 patients with cervical myelopathy and 5 patients after suspension laminotomy.(ABSTRACT TRUNCATED AT 250 WORDS)

A kidney transplant recipient with renal cell carcinoma derived from a native kidney.

A case of renal cell adenocarcinoma which originated from a left native kidney following kidney transplantation, with widespread metastases, including multiple bone and liver metastases, is presented herein. An extensive clinical investigation, including bone marrow biopsy, liver biopsy, abdominal computerized tomography, excretory urography and examination of the gastrointestinal tract failed to determine the site of the primary lesion in this patient. Examination at autopsy revealed a small adenocarcinoma of the left native kidney with lobulated nodular capsular invasion and metastases to the bone and liver.

Postoperative starch granuloma revealed as femoral herniation. A case report.

A rare case of postoperative starch granuloma in a 55-year-old woman, revealed as femoral herniation, is reported. A small, finger-tip-sized tumor showing femoral herniation was noticed 3 weeks after cholecystectomy for cholelithiasis, and no content in the hernia sac was noticed. Histologically, the granuloma situated in the wall of the hernia sac was diagnosed as a starch granuloma consisting of starch granules, which were clearly revealed by polarized light microscopy. These granules were detected in the cytoplasm of macrophages including multinucleated types, some showing a positive immune reaction with antibodies to the alpha-subunit of S-100 protein, vimentin and lysozyme. Starch granules from surgical gloves showed identical features by polarized light microscopy, thus proving the histogenesis of this granuloma. This is the first reported case in the Japanese literature of starch granuloma manifested as femoral herniation.

Oxatomide inhibits the release of chemical mediators from human lung tissues and from granulocytes.

The effects of oxatomide on the release of histamine and leukotriene C4 (LTC4) from human lung fragments and granulocytes were examined and the findings compared with the effects of the antiallergic drugs ketotifen, azelastine and tranilast. Oxatomide inhibited the release of both histamine and LTC4 from human lung fragments in cases of passive sensitization with human IgE myeloma serum upon anti-epsilon stimulation. IC50 values for the release of LTC4 from human lung fragments were as follows: oxatomide, 2.35 x 10(-5) M; azelastine, 27.2 x 10(-5) M; ketotifen, 52.1 x 10(-5) M; tranilast, 62.9 x 10(-5) M. Oxatomide also inhibited the release of both histamine and LTC4 from human mixed leukocytes stimulated by the calcium ionophore A23187. IC50 values for the release of LTC4 from human mixed leukocytes were as follows: oxatomide, 1.67 x 10(-5) M; azelastine, 3.65 x 10(-5) M; ketotifen, 12.2 x 10(-5) M; tranilast, 15.1 x 10(-5) M. The effects of oxatomide on the release of LTC4 from purified human neutrophils and eosinophils were also given attention. Oxatomide inhibited the release of LTC4 from eosinophils more effectively than from neutrophils and mixed leukocytes. As there is evidence that eosinophils play an important role in the development of late asthmatic responses and/or in the aggravation of asthma, oxatomide is expected to be an effective treatment for such conditions.

Antiinflammatory and antiallergic effects of a novel metabolite of Nocardiopsis sp. as a potent protein kinase C inhibitor from microbial origin.

The antiallergic and antiinflammatory effects of the new protein kinase C and calmodulin inhibitor K-252a (8R, 9S, 11S)-(-)-9-hydroxy-9-methoxycarbonyl-8-methyl-2,3,9,10-tetrahydro-8,11- epoxy-1H,8H,11H-2,7b,11a-triazadibenzo [a,g]cycl oocta [cde] trinden-1-one) were investigated in animal models, and the following results were obtained: 1. Oral administration of K-252a, ketotifen or oxatomide showed a dose-dependent inhibition on 48 h homologous passive cutaneous anaphylaxis in rats and anaphylactic bronchoconstriction in passively sensitized guinea pigs. 2. K-252a (1-100 mg/kg p.o.) exerted a dose-dependent protection against platelet-activating factor (PAF)-induced mortality in mice. This protective effect of K-252a was sustained for 7 h. 3. K-252a (1-100 mg/kg p.o.), as well as dexamethasone (1 mg/kg s.c.), showed remarkable inhibitory effects on rat paw edema induced by carrageenin, zymosan, PAF, and antigen-antibody reaction (passive Arthus reaction) and on the croton oil-induced rat ear edema. On the other hand, indomethacin (10 mg/kg p.o.) significantly inhibited carrageenin-induced edema, but did not inhibit edema induced by other phlogistic agents. Based on these results it is suggested that K-252a, by oral administration, has antiallergic and antiinflammatory effects.

[Two cases of adrenal myelolipoma].

Myelolipoma of the adrenal gland is a benign oligosymptomatic tumor composed of various proportions of fat and bone marrow elements. We report two operated cases found incidentally in a 76-year-old man and 49-year-old man.

Immunocytochemical demonstration of Chlamydia infection in the urogenital tracts.

The peroxidase-antiperoxidase (PAP) technique was applied in cellular samples for the detection of chlamydial infection. Urethral scrapings were obtained from 316 males with clinically suspected urethritis. Positive PAP staining was detected in 118 (37%) of 316 tested. Cellular samples from the endocervix of 25 (54%) of 46 female contacts of males with positive Chlamydia-infected cells had positive Chlamydia-PAP staining. Chlamydia was also found in 12 (25%) of 48 male with gonorrhea. Serum IgG antibodies against Chlamydia trachomatis were evaluated using an enzyme-linked immunosorbent assay. A total of 29 out of 73 patients had positive Chlamydia serology; of these, 21 were positive for PAP staining, and eight were negative. The determination of Chlamydia antigen by means of the PAP technique appears to be a satisfactory method for detecting Chlamydia in male and female genital specimens; also, the procedure can be done readily in most laboratory settings.

Suspension laminotomy: a new surgical technique for compression myelopathy.

In an attempt to eliminate the many negative aspects of conventional extensive laminectomy, we originated in 1981 and have developed a new surgical technique called "suspension laminotomy." A basic principle of this operation is preservation and repair of the soft and bony tissue of the spine with osseous enlargement of the spinal canal. Eighty-one patients were treated with suspension laminotomy. This operative technique can be applied to almost all segments of the spine and can be used for all cases that would usually have been treated with conventional extensive laminectomy. We emphasize the necessity of repairing paraspinal muscles and discuss the postoperative spinal deformities and the dural constriction, which seemed to be caused by scar formation after conventional laminectomy. Suspension laminotomy should minimize the negative aspects of conventional extensive laminectomy.
